Important facts about leg ulcers, what causes them, how they can be prevented and cured.

Leg ulcers are common -- approximately 2% of adults will have a leg ulcer at some stage in their lives. Literally thousands of people develop a leg ulcer each year in the United Kingdom and many of those get more leg ulcers as the years go by. Despite the misery they cause, leg ulcers remain a neglected problem. I am going to tell you the 7 essential facts should you know about leg ulcers. Number 1 -- What is an ulcer? The medical definition of an ulcer is a break in the epithelium of a body surface or lining. Many of us will be familiar with a mouth ulcer. This is a break in the cell lining the mouth. It can be caused by an injury such as biting the inside of the mouth by accident. Well if the skin breaks down anywhere on the body it is called an ulcer. If the ulcer is on the leg and has been present for more than 6 weeks it is called a chronic leg ulcer. Number 2- What causes a leg ulcer? Four out of 5 ulcers -- that's 80% - are caused by a problem with the leg vein pump and they are called venous ulcers. The leg vein pump is the mechanism by which blood flows out of the leg back to the heart. So problems with the deep veins such as obstruction from a deep vein thrombosis, superficial vein reflux or perforator vein problems can cause venous leg ulcers. The leg vein pump depends mainly on good ankle movement, good calf muscles and healthy leg veins. A minority of leg ulcers - about 15% - are caused by problems in the arteries that carry blood into the leg. About 95% of venous leg ulcers can be correctly identified by their appearance, by checking for pulses in the feet and by checking sensation. As most leg ulcers are venous -- that's what I am going to concentrate on. Number 3 -- Where on the leg do you get venous ulcers? Well venous leg ulcers appear on the lower part of the leg usually just about the ankle. Often, there are varicose veins or thread veins around the ulcer and the leg is usually swollen. The skin is often thickened and discoloured by varicose eczema which may actually develop many months or years before the ulcer as a warning sign that the vein circulation is not normal. Number 4 -- Who gets a venous ulcer? Well these ulcers affect people who have deep vein problems, usually from a past deep vein thrombosis (DVT), people who have superficial venous disease or those with ankle mobility problems. Leg ulcers are more common as we get older. Number 5 -- How do you treat venous ulcers? The aim of treatment is to get the ulcer healed as quickly as possible. This usually means graduated compression either with bandages or compression stockings. In most cases, antibiotics, creams and ointments are not necessary. Once the ulcer is healed it is very important to identify any problems with the superficial veins or perforator veins and that these are treated to reduce the risk of another ulcer. This means getting a full assessment with a duplex ultrasound scan. Number 6- Can you cure a leg ulcer? Well this is controversial. Some would say that if the problem is only in the superficial veins, the veins under the skin, or in the perforator veins that connect these superficial veins to the deep veins, then the ulcer can be cured. While there is good evidence that such treatment will greatly reduce the risks of getting another leg ulcer, if the skin has been very damaged and the ulcer has healed with a lot of scarring, this area of the leg may remain vulnerable and a trivial knock or injury might cause another skin breakdown even though the vein abnormality has been completely eradicated. Number 7 -- How can we avoid leg ulcers? Well there are 3 things I would suggest. Firstly, if you are unfortunate enough to suffer a deep vein thrombosis (DVT), it should be treated promptly to limit the damage to the deep veins. Unfortunately, damage to the deep veins cannot be reliably reversed by surgery. So, prompt treatment with anticoagulants -- blood thinning medication -- reduces the extent of the clot and the damage to the deep veins. Secondly, if you have had a DVT, you should consider wearing good quality medical grade compression socks -- some studies suggest that doing so may reduce the risk of leg ulcers after a big DVT. Thirdly, if you have superficial vein reflux have it treated early to reduce the chances of progression to varicose eczema which is itself a warning sign that the skin is damaged and liable to ulceration. Dealing with a leg ulcer due to ankle fracture repair basically trauma to my ankle. I been receiving treatment from wound care center and wearing compression socks for 5 months now. Soon I'll be going in for laser ablation procedure for a varicose vein that lies underneath my ulcer. Can't wait for this to be over. Its depressing and painful to go through. I am on 2 years, and between Xeroform cases, Boil Ease creams, Tea Tree oil, bandages, Neosporin, sprays and tapes of all sorts, I've easily spent over $1,000 in 2018 so far. And does not heal. Presently I have lower leg cast from 32days and with ulcer at time of accident. Now I didn't get pain from bone but suffered with ulcer. It will painfull. Can I removed the cast and do for the treatment of ulcer problem r else can I wait till 42 days course of cast.
Thank you for this video. I wish I had watched it earlier. I had discolortion of skin but my skin doctor did not warn me about a vein circulatiin problem. Eventually this lead to ulcers on my foot and not leg. The duplex ultrasound showed reflux on both deep vein as well as superficial veins. Compression bandages helped the ulcers heal and now I wear compression stockings. Fortunately the ulcers have not returned.

I suffered from DVT after my first pregnancy 9 years back and since last year im again suffering from vasculitis on my left leg with ulcers.......i did the pus swab test also ......how long will it take to heal? Shiukd i start wearing the stockings of veins now?will it help?
After a DVT, the vein circulation may be severely damaged. You should consult your own specialist for specific advice, but in general compression stockings do help. Please ensure that you are carefully measured and fitted and that there are contra-indications to wearing stockings in your case (eg artery disease or diabetes).

hello. I havd a neighbor wbo has ulcers all over her lowerlevs and complaims of vreat pain so much so that she sleeps sitting up. she said that this started about 9 months ago after a hospital where they over hydrated her with infused fluids. do you have any ideas what this is?
Elizabeth Gadus has your neighbour been to hospital yet? If not please tell her to go asap. A relative of mine was in the same situation, he couldn't sleep lying down and would sleep in a chair all the time. He was too stubborn to go to hospital and left it for months until he had ulcers on both legs.
Ive suffered from venous skin ulcers for awhile now, ive had multiple DVTs and countless surgeries to my veins since i also suffer from Factor V deficiency. Ive been doing a bit of research and im curios to know if an ulcer develops on the side of your foot would this fall into a different category? Being white and splotchy in color but painful at times. I currently see a cardiologist thats also my surgeon but he doesn't seem to know what else that could be causing this issue since all dopplers dont show any more signs vein issues. Not too sure who to turn to next and I would like some advice. Thank you in advance.
Have had chronic ulcers for around 3 years now,it did kind of heal afew year ago with Mankuna honey and compression but kept and keeps coming back,always the same two places,right on the ball of the ankle bone and one on my shin,the ankle one is so so painful at times.As of today im back in compression,just hope it works.My job is very foot heavy,12 hour shifts with little to no breaks,that wont help but bills to pay etc,pain meds aint cheap either week after week but needs must.Great vid doc.

My husband had his leg crushed as a teenager, the Drs chose to cast the compound fracture to Tibia/ Fibula they monitored the leg for gang green for 10 months. Since his first on set of the ulcers only started after he became disabled and unable to walk on his legs that his job as a warehouse and yard manager required him to do . My question is if one limb is effected by the ulcers is it a possibility that the other limb will be effected as well in time. Ever Thankful,
my mum has 2 large ulcers due to poor circulation she had an angioplasty which seems to have helped the blood flow. the ulcers are on either side of her ankle and you can see her bone through one. she is currently on maggot therapy but as she is on warfarin she has had 2 major bleeds which required an emergency call for an ambulance. I'm so worried she will lose her foot I just wish I could help her.
My father was in Coban compression wraps for 5 years-- the nurses were wrapping so tight he developed 2 infected unstageable ulcers at the calf... I have since taken over wrapping and am using 4 inch cotton conform wrap -- took me 20 days shy of a year to heal the ulcers.. now.. since I have been wrapping uniformly his leg has balanced-- I took opportunity to remove compression all together-- and his edema is under control... problem.. he complains of pins and needles to the level 10 at some times-- however other days no pain... is this just the capillarys coming back to life?-- he had severe stasis dermatitis-- however after a week the skin sloughed off to near normal-- still have dry skin issues -- I have him on B100 twice a day-- plenty water-- and only use coconut oil for moisture-- I keep his leg in a flannel sleeve( no compression) and a woolen sock on-- if I keep his limb warm he has comfort-- second question-- sheep skin therapy boots-- as I say-- heat and warm are his comfort... what do you think of sheep skin therapy boots-- I need to be able to apply graduated support from toes to just above ankle-- and can do his magic number ( around 15-20) via the velcro tabs...he is 82-- I also have him do at lest 5 min a day walking on the treadmill... need thoughts on sheepskin medical boots-- thank you so much -- he was dismissed from our Sask health care and handed to me-- infected ulcers and all-- I had to learn from utube compression therapy-- thank goodness for videos... I have tried juxa boot-- he gets shear-- tried all manner of compression stockings.. tourniquet effect-- and shear--- proper graduated wrapping with cotton did work however h has poor reaction to any synthetics-- and he seems as if he doesnt need compression anymore...well slight if any -- as for what he has-- I have asked for years-- venous insufficiency-- which covers a myriad of things... sigh.... poor health care here... boots? is it possible to have a leg ulcer on the upper thigh? I noticed a blackish bump in July on my thigh. it's never gone away, and just last night, I noticed it now is open in the center. I'm kind of freaked out about it.

Hi there, I had varicose eczema for 16 yrs from 1996 to 2012 tried lots of creams and potions they never helped so I just put up with it. I was diagnosed in may 2012 with rheumatoid arthritis, and was prescribed methotrexate. "The eczema totally vanished" however other problems I have now are small kidneys which the renal and rhemo are watching "stage 3" Renal impairment. Could this have stemmed from the varicose eczema and also could the venous problem be "underlying" to this day even though my leg has totally cleared up? Thanking you in advance..:)
I have no experience of the results of prednisilone with varicose eczema. However as varicose eczema is an inflammatory condition and as prednisilone is a powerful anti-inflammatory, it is possible that the response you have noted is connected. Thank you for sharing your experience.

I was taking prednisolone for a few months at first diagnosis, could that have rid me of this leg problem, because it was in 2012 when I started all this medication, that it totally disappeared. P.S. looking at this video really does look like what I had same look same place etc....
Thank you for sharing your experience. As far as I am aware, methotrexate does not help leg eczema due to vein problems. So, I think the initial diagnosis of varicose eczema may have been incorrect and that the eczema was caused by another problem.
hi, I started having some itching with a hard dark skin on the ankle joint , dont know if the itching is from an insect bite or venous stasis, now the skin got ruptured and started oozing with pain and edema, pain high on standing or putting weights . Sir, is turmeric helpful or neosporin ointment good, can u help me, thanks
Thank you. We have no experience of using turmeric. Do you have any information on this? With regard to neosporin, in general we do not advise topical antimicrobials. It would be best to see your local specialist for a duplex scan and wound care by an experienced nurse.
what about Santyl? A gel that helps to eat the fibrin almost 100% painlessly. I am going into my 7th month and thank God almost healed up cause of that and another cream at the Foot and Ankle Specialist Center for Eastern Carolina here in Greenville N.C
Yo Terry, I have Sickle Cell too (as you can see in my comment on the video) and I'm going through this with a huge ulcer on my ankle, gotta be wheelchair bound for almost 2 years now.

Those 2 help my leg burning more than when I wasn't taking them.
Don't get me messed up, I'm still in excruciating pain daily, but it does help somewhat.

In 4 years I have gone to Internists, neurologists (in case it might have helped - it did not), cardiologists, eczema and varicose "experts", cardio-vascular surgeons and several wound centers - and not in the last 2 years has anyone offered any advise not heard before. If you were Tricky Dick your problem would be solved. Clots need to be cleared to solve the stasis in the lower extremities. The best thing for the doctors is to treat your wound not the cause.
why is it,,, I have healed my own vienous ulcer that I had for 5/6 years it was horrific time for me,, I was under a specialist,, sue tickle for a long time and no one could get a hold on it,, I eventually found my own method and it healed my ulcer in just over a week,, it also cured all the infection without anti biotics,, I have tried to get support with this method to help make a new wound dressing but no one seems to want to help,, this method involves using a natural product from a tree,, I paid for preliminary research as it has not been thought up yet and is not formulated anywhere in the world,, this method must be very important but my NHS are no help,, mind you I wouldn't want someone to come along with a one time application and take away all the wound dressing profits either,, I want to develop this for all the people who suffer in the world not for pride or credit. what can I do
If you truly have had a CVLU for 5/6 years you should know the pain we are in. Heaven forbid you never get money. The mind knows the questions, the soul knows the answers A kind gesture can reach a wound that only compassion can heal.
I've had lower leg ulcers for 5 yrs now. I've been to clinics, had the compression wraps and stockings, but since I'm unable to treat my legs on my own I have let them go. I would be very interested in hearing what method you used for your venous ulcers. Would you please share this info?
hi I've had leg ulcers on leg FOR 10 years now.also have tried every thing.but they have now just about cleard up,I get them dressed twice a week. with honey _plus compretion stocking an am so blown away!!!I thought they would never go away..hope this may help,,

I am suffering from leg ulcers and have been using compression bandages and hydro gel in which my doctor prescribed but it has been a month and we change the bandages daily because of drainage do you think a Una boot would work better for me?

What is it likely of? is it DVT or what? What things should i do to heal that? please suggest ASAP.

However, from what you describe it is possible her problems are a result of a deep vein thrombosis. She needs to have a diagnostic duplex scan of the deep and superficial veins and then have her arterial pulses and Doppler pressure indices measured. Graduated compression therapy may appropriate. +Manoharan Thangavelu I suspect good diet is important in the healing process - fresh, un-processed and home cooked food with an abundance of green vegetables. I advise restricting sugar and alcohol and not smoking. Walking is a good exercise for the leg circulation. I am happy to receive further suggestions.
+Manoharan Thangavelu Some people have found these approaches helpful with the symptoms but to deal with the underlying issues, a duplex scan is needed and if there is any superficial venous reflux this should be addressed.
for the past 3 three months i have leg ulcer on right foot, inside ankle. it is flat with no hole,it seem like a burn, about 3 inches by 2 inches in size, and it is always leaking out lots of watery substance, would it be wise if i dress the leg with a Medicopaste bandage with Calamine, which is a unna boot dressing?

For the last 2 months I have 2 leg ulcers on the inside of my ankle. They are not large or that deep. They seemed to have scabbed over but there is still a small crater where the wound is. What are your thoughts on taking "Nattokinase" to help with circulation? I have heard some great things about this. Currently, my dermatologist has me applying antibiotic cream to the wound and bandaging it 2x per day. There are times there is no pain and times when it constantly hurts, especially at night. Just looking at it, it doesn't look bad. 2 fairly small crater like scrapes, but it's very aggrivating that it's taking this long to heal.
The dr. I was seeing wanted me to wear compression stockings. I simply will not go around with stocks on. I am an avid bodybuilder and will not be seen wearing stockings. the wounds are pretty much healed. I still put this stuff on called xenox or something like that. And I use an ace bandage. it's still a little bit tender. But its not longer scabbed but has a white coating over the wounds.

I'm dealing with a leg ulcer now; caused by varicose veins. It's chronic. It is life-changing, and often depressing and worrisome. I wish I'd had this information years ago. I've had varicose veins in my right leg for years, and had signs of the leg ulcer developing for more than 2 years, but did not know what it was.
Pigment changes are clue. Please do not scratch any area in or around the wound. I dig into my skin and not $1,000 offer from any stranger could stop me from scratching until I bleed.
