Recently, the Mayo Clinic published startling research showing that 40 percent of pancreatic cancer patients had been diagnosed with diabetes one to two years before discovering they had a pancreatic tumor. Researchers believe the diabetes is caused by tumors that simply haven't been detected yet. The problem is, diabetes is very common, and the majority of diabetes isn't pancreatic cancer, so doctors are trying to develop screening tools to tell the difference. Right now, they say family history is an important clue. If you're diagnosed with diabetes that seems to come on suddenly and you have no family history of diabetes, bring this to your doctor's attention and ask for further screening for pancreatic cancer. Yellowing of the eyes or skin. Even a small pancreatic tumor can block the bile duct in the head of the pancreas, causing bile to build up. This causes jaundice. Itchy skin, palms, and soles of feet. A little-known side-effect of jaundice is itchy hands and feet. It's due to a skin reaction to the bilirubin, the yellowish brown liver chemical that causes jaundice. Lack of appetite. What are the symptoms? How do you know if you have pancreatic cancer? Should you be screened? Over the next several minutes, I will give an overview of pancreatic cancer. Pancreatic cancer is the fourth leading cause of cancer-related deaths in the United States. The pancreas is the organ that sits in the central upper abdomen, is located right behind the stomach organ. The pancreas is about six to eight inches long and about two inches wide, and it extends from the head of the pancreas, which is on the right side of the abdomen, connected to the small intestine, near the gallbladder and the liver, and the pancreas extends to the tail of the pancreas, which is more on the left side of the abdomen. The pancreas is the organ that's responsible for producing certain hormones such as insulin, which regulates your blood sugar. The pancreas also produces certain digestive enzymes or digestive juices, which help us to digest fat. There are two types of pancreatic cancer. One, which comes from the endocrine glands or the hormone-producing cells such as an insulinoma. The other type of pancreatic cancer comes from the exocrine glands or from the pancreatic ducts, and this exocrine gland pancreatic cancer is the most common type of pancreatic cancer. What are the symptoms of pancreatic cancer? One is weight loss. Patients with pancreatic cancer often have early satiety, meaning they get full quickly off of just small amounts of food, or they may just lose their appetites altogether. Another symptom, abdominal pain. Patients often have a dull, aching abdominal pain in the central upper abdomen, and it often radiates to the back. Another symptom, jaundice. Jaundice occurs when the patient's skin turns yellow or the whites of the eyes turn yellow, and this is due to a blockage of the flow of bile by the pancreatic cancer. Patients who have jaundice, this yellow skin, will often also have a change in their stools. How can you be diagnosed with pancreatic cancer? If you have any of the symptoms I described, it is important that you consult your physician as soon as possible. Your physician will do a complete history and physical, and they will likely do some imaging studies such as an ultrasound of the pancreas or a CT, computed tomography, of the pancreas or a test called an ERCP, endoscopic retrograde cholangiopancreatography. If they are suspicious or if they see a mass that looks like pancreatic cancer, then they may do a biopsy, which is when they have a physician to take a small sample of the mass, and a pathologist is another type of physician who will look at that and determine if it is or if it isn't pancreatic cancer. The treatment for pancreatic cancer. The treatment really depends on the staging. If the cancer is caught early, there's a chance that you can have surgery or surgical removal of the cancer. and patients often will have an adjuvant therapy, which means they will receive chemotherapy and/or radiation, especially if the pancreatic cancer has spread. In cases of stage four or metastatic pancreatic cancer, a lot of those patients will also receive chemotherapy. However, that chemotherapy is usually just to shrink the tumor or to help to decrease the symptoms of the tumor. Stage four pancreatic cancer is usually not curable. So what are the risk factors for pancreatic cancer? Well, five to 10 percent of patients who have pancreatic cancer also have family members with pancreatic cancer. So, we do believe that there is some hereditary component. Other things that have been associated with pancreatic cancer include cigarette smoking, obesity, physical inactivity, the Helicobacter pylori infection. But quite honestly, we really don't always know the cause of pancreatic cancer. This ends my overview of pancreatic cancer symptoms, signs, and risk factors. I want you to take care of yourselves, and if you have any of the symptoms that I described, please consult your physician immediately. Those symptoms are weight loss, abdominal pain, jaundice. Pancreatic cancer usually spreads rapidly and is seldom detected in its early stages, which is a major reason why it's a leading cause of cancer relating death. Signs and symptoms of pancreatic cancer may not appear until it is quite advanced and complete surgical removal isn't possible. Causes of pancreatic cancer. 1. Genes - the DNA type. 2. Genes - the family type. 3. Carcinogens. 4. Other medical factors. 5. Traits, habits, and diet. Stages of pancreatic cancer. Stage I. Cancer is confined to the pancreas and can be removed using surgery. Stage II. Cancer has spread beyond the pancreas to nearby tissues and organs and may have spread to the lymph nodes. At this stage, surgery may be possible to remove the cancer. Stage III. Cancer has spread beyond the pancreas to the major blood vessels around the pancreas and may have spread to the lymph nodes. Surgery may or may not be possible to remove the cancer at this stage. Stage IV. Cancer has spread to distant sites beyond the pancreas, such as the liver, lungs and the lining that surrounds your abdominal organs. Surgery isn't an option at this stage in the disease. Pancreatic Cancer Symptoms. Pancreatic Cancer and Gastrointestinal Symptoms. -Abdominal pain. More than 80% of people with pancreatic cancer eventually experience some abdominal pain as the tumor grows. Pancreatic cancer can cause a dull ache in the upper abdomen radiating to the back. The pain may come and go. -Bloating. Some people with pancreatic cancer have a sense of early fullness with meals (satiety) or an uncomfortable swelling in the abdomen. -Pale-colored stools. If the duct draining bile into the intestine is blocked by pancreatic cancer, the stools may lose their brown color and become pale or clay-colored. Urine may become darker.- -Diarrhea. -Nausea. "Tests with the high likelihood of detecting pancreatic cancer, such as endoscopic ultrasound, are invasive, costly, and may carry risks of complications that outweigh their use as screening tests for the average person," New onset of diabetes! "When people with no risk factors for diabetes (no family history, good diet, thin body type) develop new-onset diabetes, or who have a history of well-controlled diabetes suddenly find that it becomes difficult to control, it's reasonable to evaluate why the pancreas may not be functioning as well. Abdominal pain! "The pancreas sits close to a bundle of blood vessels and nerves called the celiac plexus that can cause pain and nerve irritation. But the pain is usually pretty encompassing of your abdomen. "Patients feel pain from the epigastrium (the area just under the sternum) through to their back. Imagine a tire hiked up right under your chest and around your abdomen. Unexplained blood clots! Patients with pancreas cancer are particularly prone to blood clots. While the exact cause is not known, it is believed that cancer cells reduce the proteins the body uses to prevent blood clots. "For people who do not have a reason for the development of a clot-a recent major surgery, trauma (like a car accident), hospitalization, or prolonged period where you are laying still, or a hereditary predisposition to clotting-we recommend making sure you are up to date on your cancer screening and to check in with your doctor about any other symptoms that may help to identify the source of the clotting abnormality. Jaundice! The pancreas is located close to the liver, which produces bile. If a tumor prevents the bile ducts from draining bile appropriately, people may develop elevated levels of bilirubin in their body, which translates into jaundice, or a yellowing of the skin. "This is usually best seen in the white of the eyes, and can also lead to itching all over the body, light-colored stools, and darkened urine. Unexplained weight loss and change in eating patterns! If you experience a loss of appetite or feel full after eating very little, this also may be a sign of pancreatic cancer. The cancer is sometimes detected because of medical tests and procedures trying to find what is causing some nonspecific symptoms. Regrettably, in the early stages, pancreatic cancer often causes few (if any) distressing symptoms until later stages when the treatments (chemo and radiation) only slow the progress of the disease. Early diagnosis is important while there are treatment and surgical options available 1 Watch for jaundice. One of the first signs of pancreatic cancer can be jaundice, which is the yellowing of the skin, eyes and mucus membranes due to too much bilirubin in the bloodstream.[3] Jaundice is associated with liver disease, although pancreatic cancer often causes nearby organs, liver and gallbladder to swell, which leads to dysfunction. Look at your skin and eyes in a well-lit mirror to check for yellow discoloration. Jaundice also causes itchy skin, so be on the look out for that also. The parts of the eyes that turn yellow are called the sclera (the "whites") not the iris, which is typically colored either brown, blue or green. To confirm jaundice (if the yellowing is not so obvious), your doctor may test your urine for bile or order a blood test. 2 Take notice of a sore abdomen. One of the first signs of pancreatic cancer can sometimes be a diffuse abdominal aching and soreness, although many people don't feel any pain until the cancer is advanced. The pancreas gland is located underneath the stomach and behind intestines — pretty much in the middle of your belly. It secretes insulin (for blood sugar control), hormones and digestive enzymes. [4] If your belly soreness doesn't go away after a week, contact your doctor. Palpating (deeply touching) your pancreas for mild-to-moderate swelling is difficult and virtually useless for doctors because the gland is positioned behind and close to other organs. Because pancreatic cancer often causes the liver and/or gallbladder to swell, which are easier to palpate and detect, the condition may be misdiagnosed as liver cirrhosis or cholecystitis. Due to abdominal tenderness, fatigue and diarrhea, the early stages of pancreatic cancer can mimic infections, ulcerative colitis, Crohn's disease and irritable bowel syndrome. 3 Be alert to fatigue and weakness. Another early sign of pancreatic cancer -- as well as in most other types -- is a general feeling of tiredness, fatigue and weakness.[5] This is because pancreatic cancer may block the flow of digestive fluids -- and also, cancer cells tend to use lots of resources, burn lots of energy and trigger your immune system to work harder. 6 Pancreatic Cancer Symptoms You Should Never Ignore When was the last time you thought about your pancreas? Chances are, you rarely give much thought to this glandular organ, which helps your body digest food and convert it into energy. Located behind your stomach, the pancreas produces insulin and regulates your body's hormones that maintain glucose (blood sugar) levels. Pancreatic cancer is overall pretty rare, with 55,440 people in America expected to be diagnosed in 2018. (The most commonly diagnosed cancer is non-melanoma skin cancer, affecting more than 1 million people a year, followed by lung cancer, with more than 200,000 cases diagnosed annually.) But even though it’s an uncommon diagnosis, pancreatic cancer is soon expected to overtake colon cancer as the number-two leading cause of cancer deaths in the United States, says Valerie Lee, MD, pancreatic oncologist at Johns Hopkins Medicine. Why? First, because pancreatic cancer is very aggressive. “It has a high likelihood of spread, or metastasis, to other organs in the body," says Neil Woody, MD, oncologist at the Cleveland Clinic. The five-year survival rate is only 8.5 percent. But it may also be because pancreatic cancer is on the rise. Pancreatic cancer has been diagnosed at an increase of .5% a year for more than a decade. Part of that is due to increased detection, Dr. Lee says, but lifestyle factors may be playing a role as well. Increases in diabetes and obesity—two major risk factors for pancreatic cancer—may also be contributing to the rise in pancreatic cancer cases. Pancreatic cancer warning signs. Pancreatic cancer is notoriously difficult to diagnose. Only about 10 percent of pancreatic cancers have a genetic predisposition, and good screenings for the general population have been difficult to establish, says Dr. Woody. “Tests with high likelihood of detecting pancreatic cancer, such as endoscopic ultrasound, are invasive, costly, and may carry risks of complications that outweigh their use as screening tests for the average person,” he explains. What’s more, pancreatic cancer is tough to spot early, and many of the symptoms can be caused by other conditions. Plus, people usually have no symptoms until the cancer has spread to other organs. Still, if you are experiencing any of these warning signs, it’s worth making an appointment with your doctor. 1. New onset of diabetes “When people with no risk factors for diabetes (no family history, good diet, thin body type) develop new onset diabetes, or who have a history of well controlled diabetes suddenly find that it becomes difficult to control, it’s reasonable to evaluate why the pancreas may not be functioning as well," says Dr. Lee. 2. Abdominal pain "The pancreas sits close to a bundle of blood vessels and nerves called the celiac plexus that can cause pain and nerve irritation, says Dr. Lee. But the pain is usually pretty encompassing of your abdomen. "Patients feel pain from the epigastrum (the area just under the sternum) through to their back. Imagine a tire hiked up right under your chest and around your abdomen," says Kim Reiss-Binder, MD, an oncologist specializing in pancreatic cancer at the University of Pennsylvania. 3. Unexplained blood clots "Patients with pancreas cancer are particularly prone to blood clots," says Dr. Lee. While the exact cause is not known, it is believed that cancer cells reduce the proteins the body uses to prevent blood clots. “For people who do not have a reason for the development of a clot—a recent major surgery, trauma (like a car accident), hospitalization, or prolonged period where you are laying still, or a hereditary predisposition to clotting—we recommend making sure you are up to date on your cancer screening and to check in with your doctor about any other symptoms that may help to identify the source of the clotting abnormality," says Dr. Lee. 4. Chalotte Rae, familiar to millions of TV viewers as the housemother on the '80s sitcom The Facts of Life, was diagnosed in 2009 with pancreatic cancer, an often-silent killer with few if any symptoms until it is too late. Some 40,000 new cases of pancreatic cancer are diagnosed each year, and by the time diagnosis is made, up to 80 percent of patients are no longer candidates for treatment. "I had no symptoms," Rae says. "I had absolutely no symptoms. None whatsoever." But Rae was, in a way, lucky. Because she had a family history of the disease - her mother, uncle and older sister all died from the disease - she underwent early screening, which detected the cancer at an early stage. "We're working hard to develop tests for earlier diagnosis," says Howard Reber, M.D., director of the UCLA Pancreatic Cancer Program. The goal is to create something similar to the PSA test now done to detect prostate cancer in its early stages, before it has had a chance to spread. "In patients where we know that there's an increased likelihood of the development of the disease, we can screen them, we can get CT scans, we can get endoscopic ultrasounds," Dr. Reber says. In Rae's case, the cancer was detected and found to be contained, but it was growing fast. Surgery was performed to remove the cancer, and now, following surgery and chemotherapy, Rae is cancer free. 10 Early Warning Signs of Pancreatic Cancer 1. Diabetes, especially if it comes on suddenly. Recently, the Mayo Clinic published startling research showing that 40 percent of pancreatic cancer patients had been diagnosed with diabetes one to two years before discovering they had a pancreatic tumor. Researchers believe the diabetes is caused by tumors that simply haven't been detected yet. The problem is, diabetes is very common, and the majority of diabetes isn't pancreatic cancer, so doctors are trying to develop screening tools to tell the difference. Right now, they say family history is an important clue. If you're diagnosed with diabetes that seems to come on suddenly and you have no family history of diabetes, bring this to your doctor's attention and ask for further screening for pancreatic cancer. 2. Yellowing of the eyes or skin. Even a small pancreatic tumor can block the bile duct in the head of the pancreas, causing bile to build up. This causes jaundice. 3. Itchy skin, palms, and soles of feet. A little-known side-effect of jaundice is itchy hands and feet. It's due to a skin reaction to the bilirubin, the yellowish brown liver chemical that causes jaundice. 4. Lack of appetite. An Italian study found that six to eight months before being diagnosed with pancreatic tumors, patients reported a sudden drop in their appetite and a tendency to feel full after eating very little. 5. Changes in taste. In the same Italian study, some of the patients surveyed said they'd suddenly lost their taste for coffee, wine, and smoking. In fact, they said, they felt "disgust" for the smell and taste of coffee and alcohol. 6. Abdominal pain. Pancreatic cancer sufferers remember this pain as a gnawing pain, rather than a sharp cramp or ache, and it radiates toward the back. A characteristic clue: the pain goes away when you lean forward. 7. An enlarged gall bladder. The same blockage of the bile duct that causes jaundice can also cause an enlarged gallbladder, as the bile builds up behind the duct. The good news is that an enlarged gallbladder can be seen on imaging tests, and it may even be possible for a doctor to feel it during a physical exam. 8. Pale, floating, smelly stools. If a pancreatic tumor prevents digestive enzymes from reaching the intestine, the result is an inability to digest fatty foods. Recently, the Mayo Clinic published startling research showing that 40 percent of pancreatic cancer patients had been diagnosed with diabetes one to two years before discovering they had a pancreatic tumor. Researchers believe the diabetes is caused by tumors that simply haven't been detected yet. The problem is, diabetes is very common, and the majority of diabetes isn't pancreatic cancer, so doctors are trying to develop screening tools to tell the difference. Right now, they say family history is an important clue. If you're diagnosed with diabetes that seems to come on suddenly and you have no family history of diabetes, bring this to your doctor's attention and ask for further screening for pancreatic cancer. 2. Yellowing of the eyes or skin. Even a small pancreatic tumor can block the bile duct in the head of the pancreas, causing bile to build up. This causes jaundice. 3. Itchy skin, palms, and soles of feet. A little-known side-effect of jaundice is itchy hands and feet. It's due to a skin reaction to the bilirubin, the yellowish brown liver chemical that causes jaundice. 4. Lack of appetite. An Italian study found that six to eight months before being diagnosed with pancreatic tumors, patients reported a sudden drop in their appetite and a tendency to feel full after eating very little. 5. Changes in taste. In the same Italian study, some of the patients surveyed said they'd suddenly lost their taste for coffee, wine, and smoking. In fact, they said, they felt "disgust" for the smell and taste of coffee and alcohol. 6. Abdominal pain. Pancreatic cancer sufferers remember this pain as a gnawing pain, rather than a sharp cramp or ache, and it radiates toward the back. A characteristic clue: the pain goes away when you lean forward. 7. An enlarged gall bladder. When patients diagnosed with cancer are admitted to the hospital, many times we see them at their lowest. From chemotherapy and radiation treatments to surgeries and diagnostic test. It can be an emotional roller coster for not only the patient but also all of their loved ones… including their medical staff… I remember about 2 months ago, I was feeling down. Normally I’m an extremely upbeat person. However, working on the oncology unit was taking it’s tole on me. I realized, that I was mistakingly focusing on some of the patients who didn’t make it. Unfortunately, many of the patients who recover fully do not make their ways back in the the hospital. I can’t blame, the hospital would not be high on my list also. Recently, I attended a fundraising event with my husband and I heard a beautiful dynamic speaker by the name of Camille. Don’t tell anyone, but I actually cried while she was talking… By the time she finished speaking, I knew I had to ask her to share her story on my channel. I waited for 2 hours to talk with her and after the crowds had died down, she graciously accepted my offer. Spending time with Camille was like taking a breath of fresh air. Imagine walking into someones home for an interview and a bubbly 60 year old greets you with tons of energy, gorgeous hair and a wholesome outlook and love for life. This is who Camlile is, just a loving bundle of joy - I left her house inspired and grateful. I hope you feel the same way after listening to her story. Camille was given one of the worst diagnosis… Had I been reading her chart, I unfortunately would have not spent too much time with it… I have never met anyone that survived Pancreatic Cancer, much less; Stage 4 Pancreatic Cancer with metastasis aka METS. The first doctor she went to told her that she that with stage 4 Pancreatic Cancer with spots on in her lungs, liver and kidneys; there was nothing they could do and she was to go home and prepare to die. However, one thing that she strongly recommends is to get a second opinion, which is what she did. She went to University of Miami’s Sylvester Comprehensive Cancer center. She found a doctor that believed in her and she trusted him. She will be the first to tell you that her story is not the norm and oftentimes told me how it hurts her to know that many others have had the same diagnosis and not the same outcome. However, her main focus is to share that no matter how bad things seem, you can always and MUST always have hope. Thankfully Camille is not alone these days.
