When I consider purchasing an individual health insurance plan for myself or my family, do I have any financial obligations beyond the monthly premium and annual deductible? Answers: It depends on the plan, but some plans have the following cost-sharing elements that you should be aware of. Co-Payments: Some plans include a co-payment, which is typically a specific flat fee you pay for each medical service, such as $30 for an office visit. After the co-payment is made, the insurance company typically pays the remainder of the covered medical charges. Deductibles: Some plans include a deductible, which typically refers to the amount of money you must pay each year before your health insurance plan starts to pay for covered medical expenses. Coinsurance: Some plans include coinsurance. Coinsurance is a cost sharing requirement that makes you responsible for paying a certain percentage of any costs. The insurance company pays the remaining percentage of the covered medical expenses after your insurance deductible is met. Out-of-pocket limit: Some plans include an out-of-pocket limit. Typically, the out-of-pocket limit is the maximum amount you will pay out of your own pocket for covered medical expenses in a given year. The out-of-pocket limit typically includes deductibles and coinsurance. But, out-of-pocket limits don't typically apply to co-payments. Lifetime maximum: Most plans include a lifetime maximum. Typically the lifetime maximum is the amount your insurance plan will pay for covered medical expenses in the course of your lifetime. Exclusions & Limitations: Most health insurance carriers disclose exclusions & limitations of their plans. It is always a good idea to know what benefits are limited and which services are excluded on your plan. You will be obligated to pay for 100% of services that are excluded on your policy. Beginning September 23, 2010, the Patient Protection and Affordable Care Act (health care reform) begins to phase out annual dollar limits. Starting on September 23, 2012, annual limits on health insurance plans must be at least $2 million. By 2014 no new health insurance plan will be permitted to have an annual dollar limit on most covered benefits. Some health insurance plans purchased before March 23, 2010 have what is called "grandfathered status." Health Insurance Plans with Grandfathered status are exempt from several changes required by health care reform including this phase out of annual limits on health coverage. If you purchased your health insurance policy after March 23, 2010 and you're due for a routine preventive care screening like a mammogram or colonoscopy, you may be able to receive that preventive care screening without making a co-payment. You can talk to your insurer or your licensed eHealthInsurance agent if you need help determining whether or not you qualify for a screening without a co-payment. There are five important changes that occurred with individual and family health insurance policies on September 23, 2010. Those changes are: 1. Added protection from rate increases: Insurance companies will need to publically disclose any rate increases and provide justification before raising your monthly premiums. 2. Added protection from having insurance canceled: An insurance company cannot cancel your policy except in cases of intentional misrepresentations or fraud. 3. Coverage for preventive care: Certain recommended preventive services, immunizations, and screenings will be covered with no cost sharing requirement. 4. No lifetime maximums on health coverage: No lifetime limits on the dollar value of those health benefits deemed to be essential by the Department of Health and Human Services. 5. No pre-existing condition exclusions for children: If you have children under the age of 19 with pre-existing medical conditions, their application for health insurance cannot be declined due to a pre-existing medical condition. In some states a child may need to wait for the state's open-enrollment period before their application will be approved.

In my Real Doctor vs. TV Doctor medical drama review video, I promised that if you got that video to 10,000 likes I would record myself reacting to a full grey's anatomy episode. You got it to 20,000 likes! You have spoken and I have listened. You should know that I do get the question a lot from patients as well. Is grey's anatomy accurate? So I finally decided to take a dive and find out! In this video, I sit down and watch the first ever Grey's Anatomy episode. Basically, a real doctor reacts to greys anatomy! I grew up watching ER and other TV medical dramas so I had a suspicion I would enjoy the show. I have to say this, greys anatomy is not medically accurate but the way it describes the characters as residents is very accurate. The hospital drama is real and I feel the show did a good job highlighting the difficulties of being a resident, inside the hospital and out. The one medical scene which totally threw me is when the surgeons automatically assumed that the seizure patient will need surgery before even coming to a diagnosis. That is not real life medicine. It surprised me that the show is actually very entertaining and I actually enjoy the drama between McDreamy / Derek Shepherd and Meredith Grey. Apparently, TV doctors do have souls haha. Whichever the case may be, I'll recommend you sit tight as I walk you through just about everything you need to know. First off, the one-meal-a-day diet is exactly what it sounds like — it's a plan that restricts you to just one meal a day. In essence, you'll get to eat whatever you want but of course, you can't go beyond a single meal. For the most part, the diet plan is created to challenge your traditional views on food intake and nutrition. And according to a few doctors, it could be the key to sustainable weight loss. Well, guess what? I decided to try the diet plan for 30 days and figured I should make this video to let you in on the pros and cons. Of course, at the end of the day, you'll decide if the plan is for you or not. Let's jump right in! Pros • It's simple and straightforward As mentioned earlier, the one-meal-a-day diet comes with one simple rule — just eat one meal a day and you're good! Essentially, there's no need for calorie counting, complex meal planning or special ingredients, weighing and the likes. You just have to stick to the simple rule! • You'll get to eat all your favorite food This is perhaps the biggest highlight of the one-meal-a-day diet — it gives you the freedom to eat whatever you want, say sweet potato chips, ice-cream, cheeseburgers and more. Well, I wouldn't recommend you eat everything at once though! • Reduced appetite Well, I don't know if you're going to experience this but I discovered that after fasting for most of the day, I didn't have the urge to eat too much afterward. Well, at this point, I'll just stick to the claims that fasting shrinks your stomach and make you want to eat less. You can give the plan a shot and of course, see if you feel the same way. • Sustainable weight loss Finally, the whole idea of eating one meal a day can be incredibly effective for weight loss. But of course there's a pretty reasonable catch — you have to try not to overeat at your one meal. Remember, weight loss is the plan, so do your best to stick to it! Cons • Hunger Pains Yes, this is clearly the biggest drawback of the one-meal-a-day diet. To be honest, I felt the hunger pains at first but of course, it got better as I grew more used to the routine. So what's the point? Well, the diet plan requires you to tolerate a certain level of hunger right off the bat; this is the only way to start the journey and stick with it! Note: I wouldn't recommend you try the one-meal-a-day diet if you're prone to binge eating — you may actually end up seeing little to no result. • Low energy levels As most of us know, any diet that restricts your calories can make you feel lethargic. When you start the one-meal-a-day plan, there's a good chance of feeling more tired than usual — I also experienced it. What's more, people who rely on sugary snacks to get an energy boost may find it hard to stick with the plan. Parting Words The one-meal-a-day diet plan is clearly not for everyone but, of course, you won't be wrong to try it if you can keep up with the rule. After 30 days on the one-meal-a-day diet plan, I actually lost a couple of pounds and I felt really good about it! Who wouldn't? Doctors at St Mary's Hospital in London are pioneering the use of AR to improve skin graft surgery. It's hoped this technology will make surgery faster and safer for patients. Multiple images from CT scans are combined to make a 3D hologram which is superimposed onto the surgeons real world view. The AR headsets were developed from the technology used in a Microsoft games console but they now have life-saving applications. 02:30 - 4 Japan tackles tourism In September sports fans will turn their eyes to Japan as it hosts the Rugby World Cup. The tournament will be more than just a warm-up for the 2020 Tokyo Olympics it's central to a plan to transform the country's economy. The government hopes to boost the number of tourists to 40 million a year by 2020 - 25 percent more than in 2017. Currently around 85 percent of visitors to Japan are from Asia but it is now attracting more Western visitors with far more spending power. In preparation for this boom a taxi company in Tokyo has organized a competition to test drivers English. The winner early started learning English four years ago and he's seen a steady rise in foreign customers. The Japanese government also wants more foreigners to stay in the country. With famously low immigration rates and almost a third of citizens aged over 65, Japan is suffering from a dire labor shortage but legislation has been passed that will allow three hundred and forty-five thousand foreign workers in over the next seven years. But with the Japanese population shrinking by nearly four hundred thousand a year the country may need to welcome even more people from the rest of the world. 05:06 Testing Trump 2019 is set to be US president Donald Trump's most testing year yet when Democrats assume control of the House of Representatives and he comes under more scrutiny. Mr. Trump came to power promising to rid Washington DC of corruption and vested interests. But this could be the year the swamp really makes the president sweat. Top of the list for some Democrats is publishing the president's tax returns. But it won't just be his financial affairs under the spotlight - the real pressure on Mr. Trump in 2019 could come from the Muller investigation into Russian interference in the 2016 election. If the investigation supports serious allegations against the president then 2019 could be the year that the Democrats begin impeachment proceedings. But don't expect the Republican majority in the Senate to convict him or any charges to even make a dent in Mr. Trump's core support across the country So for Mr. Trump's political opponents the real focus in 2019 is likely to be on who they choose to run against him at the next presidential election. 07:46 - 2 Moon rush On the 20th of July the world will remember the moment 50 years ago when Neil Armstrong first set foot on the moon. In 2019 there will be new entrants in the race to return to the moon. Not to be outdone China's Lunar Lander should also touchdown in 2019, making it the first craft to land on the far side of the Moon - But there is more than just prestige at stake and it will no longer just be men taking giant leaps. 10:05 - 1 Battle for Europe The biggest story of 2019 won't be, as some might think, the UK's protracted divorce from the European Union. In 2019 a bigger broader upheaval is set to unfold across the continent ahead of the European parliamentary elections. Populist parties have shaken the traditional political order across the continent and are hoping to win more power than ever in the 2019 vote. It raises the multi-billion euro question what will all this mean for the EU's future. Some of its supporters hope that populist forces could ultimately push the EU into reforms which will save it and the continents prosperity. By the end of this video Medicare will be made clear for you. You will understand how Medicare works. You will understand Medicare Part A & Medicare Part B and the choices you have to make once you sign up for Medicare. Once you sign up for Medicare you will have to choose from one of three options. We will go over each of those three options and pros and cons you should consider before you make a decision. Once you understand the Medicare basics of Part A and Medicare Part B, you will be faced with choosing one of three paths to take with your Medicare. This will be the most important Medicare decision you have to make because it could set you on a course that will impact the quality of your medical care and can potentially be irreversible. To keep this video brief, I have a separate video that covers how, when and where to sign up for Medicare Parts A & Medicare Part B. I have it linked in the upper right of this video, you’ll see a white circle with a letter “i”. You can click on that at any time to go to the video on how to sign up for Medicare. With terminology out of the way, next in the Medicare Explained video is Medicare Part A & Medicare Part B. Medicare Part A is that portion of your health insurance that pays for your healthcare whenever you are an inpatient in a medical facility. As soon as you become an inpatient in a hospital, Medicare Part A takes over. The same when you become an inpatient in a skilled nursing facility or hospice. The one exception is that if you are not able to physically get to a medical facility for healthcare, Medicare Part A also pays for home healthcare. Think of Medicare Part A is inpatient insurance. Medicare Part B is the part of Medicare that pays for outpatient and physician services. This is the portion Medicare insurance you use when you see a doctor or have lab tests or physical therapy and so on. Medicare Part B also includes some wellness coverage. Medicare Part B is the part of Medicare people use most often because most of our medical care is via outpatient services. Even many surgeries and hospital services are now done as an outpatient and are thus covered under Medicare Part B. Your Original Medicare (Part A & B) is good anywhere in the country. You can see any doctor, go to any medical facility in any state or territory, as long as they accept Medicare. You do not need to get permission from a primary care physician. You have both freedom to go where you need and control to see who you choose. Also it is important to understand that Medicare Advantage Plans are local coverage. If you are a snowbird, and RVer or just travel the country that should rule MAPD as an option. Next in our Medicare Explained video are your three choices. You can do one of three things: # 1 Do nothing. Just keep Original Medicare (Your Part A & B). This is the only choice that makes understanding those co-pays and deductibles very important. This is not an advisable choice because Original Medicare was never intended to be stand-alone health insurance. You will find when going over the details of the copays and the limits of the coverage that there is no maximum out-of-pocket limit on your potential financial obligation. You are at substantial financial risk. # 2 You can trade in your Medicare Part A and Medicare Part B for a Medicare Advantage Plan. Medicare Advantage plans are also called Medicare Part C. #3 Your third choice is to keep you Original Medicare Parts A & B, but add a Medicare supplement plan that pays the co-pays and deductibles that come with just Original Medicare, so you don’t have to. UPDATE: Man K!lled At Wedding Reception Was My Best Friend/JBN Their wedding took years to plan and drained much of their savings, but what they expected to be one of the happiest days of their lives ended abruptly with a hail of bullets, even before the reception started. Damian Davis and his wife, Asheika, had just exchanged vows and were taking pictures on a nearby lawn at the Glistening Waters Restaurant and Marina in Falmouth, Trelawny, when explosions echoed in the area designated for their reception, sending frantic guests scampering for cover. When the explosions ended, Davis' best friend, 45-year-old Paul Campbell, lay dead, and a son of the operator of the restaurant, 38-year-old Earl Bernard of a Montego Bay, St James, address, was seen clutching a firearm, with blood running from a wound in the upper body. The incident occurred on December 28, 2018, a date the couple will never forget. Official police reports are that Campbell, who was playing music for the reception, was involved in an argument with the owner of the premises, who repeatedly asked him to turn down the volume. During the argument Bernard, the woman's son, intervened and reportedly pushed Campbell over a flower pot. Campbell then pulled his licensed firearm and shot Bernard in the neck. The injured man, who was also a licensed firearm holder, then pulled his weapon and shot Campbell repeatedly, killing him on the spot. Bernard was taken to hospital and treated. He was reportedly released from hospital last Wednesday. "It is really bad, and when I say really bad, I mean really bad. For me, I have to remain strong because my daughter and my wife, they are very distraught," Davis said last Friday as he recounted Campbell's close relationship with his family. "My wife is supposed to be at work today and she can't even manage to go. For myself, I have to be at work every day to sort of keep my mind preoccupied. But once I leave work, everything just comes back," added Davis. He described Campbell as his best friend and said that Campbell was a kind, loving father of five and business owner who not even close friends knew he had a licensed firearm over the past two years. "He has never left my side. When I wrote the wedding vows, I gave it to him, and he is the person who went through it and edited it. For us, every year December 28 is going to be more of a remembrance, a day of pain every year going forward," said Davis. "Whenever I think of anniversary, I'm going to be thinking about where we got married, what happened on the day, and that I lost my best friend on the day. He was not only a friend to me but also a friend to my family," added Davis, who said that his family spent Campbell's birthday, December 25, at his house. According to Davis, his wife is in dire need of counseling as everything reminds her of her shattered wedding day. Explosions on New Year's Eve, a usually romantic time for the couple, jolted her back to the gunshots that rang out days earlier. "It is definitely going to scar us for life," lamented Davis one week after the deadly clash. Campbell fired the first shot, but Davis said his friend, who craftily found out his wife's ring size, helped to plan the wedding and was scheduled to give a toast at the reception and would not have pulled his gun without his life being directly threatened. "I wasn't there when the incident happened, but me knowing Paul, I don't see Paul as that person who would get up and pop out a gun and shoot persons. "Something happened that drove him to that point. That is what I believe," said Davis, as he argued that Campbell would be quicker to remove himself from confrontation. "I was the one who took Paul there to play music, and I would be the person who rented the place ... if there was a problem, I would think you (the owner) would come to me," Davis added, as he struggled to make sense of the reported confrontation which led to his best friend being dead and another person nursing a gunshot wound. Last week, a senior police source in Trelawny said that the investigation into the incident is ongoing. According to the source, once the investigation is done, a file will be sent to the Police High Command or the director of public prosecutions for a ruling. My name is Emma, and in today's video I am going to teach you a very, very important thing for customer service. I used to work in customer service, and this is actually one of the most important things I learned, and this is called the L-A-S-T or LAST approach. So, to get started, let's talk a little bit about: What is customer service? So, customer service is when you have customers, of course, and you're trying to make your customers as comfortable and happy as possible. You're also trying to meet their needs and expectations, and solve any problems or situations that they might have. So, customer service is a huge category. There's many, many different jobs where you use customer service. If you work in a hotel, for example, as a clerk, you know, in the lobby, as a bellhop, you will be using customer service. If you work at a restaurant as a server, you'll be using customer service, or as a hostess. If you're the manager of a store, you'll be using customer service. If you work in a business or even in a hospital, you'll be using customer service. So, pretty much any time you're dealing with people from the public and they're customers and you're trying to help them, you're doing customer service. So, there are many different problems that a customer might have. What are some examples of some problems? Can you think of anything, a problem a customer might have? Maybe somebody charged them too much for something, maybe they're in a store and the lineups are too long, maybe a customer is at a hotel and they're very unhappy because the Wi-Fi isn't working or their bed's uncomfortable. So, there's so many different problems customers might have at different types of businesses. In this video what I'm going to teach you is: What do you do when a customer has a problem? Okay? So, a very easy thing to do when a customer has a problem is called the LAST approach. "LAST", what does it stand for? Well, if a customer has a problem, the first thing you should do is listen to their problem, the next thing you should do is apologize, solve their problem, and thank them. So: Listen, Apologize, Solve, and Thank. We're going to look at expressions we use to show we're listening, expressions to apologize, expressions that can help us solve problems, and expressions to thank customers. Okay, so the first step when a customer has a problem is to listen. So, the first thing you should do is find out what the problem is. You can ask them: "What seems to be the problem?" or "How can I help you?" Okay? Once they start explaining what the problem is, very important that you look like you're actually listening and that you do listen. Okay? So, you shouldn't look at your watch: "Mm-hmm. Mm-hmm." Okay? That's a bad idea. You shouldn't look at the sky: "Uh, when is this person going to stop talking?" Okay? You shouldn't roll your eyes. Okay? No, no, no. You need to show that you actually care about what the customer is saying. So, showing you're listening is very important. You can repeat back to the customer what they're saying to show that you understand and to make sure that you did understand. So: "So what you're saying is, you know, there's no hot water in this hotel.", "So what you're saying is the Wi-Fi's not working and you're not happy with that." Okay? These are some examples. "So what you're saying is _________." You can also say: "Let me get this right..." "Let me get this right, what you're saying is that, you know, there's a problem at your table.", "What you're saying is that you've been waiting for your food for a really long time." So it's important to show that you are listening and you acknowledge what they have said. Okay. Now, sometimes with customer service you get a customer who's very angry, and maybe they start swearing, they start using very bad language. Okay? So if this happens, very important that you don't get upset. Okay? When this happened to me in the past, I would actually pretend to be a computer. I would not take anything personally. I would just smile and pretend to be a computer, and that's how I got through angry customers. So, if the person is rude... You know, it's not right if somebody is saying something rude to you, if they're swearing at you, or they're making you feel uncomfortable. So, be polite. Okay?

They may be locally medical facilities, hospital and clinic which are very capable of dealing with cut or cold or the flue but in the event of a serious medical issue then most people will want to be evacuated perhaps south to South Africa or North to Dubai or Europe. In order to do this, it is important to have a good medical insurance plan and today there is a lot of international medical companies that will allow you to be evacuated in the event of a serious illness, but it is important to know that this cover does not always come with standards, so when you are buying the plan, it is important to select this module. Most health insurance today build their health insurance plan in a modular form. What this mean is that you can select the part of the plan that is right for you and your family. The core of any plan is typically the impatient part, and that would cover you if you go into hospital, it will cover the overnight stay, the surgery the anesthetist etc... Beyond this you can buy an outpatient module, that would obviously cover things like trips to the doctor GP, specialist, medication, diagnostic testing, further more beyond that you ca n buy modules related to maternity optical dental. Well being and finally and very importantly the module that you should definitively add-on if you are living in the Congo would be the evacuation option. They are many leading international insurance today who will offer this type of cover. Many of the brands you may be familiar with companies like Bupa, Cigna, Aetna, DKV, Allianz, Axa, but there is also a number of companies that you may not have heard of William Russell, InterGlobal , these companies are specialists in medical insurance providers. Next Step: http://hubs.ly/H01ndv70 So you've been researching health insurance plans in Ohio for what feels like three and half life times, even though it’s probably only been a couple weeks. Your search has led you to this plan from Anthem Blue Cross Blue Shield's Pathway X PPO 1250/10 plan and you want to know as much as you can about it before you put it in charge of covering you and/or your family. Network Name Anthem has taken one of the more daunting aspects of health insurance and made it worse. They made some of the craziest, possibly questionable, changes to their networks of any insurance company out there. The Pathway X portion of the plan name indicates the name of the network you are getting with the plan, regardless if you know that or not. Once you have the cipher to crack the code for Anthem’s naming structure, you can actually glean a lot of information just from the name. The X in Pathway X is short for Exchange or Health Insurance Exchange. That means this plan is I promise I’ll stop beating the dead horse that is name of this plan soon, but exchange is not even what they are called anymore as it has been renamed the Health Insurance Marketplace and even more confusing you have to go to healthcare.gov to buy from the Health Insurance Marketplace. That last part isn’t Anthem’s fault, that’s the federal government's misstep, but it felt appropriate to keep terrible name train going and mention that here. The sad part is everything I’ve just said isn’t how Anthem made the network experience even more daunting. It’s the two words tired hospital after Pathway X that will make your head want to explode. Anthem has included this Just know that you will want to do everything you can to make sure you are going to a tier 1 hospital. Plan Includes Non-Network Coverage? Yes If you happen to find yourself at a hospital that isn’t in the tier 1 or 2 portions of Anthem’s network you will still be covered, you just will be required to a pay a lot more for that treatment. Deductible (Network/Non-Network) For a single person you will be on the hook for $1,250 in-network and $5,000 out-of-network. For a family (two or more people) you will pay $2,500 or $10,000. These numbers are simply the dollar amount you have to cover first before the co-insurance kicks in. We’ll talk about what co-insurance is in a little bit. Out-Of-Pocket Limit (Network/Non-Network) This is the most important number you want to pay attention to on any policy you look at. The most you will pay of any covered in-network service during the calendar year is $3,100 for a single person. For a family it’s $6,200. If you are out-of-network it will go all the way up to $10,000 and $20,000. Co-Insurance (Network/Non-Network) Once you’ve paid your deductible, Anthem is going to split the bills with you 90/10. That means they will 90 percent of the bill and you will be left with only 10 percent in-network. Non-network treatment will be a 60/40 split. For example, if you have a $3,000 CT scan you will pay the first $1,250 which takes care of your deductible and then you will split the remaining $1,750 with Anthem at 90/10. In this case you would pay $175 and Anthem would pay $1,575. Not a bad deal.
