WHAT IS WARFARIN: Warfarin is the main oral anticoagulant used in the UK. An anticoagulant is a medicine that prevents blood clotting. Clotting (thickening) is a complex process involving a number of substances called clotting factors. Clotting factors are produced by the liver and help control bleeding. They work with cells that trigger the clotting process (platelets) to ensure blood clots effectively. Warfarin blocks one of the enzymes (proteins) that uses vitamin K to produce clotting factors. This disrupts the clotting process, making it take longer for the blood to clot. WHEN IS WARFARIN PRESCRIBED: Anticoagulant medicines, such as warfarin, are often prescribed for people who've had a condition caused by a blood clot or have an increased risk of developing harmful blood clots. TAKING WARFARIN: It's very important that you take warfarin exactly as directed. Don't increase your prescribed dose unless the doctor in charge of your care advises you to. Warfarin is taken once a day, usually in the evening. It's important to take your dose at the same time each day, before, during or after a meal. The aim of warfarin therapy is to decrease the blood's tendency to clot, but not stop it clotting completely. INR -To check how long it takes for your blood to clot. The higher your INR is, the longer it takes for your blood to clot. Avoid adding table salt to your food, salting meat when you cook it, or salting rice and pasta water. 2 Lower your heart attack risk with medications. Which medications your doctor recommends will depend on your particular situation and medical history. If you are at a high risk of having blood clots or high blood pressure, your doctor will probably prescribe medications for those conditions. Medications will not repair a leaky valve, but they can improve conditions that make the leakage worse, like high blood pressure. Possible medications include: Angiotensin-converting enzyme (ACE) inhibitors. These are common blood pressure medications for mild mitral regurgitation. Anticoagulants such as aspirin, warfarin (Coumadin, Jantoven) and clopidogrel (Plavix). Blood clots can cause strokes and heart attacks. These medications reduce the probability that you will develop a blood clot. Diuretics. These medications prevent you from retaining too much water. If poor circulation is causing your legs, ankles and feet to swell, you may be prescribed diuretics. They will also reduce your blood pressure. Diuretics can be used to relieve swelling caused by a tricuspid regurgitation. Statins. These medications lower cholesterol. High cholesterol is often associated with high blood pressure and it may exacerbate the leakage. Beta blockers. Beta blockers reduce the rate and force at which your heart beats. This reduces your blood pressure and can reduce the strain on your heart. 3 Repair a leaking valve. The standard way to repair a faulty valve is through surgery. If you have a valve repaired, be sure to go to a cardiac surgeon who specializes in valve repair. This will give you the best chances for a successful surgery. Valves can be repaired through: Annuloplasty. If you have structural problems with the tissue around the valve, it can be reinforced by implanting a ring around the valve. Surgery on the valve itself or the supporting tissues. If the valve itself has been damaged through infection or injury, it may be necessary to repair the valve itself to stop the leak. Transcatheter Aortic Valve Replacement (TAVR). This is a newer, minimally invasive option for people who are unable to undergo open chest surgery. Therefore, deficiencies affect vision and cause Night Blindness. Vitamin A also acts as a growth factor that causes the differentiation of cells. Therefore, vitamin A can be used to treat certain types of AML as it causes differentiation of immature leukemic cells. This effect of differentiation of Vitamin A also means deficiencies can affect the immune response. Vitamin A supplements for deficient patients can be an effective treatment for disease like Measles. Deficiencies can result from fat malabsorption or inadequate intake of foods high in vitamin A (Liver, fruits, & vegetables). Vitamin A is also a teratogen that can cause numerous problems for an unborn fetus. This makes sense as prenatal development is a period where cell differentiate needs to be highly orchestrated. Too much vitamin A could interrupt this delicate process due to its effects on cell differentiation. Vitamin A should not be used as a treatment for women who may be pregnant. Vitamin A toxicity can result in a wide array of non-specific symptoms like nausea, irritability, headaches, visual problems, and changes to skin or hair. Most other vitamin deficiencies or toxicities have a more specific classic presentation. Therefore, I usually just rule out the other answer choices and select Vitamin A toxicity if nothing else fits. Vitamin D [1,25-(OH)2D3] is synthesized in the body as a result of sun exposure or ingested as part of cow's milk (breast milk is actually low in vitamin D). The inactive precursor to Vitamin D undergoes enzymatic processing in the liver and the kidney to become 1,25-(OH)2D3. This active form of Vitamin D primarily increases the absorption of calcium and phosphate from the diet. Therefore, deficiencies of Vitamin D lead to decreased bone mineralization. Vitamin D deficiency in children leads to Rickets and in adults it leads to Osteomalacia. Vitamin K is responsible for the modification of numerous proteins required for coagulation. Therefore, deficiencies of Vitamin K lead to deficiencies of Coagulation Factors II, VII, IX & X as well as deficiencies in Protein C & S. This can lead to an increased PT, increased PTT, bruising (purpura, ecchymosis, perifollicular hemorrhages) and other problems related to clotting. Vitamin K is taken in through the diet & synthesized by normal gut flora. Both of these routes lead to an inactive form of Vitamin K that must be activated by liver Epoxide Reductase. Coumadin (AKA Warfarin) inhibits Epoxide Reductase to intentionally create a Vitamin K deficiency and inhibit clotting. Patients with liver failure may have difficulty activating their vitamin K also. Patients who are on wide spectrum antibiotics may have a deficiency if Vitamin K because antibiotics can kill off the normal gut flora which produce Vitamin K. Newborns also often have Vitamin K deficiency because they have not yet been colonized with the normal flora that create Vitamin K and breast milk is low in Vitamin K. This is referred to as Hemorrhagic disease of the Newborn and is why newborns are usually given a shot of Vitamin K soon after birth. And that’s what I’m going to quickly discuss next… All Natural Solutions So there are lots of natural ways you can improve poor blood flow and circulation. You don’t have to take harmful drugs like Plavix or Coumadin or even Aspirin. Stop Smoking I think by now everyone knows how bad smoking is because it increases cholesterol, hardens the arteries and causes blood vessels to constrict. So if you smoke, make a decision and commit to STOPPING once and for all! Exercise Exercising daily is vital. Cardiovascular “aerobic” exercise is very important – jogging, running, swimming, biking are all great. Of course, daily brisk walking can be done by anyone, so you have no excuse not to get outside and take a 20-30 min walk daily or at least 3x weekly. But most people don’t know or realize that “anaerobic” muscle-building exercise, is just as important and maybe even MORE important than anaerobic. The more muscle you have, the better your blood flow, the bigger your arteries, the more flexible your blood vessels… Which all translates into greatly improve circulation from head to toe. Additionally, “resistance exercise” improves your hormones. And you don’t need much. Just 20-30 minutes, 2-3x weekly using cables or dumbbells at home is great. Even your own body weight is a fantastic start. Diet So the first thing to do is reduce or eliminate animal protein and animal fat. You should also reduce your sugar intake because high insulin levels corrodes your arteries and blood vessels. You should focus on eating more natural whole grains, sweet potatoes, fruits, vegetables, legumes, fish and nuts. And don’t forget the water… You should also eliminate all caffeine beverages because caffeine constricts your blood vessels and also increases the stress hormone, cortisol. Stress Management Speaking of the stress hormone, Cortisol … Even though we can’t stop or eliminate stress, you can certainly learn to manage how you REACT to stress, which is very important. This is because when you’re under heavy emotional stress, your adrenal glands secret lots of different hormones, which end up causing constriction of the blood vessels, arteries and so forth … causing anxiety and panic attacks. Good sources include: apples, asparagus, berries, broccoli, cabbage, melon (cantaloupe, honeydew, watermelon), cauliflower, citrus fruits (lemons, limes, oranges), kiwi, fortified foods (breads, grains, cereal), dark leafy greens (kale, spinach), peppers (especially red bell peppers, which have among the highest per-serving vitamin C content), potatoes, and tomatoes. Are there any risks associated with too much vitamin C? When obtained from food sources and supplements in the recommended dosages, vitamin C is generally regarded as safe. Side effects are rarely reported, but include diarrhea, nausea, abdominal cramps, and other gastrointestinal symptoms. For most healthy individuals, the body can only hold and use about 200-250 mg of vitamin C a day, and any excess is lost though urine. At times of illness, during recovery from injury, or under conditions of increased oxidative stress (including smoking), the body can use greater amounts. High doses of vitamin C (greater than 2,000 mg/day) may contribute to the formation of kidney stones, as well as cause severe diarrhea, nausea, and gastritis. Are there any other special considerations? Adverse affects may occur between vitamin C and anticoagulant drugs such as warfarin (Coumadin), decreasing their action. Nicotine products, oral contraceptives/estrogens, tetracyclines, barbiturates, and aspirin may decrease levels of vitamin C. Vitamin C may increase absorption of iron and lutein. Although some evidence suggests that large doses of supplemental vitamin C may interfere with the absorption and metabolism of vitamin B12 found in food, other studies have shown no such effects.
