Some people who have type 2 diabetes can achieve their target blood sugar levels with diet and exercise alone, but many also need diabetes medications or insulin therapy. The decision about which medications are best depends on many factors, including your blood sugar level and any other health problems you have. Your doctor might even combine drugs from different classes to help you control your blood sugar in several different ways. 1. Metformin Generally, metformin is the first medication prescribed for type 2 diabetes. It works by improving the sensitivity of your body tissues to insulin so that your body uses insulin more effectively. Metformin also lowers glucose production in the liver. Metformin may not lower blood sugar enough on its own. Your doctor will also recommend lifestyle changes, such as losing weight and becoming more active. 2. Meglitinides These medications work like sulfonylureas by stimulating the pancreas to secrete more insulin, but they're faster acting, and the duration of their effect in the body is shorter. They also have a risk of causing low blood sugar, but this risk is lower than with sulfonylureas. Weight gain is a possibility with this class of medications as well. Examples include repaglinide (Prandin) and nateglinide (Starlix). 3. Thiazolidinediones Like metformin, these medications make the body's tissues more sensitive to insulin. This class of medications has been linked to weight gain and other more-serious side effects, such as an increased risk of heart failure and fractures. Because of these risks, these medications generally aren't a first-choice treatment. Rosiglitazone (Avandia) and pioglitazone (Actos) are examples of thiazolidinediones. 4. Sulfonylureas These medications help your body secrete more insulin. Examples of medications in this class include glyburide (DiaBeta, Glynase), glipizide (Glucotrol) and glimepiride (Amaryl). Possible side effects include low blood sugar and weight gain. 5. GLP-1 receptor agonists These medications slow digestion and help lower blood sugar levels, though not as much as sulfonylureas. Their use is often associated with some weight loss. This class of medications isn't recommended for use by itself. Exenatide (Byetta) and liraglutide (Victoza) are examples of GLP-1 receptor agonists. Possible side effects include nausea and an increased risk of pancreatitis. 6. SGLT2 inhibitors These are the newest diabetes drugs on the market. They work by preventing the kidneys from reabsorbing sugar into the blood. Instead, the sugar is excreted in the urine. Examples include canagliflozin (Invokana) and dapagliflozin (Farxiga). Side effects may include yeast infections and urinary tract infections, increased urination and hypotension. 7. DPP-4 inhibitors These medications help reduce blood sugar levels, but tend to have a modest effect. They don't cause weight gain. Examples of these medications are sitagliptin (Januvia), saxagliptin (Onglyza) and linagliptin (Tradjenta).

There are now 2 oral drugs used to treat diabetes in a new class called SGLT 2 inhibitors. They are Invokana and Farxiga. These pills block an enzyme in the kidney and result in sugar being spilled into the urine. This can account for 150-300 calories lost in the urine per day. In addition to lowering blood sugar, theyhave a diuretic effect, and can lower blood pressure. There has been some concern that these drugs may be associated with bladder cancer. The problem is that it is well established that diabetics have a higher risk of MANY cancers including pancreas, liver, colon, kidney and bladder. Diabetics take diabetic drugs, and non-diabetics don't. A reasonable comparison would be to note that people who take cholesterol drugs have more heart attacks than those that do not. Obviously, young healthy people with normal cholesterol do not take these drugs, and high risk older people with high cholesterol do. You are selecting out a group of people that inherently are at higher risk for certain diseases. That said, I have now tried these medications in the office, and am seeing significant weight loss AND improvement in blood pressure. I have tried them on non-diabetics, and they spill similar amounts of sugar.Side effects can include yeast infections and UTIs because of the presence of sugar in the urine. Taking cranberry pills may help prevent this.

And even though most people have heard about insulin spikes not being good for weight loss the average person doesn't truly understand why. Well my hope is that today I can explain this in a way that anybody can understand. It all starts with you eating food. When you eat food the chewed up food particles get broken down further in your stomach into very small parts and then they pass through your intestines at which point your body absorbs and breaks down those nutrients and calories again and dumps them into your bloodstream. These broken down nutrients can't just jump into the cells that need them for their daily functions without some help. That's where insulin comes in. Insulin is a hormone secreted by your pancreas and it gets released into your bloodstream when your body senses that blood sugar has increased. Very high blood sugar levels are not good for you and your body knows this. So insulin acts as a bridge between your cells and the sugar found floating around in your blood. Without insulin your blood sugar would be very high because your body wouldn't be able to get that sugar into the cells. This is actually pretty much what type 2 diabetes is. When people are insulin resistant which means that insulin isn't doing its job they usually wind up being labeled as a type 2 diabetic which means that they need an external source of insulin for example either a shot or a pill to get their blood sugar down. So what does all of this have to do with fat loss and weight loss. Well whether you eat pasta brown rice or drink a can of soda all of it will eventually be broken down and it will be blood sugar floating around in your bloodstream. The only difference is that whole high-fiber carbs like brown rice will take a lot longer to digest which means that the sugar will be released into your bloodstream slowly while the can of soda will pretty much be released all at once. When it's released into your bloodstream slowly you give your body a chance to actually use it overtime so you won't require quite as much insulin which leads to less of it being stored as fat. Before insulin starts storing blood glucose into fat cells it will first store as much of it as possible into your liver for later use. The liver can only hold a limited amount of blood sugar and then the excess starts to get dumped off into your muscle cells. Now your muscle cells can hold more of this glucose then your liver can however it also has its limit. By building more muscle you're going to be able to increase this limit and use more of the blood sugar for your muscles rather than storing it as fat, however again even the most muscular person on the planet is going to have a limit of how much of this blood glucose can actually be used by the muscles. Once that limit is reached we hit a point called spillover where the sugar that came from mostly carbohydrates in your diet will begin to be stored in fat cells in order to lower the sugar found in your blood stream to normal levels. So insulin is an anabolic hormone that causes weight gain and growth. The opposite hormone to insulin is a catabolic hormone known as glucagon which is released when blood sugar drops too low. Glucagon just like insulin goes in and acts as a bridge between the stored energy in your cells and your bloodstream. Except it has the inverse effect. So it pulls energy from your cells and dumps it into the bloodstream. Before your body will start pulling fat from fat cells for energy it will first pull that stored glucose from your liver and your muscle cells. This is why so many low carb diets work. When you don't have that many carbs it's easy for your body to use up the carbs stored in your muscles and your liver and then proceed straight to burning fat for energy. This is the same reason why ketogenic diets work. Except an added benefit to ketogenic diets is that your body becomes more efficient at using fat for energy which means more fat will burnt.

Effect of Whey Protein on Insulin Whey is one of the two main proteins in milk - the other is casein. Whey, like all proteins, is made up of amino acids, the building blocks of protein. When we consume protein, we then break down these amino acids and use them for muscle growth and tissue repair. Whey contains all three BCAAs along with all of the essential amino acids. Whey is highly insulinogenic - consuming whey leads to a sharp increase in insulin levels. This response does not have to do with blood sugar levels - when a food has a high glycemic index, consuming it will cause a large increase in blood sugar levels. In response to this, the body produces insulin to combat the high blood sugar and allow glucose to enter our cells for energy. This is not what is happening when we consume whey - insulin release is responding to amino acids, not glucose levels. Insulin binds to cells to allow them to take up energy, which can be glucose or amino acids from proteins. The amino acids in whey cause an insulin release. Whey also leads to the release of GIP and GLP-1, two gastrointestinal hormones that also raise insulin levels. This increase in insulin is actually a good thing! Whey is a fast acting protein, and the spike in insulin allows amino acids into the muscle cells to encourage repair, helping us to gain muscle and heal quickly. Consuming whey protein has actually been shown to lower blood glucose levels when consumed during or directly before a meal. A 2005 study in the American Journal of Clinical Nutrition found that when whey was combined in a high-sugar meal, individuals with type-2 diabetes had a lower blood glucose level following the meal than if no whey was consumed. This is thought to be because the increased insulin aided the body in glucose digestion. Generally, the better the whey, the higher the insulin spike and thus the more effective at delivering building blocks into the cells for the purpose or repair and growth. Best to consume whey directly following exercise. It is quickly broken down and made available to muscle cells for repair post workout. Whey protein isolate contains 90% protein and thus has less lactose, fat and cholesterol than whey protein concentrate. To get the most out of your protein, go for the whey protein isolate. You will experience less bloating and get more of the benefits - lean, ripped muscles. Too much protein is dangerous, so be sure not to consume excess protein. As probably already know, CLA or conjugated linoleic acid is a pretty popular weight loss supplement. But how much of what you hear is fact, and how much of it is fiction, presented to convince you to make a purchase of some product or another? This video will sort all that out for you. Before I begin, one small thing... if you're new to my videos, please note that I include links to the scientific studies that support my arguments in the description under the video. Why do I do this? Good question. It's so YOU can check my work and ensure I'm not just making this stuff up. This is the Internet after all, where anyone can claim to be an expert on anything. Alright, with that said let's dig into 9 things you absolutely, positively must know about CLA weight loss pills before you spend a penny of your hard earned cash on them. # 1) Taking the ideal dose is critical to success. The ideal dose is appears to be 3.2 grams - or 3200 mg - of CLA per day. # 2) Increasing the dose of CLA does not appear to increase its effectiveness. So if you're thinking "hey, if 3.2 grams works well, maybe 6.4 grams will work twice as well. Nope. The studies show that a higher dose doesn't make it any more effective. All you'll be doing is wasting your money. # 3) Do some simple math to make sure you're getting an effective dose. Here's what I mean... suppose you're taking a product that supplies 1,000 mg of safflower oik standardized to 70% CLA per capsule. That means each capsule contains 700 mg of CLA, not 1,000. That means you'll need to take 5 capsules to get an effective daily dose... not 3 capsules as most labels indicate. # 4) many Products that contain CLA as part of a more extensive formula usually do not contain anywhere enough of the ingredient to be effective. Watch out for this. In fact, if your chosen product contains anything other than CLA and MAYBE a very small handful of other ingredients, the CLA is most likely under dosed. # 5) If youire hearing claims that CLA weight loss pills are amazing and burn fat like crazy, you need to fire up your bullshit detector. The positive studies show that on average, supplementing with CLA leads to an average weight loss of .09 kilograms per week. How much is that exactly? .09 kiligrams is 90 grams. There are 454 grams in a pound. That means you can expect to lose a pound of weight every 5 weeks with CLA, and only if you're not overconsuming calories. # 6) Higher doses of CLA may elevate testosterone levels slightly, and as a result, some retailers will to to pass CLA off as a test booster, or that it promotes lean muscle mass. Don't be fooled. While a small single study did show an elevation in test levels, these levels are still very much within the normal range, and there's no evidence to support the argument that you're not going to pack on slabs of muscle when taking the stuff. # 7) CLA is a pretty safe supplement, but there is some older studies suggest it can elevate inflammatory markers that are precursors to many diseases. # 8) If you're going to try CLA, don't overpay for it - it's actually a pretty affordable supplement. The best way to avoid overpaying is buy it from a reputable online or offline merchant. I was able to find a 45-day supply at the effective dose for $16 USD. I'll include a link to the product in the description below, and no... this is not an affiliate link, which means I do not receive any money for making this recommendation. # 9) Despite all the hype on the Internet about CLA's weight loss abilities, not all clinical studies showed a positive result. It is inexpensive, can be used with many other diabetic drugs in combination, it can cause weight loss and has been shown to decrease cardiovascular risk and even pancreatic cancer. It can be used safely in non-diabetics, and pre-diabetics. It can cause GI side effects, so when I start someone on it, I usually give them 1000mg pills, and the goal is to take 1000mg twice a day. Start with 1/2 pill once a day at dinner, in one week, Take 1/2 pill twice a day with food, in another week, take 1/2 pill in the morning and one at dinner, Finally advance to 1 pill twice a day. Some people cannot tolerate the full dose, and have to take somewhat less of a dose. Switching to the ER version can be helpful. If you eat lots of carbs, you will have more side effects. Limiting carbs, helps to tolerate it better AND can help you to lose more weight.

If you've ever been diagnosed with stomach cancer, or any cancer for that matter, and are experiencing and going through treatment for cancer, there could be a few things in your diet that would help you to manage these symptoms. First and foremost, watch your weight and make sure you're taking in enough calories. The more you can stay at an appropriate weight, the better chance your immune system will have to help you this disease. Looking at things like making shakes of higher, dense, good nutritional value are a great way to add a lot of calories in a small volume. Also, you may experience no appetite, or you're too tired and you just don't have the desire to eat at all. These shakes come in handy. That way, you're putting in a smaller volume of food with a great nutritional value. Looking at things to try to increase the flavor, because you may have some dry mouth or sore throat, this can also help if you just bite into a lemon or have a little bit of fluid before you take that food. Stimulating the taste buds help to make you taste the food the way it is. Look at nutritional supplements at some point, if you really can't maintain weight. These are a great source of combined, packed nutrition. They have the right amount of calories, the right amount of protein, carbohydrates and fats to, again, keep you healthy as possible. Now, remember your immune system is going to be down. So, you may have heard something about a neutropenic diet. This is a diet that has no raw foods. Sometimes, when we eat raw foods, they can have little add-ons, like micro-bacteria that may cause us to get sick. So, the neutropenic diet suggests to cook all foods before consuming. Again, look at an apple. A raw apple may have something on it that you can't see. However, eating it baked, or already processed in the form of applesauce, is going to reduce the amount or chances you have of containing and eating that bacteria. So, look for shortcuts to add good nutrients. Look, adding peanut butter into that milkshake or using that nutritional supplement to make your own milkshake. Add gravies to foods. Look at healthy ways to keep that nutrient content high and healthy for you. Again, be aware of raw foods, because sometimes your immune system may be down. And also, if you're having that desire to not eat, think of food as a medicine. You need to keep a weight and keep a good nutrient intake to again, help you fight that cancer. 2018 update: Invokana MAY be associated with an increased risk of amputations. Farxiga and Jardiance have not shown this possible side effect. Its not clear weather this is a statistical anomaly, if Invoka was used in sicker patients than the other studies, or it actually has this risk. At this time, I see no reason to use Invokana when the other 2 drugs have not had any concerns about this risk. A new class of diabetes drugs called the SGLT2 or Sodium GLucose coTransport 2 have been developed. The first to come to market is called Invokana. These drugs block the reabsorption of glucose in the kidneys, and cause glucose or sugar, to be spilled in the urine. This lowers blood glucose, as well as weight loss. The loss of 40-80 grams of sugar in the urine equates to 160-320 calories per day, or 15-30 minutes of exercise equivalent. The major side effect is vaginal yeast infections and urinary tract infections from the sugar feeding yeast and bacteria. They may also increase the risk od bladder cancer. In fact another drug in this family, Daptogloflozin, was blocked from being released to market until further cancer studies can be done
