Felt awful. Could hardly walk from muscle weakness. Terrible dreams. Tingling began in forehead. Once started walking wanted to lay down. Weight loss 150bl-145 by August 20th. Full body tingling by Thursday. Could not concentrate. OFF DRUG August 7th- full body tingling stoped. Felt better but slept constantly. Minor episodes occurred. August 8th-18th- felt pretty good through these weeks. Little fatigued but did not feel like I was on drugs. August 19th-20th- probably the worst I've felt Friday night started and then Saturday was awful. Had to stop walking felt like i was going to pass out absolutely awful. Felt OK by midnight but extremely out of it. August 21-26- pretty massive fatigue felt extremely out of it. Trouble sleeping. August 27-28- felt pretty shitty on and off. Had a lot of trouble sleeping. Very fatigued. Tingling, shortness of breath, felt like zombie. August 29-1- felt pretty good besides the anxiety. September 1- felt good not too many issues September 2nd-3rd- felt good until the night of the 2nd...shortness of breath. Very uneasy. Anxious. September 4th- probably one of my better days. Did a ton and had a lot of fun. September 5th-felt great until midday...then felt shitty needed to sit down. very anxious, woke up with both arms asleep. September 6-8th- very anxious. Not much fatigue. September 9th- felt ok on and off but extremely anxious. Slept alright. Tingling forehead and shoulder blade. September 10th- felt like shit on and off...tingling still in forehead and left shoulder blade. Exercised twice. Slept like shit had brain shocks. September 11th- felt decent throughout the day. Tingling continues. Anxiety is bad but I'm traveling. September 12th- one the worst days I have had extreme muscle weakness, felt sick, could not concentrate, terrible time sleeping. Tired. September 13th- Fatigue is gone. My tingling in forehead that gets very intense. September 14th- tingling is not as intense. Feel out of it today. Stiff neck and back muscles. Better than my first two days. September 15th- felt OK today. Tingling is still there but muscle stiffness is gone. September 16th- felt OK until the afternoon then started to feel like shit. Shortness of breath. Fatigue and tired. Felt good by the night. September 17th- felt OK but didn't do much. Had a little trouble at night and was out of it but most likely because of staying up til 3am the night before. September 18th-21st woke up with sore throat. Running nose. Turned to cough by the 20th. Officially got a cold. Fuck this. Felt better on the 21st. On going(after drug)- throat irritation causing dry cough, suicidal thoughts, stomach irritation, brain "zaps", weird dreams, chest pain, pins and needles in foot(normally right). Note-extremely good support system, more exercise, increased calorie intake by 1000(2000-3000), cut all caffeine out of diet, minimal alcohol, don't smoke, never been on prescription drugs in my life, better diet, work 45 hours a week, missed one day of work. Doctor Work- 2 ER visits. 15 blood tests all negative. EKG fine. 3 urine tests all fine. One heart monitor. Fine. MRI. Normal. Throat doctor. All good.

SKIP AHEAD: 2:20 – Mechanism of Antidepressants 3:16 – General Principles of Antidepressant Use: Suicide, Mania & Serotonin Syndrome 7:51 – Tricyclic Antidepressants 9:10 – TCA Side Effects 10:40 – SSRIs 11:47 – SSRI Side Effects 13:01 - SNRIs 13:33 – Atypicals: Bupropion, Mirtazapine & Trazadone Antidepressant mechanism - One hypothesis for the pathophysiology of depression is that it is due to low levels of monoamine neurotransmitters (mainly serotonin, norepinephrine and dopamine). That is why antidepressants aim to increase the levels of these neurotransmitters in the synaptic cleft. They do this by slowing the reuptake of the neurotransmitters so that they stay in the cleft longer and interact with post synaptic receptors more often. The first drugs in this group were non-specific and increased all of the monoamines, which lead to lots of side effects and safety issues related to toxicity. Newer antidepressants are more selective and mostly only effect 1 or 2 monoamines. General principles: Unfortunately, antidepressants take at least a month to start working. Good patient education about the delayed onset of effect and close monitoring of the patient during this initial period is extremely important. Patients can become hopeless if they expect the drug to start working right away. This may be one reason why antidepressants are associated with suicide, especially in patients 25 years old and younger. Another proposed mechanism is that a depressed person may have the energy to carry out their suicide once the medications start to work. There is now a black box warning for suicide on antidepressants. Some psychiatrists argue that they don’t actually see this association with suicide in clinical practice, and that the thing that really increases the risk for suicide is not treating a depressed person with the proper medications. However, it is still standard practice to have a close follow up with patients you are starting on antidepressants. Usually this will involve a follow up visit about 2 weeks after the medication is started. At this visit the drug will not have started working yet so you can’t evaluate efficacy, but you can monitor for side effects like suicidality. Another serious side effect you have to be on the lookout for soon after initiating treatment is mania. If a bipolar individual is incorrectly diagnosed as having depression, an antidepressant may induce a manic episode. Another very serious side effect that has to be considered for antidepressants is Serotonin Syndrome. This usually occurs when you combined multiple antidepressants at the same time or combine an antidepressant with another medication that increases serotonin such as dextromethorphan or an opioid. It presents with tremor, diaphoresis, tachycardia, flushing and hypertension. If not corrected it can progress to delirium, AMS and death. Treatment includes medication cessation and the use of Cyproheptadine (a serotonin antagonist). In order to prevent this from happening you should have about a month “Wash Out” period when you are switching between antidepressants. So you taper the 1st medication down and then stop it, give the patient at least a month with no antidepressant and then start adding the new medication slowly. Most side effects begin immediately after starting the medication, but diminish over the course of a month. This is another reason why patient compliance is poor with these meds. It makes them sick and the drug doesn’t work during the first few weeks. However, if they can stick with it the medications will likely start working and the side effects will diminish over time. A principle that applies to all of the antidepressants is “start low and go slow.” This means that you start with a lower dose and slowly increase it in order to decrease side effects and increase patient compliance. The dose you start the patient on may not even be at a therapeutic level, but every month or so you can increase the dose a bit. MY STORY: My name is Noah and on May 18 2011, I had a rare reaction to a vaccine called VIVITROL and consequently spiraled into a major, agitated, suicidal depression with depersonalization. I lost 25 lbs in 4 weeks and was in full panic or near panic for 8 weeks straight mixed with the darkest most painful depression I cold have ever imagined. I immediately could not work and had to move in with my parents who along with many siblings and friends had to watch me 24/7 as I was so suicidal. I was eventually hospitalized. Getting through each day seemed truly unbearable and I knew I would surely die. I have been put on many many different SSRI's SNRI's Tricyclics, Mood stabilizers, anti psychotics, holistic meds, acupuncture and even a form of shock therapy called RTMS. I barely saw any improvement in my condition for a full year. It was decided I had treatment resistant depression and I spent nearly every moment in tears. Weeks after starting my newest round of medications (Seroquel & Nortryptaline) as a last ditch effort, I had my blood drawn for possible hormone imbalances and my Testosterone levels came back 200 ng/Dl and 150 ng/Dl. The average 25 year old male has 750 ng/Dl. With this discovery I for the first time had any type of possible explanation as to why I was not getting better and why I might be so so sick. The symptoms of such Low T are very similar to those of major depression. I started Testosterone replacement therapy soon after and have been checking in with the world and documenting my experience with treatment. I am blessed to say that I have slowly over the last 8 months been improving and becoming more stable which I never thought to be possible. My low T manifested itself in the form of Major depression, anxiety, and depersonalization/ derealization for over a year. I am glad to report that I have experienced improvements in all areas and that is a miracle. I do not consider myself to be totally healed yet but I am closer now then ever before and aim to use what I have been through to help or at least offer support to others in need.

I am having to taper off of my daily 20 mg dose of Paxil that I have been on for over a year to help treat the symptoms of my bi-polar disorder. I have been doing a lot of research and found that Paxil is one of the *hardest* SSRI drugs to get off of. The newer generation antidepressants are called Selective Serotonin Reuptake Inhibitor or SSRIs. MY STORY My Story My name is Noah and on May 18 2011, I had a rare reaction to a vaccine called VIVITROL and consequently spiraled into a major, agitated, suicidal depression with depersonalization. I lost 25 lbs in 4 weeks and was in full panic or near panic for 8 weeks straight mixed with the darkest most painful depression I cold have ever imagined. I immediately could not work and had to move in with my parents who along with many siblings and friends had to watch me 24/7 as I was so suicidal. I was eventually hospitalized. Getting through each day seemed truly unbearable and I knew I would surely die. I have been put on many many different SSRI's SNRI's Tricyclics, Mood stabilizers, anti psychotics, holistic meds, acupuncture and even a form of shock therapy called RTMS. I barely saw any improvement in my condition for a full year. It was decided I had treatment resistant depression and I spent nearly every moment in tears. Weeks after starting my newest round of medications (Seroquel & Nortryptaline) as a last ditch effort, I had my blood drawn for possible hormone imbalances and my Testosterone levels came back 200 ng/Dl and 150 ng/Dl. The average 25 year old male has 750 ng/Dl. With this discovery I for the first time had any type of possible explanation as to why I was not getting better and why I might be so so sick. The symptoms of such Low T are very similar to those of major depression. I started Testosterone replacement therapy soon after and have been checking in with the world and documenting my experience with treatment as well as giving my insight and perspective on various topics of mental health. I am blessed to say that I have slowly, over the last year and 4 months, been improving and becoming more stable which I never thought to be possible. My low T manifested itself in the form of Major depression, anxiety, and depersonalization/ derealization for over a year. Gaining some mental stability back is nothing short of a miracle as I was near death for what felt like forever. I do not consider myself to be totally healed yet but I am closer now then ever before and aim to use what I have been through to help or at least offer support to others in need. I was able to successfully come off my Seroquel and Pamelor. I work out all the time as a part of my mental health recovery!!! Weight training and all kinds of cardio rule much of my free time!
