6 Side Effects of Too Much Vitamin D 1. Elevated Blood Levels Achieving adequate levels of vitamin D in your blood may help boost immunity and protect you from diseases like osteoporosis and cancer (5). However, there isn't universal agreement on the optimal range for these levels. Although a vitamin D level of 30 ng/ml (75 nmol/l) is typically considered adequate, the Vitamin D Council recommends maintaining levels of 40–80 ng/ml (100–200 nmol/l), and states that anything over 100 ng/ml (250 nmol/l) may be harmful (6, 7). Despite the fact that more people are now taking vitamin D supplements, it's rare to find someone with very high blood levels of this vitamin. One recent study looked at data from more than 20,000 people over a 10-year period. It found that only 37 people had levels above 100 ng/ml (250 nmol/l). Only one person had true toxicity, at 364 ng/ml (899 nmol/l) (8). 2. Elevated Blood Calcium Levels Vitamin D helps your body absorb calcium from the food you eat. In fact, this is one of its most important roles. However, if vitamin D intake is excessive, blood calcium may reach levels that cause symptoms that are not only unpleasant, but dangerous. Symptoms of hypercalcemia, or high blood calcium levels, include: Digestive distress, such as vomiting, nausea and stomach pain Fatigue, dizziness and confusion Excessive thirst Frequent urination The normal range of blood calcium is 8.5–10.2 mg/dl (2.1–2.5 mmol/l). In one case study, an older man with dementia who received 50,000 IU of vitamin D daily for six months was repeatedly hospitalized with symptoms related to high calcium levels (10). In another, two men took improperly labeled vitamin D supplements, leading to blood calcium levels of 13.2–15 mg/dl (3.3–3.7 mmol/l). What's more, it took a year for their levels to normalize after they stopped taking the supplements (11). 3. Nausea, Vomiting and Poor Appetite Many side effects of too much vitamin D are related to excessive calcium in the blood. These include nausea, vomiting and poor appetite. However, these symptoms don't occur in everyone with elevated calcium levels. One study followed 10 people who had developed excessive calcium levels after they had taken high-dose vitamin D to correct deficiency. Four experienced nausea and vomiting and three had a loss of appetite (12). 4. Stomach Pain, Constipation or Diarrhea Stomach pain, constipation and diarrhea are common digestive complaints that are often related to food intolerances or irritable bowel syndrome. However, they can also be a sign of elevated calcium levels caused by vitamin D intoxication (15). These symptoms may occur in those receiving high doses of vitamin D to correct deficiency. As with other symptoms, response appears to be individualized even when vitamin D blood levels are similarly elevated. In one case study, a boy developed stomach pain and constipation after taking improperly labeled vitamin D supplements, whereas his brother experienced elevated blood levels without any other symptoms (16). 5. Bone Loss Because vitamin D plays an important role in calcium absorption and bone metabolism, getting enough is crucial for maintaining strong bones. However, too much vitamin D can also be detrimental to bone health. Although many symptoms of excessive vitamin D are attributed to high blood calcium levels, some researchers suggest that megadoses may lead to low levels of vitamin K2 in the blood (18). One of vitamin K2's most important functions is to keep calcium in the bones and out of the blood. It's believed that very high vitamin D levels may reduce vitamin K2 activity (18, 19). 6. A recent study found that increasing levels of vitamin D in people who are deficient and suffering from vertigo, greatly reduced their episodes of vertigo. Everyone seems to be talking about vitamin D lately. But what's the big deal? Why should you care about vitamin D? Well, I'll tell you why, because there are many, many consequences of vitamin D deficiency. One consequence, having a poor immune system or being sickly. You can also have muscle weakness with vitamin D deficiency. And, of course, you can have weak bones if your vitamin D is deficient. There are some observational studies that show that vitamin D deficiency may be associated with fatigue, poor wound healing, hair loss. What is vitamin D? Vitamin D is a fat-soluble vitamin that is important in your body's absorption of calcium and phosphorus, which are both essential for bone health. But beyond being essential for bone health, vitamin D is also important for your immune system and for muscle strength. How do you get vitamin D in your system? Well, one way is through sun exposure. Yes, on a beautiful sunny day like today your skin can actually synthesize vitamin D from the sunlight. You can also get vitamin D through your diet, through foods like fatty fish, cod liver oil, cow's milk, and some vitamin D can be found in eggs, and then, of course, you can take vitamin D supplements. Who is at risk for vitamin D deficiency? People who don't have enough sun exposure are at risk. So if you are a person who wears a lot of sunscreens, or if you always carry an umbrella to prevent sun exposure, then you could be at risk for vitamin D deficiency, or people with darker skin with a lot of melanin in the skin are also at risk for vitamin D deficiency. Also, if you are a person who is homebound, or bed bound like a nursing home patient, you are at risk, and people who live in cold climates where they are essentially indoors during all the winter months, and not out getting sun exposure. These people are also at risk for vitamin D deficiency. You are also at risk if you have an inadequate dietary intake of vitamin D. What happens if you don't eat fatty fish, if you don't eat eggs, if you don't drink a cow's milk, and if you don't supplement your diet with vitamin D? Then certainly you can have vitamin D deficiency. You are also at risk for vitamin D deficiency if you are unable to absorb the vitamin D in your intestines properly. If you have Crohn's disease, then you are at risk, and if you have had a gastric bypass surgery, you can also be at risk for vitamin D deficiency. Patients with kidney or liver disease are at risk for vitamin D deficiency as well. Here's why. Both the liver and the kidney have enzymes which help to transform vitamin D into a biologically active form, so if you have kidney disease or liver disease, then make sure you check with your doctor for your vitamin D level, because you are definitely at risk for vitamin D deficiency. So how can you be diagnosed with vitamin D deficiency? Well, you simply go to your physician and ask for a blood test, your vitamin D level. A normal vitamin D level is at very least 30 nanograms per milliliter, but we actually prefer your vitamin D level to be greater than 50. You have vitamin D insufficiency if your vitamin D level is less than 30, and you have frank vitamin D deficiency if your vitamin D level is less than 20. In certain studies, Americans has been found to be the #1 population in vitamin D deficiency. Some of the main symptoms of vitamin D deficiency are a weakened immune system, candida or bacterial overgrowth, emotional ups and downs, hormone imbalance, trouble putting on muscle or losing weight, weakened bones or teeth, cancer, heart disease, or diabetes. In the body, most calcium is located in bones, only about 1% is in the blood and extracellular fluid. The amount of calcium in circulation is regulated by 2 hormones: parathyroid hormone, PTH, and calcitriol. When serum calcium level is low, PTH is UPregulated. PTH acts to PROMOTE calcium release from bones and REDUCE calcium loss from urine. At the same time, it stimulates production of calcitriol, which promotes absorption of calcium in the small intestine while also INcreases RE-absorption in the kidney. Together, they bring UP calcium levels back to normal. The REVERSE happens when calcium level is high. This feedback loop keeps serum calcium concentrations within the normal range. HYPOcalcemia refers to ABnormally LOW levels of calcium in the blood and is generally defined as serum calcium level LOWER than 2.1 mmol/L. Because the total serum calcium includes albumin-bound and free-ionized calcium, of which only the LATTER is physiologically active, calcium levels must be corrected to account for albumin changes. For example, DEcreased albumin levels, such as in liver diseases, nephrotic syndrome, or malnutrition, produce LOWER serum calcium values but the amount of FREE calcium may STILL be normal. On the other hand, in conditions with high blood pH, albumin binds MORE calcium; leaving LESS FREE-ionized calcium in the serum while the total calcium level may appear normal. The most common cause of hypocalcemia is PTH deficiency resulting from DEcreased function of the parathyroid glands, or HYPOparathyroidism. HYPOparathyroidism, in turn, may be caused by a variety of diseases and factors. These include: - accidental removal or damage of the parathyroid glands during a surgery - autoimmune disorders - congenital disorders: mutations that set the “normal calcium levels” to a lower value - other genetic disorders that produce underdeveloped or non-functional parathyroid glands - magnesium deficiency Other causes of hypocalcemia include low vitamin D intake/production, and excessive loss of calcium from the circulation such as in kidney diseases, tissue injuries or gastrointestinal diseases. While chronic moderate hypocalcemia may be Asymptomatic, ACUTE and severe hypocalcemia can be life-threatening. Most symptoms of acute hypocalcemia can be attributed to the effect it has on action potential generation in neurons. Because extracellular calcium INHIBITS sodium channels, and consequently DEpolarization, REDUCED calcium level makes it EASIER for depolarization to occur. Hypocalcemia therefore INCREASES neuronal excitability, causing neuromuscular irritability and muscle spasms. Early symptoms often include numbness and tingling sensations around the mouth, in the fingers and toes. As the disease progresses, muscle spasms may manifest as tetany, wheezing, voice change, and dysphagia. Seizures may occur in severe cases. Effects of hypocalcemia on cardiac function include long QT interval due to prolonged ST fragment, congestive heart failure and hypotension. Acute hypocalcemia should be treated promptly with intravenous calcium. Chronic hypocalcemia is usually treated with oral calcium and possibly vitamin D supplementation.
