Zarontin is approved for absence seizures. Ethosuximide is considered the first choice drug for treating absence seizures in part because it lacks the idiosyncratic hepatotoxicity of the alternative anti-absence drug, valproic acid.

USMLE Neurology 27 Neuro Pathology: Seizures and Pharmacology, Sleep Disorders

24:00
Welcome to LY Med, where I go over everything you need to know for the USMLE STEP 1, with new videos every day. Follow along with First Aid, or with my notes which can be found here: https://www.dropbox.com/sh/8uams03zbpcr333/AABmgSwTFPAhgsUqHKrmFyPIa?dl=0 This is our last video on neuro pathology! We've come a long way. In this video, we will discuss sleep disorders and seizures, including seizure pharm. We start with sleep, and a quick recap on sleep physiology, including it's control by hormones and suprachiasmatic nucleus. We discuss the stages of sleep, including REM and non-rem sleep, and include a talk about the brain patterns associated with each stage. Lastly we discuss some path such as bedwetting or night terrors. We then move on to seizures. Seizures are due to abnormal excessive neuron firing. There are many different types of seizures that you should know, but they are grossly categorized by whether or not they affect one hemisphere or both. Partial seizures affect one hemisphere (often the temporal lobe) and is broken up into intact consciousness (simple) or loss of consciousness (complex). Generalized seizures affect both hemispheres and all have LOC. They instead are broken into symptoms: Absent seizures show up as staring spells that last a few seconds. EEG may show spikes, and there is no post-ictal state. Myoclonic seizures show as repetitive muscle jerks. Tonic-clonic is what we think of when we think of a stereotypical seizure. This is characterized by stiffening and falling. Tonic: stiffening Atonic: complete loss of tone and falling, drop seizures. Whew, that was a lot. Now we can talk about pharm. Because we know seizures are due to excessive firing, we aim to reduce this, either by blocking sodium channels, blocking glutamate, or increasing GABA. Drugs that block sodium channels include: Phenytoin: a powerful drug but has a lot of side effects which include gingival hyperplasia, hirsutism and fetal hydantoin syndrome. Carbamazepine: another powerful drug with a lot of SE. It is used for neuropathic pain also, but is also a teratogen, and is also known to cause agranulocytosis and SIADH. GABA drugs: Valproic acid is the most well-known. It can be teratogenic, causing neural tube defects, as well as causing severe pancreatitis and hepatotoxicity. Vigabatrin is another drug. Some miscellaneous drugs include ethosuximide (which is the best drug for absent seizures), lamotrigine, topiramate, and benzos. That does it for this video. One more and you're all done with neuro!

Depakote Side Effects

00:59
Visit http://lawsuits.lawinfo.com/Depakote/index.html to learn more. "The drug Depakote has been on the market for decades and has been used successfully to treat patients suffering from epileptic seizures. In addition to seizures, it has also been used to relieve migraines, and even help those afflicted with bipolar disorder. As with any drug, a person taking Depakote may experience minor side effects that can include headaches, nausea, joint pain and sleeplessness. Unfortunately, the use of Depakote has also been linked with causing severe birth defects. In a small percentage of documented cases women who took Depakote while pregnant gave birth to children with cleft palates, spina bifida, and even malformed hands. In extreme and unfortunate situations, the drug has been linked with fetal death. If you or a loved one is taking Depakote, be sure to consult with your doctor and take great care if you are, or think you may be, pregnant."

What Is Zarontin? | Epilepsy

01:09
Watch more Epilepsy & Seizure Disorders videos: http://www.howcast.com/videos/502070-What-Is-Zarontin-Epilepsy Zarontin or otherwise known as Ethosuximide is an anti-epileptic, an old one, that's used for abson seizures. So those are those generalized ones where I stare and zone and don't respond. And it's one of the older medications that have been used to treat this very effectively. So there was a study done a few years ago that compared a bunch of different medications for the treatment of abson seizures and actually this is the one that most people used and was the most efficacious. The biggest side effect with Zarontin is that it can upset your stomach. So some kids we give Tums when they are taking the medication, we make sure that they take the dose after they eat. It's available liquid and it's available in pills for those kids who can swallow pills. So it's a great medication, good for controlling abson seizures, those staring and zoning out episodes. Again the major side effect issue is the stomach problems. If you take it with food it's pretty well tolerated. There is always a small risk of a weight gain. But if they get too much of the medication they can get tired and fatigued. But again a good choice, first line choice for patients with abson seizures.

Carbamazepine and Other Seizure Drugs

02:51
This parody of Frank Mills' "Music Box Dancer" describes the pharmacology of the seizure drugs carbamazepine, ethosuximide, phenobarbital, lamotrigine, and gabapentin. Lyrics are below. Note that water intox. stands for water intoxification, and toxic epi. necrolysis stands for toxic epidermal necrolysis. Element abbreviations (e.g. Na) stand for their corresponding ions. Lyrics: Carbamazepine is used for bipolar manic, Trigeminal neuralgia, seizures tonic-clonic, And partial seizures. It’s broken down by the liver. It blocks Na and makes its own breaking down faster. It induces the enzyme CYP 450. Its clearance is slowed when it valproic acid meets. It can cause osteomalacia, diplopia, Agranulocytosis, aplastic anemia, CNS depression, skin rash erythromatous, Ataxia, exfoliative dermatitis, Cleft lip and palate, spina bifida, anemia, Megoblastic, water intox., and hyponatremia. Ethosuximide T-type Ca channels obscures In thalamic neurons and is for absence seizures. Valproate slows its breakdown. It can cause dizziness, Parkinsonism, headache, and also GI distress, And lastly, bone marrow depression rare, but so bad. Phenobarbital blocks AMPAs, ups GABA effects, Opens chloride channels and GABA-A receptors, And sodium and calcium channels does hinder. It’s for simple partial seizures, pre-anesthesia, Tonic-clonic seizures, for the short-term insomnia, And for status epilepticus alternatively. You shouldn’t take it with a prophyria history. Jett Travolta, Seizures, Psych Meds, and Scientology - A Public Service Message

04:58
RIP Jett Travolta - 1992 to 2009 (Age 16). Anonymous condolences to the Travolta family, and a short social education piece about seizures, the beliefs of Scientology, and psychiatric / anti-seizure medication. ----- TRANSCRIPT ------ It is with deep sadness that Anonymous notes the death of Jett Travolta, son of John Travolta and Kelly Preston, reportedly as the result of a seizure. We extend our condolences to the family and friends, and to everybody whose life was touched by Jett. We shall not intrude upon your time of grief. Ladies and gentlemen of the press - people of the Internet, and of the world! While we caution you against drawing ugly conclusions about this very sad situation on the basis of insufficient evidence, as a result of speculation in the press, Anonymous websites have received many tens of thousands of views since news of the child's death was released. Website guests, apparently searching for information about Travolta's belief in Scientology, and perhaps for a more complete understanding of the organization's attitude of disbelief in the existence of disorders such as autism and epilepsy, have made clear the need for Anonymous to publish this bulletin, if only to answer some of these questions. A seizure is a sudden disruption of the brain's normal electrical activity accompanied by altered consciousness and/or, other neurological and behavioral manifestations. For example, epilepsy is a condition characterized by recurrent seizures that may include repetitive muscle jerking commonly referred to as convulsions. According to modern medical science, there are more than 20 different seizure disorders; one in ten Americans will have a seizure at some point during their lifetime; and at least 200,000 have at least one seizure a month. Again, according to modern medical science a combination of drugs may be needed to control some symptoms, but most patients who have seizures take one or more of the following psycho-pharmaceutical medications: phenytoin (Dilantin), carbamazepine (Tegretol), Phenobarbitol, primidone (Mysoline), valproic acid (Depekene), sodium valproate (Depakote and Depakon) Clonazepam (Klonipin), or ethosuximide (Zaronotin). All of these drugs fall under Scientology's broad classification of "dangerous psychiatric drugs," are considered toxic by Scientologists, and are believed by them to remain in the body permanently. By doctrine the use of psychiatric medication is forbidden to all Scientologists and reportedly these provisions apply to the minor children of Scientology families. Many former Scientologists have reported that they were expected or even ordered to cease taking medication for various disorders, including members who suffered from serious medical conditions such as epilepsy, Lyme disease, and even Grave's disease. Scientology denies the existence of such conditions, and attributes their symptoms to other causes. Scientology claims that these symptoms can be alleviated by so-called "auditing sessions" utilizing a crude facsimile of a lie detector, a series of hours-long alternating exercises, lengthy sessions in dangerously hot saunas, and extremely large doses of vitamin tablets. Cumulatively these procedures combine in what is known within Scientology as a "purification rundown" and are likely the "detoxification based on teachings from the Church of Scientology" which Kelly Preston credits in print as "helping improve" the quality and quantity of Jett's life. Unfortunately, the disruption or absence of existing, medically demonstrable treatment regimes often means that participants in Scientology with such illnesses can suffer from recurrence or even intensification of their conditions, which can at times be dangerous for their health or even, in some cases, for those around them. The consequences for a Scientologist who takes, or does not give up, psychiatric medication are many. Their beliefs specifically exclude anyone who has ever taken psychiatric medication from many parts of the religion, and those who have taken psychiatric drugs for any length of time may be excluded from practices such as auditing. Such persons are often also considered entirely unable to advance towards the state of 'clear'. For this reason, Scientology considers psychiatrists and pharmaceutical companies to be 'criminals', and 'an industry of death.' As always; We remain; Anonymous. Legal Note: Permission is hereby granted to freely distribute, mirror, or otherwise copy for the purposes of redistribution, this video IN ITS UNALTERED FORM for all purposes OTHER than the promotion of Scientology (tm). Scientologists mirroring or otherwise utilizing this video on behalf of Scientology in ANY of its forms is prohibited by law, and will be prosecuted to the fullest extent under existing (widely abused) legislation.
