Steroid Cream For Eczema | How To Apply Steroid Cream For Eczema | How To Use Steroid Cream Safely | How To Reduce Steroid Cream Side Effects | How To Use Moisturiser and Steroid Cream | What is A Fingertip Unit Hey guys! This weeks video is about topical steroids, how to use them safely and properly. Topical steroids are used in addition to emollients (moisturisers) and are applied directly to the skin to reduce inflammation and irritation in various skin conditions such as eczema. WHAT ARE TOPICAL STEROIDS: Topical steroids work by reducing inflammation in the skin. They are used for various skin conditions including eczema. (Steroid medicines that reduce inflammation are sometimes called corticosteroids. They are very different to the anabolic steroids which are used by some bodybuilders and athletes.) There are many types and brands of topical steroid. However, they are generally grouped into four categories depending on their strength - mild, moderately potent, potent and very potent. There are various brands and types in each category. For example, hydrocortisone cream 1% is a commonly used steroid cream and is classed as a mild topical steroid. The greater the strength (potency), the more effect it has on reducing inflammation but the greater the risk of side-effects with continued use. Creams are usually best to treat moist or weeping areas of skin. Ointments are usually best to treat areas of skin which are dry or thickened. Lotions may be useful to treat hairy areas such as the scalp. HOW TO APPLY TOPICAL STEROIDS: Always follow your healthcare professionals instructions on how much to apply and how often. Most people only need to use the medication once or twice a day for a week or two, although occasionally your doctor may suggest using it less frequently over a longer period of time. The medication should only be applied to affected areas of skin. Gently smooth it into your skin in the direction the hair grows. If you're using both topical corticosteroids and emollients, you should apply the emollient first. Then wait about 15 minutes before applying the topical corticosteroid. FINGER TIP UNITS: The amount of topical steroid that you should apply is commonly measured by fingertip units (FTUs). One FTU is the amount of topical steroid that is squeezed out from a standard tube along an adult's fingertip. (This assumes the tube has a standard 5 mm nozzle.) A fingertip is from the very end of the finger to the first crease in the finger. One FTU is enough to treat an area of skin twice the size of the flat of an adult's hand with the fingers together. Two FTUs are about the same as 1 g of topical steroid. For example, say you treat an area of skin the size of eight adult hands. You will need four FTUs for each dose. (This is 2 g per dose. If the dose is once a day, then a 30 g tube should last for about 15 days of treatment.) Side-effects from mild topical steroids are uncommon. Side-effects from topical steroids can either be local or systemic. Problem Areas (#1) outer lips (being treated with triamcinolone acetonide ointment 0.5% ) (#2) inner lips (being treated with ? ) (#3) mouth "ulcers" (being treated with ? ) (#4) tongue "ulcers" (being treated with ? ) (#5) tongue bumps (being treated with ? ) (#6) thick "mucus" glue-paste-feeling in throat (being treated with ? ) My oral medicine doctor has decided to take me off of clobetasol ointment & now has me trying Triamcinolone acetonide ointment for my lips. He switched me from the clobetasol ointment to Triamcinolone acetonide ointment because he wanted to reduce the strength of the steroid I am using - I guess triamcinolone acetonide is a weaker steroid than the clobetasol. I am to use the triamcinolone acetonide 2x daily & massage the ointment into my lips for 1 minute. The consistency of the triamcinolone is essentially the same as the clobetasol except the triamcinolone seems to be less "gooey" and more "tacky" which I actually prefer as it comes off of my lips less & it seems like so far I am swallowing the triamcinolone less than I was the clobetasol because the triamcinolone is more tacky & stays on my lips a bit better, at least for the time being. I've also been using biotene "dry mouth" toothpaste. The ONLY reason I use this is because (1) it does not have any minty flavor which I love about it (2) it has fluoride so I wont get cavities. The biotene "dry mouth" toothpaste does absolutely nothing to increase the saliva in my mouth unfortunately. My oral medicine doctor is also thinking of ways to try & improve the cell regulation. I'm not sure what cell regulation he is talking about - my lips, my mouth, my tongue, all 3 - but I DO know that the doctor mentioned something about my cells not maturing or something along those lines & that the regulation of my cells seems to have gone awry. My oral medicine doctor also mentioned... - an immuno-suppressant ointment OR immuno-suppresive therapy (this was unclear & he has yet to decide or get back to me on this idea) - "dapsone" oral pill medication (i dont know his reasoning/if this is still the plan) - blood work checking (1) cbc/d (2) comp metabolic panel (3) G6PD enzyme levels

It is important to understand the causes of fungal infection, precautions to be taken and effective treatment options. They are increasingly becoming a public health hazard with OPD’s being flooded with new cases of these fungal infections. Even though there are a variety of antifungals in the market, we are quickly going to run out of effective options if we do not control these infections properly. There are many reasons for the sudden increase: Urbanization, Over crowding, increased humidity, Long commute, Use of steroid creams,Non compliance to treatment leading to resistant strains The commonly seen fungal infections of skin are caused by dermatophytes. These fungi are skin loving and break the top layer of skin to derive nutrition for survival. They occur in areas where there is water logging or sweat logging such as body folds in the groin area, armpit, in buttock area and under the breast in women. They appear as round, lesions with scaling. They are itchy and cause significant discomfort. They may become red and even causes burning sensation and irritation. What should you do? Consult a qualified health care professional who can make the correct diagnosis and administer proper treatment. Do not take medicines from a chemist. We get patients who go to the chemist/ pharmacist and tell them they have itching in the groin and the chemist hands over the medication without even looking at the lesion or knowing the history of pat medication. This is dangerous. Pharmacist study about the drug, the molecule and are experts at dispensing the correct medication. They are not equipped to make a diagnosis and prescribe medication. You will have to take oral antifungals for a period of 3 weeks to 3 months depending on severity and past treatment. Whenever you visit a doctor for recurrent infections, make sure you take all you old prescriptions along so the doctor has an idea of what all medications you have taken and how you have responded. Please understand that you have to complete the course of antifungals as suggested by the treating doctor. You might notice improvement within a week of treatment. Na d because some of these medications are expensive, people tend to discontinue it mid-way. This should absolutely not be done. The fungus becomes inactive but is not completely cleared from your skin. So, until it completely clears, there is a high chance of it coming back. So please please remember that is very very important to complete the course. I do not want to name any brand of antifungal creams as these fungal infections need to be treated by health professionals and you must visit a doctor. It is not something that you can treat at home. Important things to remember if you have fungal infection: · Take bath daily: twice a day if possible · Use a shower gel for bathing. If you use soap, use a small soap that can be discarded weekly. · Dry yourself with a thin cotton towel. Towels need to be washed and ironed everyday · Iron your undergarments, belt area of trousers before wearing · Wear loose cotton clothing · If you work out in the gym, make sure your clothes are sweat absorbent. Take a bath as soon as you finish your exercise and wash the attire daily. · All our clothes have to be washed separately and sun dried · Apply creams on the lesions and 2 cm area beyond the margin · Keep nails short · Do not scratch the lesion. The Triamcinolone is not helping my lips at all, so after 6 days of using it I am stopping it. I learned that long term use of topical corticosteroids can cause glaucoma - a condition that I am already at high risk for due to an eye injury I got in 2013 - but what makes people with exfoliative cheilitis at increased risk is (1) long term high potency corticosteroid use (2) the area of location that exfoliative cheilitis is which is the lips, not far from the eyes (3) using the steroids on the lips means that 24/7 the steroid ointment is being swallowed and eaten. The only corticosteroid which even partially helps my lips is clobetasol which is the strongest corticosteroid available & the most risky corticosteroid. For exfoliative cheilitis, long term corticosteroid use might be partially helpful or at least help with the dryness of the keratin scaling but the high risk inevitable horrific side effects make it so that it's a lose/lose scenario.

The chief symptoms of this disease include severe itching, photophobia, redness, tearing and tenacious (ropy) discharge. The important clinical signs in conjunctiva include cobblestone papillae in the upper tarsal conjunctiva, limbal conjunctival thickening with gelatinous nodules and Tranta's dots. Corneal involvement can occur in the form of punctuate keratitis, shield ulcer, scarring and pannus formation.4 In majority of children, the disease resolves spontaneously between age 2 and 10 years. Eyes with refractory and frequently recurrent VKC often develop potentially blinding complications like corneal vascularization, corneal opacity or signs of steroid over use. SUPRATARSAL INJECTION: Triamcinolone acetonide 0.5ml (1 ml = 40 mg drug, Preservative Free sloution AUROCORT) was injected in potential space between conjunctiva and Muller's muscle, 1 mm superior to upper edge of tarsus with a 27 gauge needle after proparacaine hydrochloride 0.5 % was instilled into the eye 3 times at an interval of 2 minutes. This produced ballooning of the potential space between the conjunctiva and Muller's muscle. No eye dressing or patching was used after giving the injection. Topical steroid medications were discontinued & patients were maintened with Olopatadine (Pataday, Alcon Lab.) eye drop once per day for several weeks. The recalcitrant eyes with VKC invariably develop disease related or treatment related complications with irreparable ocular morbidity and blindness. This poses a challenge for the treating ophthalmologist, especially when the patients with advanced VKC remain markedly symptomatic and debilitated despite maximum medical treatment. Supratarsal injection of triamcinolone acetonide in patients with severe refractory VKC, found it well tolerated and provided high rate of clinical response with lack of complications.
