Mayo Clinic uses stem cell therapy to treat arthritis in knee

Researchers at Mayo Clinic’s campus in Florida have conducted the world’s first prospective, blinded and placebo-controlled clinical study to test the benefit of using bone marrow stem cells to reduce arthritic pain and disability in knees. It is the first time that the belief that stem cells can provide substantial and possible regenerative relief in an ailing joint has been put to the test in such a rigorous fashion. The researchers say such testing is needed because there are at least 600 stem cell clinics in the U.S. offering one form of stem cell therapy or another to an estimated 100,000-plus patients, who pay thousands of dollars, out of pocket, for the treatment, which has not undergone demanding clinical study. The findings in The American Journal of Sports Medicine represent another first —patients not only had a dramatic improvement in the knee that received stem cells, but also in their other knee, which also had painful arthritis but received only a saline control injection. Each of the 25 patients enrolled in the study had two bad knees, but did not know which knee received the stem cells. Given that the stem cell-treated knee was no better than the control-treated knee — both were dramatically better than before the study began — the researchers say the stem cells’ effectiveness remains somewhat uninterpretable. They are only able to conclude the procedure is safe to undergo as an option for knee pain, but they cannot yet recommend it for routine arthritis care. “Our findings can be interrupted in ways that we now need to test — one of which is that bone marrow stem cell injection in one ailing knee can relieve pain in both affected knees in a systemic or whole-body fashion,” says the study’s lead author, Shane Shapiro, M.D., a Mayo Clinic orthopedic physician.

http://www.arthritistreatmentcenter.com Here’s another complementary medicine that might work for knee osteoarthritis… Arthritis Knee Pain Eased by Chinese Med Nancy Walsh writing for MedPage Today reported the traditional Chinese medicine practice of moxibustion was effective in reducing pain and improving function among patients with knee osteoarthritis, a randomized clinical trial found. After 6 weeks of thrice-weekly sessions of moxibustion, in whichthe herb Artemisia vulgaris is burned in a small device placed near knee acupuncture sites, pain scores decreased from 6.69 to 3.03 among patients receiving the active treatment, compared with a decrease from 6.27 to 4.56 in patients receiving a sham treatment, according to Xueyong Shen, of Shanghai University of Traditional Chinese Medicine, and colleagues. In addition, physical function difficulty scores decreased from 33.4 to 16.43 and from 30.99 to 21.70 in the active treatment and sham groups, respectively (P=0.015), the researchers reported in Arthritis Research and Therapy. Moxibustion is a modality that has been used by acupuncturists for thousands of years and has been considered effective for relief of arthritic pain. But previous studies of this treatment have been of poor quality, so Shen and colleagues undertook a trial intended to be rigorously blinded and controlled, using the standard outcome measure of the Western Ontario and McMaster Universities' Osteoarthritis Index (WOMAC). They enrolled 110 patients who had radiologically confirmed osteoarthritis. Only patients who had never before received moxibustion were included, so they would have no expectations about sensations experienced during the procedure, and they were told that the device used had been recently developed and they might or might not feel any heat during the procedure. Patients' mean age was 65, and two-thirds were women. In the majority of cases, both knees were affected. They were allowed to continue on their baseline pain medications throughout. Just how moxibustion might influence the arthritic joint remains unclear, but may involve factors including heat, odor, and smoke. "Moxibustion might play a role similar to that of acupuncture stimulation, although its effect on the sensory nerve would be more superficial," Shen and colleagues wrote. Tel 786-504-3084 Treatment of stage 3 and 4 Hodgkin lymphoma Hodgkin lymphoma accounts for approximately 10 percent of all lymphomas and approximately 0.6 percent of all cancers diagnosed in the developed world annually. This amounts to approximately 8830 new cases due to Hodgkin in the United States annually. The majority of patients with classical Hodgkin lymphoma present with painless localized enlarged lymph nodes, typically involving the cervical (neck) region. Hodgkin lymphoma arises from germinal center or post-germinal center B cells in the lymph nodes. Hodgkin lymphoma has a unique cellular composition, containing a minority of neoplastic cells (Reed-Sternberg cells and their variants) in an inflammatory background. It is separated from the other B cell lymphomas based on its unique clinicopathologic features, and can be divided into two major sub-groups, based on the appearance and immunophenotype of the tumor cells. The diagnosis of Hodgkin lymphoma is made by the evaluation of involved tissue, usually a lymph node biopsy. Excisional biopsies are preferred and large core needle biopsies may be adequate in select cases. Here, Dr. Tony Talebi discusses the treament of stage 3 and 4 Hodgkin lymphoma with Dr. Ann Morhbacher, associate professor of medicine at the University of Southern California and a "top US doctor" as per US world news and world report magazine. Melissa M. Galli, DPM, MHA is a board qualified and dual fellowship-trained reconstructive foot and ankle surgeon specializing in sports medicine, external (frame) fixation, trauma, total ankle replacement and reconstruction of the forefoot, hindfoot and ankle. Tel 786-504-3084 Hodgkin lymphoma Hodgkin lymphoma accounts for approximately 10 percent of all lymphomas and approximately 0.6 percent of all cancers diagnosed in the developed world annually. This amounts to approximately 8830 new cases and about 1300 deaths due to HL in the United States annually. The majority of patients with classical Hodgkin lymphoma present with painless localized enlarged lymph nodes, typically involving the cervical (neck) region. Hodgkin lymphoma arises from germinal center or post-germinal center B cells in the lymph nodes. Hodgkin lymphoma has a unique cellular composition, containing a minority of neoplastic cells (Reed-Sternberg cells and their variants) in an inflammatory background. It is separated from the other B cell lymphomas based on its unique clinicopathologic features, and can be divided into two major sub-groups, based on the appearance and immunophenotype of the tumor cells. The diagnosis of Hodgkin lymphoma is made by the evaluation of involved tissue, usually a lymph node biopsy. Excisional biopsies are preferred and large core needle biopsies may be adequate in select cases.
