"Our Supersized Kids" profiles four families whose grade school children are overweight or obese. Each of these families are actively working to help their child turn around an out-of-control weight gain. The sugar in blood if remain elevated for a longer period of time then complications like kidney failure, blindness, heart disease, amputation and other nerve damages start to set in. The quality of the life of the child depends on the ability of the parents to recognize symptoms of child diabetes at their earliest. Unfortunately child diabetes has reached to epidemic proportions. It is predicted that incidence of this chronic and deadly disease will continue to rise. Unluckily it is hard to identify the signs of diabetes in children. Read the given list so you can better understand why these symptoms of child diabetes are not easily recognized. Signs of diabetes in children. Excessive Urination: The parents generally think that their child is drinking plenty of water Frequent Urination: The parents' often confuse and think their child is drinking a lot; so he is urinating more. Recurrent Infections: Generally parents consider that this is the part of childhood. Blurriness In Vision: The vision of the children gets blurry but they are unable to explain this to their parents. Sometimes even they are unable to feel that something is going on with their vision. Slow Healing Wounds and Cuts: Simply the parents think the injury was deeper one and it will take comparatively longer time to heal. Numbness and Tingling In Feet and Hand: Once again this symptom of child diabetes is ignored by the parents and children. Children are unable to explain the weird feeling in their extremities to their parents. Unexplained and Sudden Weight Loss: If the child was previously overweight then this symptom of child diabetes is welcomed by the parents otherwise the parents think the young one is going through the growth spurt. Mood Swing or Irritability: This is generally blamed as child and parent confrontation. Some parents of teenagers think that it is the integral art of teen years. It is also important here to keep in mind that all of these signs of diabetes in children may not be obvious at once. Unfortunately there is no magic pill or spell to curing obesity but the good news is that we do have the tools to take a small but important step by getting rid of our judgement and bias towards children with obesity. [Filmed at TEDxMaastricht] Dr Anita Vreugdenhil, Paediatric Gastroenterologist at Maastricht University Medical Centre, is the founder of the Maastricht programme COACH which encompasses an ‘all-inclusive’ approach to improve the lifestyle of overweight and obese children in the South of the Netherlands. Overweight, a condition in which a person’s weight is 10%-20% higher than what is considered as “normal”, and obesity, when a person’s weight is more than 20% higher than normal, are rapidly becoming a national health threat in the Netherlands, especially among children. The latest figures indicate that 500.000 children in the Netherlands are overweight or obese. But many parents don’t realise that their child is overweight and are not aware of the severe consequences. Even when they become aware of the problem, they still don’t know how to correct and change their ingrained behaviour. That’s where Anita and her team can help. All the healthy elements of corn, wheat, and oats are removed during production — only the carbohydrates are left. -Honey can cause an allergic reaction, and sometimes honey contains bacteria, which can lead to a serious infectious disease called botulism. -Grapes are large and slippery, and a child could choke on them. Grapes are also very difficult for a child’s digestive tract to process. -No matter how harmless animal-shaped vitamins look, they should only be prescribed by doctors. -Drinking fatty milkshakes regularly can lead to the development of cardiovascular diseases. -French fries, burgers, and pizza are loaded with salt, sugar, and saturated fat. When consumed regularly, this deadly trio could cause obesity, high cholesterol, and cardiovascular diseases. -Caffeine, which is an ingredient in coffee, could affect calcium absorption, and this might weaken their bones. -Soda contains calories, artificial sweeteners, and caffeine, all of which erode tooth enamel and cause mood swings. -Processed meats like hot dogs, bacon, and bologna are rich in sodium and saturated fat. However, when consumed daily and in large amounts, they may increase the risk of heart disease, diabetes, high cholesterol, and spikes in blood pressure. -Chicken nuggets must go because they’re packed with preservatives, calories, salt, and a bit of sugar. -Potato chips contain loads of sodium and fat. Chips should never land in your grocery basket, and they should stay out of your child’s life as well. -Ketchup is made from sugar, corn syrup, food coloring, and extra fat. -Mac and cheese contains sodium, food preservatives, and noodles stripped of fiber. This same goes for canned spaghetti and ramen in plastic packages. -Canned tuna contains high levels of mercury, which is dangerous for their health. -Cake, cookies, doughnuts, candies, and fruit snacks can cause tooth decay and cavities. -According to the FDA, unpasteurized milk carries dangerous bacteria such as salmonella, E. coli, and listeria. Mom Wins Obesity and Diabetes Battle

Liz Cromwell always puts her family first: She’s currently caring for her eight children as well as her husband, who suffers from spinal stenosis. After a painful nine months of pregnancy followed by the stillbirth of her ninth child, Cromwell says she hit rock bottom. She discovered she was a Type 2 diabetic, gained weight and suffered from severe depression. “I have to always be fighting to stay on top for my family because I know if I’m gone then it’s gonna be a struggle. That is why I just decided that I need to do something for me,” says Cromwell. After trying months of boot-camp-style workouts yet failing to see much progress, Liz finally found Eton CrossFit and instantly knew it was the thing she needed to change her entire life. “We’ve got an amazing, amazing, amazing community—very supportive. … Once I walk into that box, it’s a great release,” says Cromwell, whose fitness has improved dramatically since she started training. The community at Eton CrossFit also helped Cromwell overcome additional obstacles including shoulder surgery and fibroids. In just two years, Cromwell—who “feels like a whole new person”—has greatly decreased her body-fat percentage and cut her insulin usage down from an entire pen to just 20 mL per day. Now Liz can keep up with her athletic children, who have noticed the change in her and aptly call her “Super Mom.” Cromwell is inspiring others with her desire to be active, strong, vibrant and healthy again at 40. “If I keep doing what I’m doing, I will be diabetic-free,” Liz says. A new study suggests, the diabetes drug metformin may help overweight children and teens with autism slim down. The study included 60 people with autism, ages 6 to 17. The patients were overweight due to the side effects of taking antipsychotic medications for irritability and agitation. For the study, participants were given either metformin or an inactive placebo. The study lasted for 16 weeks. Data from the most recent NHANES survey1 shows that among adult men the prevalence of obesity was 31.1% in 2003—2004, and 33.3% in 2005—2006, a small but not statistically significant change. Among adult women, the prevalence of obesity in 2003—2004 was 33.2%, and in 2005—2006 was 35.3%, again a small but not significant change. Another recent NHANES survey2 found that obesity prevalence among children and adolescents showed no significant changes between 2003—2004 and 2005—2006. Based on the study, in the combined years of 2003—2006, 16.3% of children and adolescents aged 219 years were obese, at or above the 95th percentile of the 2000 BMI-for-age growth charts. This rate of obesity raises concern because of its implications for the health of Americans. Obesity increases the risk of many diseases and health conditions. These include— * Coronary heart disease * Type 2 diabetes * Cancers (endometrial, breast, and colon) * Hypertension (high blood pressure) * Dyslipidemia (for example, high total cholesterol or high levels of triglycerides) * Stroke * Liver and Gallbladder disease * Sleep apnea and respiratory problems * Osteoarthritis (a degeneration of cartilage and its underlying bone within a joint) * Gynecological problems (abnormal menses, infertility) Two of the Healthy People 2010 national health objectives3 are (1) to reduce the prevalence of overweight and obesity among adults to less than 15% and (2) to reduce the prevalence of obesity among children and adolescents to less than 5%. Insulin is secreted from the Islets Cells of Langerhand specifically from the beta cells. Glucose enters the beta cell via the GLUT2 and is activated by glucokinase. This increases ATP which deactivates KATP pump. This leads to an influx of calcium and release of vesicles with insulin. OSU and repaglinide inhibit the same KATP channel. Insulin is synthesized in Rough Endoplasmic Reitculum. Preproinsulin has a c,b,a pre and post section. When pre and post is removed then it is known as pro insulin. Pro insulin moves on to Golgi Apparatus where the c peptide curve. a peptide and b peptide forms bond and the c peptide is removed PATHOPHYSIOLOGY OF DIABETES MELLITUS TYPE 2 Genetic causes of diabetes mellitus focuses on MODY1, MODY3, MODY5 associated with HNF. MODY2 is associated iwth glucokinase mutation and MODY4 is associated with INsulin promotor factor (IPF-1). Environmental influences includes obesity which causes releases of adipokinse such as leptin, adiponetin, TNF, resistin. SYMPTOMS OF DIABETES MELLITUS TYPE 2 Diabetes is often times asymptomatic. Most common presentation is polyuria, polydypsia, and weight loss. Other patients with Diabetes Type 2 present with blurred vision, vulvovaginits because diabetes is an immunosuppressed state. Often times it may present Hyperosmolar Hyperglycemic state associated with severe glucose level and hyperosmolality. Diabetes type 2 may also be associated with Diabetic Ketoacidosis. DIAGNOSTIC CRITERIA FOR DIABETES MELLITUS TYPE 2 The American Diabetes Association has four factor for diagnosing diabetes. HbA1c is greater than 6.5 percent is diagnosed as having diabetes. If the fast plama glucose is greater than 125 then the patient is diagnosed as having diabetes. OGTT includes giving 75g of glucose and then checking blood sugar. If patient has greater than 200mgdL than patient is diagnosed as having diabetes. If random plasma glucose is greater than 200 and the patient is symptomatic than that is enough to diagnose the patient was diabetic. SCREENING PATIENTS FOR DIABETES MELLITUS TYPE 2 Screen patient for diabetes if patient is greater than 45 years old, sedentary lifestyle, primary family history, high LDL, Low HDL, signs of insulin resistance and certain ethnicities. Screen children for diabetes if they are greater than 10 years old and overweight, family history, gestational diabetes of mother and signs of hyperinsulinemia. MANGEMENT OF DIABETES MELLITUS TYPE 2 Primary focus of management is control of blood glucose. Initial therapy focuses on Diet, Exercise and Metformin. If after three months the HbA1c is greater than 7 than add another drug to control the diabetes. If after three months the HbA1c is still not below 7, than add another drug for triple therapy. The current diet for diabetics is medical nutrition therapy (MNT). This individualized diets has been seen to be the most effective for diabetic patients. Diabetic children should get at least 60 minutes a day, while adults should have 150 minutes a week. Diabetic patients should be screened for complications of diabetes. Nephropathy check for albuminuria and start treatment for ACE inhibitor if positive. Diabetic Retinopathy can be check with routine ophthalmologic exams. Neuropathy is also common in diabetic patients. Check for sensation of arms and legs. Foot Exam is also important and should be done in every visit. Cardovascular assessment is also important in diabetic patients because they are at risk. Check for blood pressure, dyslipidemia and consider aspirin

While in developed countries, children with type 1 diabetes can lead full, productive lives, in developing countiries those without access to insulin and proper medical care can die within a year. Almost a century after the discovery of insulin, it is unacceptable that this life-sustaining drug is not accessible to all those who require it to live.

There are two types, both of which are related to the function of the pancreas. Type 1 typically occurs in children and young adults and is associated with the pancreas’ inability to produce enough insulin. The job of insulin is to push glucose (aka sugar) into your cells to use as energy, among other things. This is also known as insulin dependent diabetes. Type 2 diabetes tends to occur in overweight adults, although there is an increasing incidence in children. This version occurs when your pancreas can’t keep up with the high levels of glucose in your blood and your cells actually become resistant to the function of insulin. This particular study focus on type 2 diabetes, which has multiple risk factors for development. This includes Weight, especially BMI over 30 Body fat distribution Lack of physical activity Family history Race Age Gestational diabetes (in pregnant women) Polycystic ovarian disease Here are a few other facts about diabetes in the US (from CDC, 2014) 9.3% of the US population has DM (29.1 million people) 27% are undiagnosed 1.7 million new cases are diagnosed each year Total costs of diabetes care in the US is 245 billion annually (2012) There are two types of Diabetes Mellitus, Type 1 and Type 2. In type 1, your pancreas doesn't produce enough insulin. In type 2, the cells of your body become resistant to insulin because of chronically elevated blood sugar. Elevated blood sugar, also known as glucose, can be caused by a lot of things related to diet: fat, protein, carbs, carbohydrates, calories. Treatment includes some combination of insulin, metformin, weight loss, exercise and increased physical activity. Risk factors include obesity, overweight, high BMI, lack of exercise, inactivity, body fat distribution, race, age, genetics, gestational diabetes and PCOS. Complications of diabetes include immunocompromised, peripheral neuropathy and leg amputations, kidney disease, retinopathy and vision loss and even death.
