PART1, TREATING SWELLING CAUSED BY INJURY 1)REST THE SWOLLEN AREA. Whether your body is swollen from an injury or poor circulation, it is best to allow the swollen area a bit of time to rest. If you have a swollen foot or ankle, try not to use it in a strenuous way for at least a few days, until the swelling goes down. • If you have an injured foot, consider using crutches or a cane to take the pressure off the swollen area. • If you have a swollen arm caused by an injury, use the other arm to perform tasks, or ask for help from someone else. 2)ELEVATE THE SWOLLEN BODY PART. Any time you are sitting or lying down, prop up the swollen area on pillows, above the level of your heart. This prevents blood from collecting in the swollen area and aids in circulation. • Use a sling to keep your arm elevated if necessary. • If the swelling is severe, make a point of sitting down and elevating the swollen body part for a few hours. 3)APPLY A COLD COMPACT. High temperatures will make swelling worse, so do your swelling a favor by applying a cold compact. Avoid applying ice directly to the skin, but wrap an ice pack in a towel and apply to the swollen area. Do this for 15 minutes at a time, several times a day. 4)TAKE MEDICATION. Non-steroidal anti-inflammatory drugs (NSAIDs) are medications that decrease pain and swelling. The most common variants are ibuprofen (common brands include Motrin and Advil) and naproxen (Aleve). Note that acetaminophen (Tylenol) is not an NSAID and will not reduce swelling. Talk to your doctor about what the best medication for your situation may be. If that's not an option, make a point of getting up to walk around the office every hour or so. • When you are sitting, change positions often and keep your feet slightly elevated whenever possible. 2)MINIMIZE YOUR SODIUM INTAKE. High levels of sodium contribute to swelling, so avoid consuming foods with lots of sodium. In extremely severe cases, jaundice can also cause a loss of brain function. In older patients, this is often misdiagnosed as a dementia-related disorder. Treating liver disease-related jaundice depends on successfully addressing the underlying medical condition. While severely damaged livers will never fully regenerate, jaundice and its related symptoms will diminish in intensity as liver function improves. However, it’s important to remember that liver disease isn’t cured when jaundice disappears; patients who recover from severe liver damage must still be very careful to maintain better health. 3. Abdominal Tenderness Abdominal pain, particularly in the upper right corner of the abdomen, or to the lower right portion of the rib cage, is a telling sign of liver damage. This pain may or may not be accompanied by swelling in the abdomen, or ascites. Patients usually characterize it as a persistent throbbing or stabbing pain which can only be temporarily relieved by medications. In fact, improper use of medications can be part of the reason you’re experiencing abdominal pain symptoms. It’s important to remember that liver disease can be caused by long-term overuse of certain over-the-counter and prescription medications, including acetaminophen and narcotic-acetaminophen combinations. These medications may cause upset stomach as a general side effect, and their damaging attributes can be amplified if you drink alcohol while using them. The upset stomach side effects can snowball over the long term as liver damage appears. Eventually, this pain will become severe enough for patients to seek medical attention. 4. Urine Changes Urine may become dark yellow in color due to increased levels of bilirubin in the body’s bloodstream, which the damaged liver is unable to eliminate via excretion through the kidneys. Bilirubin is a bodily fluid which is produced through the natural breakdown of bile, and is normally eliminated through bile and urine. It is believed to function as a cellular-level antioxidant, but excessive amounts can lead to toxicity. Uncontrolled reflexes and eye movements, seizures, and neurological impairments can all result from chronically high bilirubin levels. Dark urine can be caused by many different factors, and it isn’t something to be alarmed about if it does not occur on a continuous basis. Liver disease can also cause other skin problems, including a generalized yellowing of the skin and mucus membranes (jaundice), reddened of the feet or hands, and changes in skin tone and color. These changes can result in unusually dark or unusually light patches of skin in localized areas. In some cases, patients experience the appearance of veins through the skin. If any of these symptoms appear, and particularly if they are accompanied by itching, you should speak to your doctor. He or she can check for signs of liver problems by performing a few simple, painless tests.

WHAT TO EXPECT: At the ER they will put an IV line in you to keep fluids going into your body. Your leg diameter will continue to swell for the next few days. Be sure you ask for a doppler to confirm thrombosis. Your pain levels will increase...you won't be able to walk but a few meters without passing out from orthostatic hypotension. If you are on diuretics for BP CONTINUE THEM. ASK THE HOSPITAL DOC (DAY 1) IF YOU CAN HAVE SOMEONE BRING YOU 20-30 mm thigh high COMPRESSION STOCKINGS FOR IMMEDIATE USE. You will need measurements of your ankle, calf, and thigh at the widest part. You will only use these until about two or three weeks after you get home then order some stockings at 30-40mm for the next 4-6weeks and then go to 40-50mm. As soon as you can (when the 30-40mm began to stretch and lose compression.The hospital may give you some short knee high compression socks which are useless and just slow your progress or they may use a machine with a pressure cuff to stimulate circulation---they usually don't get the cuff adjusted tight enough to do any good....take some control here and do it yourself. Each day, Ask your hospital doc if your vital signs have returned to normal (such as kidney function). If so then don't let the hospital keep putting IV fluid into your body...that just makes your leg feel like it is going to burst plus it makes it almost physically impossible for you to use the toilet in your room no matter where it is. They will argue the IV is for your benefit. Tolerate what you can but take control. You will also feel your upper body begin to retain fluid as well. This will also make going to the toilet a lot more difficult. Until you can get the fluid in your body down (ask doc to use lasix for that) you will be in great pain and pretty immobile and severely constipated and unable to pee. You will be put on Coumadin or Warfarin accompanied with a few shots of Lovenox injections for about ten days. The Coumadin you may be on forever. You will also be given medication for pain.

Ginger and/or Turmeric: The powerful anti-inflammatories present in ginger root and turmeric can be very helpful in easing gout pain and inflammation. I basically chop off a little piece the size of an inch and boil it for about 20 minutes and drink it as tea. You can also add ginger root and/or turmeric in cooking recipes. Some may also eat a small piece raw daily. Others use it topically to reduce swelling by making a paste of ginger root with water and then apply it to the affected area, leaving it on for about 30 minutes. 5. Water: Very simple, we all have it and it’s usually free. Drink plenty of it and flush excess uric acid out of your body. Staying hydrated is one effective way of limiting gout flares. Drink at least 8 glasses of water a day but if you can, why not strive for 12?! Make sure a water bottle is with you everywhere you go, I carry a bottle with me from morning to night and keep refilling it all day long. 6. Lemon Juice: Another natural home remedy that helps alkalize your body thus reducing uric acid in the blood, is drinking lemon juice. Many gout sufferers squeeze half a lemon in a glass of 8 oz. of water and drink it up to three times a day. Others will combine it with ½ a teaspoon of baking soda. Remember lemon juice is also high in vitamin C which also helps gout. 7. Celery Seeds: Another popular natural home remedy that helps lower uric acid and prevent gout according to many gout sufferers is celery seed extract. Celery seed simply stimulates your kidneys causing your kidneys to excrete more uric acid out of the body. Thus it causes you to visit the bathroom and urinate more. 8. Epsom Salt: There is no better topical treatment to treat a gout attack than epsom salt, widely used by many to relax muscles, aches and pains. Osteoarthritis is the most common form of arthritis and is associated with the aging process. Osteoarthritis is a chronic disease causing the deterioration of the cartilage within a joint. For most people, the cause of osteoarthritis is unknown, but metabolic, genetic, chemical, and mechanical factors play a role in its development. Symptoms of osteoarthritis include loss of flexibility, limited movement, and pain and swelling within the joint. The condition results from injury to the cartilage, which normally absorbs stress and covers the bones, so they can move smoothly. The cartilage of the affected joint is roughened and becomes worn down. As the disease progresses, the cartilage becomes completely worn down and the bone rubs on bone. Bony spurs usually develop around the margins of the joint. Part of the pain results from these bone spurs, which can restrict the joint’s movement as well. Osteoarthritis (OA) is a type of joint disease that results from breakdown of joint cartilage and underlying bone. The most common symptoms are joint pain and stiffness. Initially, symptoms may occur only following exercise, but over time may become constant. Other symptoms may include joint swelling, decreased range of motion, and when the back is affected weakness or numbness of the arms and legs. The most commonly involved joints are those near the ends of the fingers, at the base of the thumb, neck, lower back, knee, and hips. Joints on one side of the body are often more affected than those on the other. Usually the symptoms come on over years. It can affect work and normal daily activities. Unlike other types of arthritis, only the joints are typically affected. Causes include previous joint injury, abnormal joint or limb development, and inherited factors. Risk is greater in those who are overweight, have one leg of a different length, and have jobs that result in high levels of joint stress. Osteoarthritis is believed to be caused by mechanical stress on the joint and low grade inflammatory processes. It develops as cartilage is lost and the underlying bone becomes affected. As pain may make it difficult to exercise, muscle loss may occur. Diagnosis is typically based on signs and symptoms, with medical imaging and other tests occasionally used to either support or rule out other problems. In contrast to rheumatoid arthritis, which is primarily an inflammatory condition, in osteoarthritis, the joints do not typically become hot or red. Treatment includes exercise, efforts to decrease joint stress, support groups, and pain medications. Efforts to decrease joint stress include resting and the use of a cane. Weight loss may help in those who are overweight. Pain medications may include paracetamol (acetaminophen) as well as NSAIDs such as naproxen or ibuprofen. Long-term opioid use is generally discouraged due to lack of information on benefits as well as risks of addiction and other side effects. If pain interferes with normal life despite other treatments, joint replacement surgery may help. An artificial joint typically lasts 10 to 15 years. Osteoarthritis is the most common form of arthritis with disease of the knee and hip affecting about 3.8% of people as of 2010. Stiffness is most common in the morning, and typically lasts less than thirty minutes after beginning daily activities, but may return after periods of inactivity. Osteoarthritis can cause a crackling noise (called "crepitus") when the affected joint is moved or touched and people may experience muscle spasms and contractions in the tendons. Occasionally, the joints may also be filled with fluid. Some people report increased pain associated with cold temperature, high humidity, and/or a drop in barometric pressure, but studies have had mixed results. Osteoarthritis commonly affects the hands, feet, spine, and the large weight-bearing joints, such as the hips and knees, although in theory, any joint in the body can be affected. As osteoarthritis progresses, the affected joints appear larger, are stiff, painful and may swell, but usually feel better with gentle use but worse with excessive or prolonged use, thus distinguishing it from rheumatoid arthritis. Osteoarthritis is the most common cause of a joint effusion of the knee. Sometimes called degenerative joint disease or degenerative arthritis, osteoarthritis (OA) is the most common chronic condition of the joints.

At Kennicott Mine today, we are taking the Root Glacier hike. An amazing 5-6 mile hike through town, along the side of the mountains, near the Eric mine shaft, up on to the Root glacier. Using crampons was the only way to walk on this chunk of ice. We explored the blue ice, blue ponds, ice waves, the Kennicott glacier moraine, and an amazing moulin that was approx. 175 deep. ( we guessed as we couldn't see the bottom). Then back to the lodge to take Tylenol for swollen knees, ankles, and blisters. Yep a great day!!
