I've been practicing dermatology at Dermatology Partners in Wellesley, Massachusetts for the past 10 years. I believe that skin care doesn't have to be complicated. With a few simple steps you can have beautiful skin. You can visit Dermatology Partners on the web at dermatologypartnersinc.com. So today we're gonna talk about skin care. So, Retin A, or Renova is a medication that we commonly use in dermatology. We use it frequently to help treat fine lines and wrinkles as well as acne. So these are great treatments but they have to be used correctly. You have to remember that this is a pretty strong medication so more is not better. What you wanna do is take the size of a small green pea, put it on the tip of your finger and gently dab it on different areas of your face. Almost like if you're applying moisturizer. You want to avoid the corners of your mouth and the corner of your eyes and then you wanna slowly rub it into the skin. After it's rubbed in you want to apply a moisturizer on top. Another way that you can apply this is to take the same small, pea size amount into the palm of your hand, mix in some moisturizer, mix it together and then rub it into your face. If your skin is getting too red or irritated you want to stop a few days and give yourself a break. And generally when you start using these medications, you wanna start using them two or three times a week and gradually work yourself up into every night. So when using this medication, it's important to remember that you skin is gonna be more sensitive to the sun so make sure you're using proper sun protection. And if you're doing any sort of cosmetic procedure such as waxings or peels, you want to stop a few days before and if you're pregnant you want to discontinue using the medication immediately. So these medicines can have excellent results but is has to be used properly.

And stay tuned until the end of this episode for the question of the day; if you answer it correctly, you'll have a chance to win a free skincare product! Retin-oids, Retin-A ,Retin-ol and Renova. We all hear a lot about them, but what's the difference? For all intents and purposes, they all began with Retin-A, so that's where we'll start. Retin-A is the brand name of the original skincare product containing the prescrition-only active ingredient, tretinoin. Tretinoin is the form of Vitamin A that has been turned into an acid... So it's also known as retinoic acid... And was originally developed to fight acne, which it has successfully done for the last 30 years. In the late 1990's, it was discovered that tretinoin could also help reduce fine lines, wrinkles and other signs of photo-aging. Because this use was for a much older population with much drier skin, they put the tretinoin in a much more emollient and moisturizing vehicle, and called it Renova. So the main difference between Retin A and Renova is its vehicle. Now for the fun part! Retinol. We now know that Retin-A is a product that contains the acid derivative of Vitamin A. Retinol, on the other hand, is a non-prescription active ingredient, and a form of Vitamin A... But Retinol is not the acid form of Vitamin A like tretinoin. Instead, it's the actual form of Vitamin A found in animals. Since Retinol is a fairly soluble compound, if it's put on the skin, it can be absorbed into the epidermis where under certain circumstances, in the presence of certain enzymes, a small part of the Retinol, less that 10%, can be converted into tretinoin, which as you know is the active ingredient in Retin A and Renova... Voila! So this stuff really is all related and it's starting to make some sense. So non-prescription Retinol can convert into prescription tretinoin. But the operant word is "can." The conversion is not that predictable. So just because a product contains retinol doesn't mean it can or will do what tretinoin can do in terms of acne or anti-aging treatments... And probably won't. Yet many skincare companies tout their products containing Retinol as working as if they contained tretinoin. This is definitely a case of buyer beware. Now we get to the final term. Retinoids. Retinoids are a class of chemical compounds that are all related to Vitamin A. Retinol and Tretinoin are called first generation Retinoids because their structure is closest to Vitamin A. Less related products are second generation, third, and so on... Finally, connecting the dots, you now know that all of the products I've mentioned today are related to Vitamin A and therefore are collectively called Retinoids. Dr. Schultz used to prescribe retinoids to his patients. That's because at the time, it was the best exfoliant available. However, it can be extremely irritating and has some other side effects. In this episode of DermTV, Dr. Schultz shares some DermTV viewers' stories which serve as examples of why he no longer prescribes retinoids and instead uses the gold standard of exfoliants: glycolic acid. Dr. Schultz used to prescribe retinoids to his patients. However, time and time again, I get questions about retinoids. Retinoid products are also effective exfoliating treatments for acne and are also helpful for facial rejuvenation. Retinoids include not just Retin-A, whose generic name is Tretinoin, but also other derivatives of tretinoin like Ta zarotene, and Adapalene. The reason that I've moved to glycolics is the side effects from retinoids and is really well underscored by a few questions and comments I've recently gotten from viewers on the topic. I'd like to share them with you. Here are a few. First. "Hey Dr. Schultz. My name is Eva and I'm from Africa. I have just started using Retin-A cream and my face got worse within the third day. I got rashes on my forehead. Trying as much as possible to keep away from sunlight although it's really hot on this side of the Sahara! So, should I stop using it? Need some serious help!" Another is... "Hi Dr. Schultz. I'm on Tretinoin 0.025% topical cream for acne and acne scarring. I've been using it for one week. It's making my face darker than the rest of my body. Also flaky and stingy. How long will these effects last? Will my skin return to its normal color? And last... "I used tretinoin under my eyes and on my face. My eyes have more wrinkles, and my cheeks are dark. How can I fix this? Please help!" Obviously, all of these viewers need to stop using their tretinoin and get help from whomever prescribed it for them. Again, I have no personal bias against retinoids but unfortunately patients often have difficulty with them. They can lead to discoloration, irritation, flaking and even soreness. And one other issue of importance with retinoids: They can have potential harmful effects to a forming fetus, so you can't use any retinoids if you're pregnant. All retinoids should only be used only under the direction of a physician. Of course, if you're using retinoids under proper medical supervision and you like the results they're providing then you should certainly continuing using them but to avoid the issues I just mentioned, I prefer glycolics. However, I have been getting messages asking about Tretinoin after featuring it in one of my blogs as one of my skin care essentials found in my man-bag. Looking back, my first Tretinoin was prescribed to me by my dermatologist. Now, Tretinoin cream may be purchased over-the-counter (without prescription needed) from the local pharmacy. I use Tretinoin to address my occasional adult acne and for anti-aging purposes. Recommended usage is to apply once daily at bedtime. I only use it though once a week to minimize its side effects. Side effects may include dry skin and photosensitivity. I also read that it may cause a stinging or burning sensation. Although, I have not experienced this yet. To address dry skin, I make sure I use a good evening moisturizer. During the day, I wear sun protection such as an advanced daily facial moisturizer with SPF 15 and a cap to protect my face from the harsh UV rays of the sun. Retin A Micro for Acne & Wrinkles | Results & Side Effects I've been using retin a micro .1% for a little over 2 months now and I'm really happy with the results. It is helping to keep my skin nice and clear (along with my healthier diet) and also giving it a healthier overall appearance. My skin is firmer, healthier, and seems more resilient. I do not have much for wrinkles, yet, but this definitely helps soften the fine lines I do have. Thankfully, I do not suffer from acne like I used to (see my acne story here: https://www.youtube.com/watch?v=pz8Hf... ) but I am still prone to breakouts. I was excited to start using retin a again (I had to stop due for a few years due to pregnancy / nursing) since I had used it in the past with great results. For the first 4 weeks, I stuck to one application per week. Once my skin adjusted and stopped peeling, I increased application to 2x per week. I will probably stick to 2x per week - it seems to be the sweet spot for my skin. I don't want to over do it since retin a micro is quite powerful. In between applications, I use good moisturizers - Cerave PM is my favorite. Retino A Cream Excellent For Dark Acne Spot Scars Treatment uses benefits side effect precaution & Retino A Cream review Retino-A is the most commonly used Tretinoin cream in India. Tretinoin is an acid form of Vitamin A usually prescribed for acne vulgaris, acne scar treatment and sun-damaged skin. Both women and men can use Retino-A. Retino-A is available in 2 concentrations – 0.025% and 0.05% Tretinoin cream. The price of Retino-A 0.025% is Rs. 113 and Retino-A 0.05% is Rs. 108 for a 20 gm tube Retino-A is a prescription-only product in India, meaning it should be used only in consultation with a doctor. If your dermatologist has prescribed you Retino-A, this post will help you use it while keeping side-effects and skin-irritation to the minimum. Is Retino-A a good acne treatment cream? Retino-A is an excellent overall and spot treatment for cystic acne – those under the skin bumps that stay there for days together. A tiny amount of Retino-A on the spot suppresses a cyst, usually while it is still under the skin. That way you minimize chances of scarring from a popped pimple. Using too much Retino-A on a single spot can burn the skin in that area and cause a dark mark. Use a cotton bud (Q-tip) to scoop out a minuscule amount of Retino-A and dab it on the pimple. Does Retino-A work for pitted acne scars / pimple marks? Acne scars look depressed because of loss of collagen during healing of pimples. Tretinoin kind of “fills up” pitted acne scarring over time. This plumping action of tretinoin is not the temporary kind of plumping achieved from moisturizers. This plumping is because of increased collagen production under the skin, so the results last. It is also because of this reason that Tretinoin is a gold standard in removing wrinkles. For acne prone skin, Tretinoin / Retino A is a spot treatment, scar removal cream plus an anti-aging night-cream. It doesn’t get better than this. How to use Retino-A When it comes to prescription strength medications like Retino-A, it is best not to self-medicate. Your dermatologist will likely get your started on the lowest dose first. That is so that your skin can build up tolerance, slowly and safely. Retino-A can cause a burning sensation when you are just starting out. Wash face with a gentle cleanser. Our first choice is Cetaphil Cleansing Lotion. Wait for about 15-20 minutes after washing face. This will reduce the burning sensation a great deal. Use a very thin layer of Retino-A – half a pea-sized amount for the whole face. Avoid any cuts or open wounds. Do not use Retino-A around the eyes or on eyelids. Use a mild eye cream. Start with once every 3 nights. Then slowly increase to using it every night. It is okay to skip a couple days in between. Pat a gentle moisturizer on top if skin feels dry. Use a high-protection sunscreen during the day. Retino-A Side Effects & Precautions When using a Tretinoin cream, you may experience an “initial breakout” phase. Some people have reported a face full of pimples from this initial breakout. It doesn’t happen to everyone, and usually subsides in a couple weeks, but if it doesn’t, stop use and consult your dermatologist. Use Retino-A only at night. Never skip sunscreen during the day even if you use Retino-A only at night. Do not use any exfoliating cleansers, toners, moisturizers, whitening treatments, scrubs or peels. Retino-A makes skin extremely sensitive to basically everything, so using abrasive or exfoliating stuff alongside is skin suicide. Keep watching for signs of hypo pigmentation (lighter coloured skin patches). Stop use until hypo pigmentation fades. If you notice some swelling of the skin, it means you are either using too much Retino-A or too often. Stop use until the swelling subsides. Do not switch to a higher percentage of tretinoin before your skin can comfortably tolerate a lower strength. Retino-A is a prescription strength treatment. Read the accompanying leaflet carefully. Retino-A may cause excessive skin peeling. Use a gentle moisturizer to hide and heal the flaking. Use makeup that does not have any in-built acne treatments such as Salicylic Acid. Use Johnson’s Baby Oil or another gentle makeup remover to remove makeup. The biggest thing to remember while on Retino-A is to not use any harsh skin treatments. Ideally, your skin care routine should contain a gentle cleanser, Retino-A and a moisturizing sunscreen.
