Dr. Schultz used to prescribe retinoids to his patients. That's because at the time, it was the best exfoliant available. However, it can be extremely irritating and has some other side effects. In this episode of DermTV, Dr. Schultz shares some DermTV viewers' stories which serve as examples of why he no longer prescribes retinoids and instead uses the gold standard of exfoliants: glycolic acid. Dr. Schultz used to prescribe retinoids to his patients. However, time and time again, I get questions about retinoids. Retinoid products are also effective exfoliating treatments for acne and are also helpful for facial rejuvenation. Retinoids include not just Retin-A, whose generic name is Tretinoin, but also other derivatives of tretinoin like Ta zarotene, and Adapalene. The reason that I've moved to glycolics is the side effects from retinoids and is really well underscored by a few questions and comments I've recently gotten from viewers on the topic. I'd like to share them with you. Here are a few. First. "Hey Dr. Schultz. My name is Eva and I'm from Africa. I have just started using Retin-A cream and my face got worse within the third day. I got rashes on my forehead. Trying as much as possible to keep away from sunlight although it's really hot on this side of the Sahara! So, should I stop using it? Need some serious help!" Another is... "Hi Dr. Schultz. I'm on Tretinoin 0.025% topical cream for acne and acne scarring. I've been using it for one week. It's making my face darker than the rest of my body. Also flaky and stingy. How long will these effects last? Will my skin return to its normal color? And last... "I used tretinoin under my eyes and on my face. My eyes have more wrinkles, and my cheeks are dark. How can I fix this? Please help!" Obviously, all of these viewers need to stop using their tretinoin and get help from whomever prescribed it for them. Again, I have no personal bias against retinoids but unfortunately patients often have difficulty with them. They can lead to discoloration, irritation, flaking and even soreness. And one other issue of importance with retinoids: They can have potential harmful effects to a forming fetus, so you can't use any retinoids if you're pregnant. All retinoids should only be used only under the direction of a physician. Of course, if you're using retinoids under proper medical supervision and you like the results they're providing then you should certainly continuing using them but to avoid the issues I just mentioned, I prefer glycolics. Isotretinoin is the gold standard medicine for the treatment of nodulocystic or severe acne. In this acne which has lot of cysts and lot of scarring associated with it, we need an efficient which can permanently reduce the oil secretion of the oil gland itself. When we are looking at that, one of the most common side effects which happens with the isotretinoin use is the lips become dry and they tend to chap and peel. The skin as well tends to become dry and this is one of the most common side effects and if we do not experience it, we feel that the dose of isotretinoin is not adequate enough. So what is the dose that the doctors do to usually combat this. First thing is that they will tell you to user a very pH balance soap free face wash and a face cleanse, sulphate and a paraben free moisturizer 3 -5 times a day. Also it is very important that this dry skin is not exposed to extremes of weather. So please use a sunscreen which in addition to this moisturizer and this soap free face wash. Third thing you can do for the lips is to use like a chemical free basic petroleum jelly based lip balm. This can be used 3 -5 times a day. When the lips go dry, the natural tendency is to lick the lips or to peel the lips with your fingers or with your teeth. When this happens, remember that the saliva has lot of digestive enzymes which can digest the skin of the lip and make the whole peeling problem worse. So make sure that instead of licking the lips, sip on water, use a lot of lip balm. These are the common things that you can do to help with the redness and peeling of the skin which is associated with isotretinoin.

Acne Treatment with Topical Retinoids Retin-A

Topical retinoids are the foundation for the treatment of acne. They are effective for comedonal and inflammatory acne. Mechanism of action Retinoids normalize keratinization. They increase the turnover of follicular epithelial cells, corneocytes are shed at an accelerated rate and comedones are extruded. New comedone formation is inhibited. Retinoid Formulation The choice of vehicle is important. Creams are used for dry sensitive skin, gels are for patients with oily skin. Tretinoin was the first retinoid. It may cause irritation and improvement may not occur for 1-3 months. Tretinoin is not stable in sunlight. Tretinoin (Retin-A and generics) Cream: 0.025%, 0.05%, 0.1% Gel: 0.01%, 0.025% Solution: 0.05% Tretinoin in other vehicles Microsphere vehicle (Retin-A Micro): 0.04% gel, 0.1% gel Adapalene is a derivative of tretinoin. It is stable in sunlight and resistant to oxidation by benzoyl peroxide. Adapalene has a low irritation potential and no phototoxicity. Third-generation topical retinoids Adapalene (Differin): 0.1% gel, 0.1% solution, 0.1% pledgets, 0.1% cream Tazarotene is a synthetic retinoid that is effective for acne and psoriasis. Tretinoin and adapalene are category C drugs, and tazarotene is a Category X drug. This difference in categorization by the FDA is based on the use of tazarotene in the treatment of psoriasis Third-generation topical retinoids Tazarotene (Tazorac): 0.1% cream, 0.1% gel where medication is used over a wide area. The newer topical retinoids have an antiinflammatory effect and are first-line treatment for both comedonal and inflammatory acne. Topical retinoids are used for maintenance therapy to prevent the formation of new microcomedones. Topical retinoids are often used in combination therapy, especially with benzoyl peroxide and topical and/or oral antibiotics. Patient instruction of use of topical retinoids Proper use and application is the key to success and avoiding irritation. It is not necessary to produce irritation to obtain efficacy. Patients who are irritated will be noncompliant. Wash the face with a mild cleanser. A Soapless cleanser such as Cetaphil can be used for patients with very sensitive skin. Avoid wash cloths or abrasive cleansers. Wait for 20 minutes so that the skin becomes dry. Application of medication to wet skin enhances penetration and irritation. Apply just enough medication to cover the skin. Acne may be worse in the first few weeks as microcomedones and comedones are extruded. Consider starting with every other day application until daily application can be tolerated. Another technique is to apply the retinoid in a “short-contact” manner. Apply medication for 30 seconds to 5 minutes then wash off the drug. This can be done once or twice each day to help the skin to adapt. Patient instructions - Wash face gently with a mild cleanser - Wait 20 minutes after washing before application - Apply a very thin layer of medication. - Use noncomedogenic moisturizers to control irritation - Avoid excessive sun exposure - Apply a sunscreen daily - Protect the face from cold and wind Several studies have compared these agents. Tazarotene gel 0.1 is more effective than tretinoin gel 0.025 and the microsphere gel. The reduction of open comedones at 12 weeks was 65% for tazarotene vs 44% for tretinoin. Adapalene gel 0.1% is as effective as tretinoin gel microsphere 0.1% after 12 weeks of treatment, but tretinoin gel achieved a greater reduction in the number of comedones at week 4. Alternate-day applications of tazarotene 0.1% gel and daily applications of adapalene 0.1% gel give similar results at 15 weeks. The response to tazarotene treatment was greater during the first 3-6 weeks of treatment. Tazarotene plus clindamycin lotion is more effective than either agent used alone. Tazarotene in combination with benzoyl peroxide or an erythromycin/benzoyl peroxide gel is as effective as tazarotene used as monotherapy.

Sadly, they blame themselves and it’s not their fault. So let’s start. Here’s the question I get asked a lot. Is there anything that can reverse wrinkles from sun damage? The answer is a resounding yes! Retinoic acid, which I’ll refer to as prescription retin-A to make it easy for you is the ONLY Product given the FDA seal of approval to reverse sun damage. I’ve used it over 30 years myself – it’s powerful. Only one other store bought product has similar power and I’ll get to that in a minute. Why is (retin-A), so effective? You just need a little biology 101 to get the answer – here it is: Your skin cells have little receptors. (I’ll show chart myself) Some receptors are for hormones and other things. They also have (Retin-A) receptors. Only (retin-A), not store bought retinols can “fit” into the receptors and trigger cells to do all kinds of great Things including reversing UV damage. In fact (retin A), is picked up by proteins and brought all the way to your skin’s DNA where it commands cells to reverse sun damage. Store bought retinols claim they are as effective as retin A but need to go through a lengthy complicated metabolic process to eventually become a form of retin-A that has the configuration to fit into the receptors. Here’s the process: Vitamin A changes into Retinol-Palmitate and other esters which then changes into retinol which changes into Retinaldehyde which changes into retin-A Vitamin A (place arrow downward) Retinol-Palmitate Retinal Acetate, Retinal Linoleate, Retinal Propionate. Retinol Retinaldehyde Retin-A Whew – sounds exhausting! but it’s important to know the whole story and why that when you use a product that has retinol Palmitate in it, it just doesn’t do the job it was intended to do –get rid of wrinkles! Despite what the company might tell you. Researchers in Anti-aging tips believe retinol palmitate is 1/20 the anti-aging power of (retin A). The good news folks is that a new player has come on the scene. It’s called Retinaldehyde. It only requires 1 conversion step to become (retin-A). inside your skin. It is the only product on the market independently proven to be as effective as (retin-A). This is a game changer in Skin care because it was found to be much more skin friendly and tolerable than (retin A). Expect to see it soon as chemist’s first need to refine this important molecule! Retinaldehyde, in Avene Retrional 0.05 Cream at DermStore $61.00. But beware, researchers have not found a way to stabilize it yet so I’m not guaranteeing and results at this time. Today, for now, I would use a prescription retinoid my favorite form is Tazorac 0.1%. There’s evidence to show that because it binds longer and more tightly to the receptors it is more effective. Tazorac 0.1% is the number one player in my antiaging regimen. Long ago, a doctor told me that if you use the strongest retin A for a lifetime, your skin will stay youthful for life and he was right. Tazorac or other prescription retinoids like retin A not a quick fix folks but if you devote 6 to 8 weeks you’ll be thanking me. So subscribe to my channel. Retinol is a weaker, over the counter version of tretinoin, a prescription vitamin A derivative that is highly effective in reversing sun damage and signs of aging. Tretinoin decreases fine lines, evens skin color, improves texture, tightens pores, and stimulates blood flow and collagen production. However, quite a few people cannot tolerate tretinoin because it can be highly irritating and drying. Retinol is milder; while the results may not be as dramatic, more people tolerate it well. Like tretinoin, retinol encourages skin cell turnover; this new skin is delicate and should not be exposed to the sun. That is why these ingredients should only be found in products intended for night use. Vitamin A derivatives, including retinol, rapidly break down when exposed to the sun and air. That’s why retinol-containing products should always come in pumps or tubes with tight-fitting caps. This is Dr Rebish's patient (my husband who is also a dermatologist), who has comedonal and inflammatory acne. A typical acne treatment regimen would be oral antibiotics and topical medications including tretinoin. Tretinoin is the best treatment to help prevent the blackheads and whiteheads which are the basic building blocks to acne. Extractions are helpful, because they will aid in clearing the existing comedones, which tretinoin cannot do - tretinoin mainly is effective at preventing new blackheads and whiteheads from forming and softening present comedones up so that they are more easily extracted. The breakdown of these fibers causes the skin to lose its ability to snap back after stretching. As a result, wrinkles form. Gravity also is at work, pulling at the skin and causing it to sag, most noticeably on the face, neck, and upper arms. Cigarette smoking also contributes to wrinkles. People who smoke tend to have more wrinkles than nonsmokers of the same age, complexion, and history of sun exposure. The reason for this difference is not clear. It may be because smoking also plays a role in damaging elastin. Facial wrinkling increases with the amount of cigarettes and number of years a person has smoked. Many products currently on the market claim to "revitalize aging skin." According to the American Academy of Dermatology, over-the-counter "wrinkle" creams and lotions may soothe dry skin, but they do little or nothing to reverse wrinkles. At this time, the only products that have been studied for safety and effectiveness and approved by the Food and Drug Administration (FDA) to treat signs of sun-damaged or aging skin are tretinoin cream and carbon dioxide (CO2) and erbium (Er:YAG) lasers. Tretinoin cream (Renova), a vitamin A derivative available by prescription only, is approved for reducing the appearance of fine wrinkles, mottled darkened spots, and roughness in people whose skin doesn't improve with regular skin care and use of sun protection. However, it doesn't eliminate wrinkles, repair sun-damaged skin, or restore skin to its healthier, younger structure. It hasn't been studied in people 50 and older or in people with moderately or darkly pigmented skin. The CO2 and Er:YAG lasers are approved to treat wrinkles. The doctor uses the laser to remove skin one layer at a time. Laser therapy is performed under anesthesia in an outpatient surgical setting. The FDA currently is studying the safety of alpha hydroxy acids (AHAs), which are widely promoted to reduce wrinkles, spots, and other signs of aging, sun-damaged skin. Some studies suggest that they may work, but there is concern about adverse reactions and long-term effects of their use. Because people who use AHA products have greater sensitivity to the sun, the FDA advises consumers to protect themselves from sun exposure by using sunscreen, wearing a hat, or avoiding mid-day sun. Melasma presents as patchy brown pigmentation on sun exposed areas of the face. This condition is more commonly seen in darker skin types, and is more common in females. Though the condition may respond to treatment, it is usually recurrent. Causes of melasma The cause of melasma is multifactorial. Genetic predisposition, sun exposure, and hormonal influences all play a role in this condition. Patients of darker skin colour, including Asian, Hispanic, Middle Eastern and Mediterranean cultures are more prone to this condition. Melasma is more common in women and maybe associated with hormonal changes including the oral contraceptive pill, IVF treatment and pregnancy. Ultraviolet (UV) radiation (UVA and UVB) stimulates melanocytes (the pigment producing cells) causing them to produce melanin (pigment). Clinical Features Melasma presents as dark patches which may be light brown, tan or even dark brown/black depending on the depth of the pigment in the skin. Treatment Photoprotection is the cornerstone of management. Strict and absolute protection with a broad-spectrum sunscreen, covering both the UVB and UVA spectrum is essential. Sunscreens should be applied twice a day. Cream Treatments: these, together with sun protection form the basis of management. Hydroquinone HQ 2-8% as a cream of lotion either in a stand-alone formulation or mixed with other active ingredients is the most widely used method to treat melasma. The higher the concentration of HQ, the higher the incidence of irritant dermatitis. Application should always be titrated according to skin sensitivities. Vitamin A creams including tretinoin can be used as a stand-alone treatment or in combination with other agents. Application should be titrated according to skin irritation. Ascorbic acid or Vitamin C can be used as adjunctive treatment of melasma. It acts as an anti-oxidant, as well as a pigment-inhibiting compound. Azelaic acid 20% has been reported to improve epidermal cases of melasma. Combination topicals: Modified Kligman’s formulation consists of variations in concentration of Hydroquinone, retinoid, and a topical steroid and is considered the gold-standard for melasma management at present. Chemical peels: superficial peels are preferable to medium or deep peels form melasma given this condition affects those with darker skin. Glycolic acid peels are the most commonly used agent. Lactic acid peels may also be used. Low-strength tricholacetic acid peels Medium to deep chemical peels should not be used to treat melasma due to a high incidence of complications Laser and light based treatments: Only certain lasers should be used with conservative settings and are best performed by a dermatologist. Multiple treatments are necessary to see an appreciable benefit. A detailed discussion with your dermatologist about the appropriateness of laser therapy for your melasma is essential. 1064nm Q Switch Nd Yag laser, at low power settings is, to date, the most promising laser for melasma. Several treatments are required over many months to see improvements. Vascular laser may be used to treat the erythrotelengiectatic subtype of melasma Non ablative fractional laser is not commonly used, but has been reported in the literature Ablative Fractional lasers are only appropriate for those with light skin. Complications are high if used in those with dark skin. Intense Pulsed Light can be effective in some cases of epidermal melasma in lighter skin only. It should be noted that even if they are effective recurrence is usually seen. Tranexamic acid has been reported for treatment of melasma. Newer Q-Switched picosecond lasers may prove to be more helpful than the current nanosecond Q-Switched technology. Prognosis Epidermal melasma has a better prognosis than mixed or dermal melasma but all types are recurrent and require ongoing maintenance therapy. Melasma associated with pregnancy has the best prognosis, with pigment gradually fading over months. However, subsequent pregnancies often result in recurrences. For example, if you have: a large number of papules and pustules on your chest and back, as well as your face painful nodules A combination of antibiotic tablets and topical treatments is usually the first treatment option for severe acne. If this doesn't work, a medication called isotretinoin may be prescribed. Hormonal therapies or the combined oral contraceptive pill can also be effective in women who have acne. However, the progestogen-only pill or contraceptive implant can sometimes make acne worse. Many of these treatments can take two to three months before they start to work. It's important to be patient and persist with a recommended treatment, even if there's no immediate effect. Topical treatments (gels, creams and lotions) Benzoyl peroxide Benzoyl peroxide works as an antiseptic to reduce the number of bacteria on the surface of the skin. It also helps to reduce the number of whiteheads and blackheads, and has an anti-inflammatory effect. Benzoyl peroxide is usually available as a cream or gel. It's used either once or twice a day. It should be applied 20 minutes after washing to all of the parts of your face affected by acne. It should be used sparingly, as too much can irritate your skin. Apply to all the parts of your face affected by acne 20 minutes after washing your face. It's important to apply topical retinoids sparingly and avoid excessive exposure to sunlight and UV. Topical retinoids aren't suitable for use during pregnancy, as there's a risk they might cause birth defects. The most common side effects of topical retinoids are mild irritation and stinging of the skin. A six-week course is usually required, but you may be advised to continue using the medication less frequently after this. Topical antibiotics Topical antibiotics help kill the bacteria on the skin that can infect plugged hair follicles. They're available as a lotion or gel that is applied once or twice a day. A six- to eight-week course is usually recommended. After this, treatment is usually stopped, as there's a risk that the bacteria on your face could become resistant to the antibiotics. This could make your acne worse and cause additional infections. Side effects are uncommon, but can include: minor irritation of the skin redness and burning of the skin peeling of the skin Azelaic acid Azelaic acid is often used as an alternative treatment for acne if the side effects of benzoyl peroxide or topical retinoids are particularly irritating or painful. Azelaic acid works by getting rid of dead skin and killing bacteria. It's available as a cream or gel and is usually applied twice a day (or once a day if your skin is particularly sensitive). The medication doesn't make your skin sensitive to sunlight, so you don't have to avoid exposure to the sun. You'll usually need to use azelaic acid for a month before your acne improves.
