Education is a constant process for the liberation of human beings. Education can also empower people with chronic disorders such as diabetes. Empowering people with diabetes involves teaching them how to handle all of the daily decisions they face that have a direct impact on their quality of life. The constant swings in lifestyle that result from diabetes make an individual fragile in a demanding society. If people with diabetes have not been educated to handle these daily decisions correctly, they can become a target of discrimination. Health professionals frequently tell people with diabetes that they can lead a "normal," healthy life. Given education, this is true. Without education, however, this is, sadly, not the case. Do "normal" people have to visit a health center and miss school or work as often as do people with diabetes who have not received adequate education? It is no wonder that uneducated people with diabetes often feel incapable of functioning normally and blame diabetes, using their disease as an excuse for their actions. Their family members may also treat them differently, reinforcing the already ingrained idea that they are somehow less able. A person with diabetes who has not been educated relies on the constant attention of health care providers and thus becomes overly dependent on them. Health care professionals, often without stopping to consider the root of this dependency, may treat such individuals as incompetent or ignorant. People who have not received diabetes education are also more prone to developing chronic complications, for which they sometimes feel responsible. After all, the white coat behind the desk can seem like a judge, implicitly accusing them of not complying with their prescribed treatment. This heightens their sense of incompetence and undermines their self-confidence. The risk is that these people will identify totally with the role of the passive patient who requires constant assistance. Fear of discrimination means that some people with diabetes try to hide their condition. Indeed, it is much easier to hide it than to fight the ignorance and discrimination to which people with diabetes are subjected. Children are excluded from school sports activities because of their diabetes. Young people with diabetes may encounter difficulties finding spouses. Highly trained workers are denied positions for which they are wholly qualified, simply because of their diabetes. People with diabetes receive sympathetic looks and are expected to conform to other peoples' perceptions of what their behavior should be. However, hiding diabetes is not the solution. It only makes diabetes a taboo subject. Unless we speak up about diabetes, society will accept misconceptions, and discriminatory attitudes toward people with diabetes will continue. In today's world, people with diabetes have to learn to fight for their rights. Without proper information, it is impossible for them to understand their rights and to demand them. Education is not only an important tool for the day-to-day management of diabetes. It is also what enables people with diabetes to stand up against injustice and fear. It not only empowers people with diabetes, but also helps combat an ignorant society. Only education can crush ignorance. Unfortunately, many people with diabetes around the world do not have even basic diabetes education and training. Even in highly sophisticated and developed countries, education in diabetes often is still not covered by health care systems. In developing nations, there are almost no professional educators, and education is usually given in an ad hoc manner by well-meaning but unqualified individuals. We need to change this if we really want to enhance the lives of people with diabetes. We need recognized diabetes education programs and competent professional educators for individuals with diabetes throughout the world. We need to lobby governments to include education as an integral part of diabetes programs. All of us who are concerned about this have to fight together. Next year, on the 50th anniversary of the International Diabetes Federation (IDF), we have the opportunity to bring diabetes education into the new millennium. I am proud to announce that the 17th IDF Congress, to be held in Mexico City in November 2000, will host one entire track on education, nutrition, and the psychosocial aspects of diabetes. Video abstract of original research paper "Empowerment, motivation, and medical adherence (EMMA): the feasibility of a program for patient-centered consultations to support medication adherence and blood glucose control in adults with type 2 diabetes" published in the open access journal Patient Preference and Adherence by Varming AR, Hansen UM, Andrésdóttir G, et al. Purpose: To explore the feasibility of a research-based program for patient-centered consultations to improve medical adherence and blood glucose control in patients with type 2 diabetes. Patients and methods: The patient-centered empowerment, motivation, and medical adherence (EMMA) consultation program consisted of three individual consultations and one phone call with a single health care professional (HCP). Nineteen patients with type 2 diabetes completed the feasibility study. Feasibility was assessed by a questionnaire-based interview with patients 2 months after the final consultation and interviews with HCPs. Patient participation was measured by 10-second event coding based on digital recordings and observations of the consultations. Results: HCPs reported that EMMA supported patient-centered consultations by facilitating dialogue, reflection, and patient activity. Patients reported that they experienced valuable learning during the consultations, felt understood, and listened to and felt a trusting relationship with HCPs. Consultations became more person-specific, which helped patients and HCPs to discover inadequate diabetes self-management through shared decision-making. Compared with routine consultations, HCPs talked less and patients talked more. Seven of ten dialogue tools were used by all patients. It was difficult to complete the EMMA consultations within the scheduled time. Conclusion: The EMMA program was feasible, usable, and acceptable to patients and HCPs. The use of tools elicited patients’ perspectives and facilitated patient participation and shared decision-making. Known to friends and associates as Woody, Mr. Runner has been CEO of Kerlan-Jobe, LLC, since 2004 and is currently CEO of Kerlan-Jobe Orthopaedic Clinic. He is a principal of Cumberland Capital Corporation, a private equity investment firm that has directly or indirectly participated in transactions with aggregate values of more than $2 billion. In addition, Woody is a Director and Board Member of VALID Systems, an automated check cashing and verification solutions company serving top banks, financial services companies, and retailers. After earning a degree in business administration from West Virginia University, Woody began as a CPA at a national accounting firm. Other positions he has held include Director of Financial Services for AFG Industries, a flat glass manufacturing and distribution business; CEO for Surgery Code Solutions, a company providing coding tools for orthopedic surgeons, prior to its sale to MMX Holdings; investor representative in the start-up of Sovereign Healthcare, which provides management services to ambulatory surgery centers; and was a director of Cornerstone Assistance Network, a faith-based community services organization located in Texas. Accepting diabetes as a part of his life, Woody channels his energies into living a healthy lifestyle, not as a victim, but as the master of his disease. The recordkeeping enables him to track his progress, monitor, and modify diet, exercise, and insulin doses. Woody credits an a1C that is consistently in the 5.1-5.3 range to the awareness resulting from the process of tracking his condition. Equity and Empowerment Lens: A Tool to Create Equitable Policies and Programs

This webinar was brought to you by the NPA Federal Interagency Health Equity Team’s webinar series, Promoting Health Equity through Programs and Policies. It took place on July 21, 2016. Multnomah County, Oregon created The Equity and Empowerment Lens (with a racial justice focus) in 2012 to support the elimination of racial and ethnic disparities. The Lens is a quality improvement tool used to improve planning, decision-making and resource allocation leading to more racially equitable policies and programs. Essentially, the lens is a set of principles, reflective questions and processes that focuses at the individual, institutional and systemic levels by deconstructing factors that exacerbate health disparities; reconstructing and supporting factors that promote health equity; shifting the way we make decisions and think about this work; and healing and transforming our structures, our environments, and ourselves. This webinar will introduce practitioners interested in the lens to discuss the application and outcomes of this analysis in their work. Application of the equity lens ensures that policies impact community outcomes, and it creates the environment for candid dialogue about the root causes of inequities. The presenter will discuss the role and obligation of government to address not just social environments but also one’s own practices and approaches; scenarios/circumstances that initiate conversation about implementing the Equity and Empowerment Lens and ways in which the tool has been adapted in various settings to enhance organizational capacity and build accountability.
