This treatment is called RAI, Radioactive Iodine or I-131 and is used in my case to kill off any healthy or malicious thyroid cells left over from my thyroidectomy surgery. Which I had because my nodule decided to go and grow a mass of papillary cancer. I have created a playlist of my cancer update vlogs in an effort to increase awareness and support of this condition. I am not a doctor or any sort of cool medical professional. I simply was inspired to share my journey with the world so that anyone else out there who may be going through a similar situation can know they are not alone. But let me tell you, what a year it has been. Between the surgery, recovery and now the radio active iodine therapy, plus another surgery coming up - see my other update videos about that - I am sooo ready to leave all of this cancer mumbo jumbo behind. Well-differentiated thyroid cancer is one of the most common endocrine cancers, and is often treated using radioactive iodine. However, the risk of subsequent cancer development from radioactive iodine treatment has not been previously determined. Remco Molenaar, MD, PhD, from the University of Amsterdam, Amsterdam, Netherlands, discusses the use of this radioisotope, and explains the results of a recent study which suggested that this treatment could increase the risk of patients developing acute myeloid leukemia. This interview was recorded at the European Society for Medical Oncology (ESMO) 2017 conference in Madrid, Spain.

Your endocrinologist will examine your neck, perform an ultrasound of your entire neck and monitor your blood for your thyroid hormone levels and the marker for your thyroid cancer, thyroglobulin (for papillary, follicular or hurthle cell thyroid cancers). What your thyroid cancer surgeon found during your operation and the final pathologic review are the most important factors determining whether or not to have radioactive iodine as a targeted treatment for your thyroid cancer. Your relationship with the endocrinologist is a long-term one so he/she should be experienced in thyroid cancer and you should be comfortable with them.

In this video, Dr. Clayman discusses that an increase in thyroglobulin is just a number but it requires further evaluation. Thyroglobulin is a protein made by thyroid cells and your thyroglobulin level may fluctuate over time. Different laboratories may also have different results for thyroglobulin so comparison of the numbers may not always be reliable. If not all of your thyroid was removed during your thyroid cancer surgery, or you were not treated with radioactive iodine after the operation, there may be a little bit of thyroid tissue that is producing the thyroglobulin. Thus, having some thyroglobulin in your blood doesn’t always mean the cancer has returned. However, if your thyroglobulin number was zero after your thyroid cancer operation and has now increased, then a high-definition ultrasound of the neck should be obtained. Any abnormalities found on ultrasound can undergo needle biopsy. A CAT scan of the neck with contrast and CAT scan of the chest can also be considered to evaluate other areas that thyroid cancer can be detected that would not be detected on the ultrasound study. Causes of thyroid cancer that you were not aware of The exact cause of thyroid cancer is not known. Similar to how other cancer forms develop, changes in the DNA of cells is responsible for the development of thyroid cancer. These alterations could either be genetically inherited or occur over a period of time. But, it is well established that cancer is not contagious. 1 Thyroid cancer risk factors Exposure to radiation increases one's chances of getting thyroid cancer. A mere dental X-ray, however, does not increase the chances of developing thyroid cancer, but a radiation therapy to the head, neck or chest surely does. There has been significant amount of research to understand DNA changes that become cancerous. Most cancers are not inherited, which means that DNA gets damaged by the exposure to something in the environment such as radiation or smoke during the course of one's life. The cancer may also occur when unexpected changes take place inside a cell without a specific external cause. The following are the prominent risk factors for thyroid cancer: 2 Benign thyroid disease Thyroid conditions i.e., either underactive or overactive thyroid are not related to the risk of thyroid cancer in any way, however, non-cancerous conditions such as nodules (adenomas), goitre (an enlarged thyroid) and thyroiditis (inflammation of the thyroid) increase thyroid cancer risk. 3 Radiation and low iodine levels Thyroid cancer is more likely to develop in individuals who have had radiotherapy treatments at a tender age, though the cancer usually develops years later. Survivors of atomic explosions or accidents are also at an increased risk of developing thyroid cancer. Individuals with low levels of iodine in their body are at more risk of thyroid cancer after any exposure to radiation. 4 Family history of thyroid cancer Individuals with a family history of thyroid cancer are at a higher risk of developing thyroid cancer. 5 Bowel condition FAP Familial adenomatous polyposis (FAP), a condition caused by an inherited gene, increases the chances of developing thyroid cancer. 6 Breast condition history Women with non-cancerous breast conditions are at increased risk of thyroid cancer. 7 Cancer history Individuals who have been treated for breast cancer, oesophageal cancer, testicular cancer or non Hodgkin's lymphoma have high risk of developing thyroid cancer. In this thyroid cancer video, Dr. Clayman discusses the importance of choosing your surgeon and ultrasonographer to ensure your thyroid cancer is cured with your first surgery – and does not reappear years later. The importance of an experienced, knowledgeable thyroid cancer surgeon is key to curing you. As discussed in earlier videos in this thyroid cancer series, the knowledge of how thyroid cancer behaves and the observations of the thyroid cancer surgeon during surgery are critical to properly removing not just the thyroid, but potential involved lymph nodes that could contain cancer. Recurrence of thyroid cancer in the neck is usually not recurrence at all. Instead it is cancer that was present during the first operation that was not removed by the first surgeon and over time it grows and is detected. It is also possible that this “recurrent” thyroid cancer was present prior to the first cancer operation but was not detected by ultrasound or other scans. It is important to have an expert thyroid cancer evaluation done, which involves a high definition ultrasound and a CAT scan to determine the extent of the thyroid cancer for its complete removal. If this did not happen with your first operation you want the most experienced, expert surgeon for your next surgery. Do not make the same decisions again which brought you to have recurrent or persistent thyroid cancer.

It's important to realize, though, that the majority of thyroid cancer cases are caused by the least aggressive form of thyroid cancer. This, combined with the fact that the most common therapy to treat thyroid cancer is the removal of the thyroid gland has helped increase the 5-year survival rate of thyroid cancer to around 97 to 98%. When it comes to thyroid cancer you may not have a choice as to which therapy is ideal for you, but it still helps to understand each treatment and how it impacts your body. Treatment options available for thyroid cancer include: #1. Surgery: - By surgery, I am referring to a procedure known as a complete thyroidectomy. This surgery results in the entire removal of your thyroid gland (well close to the entire amount) and it is the most common treatment for thyroid cancer. Removing your thyroid, though, can sometimes cause side effects afterward such as weight gain or fatigue if you are not dosed appropriately. #2. Radioactive iodine ablation: - Radioactive iodine is sometimes combined with a thyroidectomy to help kill off any tissue which is left over after your surgery. Radioactive iodine is a procedure which specifically targets and destroys thyroid gland tissue. It's helpful to use in conjunction with surgery because sometimes thyroid gland tissue can exist outside of the normal anatomy. Using RAI can help kill off extra tissue especially if cancer has spread. #3. Chemotherapy and radiation: - Chemotherapy isn't used very often for thyroid cancer because most cancers can be treated with surgery and with RAI. But, some cases of aggressive thyroid cancer may require the use of chemotherapy and radiation therapy. #4. TSH Suppression: - Lastly, another technique used to treat thyroid cancer is the use of TSH suppressive therapy. The goal here is to take high enough doses of thyroid hormone to suppress the natural production of TSH or thyroid stimulating hormone. This hormone is stimulating to thyroid gland tissue and can theoretically stimulate thyroid cancer cells as well. You can suppress the normal production of TSH by taking high doses of thyroid medication and this is sometimes used for patients after surgery. Each person is different so the exact treatments that you need for your cancer will depend on the type of cancer you have, how aggressive it is, the stage at which it was diagnosed, your age and other important factors. Thyroid Cancer is different in different people. In prior videos, Dr. Clayman discussed advantages and disadvantages of removal of part of the thyroid gland, called partial thyroidectomy or thyroid lobectomy. In this video, Dr. Clayman discusses the advantages of removal of all of the thyroid gland in thyroid cancer surgery. For the most common thyroid cancers, surgery of the thyroid which removes all of the thyroid gland called total thyroidectomy provides the ability to treat thyroid cancer with radioactive iodine and the ability to watch the blood of thyroid cancer patients for the most common blood markers for thyroid cancer, called thyroglobulin (for papillary, follicular and hurthle cell cancers) and calcitonin and CEA (for medullary thyroid cancer). Total thyroidectomy may be the best surgery for thyroid tumors or thyroid cancers if there are abnormalities in both sides (lobes) of the thyroid gland or if you have a thyroid cancer that has spread to lymph nodes in the neck or the thyroid cancer has broken out of the capsule (or covering) of the thyroid gland. In this video, Dr. Clayman will discuss that total thyroidectomy may be the preferred thyroid surgery for thyroid cancers which have spread to neck lymph nodes, grown outside of the thyroid, or have spread elsewhere in the body as well as when thyroid cancers involve both sides of the thyroid gland and no significant normal thyroid tissue could or should be left in the neck.

You should not lose your voice after thyroid surgery. In this video, Dr. Clayman explains that a vocal cord nerve can become temporarily paralyzed uncommonly and you may experience hoarseness, but you should not lose your voice. If a person does lose their voice as a result of thyroid surgery, it may be temporary, or it may be due to inadvertent injury to a vocal cord nerve that is known or not known to the surgeon. You should ask your surgeon how frequently they perform thyroid surgery and how often a patient loses their voice. An excellent surgeon will tell you the risk is near zero, or way less than one percent. If the surgeon says 2% or more, then you may want to discuss this with another surgeon. The surgeons with the most experience have the lowest complications rate. Nerve monitoring does not prevent injury to a vocal cord nerve and in fact it gives a surgeon a false sense of security because they think they are protected, but what prevents damage to a vocal cord nerve is your surgeon seeing the nerve and preserving the entire nerve and all of its branches. A surgeon should be able to tell you that the risk of injury to your vocal cord nerves is less than one percent, if not, it’s not the right thyroid surgeon for you. If he/she says they are going to monitor your nerve in the operating room to help prevent a nerve injury, then know this is absolutely not true and often increases the risk of nerve injury. These tubes are used to protect the surgeon from lawsuits, not the patient from injury.

Thyroid nodules are discrete lesions within the thyroid that can be thought of as a mass or a lump. They can represent many different things, as opposed to just thyroid enlargement, which is sometimes called a goiter. Thyroid nodules are more common in older individuals and women are more likely to develop thyroid nodules than men. What are the risk factors for developing thyroid nodules? There are a number of things that may increase the risk of having a thyroid nodule, one of which is iodine deficiency, although that’s rare in the United States. People who have had radiation exposure to the area in the neck where the thyroid is located, particularly if that happened during childhood, are more likely to have thyroid nodules. Also, people with an underlying autoimmune thyroid disease, which is sometimes called Hashimoto’s, or lymphocytic thyroiditis, may develop thyroid nodules. What are the symptoms of thyroid nodules? Most thyroid nodules are asymptomatic and they're often found incidentally on different kinds of testing but a number of them can be either felt by a physician as a small firm lump where the thyroid is located in the neck. How are thyroid nodules evaluated? Almost all thyroid nodules are benign. The primary way to evaluate a thyroid nodule without removing it surgically is what's called a fine-needle aspiration biopsy. In this procedure a very small needle is put into the neck where the thyroid nodule is located and cells are collected for evaluation. It's a short well-tolerated procedure that's usually done under local anesthetic. Patients are able to go home afterwards with very few issues. When is surgery needed? Surgery can be required for the treatment of thyroid nodules in a variety of situations. If the thyroid nodule is found to be a malignancy, typically a thyroid cancer, the standard treatment at this point is a surgery to remove that nodule along with the thyroid in its entirety. There are other cases in which surgery will be necessary. Some nodules, even after biopsy can't be definitively described as benign or malignant. In this case the nodule may need to be removed surgically to determine what it is. There are also cases in which benign thyroid nodules need to be removed surgically because they are compressing in a place that's causing symptoms that aren't well tolerated. Weight loss after thyroidectomy can be almost impossible unless you know exactly what to look for and how to treat it. It's considered "normal" to gain 20 pounds after thyroidectomy and Doctors tell patients that this is normal and in the same breath then say that the thyroid isn't causing the weight gain. How can this be? It is possible to lose weight after thyroidectomy but it takes a very different approach. Eating less and exercising more will NOT lead to significant weight loss if you've had a thyroidectomy or your thyroid ablated with radioactive iodine. Instead you need to be focusing on hormone imbalances that lead to weight gain. I've also included a diet guide to follow in addition to the following 5 tips which I explain in detail in the video. 5 Tips for weight loss after thyroidectomy: 1. Get on the right type and dose of thyroid medication - for most patients this means a combination of T3 and T4. So medications like cytomel or liothyronine in addition to levothyroxine/synthroid or the use of natural desiccated thyroid (NDT) will most likely be necessary. 2. Increase your free T3 - you need to be doing whatever is in your power to naturally increase your free T3. The more t3 you can in your system the higher your metabolism will be and the more weight you will lose. 3. Balance all other hormones in the body - I list several hormones, but the main ones I want you to focus on are insulin resistance and leptin resistance. Until these 2 are treated it will be almost impossible for you to lose weight. 4. Be strict with your diet but don't restrict calories - one of the worst things you can do is restrict your calories down to 1,000-1,200 calories per day. This will cause high reverse T3 levels and further reduce your metabolism. Instead be strict with WHAT you put in your mouth but don't restrict your calories. 5. When you have energy make sure you make exercise count - that means a combination of low intensity daily plus high intensity 1-3x per week. Here I am about to take the first-very low-dosage of Radioactice Iodine for my Thyroid Cancer treatment 5/22/09. I have been off of my Cytomel since 5/9 and my face has gotten swollen, my voice is very groggy and I feel like a 500lb., drunk, smoker. Being silly and finding humor during this treatment really helped me get rid of my fears. My husband and my mom were there to support me. Thyroid cancer occasionally occurs in pregnant women. This poses a specific challenge for the management of this thyroid cancer. OB/GYN doctors are very good at examining young women’s necks and frequently find lumps in the thyroid gland (thyroid nodules). They often diagnose of papillary thyroid cancer in their women patients and thus thyroid cancer can be diagnosed during a pregnancy. What is important is that these are young, healthy women and in the vast majority of cases have a very curable thyroid cancer. In Dr. Clayman’s practice, he has never performed thyroid surgery on a pregnant woman. The young woman will have an ultrasound when she is diagnosed and have strict measurements taken which will be monitored with ultrasound throughout the pregnancy. Dr. Clayman tells the pregnant young woman with thyroid cancer to enjoy her pregnancy, to eat well, and to breast feed. Once she has fulfilled her commitment to breastfeed her child, then they will proceed with surgery. Many people ask can thyroid grow back after thyroidectomy, the answer is ye and no. If you do a partial thyroidectomy or a hemi thyroidectomy there could be compensatory hypo-trophy of the reveling gland or growth of the remnant thyroid a part of the thyroid gland which is remained behind. But if you are looking at total thyroidectomy if the thyroid gland has been entirely removed the chances of thyroid gland actually growing back are very less. So unless there is been error during the surgery unlikely there will be a recurrence in the thyroid gland growth.

The thyroid gland is found in the neck, below the thyroid cartilage(which forms the Adam's apple). The thyroid gland controls how quickly the body uses energy, makes proteins, and controls how sensitive the body is to other hormones. It participates in these processes by producing thyroid hormones, the principal ones being triiodothyronine (T3) and thyroxine (sometimes referred to as tetraiodothyronine (T4)). These hormones regulate the growth and rate of function of many other systems in the body. T3 and T4 are synthesized from iodine and tyrosine. The thyroid also produces calcitonin, which plays a role in calcium homeostasis. People can suffer from hyperthyroidism. An overactive thyroid, is defined as an overproduction of the thyroid hormones T3and T4. This condition is most commonly caused by the development of Graves' disease, an autoimmune disease in which anomalous antibodies stimulate the thyroid to secrete excessive quantities of thyroid hormones.The disease can progress to the formation of a toxic goiter as a result of thyroid growth in response to a lack of negative feedback mechanisms. It presents with symptoms such as a thyroid goiter, protruding eyes (exophthalmo), palpitations, excessive sweating, diarrhea, weight loss, muscle weakness and unusual sensitivity to heat. The appetite is often increased. People can also suffer hypothyroidism. Hypothyroidism is the underproduction of the thyroid hormones T3 and T4. Hypothyroid disorders may occur as a result of # Congenital thyroid abnormalities # Autoimmune disorders such as Hashimoto's thyroiditis, # Iodine deficiency # The removal of the thyroid following surgery to treat severe hyperthyroidism and/or thyroid cancer. Typical symptoms are abnormal weight gain, tiredness, baldness, cold intolerance, and bradycardia. Hypothyroidism is treated with hormone replacement therapy, such as levothyroxine, which is typically required for the rest of the patient's life.
