How do our bodies balance the storage and burning of dietary fat? Fat carries information about how it should be used. Saturated fats are hard to break down, so they tend to get stored, while unsaturated fats are more readily consumed for energy. Too much stored fat leads to elevated blood glucose levels, which triggers insulin resistance -- the first step toward diabetes. Dr. Evans explores how diet and exercise influence the relationship between fat and muscle, precipitating diseases such as syndrome X, a disorder involving high blood pressure, heart disease, atherosclerosis, and insulin resistance.

You are checking your blood sugar levels and they are within target range. Your A1C is less than 7%. You are scheduled to see your healthcare provider for your routine visit. You're feeling good. "Everything looks good, it's just that your blood pressure has risen over the last few months", you're healthcare provider reports, "We are going to have to start you on blood pressure medications." You're bewildered. How can this be? Everything seemed to be going well. Why have you just been diagnosed with hypertension? Is there a link between diabetes and hypertension? The short answer to this question for people living with type 2 diabetes is YES. The link is explained by what clinicians call metabolic syndrome. I know that this word may mean very little to you. But this information is very important for anyone living with type 2 diabetes. I am going to explain what this means in as simple terms as possible. It is my intention that the knowledge is presented in an easy to understand format. That way you are better able to take action. It is only by taking action that you can live powerfully with diabetes. So let's dive right back into what is metabolic syndrome and how it links diabetes and hypertension. What is metabolic syndrome? Metabolic syndrome is a group of characteristics. It is not really medical illness. The important thing is that these characteristics put you at an increased risk for developing certain diseases. These diseases include type 2 diabetes, hypertension and heart disease. Metabolic syndrome is also called metabolic X syndrome, syndrome X and also insulin resistance syndrome. Most clinical experts define metabolic syndrome as having three or more of any of the following: Obesity in the abdomen. Greater than 32 inches in women and 38 inches in men. High fasting blood sugar levels more than 100-125 mg/dl A high blood pressure of more than 130/85 mm hg or if you are taking blood pressure medication. A high triglyceride level of more than 150 mg/dl or a low HDL level of less than 40 mg/dl. The prevalence of Metabolic Syndrome. More than 2/3rd of the adult American population is either overweight or obese. The number of people with metabolic syndrome has risen just as the obesity epidemic in this country. Currently that rate is at 34% of the adult population. The following factors put you at risk for developing metabolic syndrome: Being overweight. In other words a BMI of more than 25. Smoking. Eating a diet that is high in carbohydrates. Smoking. Lack of exercise. Menopause. Family history of diabetes or metabolic syndrome. Increasing age. What health risks are associated with Metabolic Syndrome? People with metabolic syndrome are more likely to develop type 2 diabetes. They are also at risk for developing cardiovascular disease. Cardiovascular disease consists of several diseases. I like to compare the cardiovascular system to the plumbing system in your house. Your symptoms depend on what part of the arteries gets clogged up. Cholesterol gets deposited in the arteries as fatty plaques. When the arteries in the heart get clogged up with plaque, this is called coronary artery disease. If the plaque collects in the blood vessels in the brain, it is called cerebrovascular disease and can cause a stroke.

Responsible scientists began to ask questions about what causes it. The discoveries they are now making are nothing short of revolutionary. They not only offer us the power to slow the rate at which our bodies age. They are experimenting with protocols that can, potentially reverse negative conditions from hypertension, insulin resistance to cholesterol imbalances and depression, simply by changing the way you eat and live. This experience is one I became fascinated with when, several years ago, I made a television documentary in the Southern Hemisphere called To Age or Not To Age. It asked the question, ?Can we slow aging by making simple nutritional changes?? We also wanted to know ?Might it also be possible to reverse age-related degeneration that has already occurred?? And, finally, ?Can these things be verified in medically measurable ways?? We decided to carry out what is known as a base-line study to find out. We sent all participants to have the standard test of medical parameters checked before the project began?from fasting insulin and blood sugar levels through cholesterol, triglycerides and blood pressure. These were carried out at a hospital which also had an excellent physiology laboratory, where exercise physiologists were able to establish each participant?s VO2Max, body fat percentage and lean body mass to fat ratio. To my surprise, every participant?including a 50 year old top athlete?showed abnormalities associated with Syndrome X. We then introduced our group to an Insulin Balance way of eating, which I devised. The experiment lasted only 5 weeks. Then we sent all our participants back to the hospital to have their parameters checked again. The results were mind-blowing. We had no idea that, after only 5 weeks of dietary changes, every abnormal medical parameter had normalized, except for cholesterol measurements in one woman, which took another few weeks to normalize. For me personally, the experiment confirmed yet again that the human body has a phenomenal capacity to heal itself, when given the opportunity to experience transformation at the deepest levels. This is a truth which, for the past four years, I have applied to helping to transform the lives of men and women all over the world, whom I have been mentoring on Cura Romana. Dr. Ebraheim’s educational animated video describes the condition of thoracic outlet syndrome. Thoracic outlet syndrome is a diagnosis of exclusion based on the patient history and symptoms. This can cause pain in the shoulders, neck and numbness in the fingers as the arm is moved. The most common causes: • Structures may be compressed in the thoracic outlet by the cervical rib • Anomalies of the scalenus muscles It is a neurovascular compression neuropathy of the brachial plexus in the thoracic outlet in the retroclavicular region with either a neurogenic or vascular etiology. The thoracic outlet space is created by: •Clavicle •First rib •Subclavius muscle •Costoclavicular ligament •Anterior scalene muscle This space also contains the subclavian vessels and the thoracic duct. It also contains the lower trunk of the brachial plexus (C8, T1). Two types: 1.Neurogenic 2.Vascular The neurogenic type is caused by compression of the neurovascular bundle as it passes over the first rib or through the scalene muscle. Causes of compression: •Cervical rib •Elongated vertebral transverse process (C7) •Anomalies of the scalene muscle insertions •First rib malunion •Abnormal fibrous band on or near the two scalenous muscles •Repetitive shoulder movement •Extreme arm positions •Abnormal pectoralis minor muscle •Weight lifting •Rowing •Swimming Vascular Entity: Caused by a compressed subclavian vessel or by an aneurysm. Where is the compression site? •Where the brachial plexus passes over the first rib. oUsually at site of scalene triangle oThe brachial plexus and subclavian artery pass through the triangle (subclavian vein does not pass through the triangle) •Under the clavicle by the subclavius tendon •Underneath the conjoined tendon inserting into the coracoid process Diagnosis: •Symptoms are usually vague •Pain in the shoulder and neck that usually radiates to the forearm and hand (paresthesia radiating along the arm) •Loss of sensation of the little and ring fingers •May be some vascular symptoms such as arterial ischemia, venous congestion, Raynaud’s phenomenon (changing colors of the hands or chronically reduced pulse) •Look for ulnar nerve sensory changes and intrinsic weakness •Look to see if the patient has intolerance to cold (Raynaud’s phenomenon) Differential Diagnosis: •C8 radiculopathy or ulnar nerve compression at the elbow oCombination of weakness involving the median and ulnar nerve innervated muscles may confirm a more proximal injury to the brachial plexus •Rule out double crush syndrome with carpal tunnel syndrome and thoracic outlet syndrome Compression of the medial antebrachial cutaneous nerve could occur with compression of the thoracic outlet. Provocative Tests: have a high rate of false positives and are of limited clinical value if used alone. •Adson’s Test oMost commonly used test oAbduct, extend and externally rotate the arm while feeling the radial pulse oRotate the head towards the tested arm and may also extend the neck oDecreased interscalene space by tensing of the middle and anterior scalenus muscles oThis test is positive if the pulse disappears with reproduction of the symptoms oRadial pulse obstruction is not specific •Wright Test oAbduction, external rotation of the arm with the neck rotated away that will lead to the loss of pulses and reproduction of symptoms •Roos Test (Elevated Arm Stress Test” or “EAST”) oElevated arm stress test oRaise both arms up and hold this position for one minute oOpen and close the fingers for three minutes while holding them overhead oTest is positive if there is reproduction of pain and numbness of the shoulders as well as fatigue Imaging •Cervical spine may show a cervical rib •Chest x-ray may show a Pancoast tumor (apical lung tumor) that could put pressure on the brachial plexus causing ulnar nerve symptoms EMG and Nerve Studies •Results are usually not very helpful Vascular Studies •May identify a vascular form or thoracic outlet syndrome Treatment •Physical therapy oStrengthen the shoulder girdle muscles oThis is usually the first form of treatment •Maintain proper posture •Activity modification •Correction of postural imbalances is needed •Surgery oDecompression is indicated in cases of intractable pain, neurological deficit, or persistent vascular insufficiency in addition to failure of nonoperative treatment oResection of the first rib or cervical rib if present oRelease or excise the anterior and middle scalene muscles oExcision of any abnormal structures oSurgery can be done through a transaxillary or supraclavicular approach Become a friend on facebook: http://www.facebook.com/drebraheim Follow me on twitter: https://twitter.com/#!/DrEbraheim_UTMC Donate to the University of Toledo Foundation Department of Orthopaedic Surgery Endowed Chair Fund: https://www.utfoundation.org/foundation/home/Give_Online.aspx?sig=29

11 Effects of Rheumatoid Arthritis on the Body Overview Rheumatoid arthritis (RA) is more than just joint pain. This chronic inflammatory autoimmune disease causes your body to mistakenly attack healthy joints and leads to widespread inflammation. While RA is notorious for causing joint pain and inflammation, it can also cause other symptoms throughout the body. Read on to learn more about the possible symptoms of RA and its overall effects on the body. The effects of rheumatoid arthritis on the body RA is a progressive autoimmune disease that mainly affects your joints. According to Arthritis Foundation, about 1.5 million U.S. people live with RA. Anyone can get RA, but it generally begins between the ages of 30 and 60. It also tends to affect women nearly three times more than men. The exact cause of RA is unknown, but genetics, infections, or hormonal changes may play a role. Disease-modifying medications can help slow the progression of RA. Other medications, combined with lifestyle changes, can help manage the effects and in turn improve your overall quality of life. Skeletal system One of the first signs of RA is inflammation of the smaller joints in the hands and feet. Most of the time, symptoms affect both sides of the body at once. Common symptoms include pain, swelling, tenderness, and stiffness, which is more pronounced in the morning. Morning RA pain can last for 30 minutes or longer. RA can also cause tingling or burning sensations in the joints. Symptoms can come and go in “flares” followed by a period of remission, but the initial stages can last at least six weeks. Symptoms of RA can occur in any of the body’s joints, including your: fingers wrists shoulders elbows hips knees ankles toes RA can also result in: bunions claw toes hammer toes As the disease progresses, cartilage and bone are damaged and destroyed. Eventually, supporting tendons, ligaments, and muscles weaken. This can lead to a limited range of motion or difficulty moving the joints properly. In the long term, joints can become deformed. Having RA also puts you at greater risk of developing osteoporosis, a weakening of the bones. This in turn can increase your risk of bone fractures and breaks. Chronic inflammation of the wrists can lead to carpal tunnel syndrome, making it difficult to use your wrists and hands. Weakened or damaged bones in the neck or cervical spine can cause chronic pain. Your doctor may order X-rays to investigate the extent of joint and bone damage from RA. Circulatory system RA can affect the system responsible for making and transporting blood throughout your body, too. A simple blood test can reveal the presence of an antibody called the rheumatoid factor. Not all people with the antibody develop RA, but it’s one of many clues doctors use to diagnose this condition. RA increases your risk for anemia. This is due to a decreased production of red blood cells. You may also have a higher risk of blocked or hardened arteries. In rare cases, RA can lead to inflammation of the sac around the heart (pericarditis), the heart muscle (myocarditis), or even congestive heart failure. A rare but serious complication of RA is inflammation of the blood vessels (rheumatoid vasculitis, or RA rash). Inflamed blood vessels weaken and expand or narrow, interfering with blood flow. This can lead to problems with the nerves, skin, heart, and brain. Skin, eyes, and mouth Rheumatoid nodules are hard lumps caused by inflammation that appear under the skin, usually near joints. They can be bothersome, but usually aren’t painful. As many as 4 million U.S. people have an inflammatory disease called Sjogren’s syndrome, according to the Sjogren’s Syndrome Foundation. About half of these individuals also have RA or a similar autoimmune disease. When the two diseases are present, it’s called secondary Sjogren’s syndrome. Sjogren’s causes severe dryness — especially of the eyes. You may notice a burning or gritty feeling. Prolonged dry eyes increases the risk of eye infection or corneal damage. Though it’s rare, RA can also cause inflammation of the eye. Sjogren’s can also cause a dry mouth and throat, making it difficult to eat or swallow, especially dry foods. Chronic dry mouth can lead to: tooth decay gingivitis oral infections You may also experience swollen glands in the face and neck, dry nasal passages, and dry skin. Women may also feel vaginal dryness. Respiratory system RA increases the risk of inflammation or scarring of the linings of the lungs (pleurisy) and damage to lung tissue (rheumatoid lung). Other problems include: blocked airways (bronchiolitis obliterans) fluid in the chest (pleural effusion) high blood pressure in the lungs (pulmonary hypertension) scarring of the lungs (pulmonary fibrosis) rheumatoid nodules on the lungs
