Risk assessment with an older man after a suicide attempt

John is a 62 year old man who has been admitted to a mental health unit after taking an overdose of paracetamol in an attempt to kill himself. He is being assessed by a psychiatrist, who is exploring John's suicide attempt as well as whether or not he is depressed. Watching the film, try and pick out the clinical risk indicators that highlight John's degree of ongoing risk. You might also find it useful to think of what it is in John's experience that helps the psychiatrist determine if he is depressed. The psychiatrist uses a particular interviewing style but there are examples here of how to 'drill down' into a subject e.g. John's sleeping pattern, and ask difficult questions. Suicide is the 10th leading cause of death in the United States and the most frequent crisis encountered by mental health professionals. This home study reviews basic information about the assessment, management, and treatment of patients at risk to die from suicide. It fulfills Act 74 requirements for Pennsylvania licensed psychologists, social workers, marriage and family therapists, and professional counselors. Disclaimer As an educational program, this podcast/video does not purport to provide clinical or legal advice on any particular patient. Listeners or viewers with concerns about the assessment, management, or treatment of any patient are urged to seek clinical or legal advice. Also, individual psychotherapists need to use their clinical judgment with their patients and incorporate procedures or techniques not covered in this podcast/video, or modify or omit certain recommendations herein because of the unique needs of their patients. This one-hour video/podcast provides a basic introduction to the assessment, management, and treatment of patients at risk to die from a suicide attempt. This podcast/video may be a useful refresher course for experienced clinicians. However, listeners/viewers should not assume that the completion of this course will, in and of itself, make them qualified to assess or treat individuals who are at risk to die from suicide. For those who do not have formal training in suicide, this podcast/video should be seen as providing an introduction or exposure to the professional literature on this topic. In addition, the Pennsylvania Psychological Association will be offering a three (3) hour expanded home study on suicide and more specialized home studies dealing with suicide in children and adolescents, and suicide with older adults. Proficiency in dealing with suicidal patients, like proficiency in other areas of professional practice, is best achieved through an organized sequence of study including mastery of a basic foundation of knowledge and attitudes, and supervision. It is impossible to give a fixed number of hours of continuing education and supervision that professionals need to have before they can be considered proficient in assessing, managing, and treating suicidal patients. Much depends on their existing knowledge base and overall level of clinical skill. It would be indicated to look at competency standards from noted authorities, such as those developed by the American Association of Suicidology ( http://www.sprc.org/training-events/amsr), by David Rudd and his associates (Rudd et al., 2008), or Cramer et al. She is continuing to see Peter on his own while her colleagues are seen here working with Peter and his family, trying to help them understand what has happened to Peter, as well as endeavouring to help him keep himself safe. The clinicians use a number of specific family work techniques to help Peter talk with his parents and them to talk to him, to help them all manage their distress and anxiety safely as the sadness and desperation of their plight emerges. The session presents suicide risk assessment methods consistent with the VA/DoD Clinical Practice Guidelines for the Assessment and Management of Suicide Risk, as well as rationale and tips for augmenting the clinical interview with objective assessment measures. The presenter highlights the importance of stratifying suicide risk with respect to both severity and temporality and shares recommendations for documentation. Finally, the presentation offers an overview of safety planning as an evidence-informed risk management intervention. Risk assessment is a process used by professionals in mental health and healthcare roles to help determine a person's risk of harm to themselves, others or their risk of being exploited by another person. In this overview, Dr. Cheryl King provides an overview of the risk and protective factors for suicide. She also describes recommended assessment procedures and measures for suicide assessment. Financial disclosures: none. Cheryl King, Ph.D. is Director of the Youth Depression and Suicide Prevention Program in the Department of Psychiatry at the University of Michigan. An active clinical educator, researcher, and public policy advocate, Dr. King is the recipient of multiple federal and private foundation grant awards for her clinical research pertaining to adolescent and young adult suicide prevention, and has published widely in this area. In addition to developing a social network intervention for acutely suicidal adolescents and a screening intervention for adolescents treated in emergency medical settings, Dr. King is working with her colleagues to develop an online screening and treatment linkage intervention for college students at elevated risk for suicide. She is also implementing a large-scale trial to evaluate the effectiveness of a community-based intervention for adolescents who are at elevated risk due to bullying or bullying victimization. Dr. King is on the Scientific Advisory Council of the American Foundation for Suicide Prevention. She is also a Past President of the American Association of Suicidology, the Association of Psychologists in Academic Health Centers and the Society for Clinical Child and Adolescent Psychology.
