(3BL Media / theCSRfeed) Atlanta, GA -- December 21, 2011 -- The American Cancer Society has revised its guideline formation process to achieve greater transparency, consistency, and rigor in creating guidance about cancer screening. The new methods align with new principles from the Institutes of Medicine (IOM) by creating a single generalist group for writing the guidelines, commissioning systematic evidence reviews, and clearly articulating the benefits, limitations, and harms associated with cancer screening tests. The new process is outlined in a Special Communication in the December 14, 2011 issue of the Journal of the American Medical Association. The new process stresses transparency, with activities and timelines posted publicly on the American Cancer Society web site. To align with the new IOM standards, the ACS process will separate expert input from the writing of the guideline, so an independent writing group will receive appropriate subspecialty expertise while protecting it from the appearance of professional conflict of interest. The process will incorporate a systematic evidence review that will use methods consistent with IOM standards, and the guidelines group will grade the strength of its recommendations. The guidelines will explicitly describe potential benefits and harms of screening and will articulate any differences between its recommendations and those of other groups and the reasons for those differences. The new process will conclude with a formal review that will include opportunities for experts and professional organizations to comment on draft guidelines. Finally, the guideline process itself will be reviewed periodically by an independent advisory group to assure clarity, utility, and influence of the guidelines. There will be a formal review and rewriting of every ACS cancer screening guideline at least every five years. "Historically, the ACS has convened ad hoc screening guideline groups to come up with its recommendations for methods of cancer screening," said Tim Byers, M.D., MPH, of the Colorado School of Public Health. Dr Byers headed the Guidelines Process Workgroup convened by the ACS Board of Directors in 2010 and co-authored the new report. "Although that approach has resulted in highly credible and useful guidelines, we saw that the ACS process could be improved in terms of consistency, transparency, scientific rigor, and communications. This new process should ensure that ACS will remain the national leader in creating and communicating trustworthy information to guide clinical practice, personal choices, and public policy about cancer screening." Cancer specialists have discovered a new way to activate the body's immune system. Immunotherapy may finally be the weapon needed to conquer one of medicines' fiercest foes. Doctors diagnosed sarcoma in Sudhakar Iyer it is a rare and deadly soft-tissue cancer. He endured debilitating rounds of standard cancer treatment known as chemotherapy. It put his cancer in remission but left him weak. Then the cancer returned. This time it spread to his lungs. Iyer qualified to take part in a clinical trial run by Dr. Gary Schwartz, chief of hematology and oncology at New York Presbyterian/Columbia University Medical Center. He used a promising new treatment known as immunotherapy. Administered by an IV, the patient takes a drug that activates normally dormant T-cells. A T-cell is a type of white blood cell essential to the immune system. In just a few months, immunotherapy seemed to be working for Iyer. The tumor on his lung went away. Dr. Schwartz said doctors are seeing response rates that are extraordinary and unparalleled survivals. In 2016, more than 1.5 million people in the United States will be diagnosed with some form of cancer and nearly 600,000 people will die of it, according to the American Cancer Society. During immunotherapy, doctors administer drugs that will actually boost your immune system, training your own body to fight the cancer. Intraoperative Radiation Therapy is an evolving technique of radiation therapy for women having a lumpectomy for favorable, early stage breast cancer. “Whole Breast Radiation” is still considered the standard of care. IORT is slowly becoming more available at hospitals across the country. It is appropriate for only a small proportion of patients with small breast cancers. Also known as a type of Accelerated Partial Breast Irradiation (APBI), IORT is a technique of delivering a one-time dose of radiation to the lumpectomy area during surgery versus whole breast radiation – a treatment that takes usually four to six weeks. IORT is still an investigational therapy and not yet incorporated into national breast cancer guidelines. When a lumpectomy is performed for breast cancer, radiation is usually recommended to reduce the chance of cancer re-growing in the lumpectomy area of the breast. IORT applies a single dose of radiation to the operative lumpectomy site while under anesthesia in the operating room. Only women who meet the strict criteria for IORT should consider this mode of radiation. What are the general criteria to have IORT? This therapy may be an option for women older than 45, a “favorable” tumor smaller than 2.5 cm, and no evidence of cancer in the lymph nodes. There are other, specific criteria that can exclude eligibility. The decision to consider IORT must be made with your breast surgeon and radiation oncologist before surgery. It takes a great deal of coordination with your surgeon, radiation oncologist, and hospital to be prepared to perform IORT during your lumpectomy surgery. Most hospital facilities currently do not offer this form of breast cancer radiation. What are the Advantages of IORT? The most attractive aspect of IORT is completing a surgical lumpectomy, sentinel node biopsy, and radiation therapy in one day. If successful, you can avoid the 4 to 6 weeks of daily trips to your radiation center to receive whole breast radiation. Another advantage is that IORT applies the radiation to only the site of the surgery, where cancer cells could be unknowingly left behind and grow again in the future. According to the American Cancer Society, over 1,600 people in the United States die from cancer every day, many of whom were receiving the current standard of care, which is radiation, chemotherapy, or a combination of both. Thomas Seyfried is a professor in the biology department at Boston College and strong proponent of using metabolic therapy as a form of treatment for every type of cancer, arguing that it’s more effective and less toxic than radiation and chemotherapy. So, what exactly does metabolic therapy entail? It’s based on a simple concept: growth requires fuel, so in order to stop the growth of cancer cells, their sources of fuel--namely glucose and glutamine--must be reduced or eliminated. Reducing the availability of glucose and glutamine results in an increase in ketones, which can be effectively used by the body’s normal cells but not by cancer cells. Seyfried explains how the primary difference between cancer cells and normal cells makes this an effective form of treatment for cancer, discusses how to achieve a state of therapeutic ketosis, and describes what to do once a state of therapeutic ketosis has been achieved. 38.4% of people in the U.S. will be diagnosed with cancer at some point during their lifetimes (Cancer.gov, May 2018), and nearly 66% of U.S. cancer patients will receive radiation therapy (American Society for Radiation Oncology, 2015 Legislative Priorities Brief). External Beam Radiation Therapy (EBRT) is one of the most frequently used radiation treatments, yet the process is opaque and complex. Providing understandable educational material to help anxious patients and caregivers navigate the process is vital; current patient education material is voluminous and overwhelming. Typically written at a 12th grade reading level, with occasional stock photographs, explanation of the treatment is handed to patients in their first meeting with their doctor, a point of high emotional stress. In a project funded by UChicago Medicine’s Bucksbaum Institute for Clinical Excellence, an IIT Institute of Design (ID) student team partnered with Dr. Daniel Golden of UChicago Medicine to explore how visual narratives could better educate patients about radiation therapy. • By surveying existing visual narrative styles, along with theoretical teachings from Scott McCloud, a renowned comic theorist, the team explored visual narratives, more popularly known as ‘graphic novels’ or ‘comics’, as legitimate emerging media for serious topics, in healthcare, mental illness, death, abortion, abuse, and cancer survivorship. The class learned that graphic storytelling is its own visual language that can be leveraged to display concepts such as time passage, transitions, simultaneity and projection. • They also researched how, as healthcare delivery adopts patient-focused care, principles for public-facing communication have emerged. However, many guidelines are focused on primarily text-based materials. • The team interviewed 26 stakeholders: patients/caregivers and medical and non-medical staff. They analyzed audio files and transcripts, extracted insights, distilled them into design principles, ultimately creating a goal-oriented framework that streamlined the process and ensured that decisions for content representation were sound and principle-driven. Research yielded design principles reflected in the design of a multicolor illustration-based discussion guide prototype. In easy-to-follow framing using 5th grade reading level language and simple graphic narrative-style illustrations, the team’s prototype distills what had been dozens of loose pages into one two-sided takeaway document designed to support clear communication and expectation-setting between doctor and patient. Dr. Golden is now testing the prototype with patients. He will present “COMMUNICATING THE EXTERNAL BEAM RADIATION EXPERIENCE (CEBRE): A NOVEL GRAPHIC NARRATIVE PATIENT EDUCATION TOOL” (for which the ID students are listed as co-authors) to fellow radiation oncologists at two North American forums this year in order to disseminate the approach, receive feedback for future iterations, and instantiate it as standard practice. Patients have reported that the tool has made a stressful interaction easier, helps them to understand how external beam radiation works, and sets expectations for the process. In 2018, an estimated 1,735,350 new cases of cancer will be diagnosed in the U.S. (Cancer.gov, May 2018). Given that external beam radiation is one of the most common treatments, this work could improve the experience of hundreds of thousands of cancer patients annually.
