CINCINNATI (WKRC) - A breakthrough study by researchers at the Cleveland Clinic has important information for anyone who takes aspirin. This study looked at how aspirin interacts with other pain relievers. It was published in the Journal of the American College of Cardiology. Researchers looked at what happens in the body when aspirin is added to popular prescription arthritis drugs such as Celebrex, ibuprofen, or Naproxen. What they found was reassuring, according to the Cleveland Clinic team that conducted the research. Those who suffer from arthritis and take these medications can add aspirin without it being considered harmful to the body. The researchers did find the aspirin does slightly change how these arthritis medications interact with your cardiovascular system. In this study, researchers analyzed data from the 2016 precision trial. Authors in that trial studied more than 24,000 people with osteoarthritis and rheumatoid arthritis who were at high risk for heart disease. Participants took daily prescription doses of either ibuprofen, Naproxen, or Celebrex. That study showed that the heart risk for the drugs was pretty much equivalent, and that Celebrex had the fewest gastrointestinal and kidney complications. The new study looks at precision participants who were also taking aspirin. They investigated investigates how aspirin may change the safety of the three prescription pain relievers. Results show that taking aspirin with any of the three drugs is relatively safe, however the safety of Celebrex, compared to ibuprofen or naproxen, is narrowed a bit when taken with aspirin. As always, no matter what you take, talk to your own doctor or pharmacist about any potential interactions. Since dosages of all of these may influence results, the authors of this study also say don’t add aspirin without telling your health care provider.

The choice of which medication a PCP recommends hinges on the patient’s presentation, patient preference driven from advertisements or prior experiences, and/or the PCP’s own preference. Although it’s becoming increasingly common to have a PCP refer a neck pain patient for chiropractic care, this still does not happen for all neck pain patients in spite of strong research supporting the significant benefits of spinal manipulation to treat neck pain. One such study compared spinal manipulation, acupuncture, and anti-inflammatory medication with the objective of assessing the long-term benefits at one year of these three approaches in patients with chronic 13 weeks or more of neck pain. The study randomly divided 115 patients into one of three groups that were all treated for nine weeks. Comparison at the one-year point showed that ONLY those who received spinal manipulation had maintained long-term benefits based on a review of seven main outcome measures. The study concludes that for patients with chronic neck pain, spinal manipulation was the ONLY treatment that maintained a significant long-term (one-year) benefit after nine weeks of treatment! In a 2012 study published in medical journal The Annals of Internal Medicine, 272 acute or sub-acute neck pain patients received one of three treatment approaches: medication, exercise with advice from a health care practitioner, or chiropractic care. Participants were treated for twelve weeks, with outcomes assessed at 2, 4, 8, 12, 26, and 52 weeks. The patients in the chiropractic care and exercise groups significantly outperformed the medication group at the 26-week point AND had more than DOUBLE the likelihood of complete neck pain relief. However, at the one-year point, ONLY the chiropractic group continued to demonstrate long-term benefits! The significant benefits achieved from both exercise and chiropractic treatments when compared with medication make sense as both address the cause of neck pain as opposed to only masking the symptoms. De Quervain’s tenosynovitis is a painful condition affecting the thumb side of the wrist. Swelling of the tendons around the base of the thumb can irritate the sheath in which they move, resulting in difficulty squeezing, grasping or turning the wrist. “De Quervain’s tenosynovitis is a condition we commonly see and treat in our practice,” says Check C. Kam, MD, FAAOS, a board-certified orthopaedic surgeon with South Florida Orthopaedics and Sports Medicine. “The pain is located along the thumb side of the wrist, and it can cause significant swelling.” Causes of de Quervain’s Tenosynovitis The actual causes of de Quervain’s disease remain a mystery. However, many physicians link it to overuse, a severe blow to the thumb, or any activity involving constant, repetitive hand or wrist movements such as gardening, hammering, playing golf, tennis, or even lifting heavy objects. A common modern exacerbator of de Quervain’s is associated with increased text messaging. “Texting Thumb” can result from the awkward positioning or hyperextension of either thumb, causing tendons to become inflamed. Pregnancy, diabetes, and inflammatory arthritic conditions such as rheumatoid arthritis have also been connected to de Quervain’s. “De Quervain’s is also known as “mommy’s wrist” because it often afflicts new mothers and causes pain when they lift up their child,” Dr. Kam explains. Symptoms and Diagnosis De Quervain’s can cause pain when turning or twisting the wrist. Patients may also experience pain and difficulty when grabbing something between their thumb and fingers. In some cases, patients may hear a squeaking, cracking, or snapping sound when they move the wrist or thumb. The bottom area of the thumb or the side of the wrist may also be sore or swollen, and sometimes the condition can cause a small bump which are commonly associated ganglion cysts on the thumb side of the wrist. An orthopaedic hand and wrist specialist can diagnose de Quervain’s disease by feeling along the tendons on the thumb-side of the wrist for tenderness. They may also ask the patient to make a fist with their fingers clasped over their thumb and then to bend the wrist in the direction of the pinky finger. This maneuver is also known as the Finkelstein Test, and a patient with de Quervain’s will find the action very painful. Treatment for de Quervain’s Tenosynovitis The goal of treatment for de Quervain’s is to relieve swelling and pain in the wrist, which helps to restore normal function. If left untreated, the pain can spread up the forearm or down into the thumb. “When I see de Quervain’s, I typically start by treating with a cortisone injection and a splint to immobilize the thumb and help reduce inflammation,” says Dr. Kam. Other conservative treatment options for de Quervain’s can include: Use of nonsteroidal anti-inflammatory medications like ibuprofen or naproxen in either oral form or cream that can be applied to the affected area Cessation of any activities that exacerbate the condition Applying ice or heat to the concerned area “If this initial treatment doesn’t work and the pain continues, or there’s a recurrence, I typically recommend a first dorsal compartment release,” says Dr. Kam. “This procedure can be performed safely as a wide-awake surgery using only local anesthesia. It takes about five to ten minutes to perform in an outpatient surgery setting.” During a first dorsal compartment release surgery, the orthopaedic surgeon makes an incision below the tip of the radial styloid (the part of the wrist bone that protrudes just under the thumb) to open the first dorsal compartment. This releases the tendon sheath and relieves the constriction of the tendons that pass through it. Additionally, ganglion cysts are also excised if present. After the incision is closed with sutures, a splint is applied and then covered with an ace wrap. “The recovery is typically fairly brief and requires about 7 to 10 days for the sutures to be removed. Therapy may or may not be needed depending on whether or not there is significant stiffness,” says Dr. Kam. Occupational therapy can help patients learn how to use their wrist more efficiently to avoid recurrence. A physical therapist can help guide patients through gentle stretching exercises that can be helpful in preventing stiffness or other range of motion issues. “Most patients do very well after this procedure,” affirms Dr. Kam. Everyone, well at least almost everyone, has had headaches from time to time, and we all know how miserable they can make us feel. In fact, at some point in time, 9 out of 10 Americans suffer from headaches that range between mild and dull to throbbing, intense, and debilitating, sometimes to the point of requiring bed rest in a dark, quiet room. The common reflex is to reach for that bottle of pills and pray the headache subsides so you don’t have to call in sick and lose another day of productivity when you have so much to do. Unfortunately, between the side effects of many medications designed to help headaches and the pain associated with the headache, this approach is frequently NOT the answer. So what is? The good news is that many studies have identified spinal manipulation therapy (SMT), the main type of care utilized by chiropractors, as being very effective for popular types of headaches—in particular tension-type headaches that arise in the neck. An important 2001 study reported that SMT provided almost immediate relief for headaches that arose in the neck with SIGNIFICANTLY fewer side effects and longer-lasting results compared with commonly prescribed medications. Another interesting study that found similar results included tracking the prevalence/frequency of headaches after treatment stopped. The authors of the study reported the patients receiving SMT had continued to experience sustained benefits throughout the following weeks, and even months, in contrast to those in a medication treatment group where headaches came back almost immediately after they discontinued treatment. The most commonly prescribed medication for tension-type headaches are non-steroidal anti-inflammatory drugs (NSAIDs). Common over-the-counter options include ibuprofen (Advil, Nuprin, etc.) and Aleve (Naproxen). For those who can’t take NSAIDs because of blood thinning and/or stomach-liver-kidney problems, doctors commonly prescribe acetaminophen (Tylenol), but it can be hard on the liver and kidneys, especially when taken over time. So, what can you expect from a visit to a chiropractor for your headaches? The typical approach begins with a thorough history and examination with an emphasis of evaluating the neck and its associated function. Your doctor of chiropractic may also perform tests designed to reduce pain and some that provoke a pain response to identify the “pain generator” or cause! CAUSES OF CARPAL TUNNEL PINS AND NEEDLES The symptoms described as carpal tunnel pins and needles, numbness and tingling are the terms generally used to describe a sensation caused by carpal tunnel syndrome -- a type of peripheral neuropathy. Part 2 Treating & Diagnosing PreMenstrual Dysphoric Disorder (PMDD) *Disclaimer: I am no medical professional as am just an ordinary young woman just sharing my stories and life experiences with my condition and that I am sharing what works for me. If you see any signs/symptoms to any of these mental health topics, do seek professional help and/or advice for with a professional. This is the second part of the video based on this topic I would like to share. If you have any of the listed PMDD symptoms in my first video, you should see your doctor. He or she will review your symptoms and medical history and give you a thorough medical exam. Psychiatric evaluation may also be included. Before a doctor makes a diagnosis of PMDD, he or she will rule out other emotional problems, such as depression or panic disorder, as the cause of the symptoms. In addition, underlying medical or gynecological conditions, such as endometriosis, fibroids, menopause, and hormonal problems that could account for symptoms, also must be ruled out. PMDD is diagnosed when at least five of the above listed symptoms (including at least one of the first four) occur for most of the time during the seven days before menstruation and go away within a few days of the start of the menstrual period. If these symptoms are present every day and do not improve with menstruation, they are unlikely due to PMDD. How does PMDD get treated? Many of the same strategies used to treat PMS may also be helpful in relieving symptoms of PMDD. The four main forms of treatment are: -Good nutrition. Many health experts recommend that women with PMDD limit their intake of salt, caffeine, refined sugar, and alcohol. Supplements, such as calcium, vitamin B6, vitamin E, and magnesium may be recommended. The effectiveness of any of these approaches hasn't been well established -Exercise. Regular aerobic exercise such as walking or swimming appears to improve premenstrual symptoms. It's unclear whether it can treat PMDD, however exercise is generally recommended as it has additional benefits. -Medications. Several antidepressants may be used to treat PMDD. There are three drugs approved by the FDA for the treatment of PMDD: Sarafem, Paxil CR, and Zoloft. These antidepressants can relieve the symptoms of PMDD. Other antidepressants have also been effective in relieving symptoms of PMDD. These medicines can be taken continuously or intermittently (only during the 14-day premenstrual period). Taking them intermittently may decreases the side effects of these drugs. Some over-the-counter pain relievers such as aspirin, ibuprofen (Motrin), and naproxen (Aleve) may help some symptoms such as headache, breast tenderness, backache, and cramping. Diuretics, or water pills, can help with fluid retention and bloating. If anxiety is a component of the symptoms and does not respond to approved medications, an anti-anxiety drug may be recommended. Currently, none are approved to treat PMDD and are typically limited to just a few days of each cycle to avoid dependence. Hormones can be used to treat PMDD. Ovulation can be stopped either using medication or surgically (as a last resort). Medicines used to stop ovulation include birth control pills, Danazol, Zoladex, Synarel, and Lupron. The second hormonal approach to treat PMDD is the use of progesterone or estrogen to relieve symptoms. It's unclear whether this approach is effective. -Counseling. Therapy to help women with PMDD develop effective coping strategies may help some with PMDD. Relaxation therapy, meditation, reflexology, and yoga may also help, but these approaches have not been widely studied. Thanks for watching and your support.
