Jhpiego has worked with stakeholders and partners to pioneer the single visit approach (SVA) for cervical cancer prevention. The SVA consists of visual inspection using dilute acetic acid (the main component of vinegar) to detect pre-cancerous lesions on the cervix, followed by the offer for treatment using a freezing technique (cryotherapy), all in one visit. This practical, clinically safe and cost-effective approach to cervical cancer prevention in low-resource settings helps save women's lives worldwide by reducing overall cost of detection and treatment, including turning what would normally require four or five clinic visits to just one.

The US Preventive Services Task Force (USPSTF) is a government organization whose function is to develop guidelines for health screening tests. Guidelines for cervical cancer have been developed and make a lot of sense. Women 21-65 should have a routine Pap smear every three years unless they also had an HPV screen at the same time; in that case every 5 years is sufficient. Women over 65 don't need either a Pap or HPV test if they have done routine exams and have been normal. There is no need for HPV screening before age 30 because so many women have this infection and most of the time it clears on its own. The “VIA VILI examination and conization” is a video tutorial displaying the visual inspection and management of a pathological case of CIN 2. The VIA/VILI examination is presented and the acetowithe and iodine negative lesions are described. The patient underwent a conization. However, cryotherapy or cold-coagulation are also good options for treatment. A low-tech approach to screening women in resource-strapped settings helped to reduce cervical cancer deaths by almost a third. Female health care workers in India were trained to perform visual inspection of the cervix with acetic acid in 150,000 poor women aged 35-64 years with no history of cancer. A control group of women received usual care for that country -- no screening. In intention-to-treat analysis, screening with VIA resulted in a 31% reduction in the cervical cancer-specific death rates (11.1 vs. 16.2 deaths per 100,000). Dr. Surendra Srinivas Shastri of Tata Memorial Center in Mumbai, India, presented the study at the ASCO 2013 annual meeting.

Advances in the Prevention, Diagnosis and Treatment of Breast and Gynecologic Cancers (Module 7) Breast cancer remains the most frequently diagnosed solid tumor and second leading cause of cancer death in U.S women. If these rates continue, a female child born today will have about a 1 in 8 chance of developing breast cancer at some point during her life. Greater public awareness of signs and symptoms have increased early diagnosis, breast cancer treatment is still dependent on clinical factors such as stage, histology, HER-2 and hormonal status. It is estimated that 20 million Americans may have the human papillomavirus (HPV). Yet more than 76 percent of U.S. women have never heard of this sexually transmitted virus which causes nearly all cervical cancers. Women who are not diagnosed with cervical cancer through screening typically present with advanced disease. In 2010, the NCI placed the number of newly diagnosed cases of cervical cancer at 12,000 with related deaths at 4,000. The scientists and clinicians at City of Hope have a long history of developing and advancing new techniques to screen for, diagnose and treat gynecologic and breast cancers. With the goal of contributing to the improvement of quality of life and overall outcomes for these patient populations, these activity has been designed by these experts, representing multiple disciplines, to profile and discuss the current advances and future directions in the treatment of gynecologic and breast cancers, and to engage with the learner in discussion as to their incorporation into current treatment paradigms. The Amazon Community Based Participation Cervical Cancer Screen-and-Treat (ABCS) Program addresses the lack of education, screening and treatment of cervical cancer for women of reproductive age in 25 isolated villages along the Lower Napo River in the Peruvian Amazon, the first of its kind to provide rapid, local diagnosis and prevention in this region. Our project will save the lives of women through reducing the incidence of cervical cancer. Working beside community members, we will make local cervical cancer screening and treatment accessible and sustainable. We will bridge the gap between cutting-edge technologies in Human papillomavirus (HPV) diagnosis, prevention and treatment and the remote, under-resourced Amazonian communities. Our project will not only improve the wellbeing of women and girls, but also empower and educate community members about reproductive health. The overall goals of this project are: to create a replicable model of cooperation between local government, national government, and non-government organisations to serve isolated women who are suffering from or at risk of cervical cancer; and, to reduce the number of deaths from cervical cancer through education, screening and treatment. We will utilize a four-pronged strategy: baseline data collection, community participation and education, HPV vaccination, and HPV testing with on-site cryotherapy treatment for positive results. Our actions will continue to promote human rights and respect of local traditions and knowledge, and also to contribute to the broader picture of reducing the burden of cervical cancer in Peru. It is recommended that women should get a pap smear screening every 3 years to detect any abnormal cells that might lead to cervical cancer.
