Tonsil cancer: Symptoms, Causes and Treatment Tonsil cancer is a rare form of cancer with symptoms that are often similar to other illnesses. Like many cancers, the causes are unclear, but there are some key risk factors. We find out more about symptoms, treatment, and outlook. Tonsil cancer is part of a group of cancers that originate in the mouth and throat. These are known as oral and oropharyngeal cancers and are often grouped under head and neck cancers. The tonsils are part of the immune system. They defend the body against bacteria and viruses that enter the mouth or throat. If cancer is found at an early stage, there are usually more options available to treat it. This increases the chance of successful treatment and recovery. What is tonsil cancer? Tonsil cancer begins with cancerous cells or a tumor in the tonsils. It can still affect people who have had their tonsils removed, as some tissue is often left behind after surgery. People who drink alcohol, smoke, or have human papillomavirus (HPV), have a greater risk of developing tonsil cancer. The tonsils are found towards the back of the throat, one on either side. Each tonsil is made up of a collection of lymphoid tissue. This is tissue containing lymphocytes, which are cells that fight off disease. Tonsils catch and destroy bacteria and viruses. They can change in size and often swell with blood to help trap germs, such as when someone has a cold.

New studies show throat and neck cancers are on the rise. But unlike most cancers, doctors say the most common cause isn’t genetics or lifestyle, but a virus that’s typically known for causing cervical cancer in women. Dr. Anthony Anfuso, an oncologist on staff with Lee Health, says it’s becoming a common diagnosis. “We’re seeing a lot of younger people who are non-smokers actually get this type of cancer and the reason in those people often times is a virus called HPV.” The human papillomavirus is the same virus that causes cervical cancer in women. Now, doctors say the sexually transmitted virus is also responsible for throat and neck cancer in men and women. “One difference between HPV induced oral cancer and cervical cancer is that there are screening tests, there are pap smears for women, to try to pick up premalignant lesions. Unfortunately, for oral cancer that has not been developed yet,” said Dr. Anfuso. But there is good news. Patients who are diagnosed with HPV oral cancer have a higher chance of survival. “Someone who has an HPV negative oral cancer, which again is a cancer of the tonsil or the back of the tongue, their 5 year survival rate is 35 to 45 percent. If you have a HPV positive tumor it actually jumps to 90 percent for a patient who is a nonsmoker,” said Dr. Anfuso. Doctors say that’s because HPV has not been found to be an aggressive cancer. Radiation and chemotherapy are common treatments for oral cancer. “The fact of the matter is, probably over 90 percent of the population has been exposed to the HPV virus, even the high risk HPV virus. Most people will clear the infection and never develop symptoms,” said Dr. Anfuso. But those who don’t are at risk for developing oral cancer, making it important to notify your doctor about any changes or pain. In general, when we’re talking about the neck cancer, we are talking about cancers of the oral cavity, oral pharynx, hypopharynx, larynx, and nasal pharynx. We are not talking about brain tumors—those really fall under a separate category of cancers. What are the major risk factors? Smoking is the major reason why patients get head and neck cancer. Smoking is one of the major risk factors. The second risk factor is alcohol abuse. We knew all along that patients that smoke and drink have the highest risk of developing head and neck cancer. Over the past decade, we’ve also become aware of another reason why patients develop head and neck cancer, and that’s infectious. We now know that infection with a particular type of virus called human papillomavirus, or HPV—specifically HPV type 16 is another reason why patients get cancers of the oral pharynx, the base of the tongue and tonsil. What are the warning signs? A mass or a lump in the neck is always one of the common signs of head and neck cancer. So really, a change in voice, change in eating, swallowing, chewing, pain on eating, pain on swallowing, sore throat—those would be signs and symptoms of head and neck cancer. There’s also a few of the entities that we call ‘pre-malignant.’ These are lesions that if you see these in somebody’s mouth, you have to be concerned that they could develop later on into cancer. Those are what are known as ‘white spots,’ or leukoplakia. Those are often identified during a dental exam. Who is at risk? The ratio of males to females is typically 3 to 1, so it’s obviously much more common in males compared to females. The age for this cancer when the cancer is due to smoking and alcohol use, the typical age for these patients is 60 to 70 years old. When the reason to get the cancer is HPV, these patients are much younger, typically in their late 30s or early 40s.

A snapshot of HPV is not a pretty picture. The first nationwide accounting found at least 50-percent of the sexually active population will have genital HPV in their lifetime. "It's really an asymptomatic infection. Most women who have HPV have absolutely no symptoms. Whereas other STDs, like herpes, certainly has symptoms if there's an outbreak," says Dr. Kevin Fleishman, gynecologist on Lee Memorial Health System's medical staff. Many of them clear without ever being detected. Some are more likely to turn bad. The human papilloma virus has more than 100 strains. "Some are low risk, some are high risk. The high risk types are known to cause almost all cervical cancer," says Dr. Fleishman. This new study revealed a rising number of people with oral HPV. It's estimated 16-million Americans between 14 and 69 are infected. This raises their risk for cancers of the mouth. "Many, many of these cancers are caused by HPV and these are largely cancers of the tonsils or the base of the tongue. Way back in the back of the throat where you can't see with a tongue blade," says Dr. Phillip Andrews, otolaryngologist on Lee Memorial Health System's medical staff. "Human papillomavirus -positive oropharyngeal cancer " also known as "HPV16+ oropharyngeal cancer" or "HPV+ OPC" is a recognized subtype of oropharyngeal squamous cell carcinomas , associated with the HPV type 16 virus. HPV oral infection precedes the development of HPV+ OPC. Slight injuries in the mucous membrane serve as an entry gate for HPV, which thus works into the basal layer of the epithelium. People testing positive for HPV16 oral infection have a 14 times increased risk of developing HPV+ OPC. Immunosuppression seems to be an increased risk factor for HPV+ OPC. Individuals with TGF-β1 genetic variations, specially T869C, are more likely to have HPV16+ OPC. TGF-β1 plays an important role in controlling the immune system. A 1993 study has found that patients with human papillomavirus -associated anogenital cancers had a 4.3-fold increased risk of tonsillar squamous-cell carcinoma. Although evidence suggests that HPV16 is the main cause of OPC between non-smokers and non-drinkers, the degree to which tobacco and/or alcohol use may contribute to increase the risk of HPV+ OPC is unclear. Concomitant human herpesvirus-8 infection can potentiate the effects of HPV-16. A prospective study has found that increased HPV+ OPC risk was observed more than 15 years after HPV exposure, pointing to a slow development of the disease, like in cervical cancer. HPV associated cancers are caused by the expression of HPV's E6 and E7 proteins that bind to and inactivate tumor suppressor proteins p53 and retinoblastoma protein , respectively, leading to malignant transformation of HPV infected cells. Cervical cancer may be the most common cancer linked to HPV, but it is not the only one. As it turns out, men are at higher risk to get another form of cancer from the same virus. The human papillomavirus - it's most closely linked to women and cervical cancer. "HPV, there are 140 different types; some are low-risk, some are high-risk. The high-risk types are known to cause almost all cervical cancers," says Dr. Kevin Fleishman, a gynecologist on Lee Memorial Health System's medical staff. Studies show that half of all American men are infected with HPV and it puts them at greater risk of cancer too. "Absolutely, more and more cancers have been shown to be associated with the HPV virus," says Dr. Fleishman. Among them are head and neck cancers. "Many of these cancers are caused by HPV. Prior to 15, 18 years ago, probably 90 to 95 percent of my new cancer patients were smokers. But now about 45 percent are non-smokers and the majority of those are caused by HPV," says Dr. Phillip Andrews, an otolaryngologist on the medical staff of Lee Memorial Health System. Two-thirds of cancers of the tongue and tonsils are caused by HPV and 80 percent of those cases are found in men. If caught early, the prognosis is good. "Some of the smaller cancers in the base of the tongue and tonsils we'll treat with radiation alone, particularly if it's HPV related," says Dr. Andrews. The HPV vaccine was originally promoted as a 'girls' vaccine. Since then, the CDC recommended it for boys as well. And not just to stop the spread of cervical cancer but to keep them safe, too. WHAT DO YOU KNOW ABOUT HPV-RELATED OROPHARYNGEAL CANCER? What is HPV? HPV (the human papillomavirus) is the most common sexually transmitted infection in the world. About 80% of sexually active people will have at least one HPV infection in their lifetime. There are over 100 strains. Most are harmless. Some cause genital warts. Other high risk strains (#16 and #18) can cause cancer in the cervix, vulva, penis, anus, AND in the oropharynx (back of mouth, base of tongue, tonsils, throat). How common is HPV-related OPC? Since 1984, the overall incidence of HPV OPC has increased by 225% in the US (Chaturvedi et al, 2011). By 2020, it is predicted that HPV OPC will be more common than cervical cancer. How do I know if I have an oral HPV infection? There is currently no screen available to test for an HPV infection in the oropharynx. Most people are asymptomatic until a tumour develops. Who is at risk? Both men and women are at risk for developing HPV OPC; however, HPV OPC is more common in men, and increasingly in younger men. What can I do? Talk to your doctor about getting vaccinated against HPV. Educate yourself about safer sex practices. Oral cancer, also known as mouth cancer, is a type of head and neck cancer and is any cancerous tissue growth located in the oral cavity.It may arise as a primary lesion originating in any of the tissues in the mouth, by metastasis from a distant site of origin, or by extension from a neighboring anatomic structure, such as the nasal cavity. Alternatively, the oral cancers may originate in any of the tissues of the mouth, and may be of varied histologic types: teratoma, adenocarcinoma derived from a major or minor salivary gland, lymphoma from tonsillar or other lymphoid tissue, or melanoma from the pigment-producing cells of the oral mucosa. There are several types of oral cancers, but around 90% are squamous cell carcinomas, originating in the tissues that line the mouth and lips. Oral or mouth cancer most commonly involves the tongue. It may also occur on the floor of the mouth, cheek lining, gingiva (gums), lips, or palate (roof of the mouth). Most oral cancers look very similar under the microscope and are called squamous cell carcinoma, but less commonly other types of oral cancer occur, such as Kaposi's sarcoma. In 2013 oral cancer resulted in 135,000 deaths up from 84,000 deaths in 1990. Five-year survival rates in the United States are 63%. In its early stages, it can go unnoticed. It can be painless with slight physical changes. But the precursor tissue changes, can be noticed by the doctors. Early stage symptoms can include persistent red or white patches, a non-healing ulcer, progressive swelling or enlargement, unusual surface changes, sudden tooth mobility without apparent cause, unusual oral bleeding or epitaxis and prolonged hoarseness. Late stage symptoms can include an indurated area, paresthesia or dysesthesia of the tongue or lips, airway obstruction, chronic serous otitis media, otalgia, trismus, dysphagia, cervical lymphadenopathy, persistent pain or referred pain and altered vision.
