It is important to know how this works with other insurance you have in order to get the best coverage. Here is what you need to know about this option. How Do You Get Coverage You have two options to get drug coverage including through Medicare Part C Advantage Plan or through Part D Prescription Drug Plan. The most common way is through Part D. This plan can be used with original Medicare and other plan option such as a Medical Savings Account (MSA) or Medicare Cost Plans. Part C or Medicare Advantage Plan includes using a PPO or HMO along with benefits similar to Part A and B. You may be required to sign up for Part A and B in order to obtain Part D with Part C. Medicare drug coverage options may vary and could change depending on what Part C covers. Associated Costs and Getting Help Your income will determine how much you pay toward prescription drugs. If you receive Social Security you can have costs deducted from your Social Security payment. You may be required to satisfy an annual deductible before the drug coverage starts to pay. You may be required to pay a copay or coinsurance after paying a deductible (if your plan has this requirement). There are drug plans with different tiers that show how much you could pay depending on related costs. Generic drugs may have cheaper copay than name brand. An example may be 20 or 25 percent. When you pay for drugs you are paying for a month supply unless stated otherwise. Signing up for Medicare may include learning more about associated costs. Medicare Part A and B cost may vary from one person to another when considering out-of-pocket costs and coinsurance. On average a Part B premium payment is about $104.00 per month. The deductible for Part B is about $147.00. You may end up paying more if you do not sign up for coverage during the enrollment period or upon becoming eligible (late enrollment penalty). For qualifying beneficiaries you can get help paying Part A and Part B costs. Costs and Services for Part A There is a monthly premium payment but if you qualify you don’t have to pay it. There is also a late enrollment penalty if you don’t sign up for coverage during the designated time frame. With Part A you could pay roughly $407.00 per month. If you sign up for Part A coverage you are required to sign up for Part B. Part A helps pay for services such as hospital stays and care received in special facilities such as skilled nursing or hospice. Home health services and certain supplies may be covered. Costs and Services for Part B For Part B a monthly premium is required in the amount of $104.00 and the deductible is $147.00 per year. You may automatically qualify for this plan or sign up if you are eligible. The amount you pay may be based on income earnings and whether you have a spouse. While there are differences in Medicare Part A and B cost of services, there are limitations on how much coverage is provided for services such as occupational therapy, physical therapy and speech and language services. Part B helps pay for some services related to preventative care, mental health, outpatient and clinical research. Options to Pay Part A and Part B Premiums Premium payments can be deducted from income such as Social Security or Railroad Retirement Board benefits (RRB). You will receive a notice if you do not receive such benefits. You can make this payment online or sign up for Medicare Easy Pay service. You can also pay by mail or use a debit card. Your plan will remain active as long as premium payments are made. There are options to consider if you need help with Medicare Part A and B cost. There is government programs that can help you based on income or medical needs. If you have low income options such as Medicaid, Medicare Savings Programs, and Program if All-inclusive Care for the Elderly (PACE) can help cover copayments, deductibles and other costs. If you are enrolled in Part A and/or B coverage and receive Part D (drug coverage) you may qualify for help covering drug costs. Certain qualifications apply. The 5 Star Special Enrollment Period allows you the opportunity to switch into or enroll in a 5 Star-rated plan once per year. Plans are rated from one to five stars by Medicare and are a reflection of the experience people who have been in enrolled in the plan have had. You are allowed to enroll between December 30th and November 8th. If you qualify for Medicare extra help you can change plans anytime. Fourth. You must know how to conduct a thorough comparison of Medicare Part D plans. You can see all available plans for your service area at the Medicare Plan Finder at medicare.gov. But don't choose a plan based on premium alone. There's a lot more to plan cost than the monthly premium. Start your search for the best plan by exploring the Medicare Part D formulary for each plan. Plan N also does not cover your $183 Part B deductible much like Plan G… There is also a $20 copay for doctor visits and a $50 copay for emergency room visits that do not result in impatient admission. Lastly Plan N does not cover part Part B excess charges. Part B excess charges are when doctors or specialist charge up to 15% more then what Medicare is willing to pay for a test, sugary, or procedure. Plan F was on average the most expensive at $130 dollars, Plan G was $104, and least expensive was Plan N at $85. This is very consistent with what you will find throughout the United States. Now lets compare the three. Comparing Plan F to Plan G is very easy. All you need to do is some simple math to find out that Plan G will cost on average about $312 less a year than Plan F. Then subtract the only difference between the two. And if you remember the only difference is the $183 Part B deductible. So your net savings with Plan G after taking into account all benefit differences is $129. So with a Plan F you are paying an insurance company $129 a year to pay for your Part B deductible. And you can even pocket the whole difference of $312 if you do not go to the doctor at all… Some other factors to look at when comparing these plans is that Plan F will no longer be available to new enrollees in 2020, but you can keep your plan as long as you like if you signed up before 2020. Plan F ending will create a closed risk pool most likely leading to higher rate increases for Plan F after 2020. It is also important to point out that Plan G and N have had historically lower rate increases than Plan F in the past as well. So just because plan F is the most popular plan on the market does not make it the best. Comparing Plans G and N is a little more difficult. Again after some simple math you find out that you can save $228 a year with Plan N over a Plan G. But you need to factor in the Part B copays with Plan N. The $228 dollar difference between plans G and N is equivalent to 11.4 $20 office visit or 4.6 $50 emergency room visits. So you would have to have quite a few office or ER lists to make up the difference in cost, but do not forget about the Part B excess charges with Plan N. You can avoid these charges by only working with Medicare Assigned doctors, but the risk of being charged 15% more for a test, sugary, or procedure is one of the reasons why Plan N is less expensive. So excess charges are an important factor when deciding between Plan G and N, but if you are in great health then Plan N could save you a lot of money especially if you continue to stay healthy 5 to 10 years into your Medicare enrollment. Now which is the best of the three. Well I would have to pick Plan G in most cases. It is close between Plan G and N, but the safer long term bet is with Plan G. Now if you are exceptionally healthy you should seriously consider Plan N especially if the cost difference is over $200 a year. But remember if you do pick Plan N just always make sure your doctor except Medicare Assignment to avoid any Excess Charges. And finally Plan F. Knowing your costs for Medicare Part B, Part D, and supplemental coverage can help you plan ahead and budget accordingly. Medicare Part A Costs Medicare Part A is usually free as long as you have worked for 10 years (40 Quarters) and paid FICA taxes. Essentially, you have already "pre-paid" for these benefits and will not have any premiums due in retirement. If you have not met the qualifications to get Part A for free, you will pay a monthly premium of up to $413 for 2017. If you have worked for more than 30 quarters but less than 40 quarters you can get a pro-rated premium for Medicare Part A. Medicare Part B costs are based on income. Premiums for Medicare Part B are based on your Modified Adjusted Gross Income (MAGI). Since enrollment in Medicare is done through the Social Security Administration, Medicare will pull your latest income numbers directly from the IRS. You will get a letter each year around December from Social Security telling you how much your Part B premium will be for the upcoming year. Modified Adjusted Gross Income (MAGI) is calculated by monies you have earned through wages, Social Security Benefits, required minimum dividends from investments, interest, capital gains, and tax-deferred pensions. Any distributions from Roth IRA (or Roth 401k) accounts, life insurance, reverse mortgages, and health savings accounts DO NOT count in the MAGI calculation. Roughly 95% of Americans fall into the standard income bracket for Medicare Part B premiums leaving only 5% paying higher premiums due to the Income Related Monthly Adjustment Amount (IRMAA). The standard monthly premium for someone new to Medicare in 2017 is $134. This premium will be automatically deducted from your Social Security benefit each month. If you are not already receiving Social Security benefits you can be billed quarterly or setup and EFT to pay for Part B directly from your checking account each month. If you have a higher income you will pay an Income Related Monthly Adjustment Amount in addition to the standard Part B premium. The chart below which was pulled directly from the Medicare website. Medicare Part D costs are also based on income. Similar to Part B, Medicare Part D also has its own rules for higher income earners. Medicare Part D premiums vary by plan and you can usually find plans starting at $15-17/month. The national average Part D premium though is $34/month. In addition to the plan's premium, those in the higher income bracket will also pay a Part D- IRMAA. The chart below can help give you an idea what to expect if your income is above the standard threshold. Planning ahead can help you make sure to have enough savings for the future. If you find all of this just a little overwhelming, don't worry it definitely can be. We can help you determine your costs ahead of time so you are prepared to start Medicare on time and on budget. Many people find that the combined costs for Medicare Part B, Part D, and supplemental coverage is still a lot less than they are paying for private insurance before they turn 65. Medicare Part B monthly premiums for 2017 and Medicare Part A -B Deductibles are increasing... CMS (Medicare) has just announced the new premiums for your Medicare Part B and the new deductibles for your Medicare Part A and Part B The majority of people have been paying $104.90 for Part B for the last few years but will now be paying $109.00 with the 2017 Medicare increase. The rest of Medicare beneficiaries (about 30%) will now pay the new 2017 Medicare Part B premium of $134 per month. This premium can be debited from your monthly Social Security benefit check or can be paid quarterly by mailing in your premiums with a check or money order. Medicare has also announced that the annual Medicare Part B deductible will be increasing to $183 VS 2016 Medicare Part B deductible of $166. Even with this slight increase in the Part B Deductible, I suggest Medicare Supplement Plan G to most of my clients. I still believe that Plan G is the most cost effective plan to get since the insurance carriers will be forced to increase premiums on Plan F to cover the deductible increases. This is especially true in states that have specific laws reducing the value of Plan G. In addition, some insurance companies are preparing for Medicare Supplement Plan D to be one of the top selling plans in Medicare. To be clear, I am referring to the Medicare Supplement Plan D and NOT Medicare Part D prescription drugs. Medicare supplement Plan D is a direct competitor to Medicare supplement Plan N. Is Medicare Supplement Plan N the right plan for you? Let’s take a look. When comparing Medigap Plan N vs. Plan G, people that prefer the Plan G to the Plan N tend to be people who would rather pay a little extra each month and avoid having to do the extra work to avoid excess charges. The plans are identified by letter across the top row. The category of Medicare services are in the left hand column. In the column below the Plan letter you can see the percentage of insurance coverage offered by that plan. You can find this table on our website and in various Medicare publications. With Medigap Plan N we see that it covers 100% “Medicare Part A coinsurance and hospital costs (up to an additional 365 days after Medicare benefits are used up.). Notice that all the supplements have this coverage. Also, notice down here that Medicare Supplement Plan N pays the Medicare Part A deductible. In 2017 that deductible is $1,380 per event. It will change every year, but with a Medigap Plan N that doesn’t matter. Your supplement will pay it. Without a supplement you pay the deductible up front, then have 60-days of hospital coverage before you start paying a copay. With Medicare supplement Plan N you can be an inpatient in the hospital for more than 365 days with 100% coverage. You will have no out of pocket costs for Part A. The asterisks indicate that you have the Medicare supplement Plan N copay. You will be required to pay up to a $20 co-pay for every doctor visit and a $50 copay for every emergency room visit unless you end up an inpatient in the hospital. Then the $50 copay is waived. The important qualifying words here are “up to”…$20. Here is what you can expect. So notice that with Medicare supplement Plan N you have 100% coverage until you get down to this line item referring to the Medicare Part B deductible and Medicare Part B excess charges. Here Medigap Plan N has no coverage at all. You are expected to pay those fees out of pocket. That’s important. Later in this video I am going to go over a real life example using a common joint replacement that will clear up some of the most common questions and misunderstandings about Medigap Plan N. In order to evaluate if Medicare supplement Plan N is right for you, you need to know what the Medicare Part B deductible is and what it is expected to be in the future. It’s also critical that you understand Medicare Part B excess charges. So let’s focus on these and then go over a real life example to show you how Medicare supplement Plan N works with office visits, hospital stays and physical therapy. If a doctor or hospital does not accept Medicare assigned rates, they are paid 5% less from Medicare, but allowed to charge you 15% more. The amount the doctor or hospital charges you that is more than what Medicare pays the doctor for that service is called an excess charge. Excess charges are only allowed on Medicare Part B services. Those are outpatient and physician services. There are also some states that either forbid or severely restrict Medicare Part B excess charges. These state laws are called Medicare Override Measure or MOM laws. If you use non-par providers your out of pocket expenses could be considerably higher. Part B is for outpatient health care treatment. Doctor visits, labs, X-rays, MRI, CT scan (CAT scan), ambulance, ER, outpatient clinics, etc. Essentially any Medicare approved health care received outside the hospital setting. Treatment considered medically necessary under Medicare guidelines. Medicare Part B has a calendar year deductible. Once satisfied, Medicare pays 80% of the approved charge, you are responsible for the remaining 20%. Most seniors worry about hospital stays, but your major expenses will more than likely fall under Medicare Part B outpatient expenses. Original Medicare stand alone has no cap on your out of pocket expenses. You pay until you run out of money, get well or die. The American Cancer Society tells us that more than half the 1.4 million new cancer diagnoses occur in people age 65 and older. Roughly 20% of retiree deaths are a result of cancer.
