This lecture will detail the pathophysiology of asthma along with the early warning signs vs active signs of an asthma attack. In addition to a sample asthma action plan along with using a peak flow meter to monitor asthma, asthma triggers, nursing interventions for asthma, and all the medications used to treat asthma, which are bronchodilators and anti-inflammatory medications. What is asthma? Asthma is a chronic lung disease (no cure) that causes narrowing and inflammation of the airways (bronchi and bronchioles) that leads to difficulty breathing. How does it happen? Normally, when you breathe in air, it travels down through your upper airway to your lower airway, which includes the trachea, bronchi, bronchioles, and alveoli (where gas exchanges happens). The oxygen you breathe in crosses over into your blood stream, and the carbon dioxide in your blood crosses over into the airway to be exhaled. In patients with asthma, the bronchi and bronchioles are chronically inflamed and can become so inflamed that it leads to an asthma attack (wheezing, chest tightness, shortness of breath, coughing), which is usually due to a trigger of some type. When exposed to something they’re allergic to, some people experience a potentially life-threatening reaction called anaphylaxis. As a result, their immune system releases chemicals that flood the body. This can lead to anaphylactic shock. When your body goes into anaphylactic shock, your blood pressure suddenly drops and your airways narrow, possibly blocking normal breathing. This condition is dangerous. If it isn’t treated immediately, it can result in serious complications and even be fatal. What are the symptoms of anaphylactic shock? You will experience symptoms of anaphylaxis before anaphylactic shock sets in. These symptoms shouldn’t be ignored. They include: skin reactions such as hives, flushed skin, or pale skin suddenly feeling too warm feeling like you have a lump in your throat or difficulty swallowing nausea, vomiting, or diarrhea abdominal pain a weak and rapid pulse runny nose and sneezing swollen tongue or lips wheezing or difficulty breathing a sense that something is wrong with your body tingling hands, feet, mouth, or scalp If you think you’re experiencing anaphylaxis, seek medical attention immediately. You should also be aware of the symptoms of anaphylactic shock. These symptoms include: struggling to breathe dizziness confusion sudden feeling of weakness loss of consciousness What are the causes and risk factors of anaphylaxis? Anaphylaxis is caused by an overreaction of your immune system to an allergen, or something your body is allergic to. In turn, anaphylaxis can result in anaphylactic shock. Common triggers for anaphylaxis include: certain medications such as penicillin insect stings foods such as tree nuts, shellfish, milk, and eggs agents used in immunotherapy latex In rare cases, exercise and aerobic activity such as running can trigger anaphylaxis. Sometimes a cause for this reaction is never identified. This type of anaphylaxis is called idiopathic. If you aren’t sure what’s triggering your allergy attacks, your doctor may order an allergy test to look for what’s causing them. Risk factors for severe anaphylaxis and anaphylactic shock include: a previous anaphylactic reaction allergies or asthma a family history of anaphylaxis How is anaphylactic shock treated? The first step for treating anaphylactic shock will likely be injecting epinephrine (adrenaline) immediately. This can reduce the severity of the allergic reaction. At the hospital, you may receive cortisone and antihistamines intravenously. These help reduce inflammation in the air passages, improving the ability to breathe. Your doctor may give you beta-agonists such as albuterol to make breathing easier. You may also receive supplemental oxygen to help your body get the oxygen it needs. Your Medication Combivent Respimat Combivent Respimat (Ipratropium and Albuterol) Combivent Respimat is also known by the generic names: ipratropium and albuterol. Combivent Respimat comes in an inhaler containing 18mcg of ipratropium and 90mcg of albuterol per inhalation. Combivent Respimat is most commonly used for opening the airways in lung diseases where spasm may cause breathing problems. It is breathed into the lungs. Before using your inhaler you will need to spray it towards the ground until mist is seen. Once the mist is seen, repeat 3 more times then you may inhale the dose through your mouth. Always remember to put the cap back on the inhaler after you are done using your dose. Standard dosing of Combivent Respimat ranges from 1 inhalation 4 times daily up to a max of 6 inhalations per day. If you miss a dose of Combivent Respimat: • If you use this drug on a regular basis, use a missed dose as soon as you think about it. • If it is close to the time for your next dose, skip the missed dose and go back to your normal time. • Do not use 2 doses at the same time or extra doses. • Many times this drug is used on an as needed basis. Do not use more often than told by the doctor. Some common side effects with Combivent Respimat include bronchitis and upper respiratory tract infections. Some less common side effects include chest pain, dizziness, fatigue, insomnia, diarrhea, nausea, and vomiting. The best way to store Combivent Respimat: • Store at room temperature. Do not store in the bathroom or refrigerator. • Protect from light • Throw away after 3 months from first use or after the most number of sprays have been used, whichever comes first, even if it feels like there is more drug in the canister. • Keep all drugs in a safe place. Keep all drugs out of the reach of children and pets. • Check with your pharmacist about how to throw out unused drugs. When taking Combivent Respimat, remember it is important to tell your doctor or pharmacist if you have: • Signs of an allergic reaction, like rash; hives; itching; blistered, or peeling skin with or without fever • Signs of high blood pressure like very bad headache or dizziness, passing out, or change in eyesight • A heartbeat that does not feel normal • Very nervous and excitable • Seeing halos or bright colors around lights

Transcript: Did you know that 9 out of 10 cases of bronchitis are caused by a virus rather than bacteria? Hi, I’m Dr. Amit Desai in partnership with Washington State Department of Health, I’d like to talk to you about bronchitis or what is commonly called a chest cold. The most important information for you to remember today is that most bronchitis does not need antibiotics. This is because most cases of bronchitis are viral, and antibiotics are ineffective against viruses. Most symptoms of acute bronchitis last for up to two weeks, but the cough can last up to eight weeks in some people. Common signs and symptoms of bronchitis include: • a cough that produces mucus • soreness in the chest • being unusually tired • mild headaches • mild body aches • low grade fever (usually less than 101 F) • watery eyes • sore throat Bronchitis can certainly be uncomfortable, so here are some ideas to help ease symptoms: get lots of rest and drink lots of warm fluids; over the counter medications such as ibuprofen and acetaminophen can be helpful to alleviate mild body aches, but be sure to check with your child’s pediatrician for correct usage before giving to a child. HELLO and welcome back to my channel today I am updating you on current health condition. If you didnt know, I had the Influenza A virus from January 19-26, January 27-30 i had no voice and was getting short of breath so i went back to the doctor on the January 31, where i was seen for chronic Laryngitis (throat swelling) and i contracted right lobe pneumonia in my third and lowest lobe. I started antibiotic and felt okay so then February 2nd i got a new antibiotic thinking the first one didnt work, so i got put on a stronger one, the next day February 3rd i went back to the URGENT care to get checked for my neck pain and ear infectiona s i couldnt hear from my left ear. I got xrays and the pneumonia spread to both of my lungs, andi was put on an even stronger antibiotic, i also have a pinched nerve in my neck and severe ear infection AND laryngitis at this point. So now February 4 i posted a update video and you guys urged me to go to emergency room but i did feel very safe and okay at home, the next day February 5 i admitted myself to the ER as i couldnt breath when dropping my son off at daycare, my Oxygen level was 78 and heart rate was 170 this was scary for me, but i got fast treatment, new xrays for pneumonia, a EKG to see if my heart was okay, 3 breathing treatments, steroids for my lungs and 2 IV fluid bags, my heart was racing because i was again dehydrated. I was released from the hospital at 2pm after a 6 hour stay and went to see my Primary care doctor who prescribed me nebulizer albuterol to help me breath. I am not on a mend and thankfull for the ER TRIP i feel better. I am secluded at my house only allowed to drop my son off and pick him up from daycare, and focussing on my breathing and resting. thank you so much for your love and prayers. HELLO and welcome back to my channel today we are talking about my time with the Influenza A h3n2 virus + Double Pneumonia. I have now spent $300+ on medicines, $300+ on doctors copays and who knows what for an Emergency room visit....all from the flu and its complications in the month of jan/feb 2018 I actually wrote his for my health care policy homework but I find it important to share my story and urge even HEALTHY young adults to get help ASAP..... I found an interesting article in regards to the H3N2 Influenza virus however prior to discussing the article I want to give you a background summary of why this touches so close to home for me. SHARE THIS with your friends:... January 19-26 2018 I had the H3N2 Influenza virus. During this time I felt as though my chest was caving in, I was throwing up and had uncontrollable gastrointestinal issues. I first saw my doctor for the FLU and was diagnosed January 21st just two short days after it started, as I felt like I was going to pass out at this same time my 3 year old got the H3N2 Influenza as well and we were a pair of very very sick people. January 27-30 flu symptoms subsided but i had no voice and was getting short of breath so i went back to the doctor again on the 31, where i was seen for chronic Laryngitis and I was diagnosed with lower right lobe pneumonia. I started antibiotic and felt okay but I couldn’t stop coughing and couldn’t catch my breath this went on for a few days February 3rd i went back to the URGENT care to get checked for severe neck pain and ear infection as i couldn’t hear from my left ear and at this point I felt so sick that I was scared. I got x-rays again and the pneumonia spread to both of my lungs, and i was put on an even stronger antibiotic (Levaquin), i also have a pinched nerve in my neck and severe ear infection AND laryngitis at this point. February 5 i admitted myself to the ER as i couldn’t breath when dropping my son off at daycare, my Oxygen level was alarmingly low and heart rate was 170 this was scary for me, but i got fast treatment, new xrays for pneumonia, a EKG to see if my heart was okay, 3 breathing treatments, steroids for my lungs and 2 IV fluid bags, my heart was racing because i was again dehydrated. I was released from the hospital at 2pm after a 6 hour stay and went to see my Primary care doctor who prescribed me nebulizer albuterol to help me breath. February 7, I am on the mend however my three year old son starts showing signs of coughing. I take him in on February 9th and I was scared that he might of contracted Pneumonia from me. Sure enough he had single lung pneumonia and was put on antibiotics, the next day February 8 my son couldn’t stop throwing up and coughing, I called nurse on call and they sent us straight to Urgent care where his pneumonia spread to both lungs, he was them placed on steroids and double antibiotics. He is finally on the mend as of February 13 2018. Now that I have “aired my garbage” in a sense, you might be asking yourself why I am sharing personal information with you all. I am sharing this because I want people to know the importance of catching Flu symptoms as soon as possible. In the article linked below you will read about the severity of the Flu outbreak in Wisconsin. I am unsure if Minnesota’s flu season has been severe, however I live in Madison Wisconsin and people are dropping like flies. Now I must add that I got my flu shot, my son did not get a flu shot and I regret that decision. My son and I have been sick for a MONTH, and we are just now on the mend. One thing I disagree with in this article is the author states “if you’re generally healthy and you get the flu, you may only need to see a doctor if your fever goes higher, your cough gets worse or you have trouble breathing” The reason I disagree with this statement is because I am a “generally” healthy individual who doesn’t get sick often, my immune system is strong and I tend to bounce back from general colds and coughs quite quickly. I am a 26 year old young and healthy adult and I was taken out for a MONTH with the flu, and if I hadn’t been on top of my health I could have if people are just staying home and “Waiting it out” they can cause themselves more harm than good. Children and elderly are more susceptible to secondary illnesses due to weak immune systems however healthy adults are coming down fast with the illnesses as well because their immune system hasn’t been subject to this strand of the flu yet. This NCLEX pharmacology review question will test your nursing knowledge about how to properly administer a bronchodilator and corticosteroid metered-dose inhaler. To answer this question correctly, you must know the mechanism of action of both the bronchodilator (Albuterol) and corticosteroid (Fluticasone). What is Albuterol? It is a short-acting bronchodilator. A bronchodilator (just as its name says) opens the bronchi and bronchioles of the lungs. When this happens more air is able to flow through the lungs. This is vital for the patient who has asthma because they experience bronchoconstriction. What is a Fluticasone? It is a corticosteroid used to prevent symptoms in patients with asthma. It will work by decreasing inflammation in the small airways. To administer these medications, the bronchodilator will be administered FIRST and then 5 minutes later the corticosteroids will be administered. The bronchodilator will open the airway first so when the corticosteroids are inhaled they can do their job within the lungs. It is important to remember that AFTER administering inhaled corticosteroids, the patient must gargle and rinse the mouth with water and spit (or a spacer can be used). The reason for this is because corticosteroids can cause thrush.
