What will the doctor do at my first prenatal appointment?

A lot of women want to know what is going to happen at the first prenatal appointment. And it's a very important appointment that usually happens between 8 weeks and 12 weeks gestation. The first thing they're going to do is have you take another pregnancy test to verify that you are indeed pregnant. And then they're going to establish a due date. This is probably going to be based on your last menstrual period, if you were regular. And if you were irregular and have no idea where you were in your cycles, then they might do an ultrasound to determine how big your baby is at that point. And if an ultrasound is done early in the pregnancy, then it's a pretty reliable factor in determining how far along you are. You can expect the doctor to ask you a lot of questions about your health history, about your family's health history, any genetic problems that might run in the family, if you've had surgery before, if you're taking medication, and if you've been pregnant before. They'll review that history, and that helps them determine if there's things they need to watch you for during the pregnancy, like things you're at greater risk for. They'll also have you do a urine test, and they'll draw your blood, and they're looking for additional risk factors that might need to be monitored throughout the pregnancy. If you're over 12 weeks, then the doctor will try to hear the baby's heartbeat using a doppler. Before then, it's hard to hear, so if the doctor tries and can't hear it, then that's okay. It doesn't mean that there is no heartbeat, it's just hard to hear until that point in pregnancy. This appointment is also an opportunity for you to ask questions, and for the doctor to educate you on topics like safe seatbelt use, exercise, weight-gain during pregnancy, things to avoid - like if you're using alcohol, drugs or tobacco, you should stop. You should think twice about everything you put in your mouth really, like over-the-counter medications. They'll review your prescriptions with you and make sure that they're all safe, and talk to you about thinking twice about even herbal supplements. You should always make sure those things are safe to take while you're pregnant before you take them. They'll also discuss typical discomforts of pregnancy like nausea, vomiting, fatigue, reflux, all of those things, and discuss different ways you can overcome them or combat them during pregnancy. They'll talk to you about things you can expect to be normal, things that are abnormal, reasons to get checked out, and of course, come with questions that you've been thinking about. So if you have them, write them down and pull them out at your appointment, because doctors always say, "Do you have any questions?", and usually our minds are just blank, and we say "no", we walk out, and we think, "Oh, there's a million things that I did want to know." So write down your questions, bring them to your appointment, and make sure you've had all of your questions answered before you leave. Remember 13 by 13 - see the doctor by 13 weeks gestation at the latest, and plan on about 13 weeks over the course of the pregnancy, possibly more if you need additional monitoring. Prenatal care is so important to your well-being and the well-being of your baby. Now there's a lot that goes into conception. It's a miracle that it happens and goes well so frequently. It's all about timing. A woman is able to get pregnant for 6 days out of each cycle (5 days leading up to ovulation and on the day of ovulation), and you have to time intercourse with your fertile window in order for pregnancy to be possible. Even when things are timed just right though, the chances of getting pregnant are only 30% to 35%, so that means that you might have to try for a couple of months, because even when things are timed just right, the chance isn't 100%. So on average, most couples have to try for up to 6 months. If you've been trying for just a few months, then don't get frustrated. It's okay. And chances are that you will get pregnant in the next few months, so keep trying. Once you've been trying for a year or more though, then it's time to talk with your doctor about underlying causes that may be leading to infertility. About 50% of the time, it has to do with the woman's reproductive system. And in most cases, it comes down to the fact that she's not ovulating or the fallopian tubes are blocked, and there are treatments for both of those things. In 35% of cases, it comes down to the male's reproductive system. And there's either a low sperm count, or they don't swim well, or maybe they're abnormally shaped, and that's all leading to infertility. The supplements that I mentioned earlier don't help with those problems, so again, you have to get to the underlying cause, and the treatment will depend on what they think the cause is. So if you've been trying for a year or more, talk with your doctor about it. And after asking you and your partner more specific questions about your history and your lifestyle, they'll decide if further investigation or treatments are necessary in order for you to achieve your dreams of having a baby. As for herbal supplements and over-the-counter remedies that claim to increase fertility, as long as they're not going to hurt anything, then your doctor may go ahead and say, "Yeah, it's worth a shot." But there's insufficient evidence to prove at this time that they're helpful. That's so exciting that you're thinking about having a baby and in preparation to get pregnant there are a few things that you can do to help increase the chances that both you and your baby will be healthy during the pregnancy. You mentioned taking prenatal vitamins and that is very important. If you're ever having unprotected intercourse or there is a chance that you could get pregnant it's best to be taking a prenatal vitamin with at least 400 micrograms of folic acid in it. This will help prevent things such as neural tube defects like spina bifida. It's also good to exercise and get active now because if you're active now you can continue with that activity during the pregnancy in most cases, unless you have complications down the road. Once you get pregnant it's not the time to start an exercise routine. So start now and if you're not at a healthy weight, you could be underweight or overweight and both have complications that are associated with them. Talk to your doctor about what weight you should be at and try to do what you can to get within a healthy range before you get pregnant. For women who are overweight you have an increased risk of things such as gestational diabetes and pre-eclampsia. There are other conditions that can affect the baby as well. Overall it can make your labor, delivery, and recovery a little more difficult if you're overweight. It's best, again, to be at a normal weight. There is also something to be said about medications in general. If you're taking any prescription medications talk to your OB provider about those that are safe to take while pregnant and sometimes there are things you need to be taken off of as soon as you find out you're pregnant. Also before taking any over-the-counter medications make sure to consult with your OB provider because there is that small window of time before you might know you're pregnant but you could be pregnant and there are certain medications that should not be taken at that time.
