Colostomy and ileostomy nursing care video for the NCLEX exam. Learn about the types of gastrointestinal ostomies, such as the colostomy and ileostomy. The types of colostomies include the ascending colostomy, transverse colostomy, double-barrel colostomy, descending colostomy, and sigmoid colostomy. An ostomy is a surgical opening created on the surface of the abdomen to allow stool (waste) to exit the body rather than through the rectum. Gastrointestinal ostomies will be formed from the small or large intestine. A colostomy is a surgical opening created to bring the LARGE intestine (also called colon) to the surface of the abdomen. An ileostomy is an opening created to bring the SMALL intestine to the surface of the abdomen (specifically the ileum). As a nurse providing care to a patient with a GI ostomy, it is important to know the pre-opt/post-opt care along with diet, medication restrictions (no enteric-coated and sustained-released medications) for the ileostomy, the different types of ostomies, stoma care, and the stool consistency expected from the ostomy. Depending on the type of ostomy and its location, the patient should have the following stool types: Ascending: liquid stool, Transverse: lose to partly formed stool, and Descending/Sigmoid: similar to normal consistency, and ileostomy: liquid stool. New nurse time management tips and advice: I will discuss some new nurse tips and tricks for time management you can use as a new nurse. Many new nurses struggle with time management issues during the first six months on the job. The reason is because most new nurse graduates are trying to develop a nursing routine that works for them while trying to learn how to be a nurse. It can be overwhelming for the new nurse and they may find themselves feeling like you don't have enough time to get their job done. Some signs and symptoms of time management issues can include: rushing to get everything done at the end of the shift, forgetting to complete tasks, clocking out late all the time, never getting breaks, always needing help, and feeling so overwhelmed with the job that you want to quit. Some tips to help you become better with time management include: asking the right questions in report, creating a sheet to help you keep track of your day, set goals of when things should be completed, try to streamline medication administration times, take it hour-by-hour and don't think about it all at once, and know that experience helps with time management. Stroke nursing (CVA) cerebrovascular accident lecture on ischemic and hemorrhagic strokes along with nursing care, tPA, symptoms, treatments, and pathophysiology. A stroke occurs when blood rich in oxygen can NOT reach brain cells (due to either a blockage or bleeding) and the cells begin to die. Types of stroke: Ischemic (most common) is due to a blood clot within the blood vessel or stenosis of an artery feeding the brain tissue. This limits the blood that can reach the brain cells. This can happen due to: -Embolism: where a clot has left a part of the body (example the clot has left the heart due to a heart valve problem or atrial fibrillation) and has travelled to the brain, which stops blood flow. -Thrombosis: a clot forms within the artery wall within the neck or brain. This is seen in patients with hyperlipidemia and atherosclerosis Hemorrhagic: there is bleeding in the brain due to a break in a blood vessel. There will be limited blood flow to the brain cells and this can cause excessive swelling in the brain from the leakage of blood in the brain tissue. A ruptured brain aneurysm or uncontrolled hypertension can cause this. In addition, a patient can experience a TIA (transient ischemic attack) also called a mini-stroke. This is where signs and symptoms of a stroke last only a few minutes to hours and resolve. It is a warning signs an impending stroke may occur. Stroke symptoms include: facial drooping, arm numbness or weakness, slurred speech or difficulty talking etc. Treatment for stroke (specifically an ischemic type) is tPA (tissue plasminogen activator). This medication works to dissolve the clot that has caused th decreased blood flow. However, tPA must be administered within 3 hours of the onset of stroke symptoms. Nursing care for stroke includes: monitoring vitals (especially blood pressure, respiratory status), watching and educating the patient with neglect syndrome (this most likely occurs with patients who've had a right brain injury), diet and swallowing techniques, hemianopsia, and how to communicate with a patient with aphasia (watch the video for more in depth nursing interventions for stroke). NCLEX practice question on how to answer a NCLEX priority question that deals with requiring immediate nursing action. This video is part of a weekly NCLEX review series where I will be going over NCLEX style questions with you. I will be helping you analyze each question, and walk you through how to select the correct option. NCLEX questions require critical thinking and you must know how to use your nursing knowledge to gather the facts and analyze what the question is asking. This specific question is a type of question that wants to know based on your morning assessment findings which finding is abnormal and requires immediate nursing action. NCLEX Question: You have just received nursing report from the previous shift and are performing your morning patient assessments. You have a total of 4 patients that are either post-opt or pre-opt for surgery. Which assessment finding requires further nursing action? A. Acute kidney injury (also called acute renal failure) nursing NCLEX review lecture on the nursing management, stages, pathophysiology, and causes (prerenal, intrarenal, postrenal). What is Acute Kidney Injury? It is the SUDDEN decrease in renal function that leads to the build up of waste in the blood, fluid overload, and electrolyte imbalances. What are the causes of Acute Kidney Injury? There are three causes, which are based on location. The first is known as prerenal injury and this is an issue with the perfusion to the kidneys that leads to decreased renal function. A second cause is known as intrarenal injury, and this is due to damage to the nephrons of the kidney. Lastly, postrenal injury is due to a blockage located in the urinary tract after the kidney that can extend to the urethra. This is causing the back flow of urine, which increases the pressure and waste in the kidneys. Stages of Acute Kidney Injury: There are four stage of acute kidney injury, which include initiation, oliguric, diuresis, and recovery stage. The initiation stage starts when a cause creates an injury to the kidney and then signs and symptoms start to appear. This leads to the oliguric stages. The patient will void less than 400 mL/day of urine during this stage and will experience increased BUN/creatinine levels, azotemia, hyperkalemia, hypervolemia, increase phosphate and decreased calcium levels along with metabolic acidosis. After this stage the patient can progress to the diuresis stage and this is where the patient will void 3-6 Liters of urine per day due to osmotic diuresis. The patient is at risk for hypokalemia, dehydration, and hypovolemia. The last stage is recovery and the patient's glomerular filtration rate has returned to normal. Therefore, the kidneys are able to maintain normal BUN and creatinine levels, electrolyte, and water levels. Surgical suture (stitches) removal is a common nursing skill that you will perform for patients who have received sutures due to an injury or surgery. People sometimes ask if nurses remove sutures. The answer is yes, I've removed stitches many times. This nursing skill video will demonstrate the following: -How to remove surgical sutures (stitches). This is demonstrated on a simple interrupted suture type. -Supplies used to remove sutures. -How to open supplies and don sterile gloves -Common types of sutures you'll encounter as a nurse, including simple interrupted sutures, vertical mattress sutures, horizontal mattress sutures, simple continuous sutures, continuous blanket sutures (also called Ford interlocking), and interrupted cruciate (cross-shaped sutures). -Examining the patient's wound for signs of infection. -The importance of properly disposing of sutures and completing your nursing documentation. You'll always want to confirm that the doctor has ordered the suture removal, verify the patient, obtain consent, and explain the procedure. Suture removal is typically not painful, but in some cases, an order for pain medication may be available for the patient. This NCLEX review question will test your nursing knowledge on how to educate a patient about decreasing signs and symptoms experienced in dumping syndrome. What is dumping syndrome? It is a condition where food from the stomach enters too rapidly into the small intestine. Seizures (epilepsy) in nursing - NCLEX review: learn about generalized vs focal (also called partial) seizures, as well as the different types and stages of seizures. Seizures occur when abnormal electrical signals are fired by neurons in the brain. Generalized seizure types can include tonic-clonic seizures (formerly known as grand mal seizures), petit mal seizures, absence seizures, myoclonic seizures, tonic seizures, and clonic seizures. Focal seizure types (formerly called partial seizures) can include the following: focal onset aware seizures (also called simple partial seizures) and focal impaired awareness (also called complex partial seizures). Seizures can occurs in anyone (children and adults) due to a severe acute condition, such as a high fever, illness (especially central nervous system types), hypoglycemia, acidosis, alcohol withdraw etc. Once the condition is corrected the seizures tend to stop. What is epilepsy? Epilepsy is when a patient experiences frequent seizure activity due to a chronic condition. This can be from a massive stroke, traumatic brain injury, congenital defect, effects of a central nervous system infection etc. Types of generalized seizures: Tonic-clonic (formerly called grand-mal): the patient loses consciousness and the patient may experience an aura (warning sign) before the seizure. Then the patient will experience: tonic episode: body stiffens (may bite the inside of the cheek or tongue, cry out, and experience apnea), and this is followed by the clonic episode, which is recurrent jerking of the extremities. The patient may have incontinence as well. The post ictus stage can take hours to days, and the patient may report feeling very tired, sore, have a headache etc. Absence (petit-mal): most common in children. Hallmark is staring (patient may appear to be daydreaming). This type of seizure is very short and may go undetected by others. The post ictus stage is immediate recovery. Atonic (drop attacks): this is complete loss of muscle tone. The patient will suddenly lose muscle tone and is at risk for head injury. Patients should be advised to wear a helmet with this type of seizure. Myoclonic: this is jerking of the muscles. The patient is aware of their surrounding during this type of seizure. In addition, patients can experience just a tonic or clonic seizure. Focal (partial) seizures: Focal Onset Aware (simple partial): the patient is AWARE of their surroundings during the seizure. The signs and symptoms vary due to the specific location of the brain being affected. For instance, some patients may experience vision changes if the occipital lobe is being affected. The seizure is very short and the post ictus stage is immediate. This is sometimes referred to as an aura, and can happen prior to a complex partial seizure (focal impaired awareness). Focal Impaired Awareness (complex partial): this is different from the other type of focal seizure because the patient is NOT aware of their surroundings and will experience motor symptoms (automatisms). Automatisms are movements performed by the patient that they are don't know they are performing, such as hand rubbing, smacking of the lips, grasping for an object that isn't there. Burns nursing care NCLEX review lecture covers burns treatment, pathophysiology, nursing interventions, degrees of burns (1st, 2nd, 3rd, and 4th degree burns with affected skin areas), circumferential burns, different causes and types of burns, and more. What are burns? Burns occur when there is damage to the skin integrity due to some type of energy source. Burns can be caused by various sources such as: -Heat (thermal): hot liquids, steam, fire etc. that comes into contact with the skin…most common type. -Electrical: electric current passes through the body and damages tissues (watch for iceberg effect), heart dysrhythmias, bone fractures (cervical spine injuries) and acute tubular necrosis. -Chemical: toxic substances come into contact with the skin (powders, gases, or certain foods). -Cold: coldness comes into contact with the skin for too long: frostbite -Radiation: sun, treatments for cancer -Friction: force abrasion to the skin….car accident…road rash, rope burn Burn severity depends on the depth of damage to the skin (burn degree), percentage of the total body surface area (rule of nines) that is burned, patient’s age, medial history, location of the burn, and did the patient experience an inhalation injury? -1st Degree (superficial): affects the top layer of the skin “epidermis". The skin will be very red or pink, painful, warm to touch, and has no blisters. Capillary refill is brisk. -2nd degree can be superficial or deep depending on how far the damage occurred in the dermis (partial-thickness). These burns involve the epidermis and dermis. The burn will have redness that will blanch and be shiny red/pink and moist in areas. -3 to 4th degree (full-thickness): for these type of burns watch for eschar (hard dead tissue that can restriction blood flow and respiratory effort) and for acute tubular necrosis because myoglobin and hemoglobin can be released into circulation and go to the kidneys and block perfusion, which leads to acute kidney injury. -3rd degree burns affect all skin layers to the subcutaneous fat (hypodermis). The burn can be yellow, red, and black. It will appear dry rather than moist/shiny. Pain sensation is gone or decreased. Skin grafting is required. -4th degree (deep full-thickness): All skin layers are affected along with the muscle, bones, and ligaments. Burn will appear charred and black. No sensation is present. It is important to be aware of the burns location and if the burn is a circumferential burn. This type of burn circles around an area of the body.
