Safety Management System (SMS) for Small Operators

Original Release Date: January 2013 The Federal Aviation Administration (FAA) is always working to take the U.S. aviation system to the next level of safety. Safety Management Systems (SMS) are beginning to play a key role in making an extraordinarily safe system even safer. With SMS, the FAA and the aviation industry can examine data from everyday operations to isolate trends that may be precursors to incidents or accidents. What is a Safety Management System? Safety Management Systems for the FAA, air carriers, airports and other certificate holders integrate modern safety risk management and safety assurance concepts into standardized, proactive systems. In SMS, safety management is a fundamental process on a par with all other management processes. An SMS has four key pieces: Policy An SMS presumes there will always be risks in aviation that must be found, and to the greatest extent possible, controlled. To do that, it's critical to have a strong safety policy in place that provides a detailed account of top management's objectives and expectations for safety. It also sets up a framework for plans, organizational structures and accountability for all employees. Risk management An SMS uses safety risk management in all activities, including training, maintenance, dispatch and operations. It identifies hazards by drawing data from sources such as operations analysis, employee reporting systems, incident reports and audits. The next step is gauging how likely the hazard is to result in an incident and the potential consequences. Finally, risk management determines how much, if any, risk is acceptable. Armed with this information, managers can develop ways to implement and track risk controls. Safety assurance Making sure an SMS remains effective after risk controls are in place is critical. The safety assurance process analyzes data and reviews decision-making not only to make sure risk control strategies are still working, but also to highlight ways to improve or correct those methods. Promotion An SMS must promote safety and establish a safety culture. It must include processes for training people in safety-critical areas, and effective two-way communications channels between managers and employees on safety matters. More Than Compliance The FAA is moving to implement SMS and system safety-based oversight. The agency issued an Advance Notice of Proposed Rulemaking (ANPRM) on SMS in 2009 that asked for public input on questions such as who should be required to implement an SMS, what type of records and documents should be kept, what regulations may already cover SMS and any concerns about making sure such systems are effective. Subsequently, an Aviation Rulemaking Committee, an industry advisory group the FAA chartered to provide recommendations, reviewed the comments resulting from the ANPRM. In its March 2010 report, the committee recommended that the agency proceed to implement SMS -- with appropriate protections -- for most segments of aviation. On October 7, 2010, the FAA issued a proposed rule that would require airports certificated under Part 139 to establish SMS for all airfield and ramp areas. Congressional action has mandated that the FAA develop a rule requiring all Part 121 operators to implement an SMS. The legislation directs the agency to publish a final rule by August 2012. The proposed rule was published in November 2010. The FAA is considering SMS regulations for other groups of aviation service providers, including Part 135 operators and Part 145 repair stations. The agency is sponsoring voluntary SMS implementation by Part 121, 135, and 145 organizations to provide learning and experience for both industry and the FAA in SMS development, implementation and oversight

Safety incident reporting and investigation - Digitised and simplified using Dashpivot

Safety incident reporting and investigation is critical for construction and industrial companies, who work in inherently dangerous conditions and environments filled with potential hazards. But managing safety incidents can be an admin and organisation nightmare - from managing incident notification and reporting on site - to organising and managing these incidents in a way which makes sending them to the right people, actioning them, investigating them and ultimately understanding what can be improved possible. Incidents are an unfortunate part of working on complex projects, and although we hope that they never happen, from time to time they do. So in this video we're going to look at how incident workflows in Dashpivot can track incidents and notify the right people when they happen so that safety and compliance issues never get missed and all of your safety incidents are recorded and actioned where required. So I'll get stuck in, so we've already set up an incident template inside of Dashpivot, it's been set up as a workflow which means that if we add a new incident it's first of all going to sit in this first column right here and then as it's signed off by the relevant person; in this case the engineer, then the SP, and then the PM, it's going to move from the first to the second to the third and so on moving from this side of the page to this side of the page through all of the columns, so I'm going to get stuck in. So if I go "add incident", you see that we've already got a template set up, I'm gonna fill in a couple of these fields, maybe there was a near miss at compound entry for example, time and date of incident, let's say it was this morning, maybe it was at 7:00 o'clock this morning, the time it was reported half hour after that, and then we've got a series of checks so we can go through and and fill those out, once we're happy with all of the information which has been added, for example, we can we can add some photos if we need to and on the phone that's really easy to do, you just open it up as you're filling out the form, you can click add photo, take a photo there and then while you're in the field and then that will get attached to this form, and once you're ready, once you're happy rather, with all of the information that's in here we can click "Save" and you'll see that it's now sitting inside this first column and now of course it needs to get signed off by the engineer in order to move to the next column, so because we've got four columns set up, we actually have three of what we call 'approval signatures' that's what these guys are, it says sign to move this incident to the next column so it's very clear about what we need to do in order to move it across, if I click sign, that now has stamped my signature and my name and time and date there, we can save it and you'll see that it's now moved to this second column, and if I open it up again you might have noticed that the the second signature is now enabled, and of course the third one is is disabled just yet because we haven't signed the second one but that's basically how we move it from one column to the next; just a quick note on this, one of the great things about having it in Dashpivot is that instead of filling out a word doc or an Excel spreadsheet, or writing it on a bit of paper, and then scanning it in and emailing it to someone back and forth, you're avoiding all of that and things aren't getting lost and not actioned because it's all sitting inside of Dashpivot (character limit)

This public workshop, convened under a cooperative agreement with the Food and Drug Administration, is being held in response to public comments received on the draft guidance “Safety Assessment for IND Safety Reporting” issued in December 2015 requesting a public meeting to discuss the draft guidance and its implications. Broadly, this workshop will provide an opportunity for the stakeholder community to continue discussions around key challenges related to IND reporting practices proposed in the guidance. Sessions will highlight industry experience implementing these approaches, and discussion topics will include but not limited to safety assessment and aggregate analysis, as well as potential best practices for unblinding of data and trial integrity, identifying expected/anticipated events, and determining reporting thresholds.

Now on this particular system here, we've got a bypass valve that's effectively a gate valve. So that means that you set it at one point, and then leave it for the rest of it's life. But you can also get automatic bypass valves that are a lot better because they're a lot more efficient for the heating system, but for the purposes of this video, to see where they fit in on a heating system, this particular layout that we've got behind us here is just fine. So if you come over here and have a closer look, I'll give you an idea about what bypass valves do, where they sit in the heating system, and then we'll go back to the white board to show you this lot all laid out schematically. So all right, on this system here, we've got our flow coming up from the boiler here. It goes through the pump, and the up. We've got an automatic air vent up there so that just vents the pump out. And it comes back down here, and this is a S-plan system. At this point here, it's really important.

Six measures launched to improve train safety

Today the Taiwan Railways Administration announced six measures designed to improve train safety in the wake of a deadly rail accident last month. In a press conference led by Transportation Minister Wu Hong-mo , officials said there would be an overhaul of the speed-regulating ATP system on express trains. They promised that all the upgrades will be complete by the end of next year. Until then, two drivers will be sent out on every Puyuma express to ensure safety.The transport minister and TRA officials held a press conference today to report on their latest action plan for improving train safety. Chang Cheng-yuanTRA Director-GeneralPuyumas, Tarokos, and the Tze Chiang push-pull trains on the West Coast Main Line will be optimized by the end of next year. As for the two-driver system, until all speed limiters are installed, we will keep two drivers on duty.Transport authorities said they would carry out six measures focusing on Puyuma trains’ speed regulation systems, air compressors, and other safety features. Two drivers will be at the control panel on every Puyuma train trip until the safety upgrade is complete at the end of next year.

Understanding "under-reporting" in occupational health and safety, November 14, 2012

Under-reporting of occupational health problems can have significant consequences including under-estimation of the occupational injury and disease burden, sub-optimal prevention and treatment, reduced access for workers to compensation, and displacement of costs from the compensation system to the public health system. But to date we do not have sufficient understanding of why and how under-reporting occurs. This plenary discusses a project that took this issue on and set the stage for future research investigation by examining key concepts, stakeholders' language and logic, new potential units of analysis and determinants, and the socio-politics of doing research in this area.
