Safety Management System (SMS) for Small Operators

This presentation was developed by the FAA SMS Program Office (AFS-900). Original Release Date: January 2013 The Federal Aviation Administration (FAA) is always working to take the U.S. aviation system to the next level of safety. Safety Management Systems (SMS) are beginning to play a key role in making an extraordinarily safe system even safer. With SMS, the FAA and the aviation industry can examine data from everyday operations to isolate trends that may be precursors to incidents or accidents. What is a Safety Management System? Safety Management Systems for the FAA, air carriers, airports and other certificate holders integrate modern safety risk management and safety assurance concepts into standardized, proactive systems. In SMS, safety management is a fundamental process on a par with all other management processes. An SMS has four key pieces: Policy An SMS presumes there will always be risks in aviation that must be found, and to the greatest extent possible, controlled. To do that, it's critical to have a strong safety policy in place that provides a detailed account of top management's objectives and expectations for safety. It also sets up a framework for plans, organizational structures and accountability for all employees. Risk management An SMS uses safety risk management in all activities, including training, maintenance, dispatch and operations. It identifies hazards by drawing data from sources such as operations analysis, employee reporting systems, incident reports and audits. The next step is gauging how likely the hazard is to result in an incident and the potential consequences. Finally, risk management determines how much, if any, risk is acceptable. Armed with this information, managers can develop ways to implement and track risk controls. Safety assurance Making sure an SMS remains effective after risk controls are in place is critical. The safety assurance process analyzes data and reviews decision-making not only to make sure risk control strategies are still working, but also to highlight ways to improve or correct those methods. Promotion An SMS must promote safety and establish a safety culture. It must include processes for training people in safety-critical areas, and effective two-way communications channels between managers and employees on safety matters. More Than Compliance The FAA is moving to implement SMS and system safety-based oversight. The agency issued an Advance Notice of Proposed Rulemaking (ANPRM) on SMS in 2009 that asked for public input on questions such as who should be required to implement an SMS, what type of records and documents should be kept, what regulations may already cover SMS and any concerns about making sure such systems are effective. Subsequently, an Aviation Rulemaking Committee, an industry advisory group the FAA chartered to provide recommendations, reviewed the comments resulting from the ANPRM. In its March 2010 report, the committee recommended that the agency proceed to implement SMS -- with appropriate protections -- for most segments of aviation. On October 7, 2010, the FAA issued a proposed rule that would require airports certificated under Part 139 to establish SMS for all airfield and ramp areas. Congressional action has mandated that the FAA develop a rule requiring all Part 121 operators to implement an SMS. The legislation directs the agency to publish a final rule by August 2012. The proposed rule was published in November 2010. The FAA is considering SMS regulations for other groups of aviation service providers, including Part 135 operators and Part 145 repair stations. The agency is sponsoring voluntary SMS implementation by Part 121, 135, and 145 organizations to provide learning and experience for both industry and the FAA in SMS development, implementation and oversight

Safety Risk Assessments Are prepared to identify and evaluate risks, to adopt control measures to reduce or eliminate risks. Shown is a Risk assessment for excavation. COSHH Sheet The purpose of the Control Of Substances Hazardous to Health sheet is to identify how to prevent or adequately control workers exposure to hazardous substances. Permit to Work. Is a management system used to control high risk activities by ensuring safety measures are in place prior to starting work. Types of work for which permits will be required include: Excavation and the digging of trenches. Working at height, including on roofs. Working in confined spaces, for example entering a manhole. Hot work, which is welding, soldering or cutting steel with angle grinders or by gas torch.

Developing a Biological Safety Evaluation

The three main steps in developing a Biological Safety Evaluation (BSE) are 1) Biological Evaluation Plan (BEP), 2) Perform Testing and Written Risk Assessments, and 3) Biological Evaluation Report (BER). A BEP is developed based on risk of the device to the patient. With this in mind, several factors need to be considered to ensure that unnecessary testing is avoided. A useful testing plan will address the device as a whole, starting with materials and processes and ending with patient exposure (type and duration). An understanding of testing options is crucial as the approach to determining the safety of a device is moving away from check box based decisions to scientific justification for when testing is necessary and when it is not. This will include a discussion about changes in the regulatory world that include an important use for chemistry testing. As BSEs become more complex, a summary report is essential to draw the right conclusions from the data submitted to support the overall safety of a medical device. A written assessment is requested in ISO 10993-1 as well as in the FDA guidance document. We will go over the information needed and the important points that should be included in the report. This webinar will also include discussion of how to assess biocompatibility of device and process changes. Composed of a 50 minute presentation followed by a Q&A session, the webinar will cover the broad scope of developing a BSE. This webinar will be based on ISO 10993-1 and the FDA Guidance document on ISO 10093-1 (effective September 2016).

So and so a chemical, physical or a biological agent poses a risk; This or that product is a hazard or behaving a certain way is unsafe. A lot of the time “hazard” and “risk” are freely used to mean the same thing. However they are not. Hazard is the potential to cause harm. Risk on the other hand is the likelihood of harm in defined circumstances. In a nutshell, hazard and risk are not always aligned as the risk depends on the exposure to the hazard ; similarly, risk, safety and perception of risk are not always aligned; this can make political decisions regarding safety levels difficult to make, in these cases it is especially important to base them on facts rather than on subjective perceptions. http://www.greenfacts.org

HAZARD REPORTING SOLUTION The hazard reporting tools are near the core of the Hazard Reporting solution. Hazard reporting with SMS Pro can take anywhere from 15 seconds to 4 minutes. There are 7 ways to report issues, all of which are configurable: • Standard hazard reporting • Quick reporting, which can be done in as little as 15 seconds • Email reporting • Public Issue reporting • Offline reporting • Audit Issues • Gap Analysis To submit a safety hazard report, you will go to the Submit New Issue module. First, note how fast a safety issue can be generated with the Quick Report. Let’s say there has been a security breach somewhere near the runway, and it needs to be reported immediately. We would click on quick report. Here we only need to give: • A Title • A Summary If we would like, we can also add a severity and initial actions taken. In this case I will mark the issue as critical and mention that security has been notified, and then submit by clicking finish. As you can see this is an extremely quick way to submit safety issues. After we submit the issue, it will appear in the issue manager, and safety managers will be notified. Furthermore, because this issue was marked as a critical issue, all employees using SMS Pro will be notified about the issue because we have configured it this way. Now let’s go over a standard issue report, using our same security breech example from earlier. Most of the options in the standard issue report are fully configurable by admin users. First, you will have a configurable map interface to mark where the issue occurred. You can select multiple locations, and drag the issue locations where you need. (WTB 2 locations) We then save our markers, and click next. Next we will fill out about the issue. Reporters have the ability to make the issue: • Public, for others users to see, or • Private for only management to see. • They can submit the issue anonymously, whereby the issue will be seen as reported by an anonymous user. • They can also submit issues as being confidential, where only one single user will be able to see the issue, even if the issue is check as being public. Admin users configure which user sees confidential issue. Users are able to submit multiple variations here, such as submitting a confidential issue anonymously, or a private issue anonymously. Reporters can choose to be notified when certain updates are made as the issue is managed. Next, safety issue reports can be automatically routed to outside agencies that Admin users have configured as available options, such as the FAA or Transport Canada. In the next section we can categorize our issue – all options here are fully --. • First we choose a Division that is most relevant for our issue. • We can select a base or location if applicable • As well as a vendor/client if applicable • Next we select which type of issue it is, in this case a security issue • And finally, we can select whether we like to use the General safety report, which looks the same as the Quick Report, or whether we would like to use a custom report, such as a Bird Strike or Fatigue report. On the next page we fill out the report we chose. We then have the option of giving a location, date, and time for the issue if applicable. Next we can add any people or vehicles/aircrafts involved with the issue. And finally, we can review a summary of the issue report and attach any documents. After we submit by clicking Finish, we receive two success message. The first is a success message in the lower right hand corner of the screen to let you know that the issue was submitted successfully. Next, the user will receive an email message, with a link to their issue in the Issue Manager module. Users can monitor the status of their issue in here, as well in the Safety Workspace module, which lists all submitted issue and the status of each issue.

How to Easily Monitor Employee Safety Performance

Aviation Safety Risk Management Software SMS Pro Features for Best Aviation Safety Management System we have created a fantastic module to address this, called the Employee Safety monitor Module, which allows safety managers to easily monitor any employees’ safety performance. It is designed to correct: • Poor hazard reporting culture and • Lack of employee involvement; • And encourage safety management’s participation in the SMS program. The idea is that if employees know that their safety performance is being monitored then they are significantly more likely to care about their participation. In this module safety managers can: • Track individual employee’s number of logins and reported issues within any given time frame; • Track employees’ progress in the critical areas of a program, such as on time Corrective Action completion, training, meeting, policy review, and on time issue completion. Let’s take a brief look at how easy it is to monitor performance. When you first enter the Employee Performance Monitor module, the first thing you will want to do as a manager is navigate the to the employee whose performance you would like to see, and then choose the time frame. You can see performance for any number of years, as well as a specific month range. This allows you to see long term… and recent performance. The most important things a manager will want to see are found here: • The number of reported issues and logins in the given time period, and the last login. Note that your companies averages are listed here as well, and you should be able to see right away how involved the employee is. You can also see the My Safety Score dial to the right. This dial takes into account how often the user logs in, how often they report issues, as well as the safety performance tiles below, each of which cover a critical area of safety performance, such as: • How many mandatory trainings were completed on time • Meeting attendance • How many CPAs were assigned and completed on time • How many messages were acknowledged on time • And how many assigned issues were completed on time. Only managers will see this tile because regular users cannot be assigned issues. These tiles are fantastic because you can add or remove them from the tile pane, and they will change performance scores. For example, if we did not want On Time Training to be counted for safety performance, we would click the edit icon, move that tile out, and then save. Now that this user doesn’t have a late training counting against them, their performance score improves. The performance tiles and login and reporting information are the core of this module, and give you a solid idea of any employees performance in about 5-10 seconds. Beyond this, you can also • See their CPAs and/or Issues completed on time; • Update contact information • See whether they have completed their yearly review of duties and responsibilities, policies, and the company org chart; and • Create a safety plan. Let’s say this current user was under review – I would probably create a safety plan for them to finish their yearly review of policies and the org chart and submit 5 issues before the next review. Once I save, this information will be stored for the next review. This module is not simply for mangers either. It’s important that regular users also have access to see their performance, and know how they are being evaluated and monitored. This way they will always know when they need to put in more of an effort, and come review time there are no surprises and no excuses. Regular users can quickly see their login and reporting performance, view the performance tiles that management has configured, access any reviewable documents… after which a checkmark will appear, and view the safety plan that management created. Regular users will not be able to see other users, update their safety plan, or configure performance tiles. Lastly, any users can use the employee performance monitor for quick access to important modules, such as issue submitting and assigned CPAs. The Employee Safety Monitor is designed to assess an employee’s performance as soon as possible, the benefit being that in larger organizations successfully monitoring many employees is neither time consuming nor burdensome. Moreover, because this module is available to all users, it gives employees and managers strong incentives to play a more active role in their safety program. Safety incident reporting and investigation - Digitised and simplified using Dashpivot

Safety incident reporting and investigation is critical for construction and industrial companies, who work in inherently dangerous conditions and environments filled with potential hazards. But managing safety incidents can be an admin and organisation nightmare - from managing incident notification and reporting on site - to organising and managing these incidents in a way which makes sending them to the right people, actioning them, investigating them and ultimately understanding what can be improved possible. Incidents are an unfortunate part of working on complex projects, and although we hope that they never happen, from time to time they do. So in this video we're going to look at how incident workflows in Dashpivot can track incidents and notify the right people when they happen so that safety and compliance issues never get missed and all of your safety incidents are recorded and actioned where required. So I'll get stuck in, so we've already set up an incident template inside of Dashpivot, it's been set up as a workflow which means that if we add a new incident it's first of all going to sit in this first column right here and then as it's signed off by the relevant person; in this case the engineer, then the SP, and then the PM, it's going to move from the first to the second to the third and so on moving from this side of the page to this side of the page through all of the columns, so I'm going to get stuck in. So if I go "add incident", you see that we've already got a template set up, I'm gonna fill in a couple of these fields, maybe there was a near miss at compound entry for example, time and date of incident, let's say it was this morning, maybe it was at 7:00 o'clock this morning, the time it was reported half hour after that, and then we've got a series of checks so we can go through and and fill those out, once we're happy with all of the information which has been added, for example, we can we can add some photos if we need to and on the phone that's really easy to do, you just open it up as you're filling out the form, you can click add photo, take a photo there and then while you're in the field and then that will get attached to this form, and once you're ready, once you're happy rather, with all of the information that's in here we can click "Save" and you'll see that it's now sitting inside this first column and now of course it needs to get signed off by the engineer in order to move to the next column, so because we've got four columns set up, we actually have three of what we call 'approval signatures' that's what these guys are, it says sign to move this incident to the next column so it's very clear about what we need to do in order to move it across, if I click sign, that now has stamped my signature and my name and time and date there, we can save it and you'll see that it's now moved to this second column, and if I open it up again you might have noticed that the the second signature is now enabled, and of course the third one is is disabled just yet because we haven't signed the second one but that's basically how we move it from one column to the next; just a quick note on this, one of the great things about having it in Dashpivot is that instead of filling out a word doc or an Excel spreadsheet, or writing it on a bit of paper, and then scanning it in and emailing it to someone back and forth, you're avoiding all of that and things aren't getting lost and not actioned because it's all sitting inside of Dashpivot (character limit)
