Our Video Topic: Mouth Cancer Pictures from Smoking - Oral Cancer Symptoms and Causes Mouth cancer, or oral cancer, can occur anywhere in the mouth, on the surface of the tongue, the lips, inside the cheek, in the gums, in the roof and floor of the mouth, in the tonsils, and in the salivary glands. What Are the Symptoms of Oral Cancer? The most common symptoms of oral cancer include: A sore that doesn't heal A sore that bleeds A growth, lump or thickening of the skin or lining of your mouth Loose teeth Poorly fitting dentures Tongue pain Jaw pain or stiffness Difficult or painful chewing Difficult or painful swallowing Sore throat Causes of mouth cancer Cancer results when a genetic mutation instructs cells to grow without control. Untreated, oral cancer will start in one part of the mouth, then spread to other parts of the mouth, to the head and neck, and the rest of the body. Mouth cancers typically start in the squamous cells that line the lips and the inside of the mouth. This is called squamous cell carcinoma. The exact cause of the mutations is unknown, but there is evidence that certain risk factors increase the chance of mouth cancer developing. Risk factors for mouth cancer Tobacco and alcohol use are prominent risk factors for mouth cancer. Smokers are three times more likely than nonsmokers to develop mouth cancer, and people who smoke and drink alcohol have up to 30 times higher risk than those who do not smoke and drink. Other risk factors include: Ultraviolet exposure on the lips, from the sun, sunlamps, or sunbeds A diet rich in red meat, processed meat, and fried foods Gastro-esophageal reflux disease (GERD), a digestive condition where acid from the stomach leaks back up to the gullet Human papillomavirus (HPV) infection Prior radiation treatment, or radiotherapy, in the head, neck, or both Regularly chewing betel nuts, a popular habit in some parts of Southeast Asia Exposure to certain chemicals, especially asbestos, sulfuric acid, and formaldehyde Subscribe our channel: https://goo.gl/gZH38F Music no-copyrights by Dj Quads: Soundcloud - https://soundcloud.com/aka-dj-quads Disclaimer: The materials and the information contained on Sheryl Elanda channel are provided for general and educational purposes only and do not constitute any legal, medical or other professional advice on any subject matter. Please SUBSCRIBE, SHARE, and COMMENT on this video. If you prefer to read, rather than watch the video, here’s the transcript. 0:17 – DEFINITION: In geometry, a torus is a surface that is generated by a circle as it is rotated about an axis that is in the same plane as the circle. A torus is a donut (doughnut) shaped structure. But in medicine, torus refers to a ridge or linear elevation. 0:24 – ANATOMY: The middle part of our face is made up of the maxillary bones. The maxilla forms the front anterior three-quarter of the hard palate (palatine process of maxilla). The palatine bones form the back posterior one-quarter of the hard palate. The incisive foramen is where the incisive nerve and blood vessels pass through. Posterior nasal spine. The inter-maxillary suture line stretches from the incisive foramen to the posterior nasal spine. 0:58 – A torus palatinus arises from the line of union that forms the inter-maxillary suture line. It is a downward extension of the bones that make up the hard palate. A torus palatinus has to satisfy three criteria: 1) A bony growth, 2) that lies on the midline, 3) of the hard palate. We have to distinguish a torus palatinus from other growths on the palate, e.g. necrotizing sialometaplasia (an inflammatory condition of the salivary glands on the palate), cysts of the incisive canal, minor salivary gland cancers (e.g. adenoid cystic carcinoma, mucoepidermoid carcinoma), and other types of cancers (e.g. lymphoma, Kaposi’s sarcoma). 1:12 – INDICATIONS FOR SURGERY: A torus palatinus only needs to be treated when: 1) there is frequent ulceration and pain on the torus, 2) when food gets stuck in the grooves on the sides of the torus, and 3) when it interferes with the fitting of dentures. 1:23 – Case Presentation: A 42 year old woman presents with a large mass on her palate. She wanted surgery because food was getting stuck in the grooves at the sides of the torus, giving rise to bad breath and poor oral hygiene. 1:26 – Surgery video demo: Lignocaine and adrenaline injection to reduce bleeding during surgery. Midline palatal incision made. Palatal mucoperiosteal flaps are elevated to expose the bony torus palatinus. The torus is removed with a drill, using an out-to-in approach. The outer part of the torus is made up of very hard cortical bone. More effort is needed to drill away the hard cortical bone. The good thing is that cortical bone is less vascular (fewer blood vessels). So there is hardly any bleeding in this stage of the surgery. As I get deeper, the torus is made up of soft medullary bone. Soft medullary bone is more vascular (more blood vessels). Surgery at this stage results in more bleeding. Now that I have an idea of the depth of bone removal from doing the left side, the torus removal on the right side should be easier and quicker. Instead of an out-to-in approach as in the left side, I can now do an in-to-out approach in the right side. This means that I drill the soft medullary bone first, and egg-shell the hard cortical bone. The egg-shelled hard cortical bone can now be fractured and removed en-masse. I now flush the area thoroughly to remove all bone chips and fragments. I trim away the excess mucoperiosteal tissue. Finally I close the wound with absorbable sutures. The wound looks ugly now, but in six weeks, the healing will be very impressive. 5:21 – Let’s compare what the torus looks like before surgery, and how the wound looks like six weeks after surgery.

Please SUBSCRIBE, SHARE, and COMMENT on this video. If you prefer to read, rather than watch the video, here’s the transcript. 0:13 – DEFINITION: A torus palatinus has to satisfy three criteria: 1) a bony growth, 2) on the midline, and 3) of the hard palate. It tends to grow larger over time. A torus palatinus is a BONY growth. If the growth is soft, it is important to exclude a malignant tumor of the minor salivary glands. It affects the HARD palate, and not the soft palate. It involves the MIDLINE of the hard palate. If it involves one side of the palate, it is important to exclude a malignant tumor. 0:51 – Torus palatinus comes in many shapes and sizes. A painful ulcer may form on its surface. The palate separates the nasal cavity from the oral cavity. During fetal life, the hard palate is separated by a cleft. The cleft gradually closes during fetal development. A torus forms where the two palate halves join together. A small torus has an obtuse angle at its edge. As the torus enlarges, it casts a ninety degree right angle. Finally an acute angle is formed. The acute angle is unhygienic because it can trap dirt and food particles. An ulcer may form on the torus surface. 2:13 – Case Presentation: A 36 year old woman presents with a bony mass on her hard palate. She wants surgery because: 1) painful ulcers form on her torus, 2) smelly food debris accumulates along the sides of the torus (results in bad breath or halitosis). 2:26 – The palate receives sensation and blood supply from the palatine blood vessels and nerves (greater and lesser palatine arteries). A midline palatal mucosal and periosteal incision enhances palate flap survival. The midline incision can be extended to improve exposure. Avoid incisions that compromise palate flap survival. 2:54 – Surgery video demo: The palatine nerves are injected to provide pre-emptive post-operative anaesthesia. Midline mucoperiosteal palatal incision is made. The flaps are carefully elevated. The torus is slowly reduced in stages with a cutting burr. Egg shelling the torus. The main risk is overzealous bone removal, resulting in an oro-nasal fistula. The cutting burr drill generates a lot of heat. Copious amounts of normal saline is dripped onto the burr drill to prevent over-heating. Extension incision is made to improve exposure. A stay suture is used to keep the flap out of the way. 5:57 - Next, we tackle the other half of the torus. The palate flaps are stitched together. Excess tissue is trimmed away.

Let me introduce my dear patient, Mrs. Ola Sawitsky. Mrs. Sawitsky underwent maxillectomy surgery for cancer of the hard palate. She had the left side of her hard palate and oll of the teeth on the left side of her upper jaw removed making a communication between her mouth and her nose. That means that every time she tries to speak she sounds like she's muffled. Every time she tries to eat everything comes out her left nostril. What I did for her was I made her a surgical obturator which closes the defect immediately at the time of surgery. I also replaced all of her teeth removed with the tumor back in her mouth at the time of surgery. When Mrs. Sawitsky awoke from surgery she was able to speak and swallow as though nothing had had occurred. In her own words, Mrs. Switzky will now tell you about her journey and how she has come from prior to surgery, receiving the diagnosis of cancer, being terrified that she would never again be able to live her life as a functioning participating member of society, to post surgery, where she awoke speaking and eating right away. Mrs. Sawitsky is now out having a wonderful time with her friends and family since the day she came home from the hospital. Please sit back and listen to this dear woman's story and know how honored I am to be involved in her care.

Pain or discomfort in your mouth that doesn't go away is the other most common symptom of mouth cancer. Ulcers that do not heal. A broken area of skin (ulcer) that will not heal can be a symptom of mouth cancer. About 80 out of 100 people (80%) with mouth cancer have this symptom. White or red patches in the mouth or throat. An abnormal looking patch could be a sign of cancer or precancerous changes: White patches are called leukoplakia. Red patches are called erythroplakia. These patches are not cancer, but if left untreated they may lead to cancer. Weight loss. It is a common symptom of many different types of cancer. Mouth and oropharyngeal cancer can make it painful to eat and diffcult to swallow. This might cause weight loss. Difficulty swallowing. Mouth cancer can cause pain or a burning sensation when chewing and swallowing food. Or you might feel like your food is sticking in your throat. Difficulty swallowing can also be caused by a harmless narrowing of the foodpipe (oesophagus). Speech problems. Cancer in your mouth or throat can affect your voice. Your voice might sound different. It may be quieter, husky, or sound as if you have a cold all the time. Or you might slur some of your words, or have trouble with pronouncing some sounds. A lump in your neck. You may have a lump in your neck caused by an enlarged lymph node. Swelling in one or more lymph nodes in the neck is a common symptom of mouth and orpharyngeal cancer. A hot red painful lump usually means an infection, rather than a cancer. Lumps that come and go are not usually due to cancer. Cancer usually forms a lump that slowly gets bigger. Bad breath. Most people have bad breath at some point in their life and it is not cancer. But if you have cancer, bad breath might be worse and happen more often.

A snapshot of HPV is not a pretty picture. The first nationwide accounting found at least 50-percent of the sexually active population will have genital HPV in their lifetime. "It's really an asymptomatic infection. Most women who have HPV have absolutely no symptoms. Whereas other STDs, like herpes, certainly has symptoms if there's an outbreak," says Dr. Kevin Fleishman, gynecologist on Lee Memorial Health System's medical staff. Many of them clear without ever being detected. Some are more likely to turn bad. The human papilloma virus has more than 100 strains. "Some are low risk, some are high risk. The high risk types are known to cause almost all cervical cancer," says Dr. Fleishman. This new study revealed a rising number of people with oral HPV. It's estimated 16-million Americans between 14 and 69 are infected. This raises their risk for cancers of the mouth. "Many, many of these cancers are caused by HPV and these are largely cancers of the tonsils or the base of the tongue. Way back in the back of the throat where you can't see with a tongue blade," says Dr. Phillip Andrews, otolaryngologist on Lee Memorial Health System's medical staff. The FDA approved an HPV vaccine for boys and girls. First aimed at preventing cervical cancer in girls, doctors now think it could also prevent 10,000 new cases of oral cancer each year. But fewer than half the people who could get the vaccine take even the first of 3 doses. "The best time to do a vaccine is before somebody's been exposed to what you're vaccinating against. And therefore, when it comes to sexually transmitted diseases, the best time to vaccinate is before there's been any sexual activity," says Dr. Fleishman. The findings concluded the rise in HPV cancer rates combined with low vaccination rates require an intervention of public education. Oral cancer appears as a growth or sore in the mouth that does not go away. Oral cancer, which includes cancers of the lips, tongue, cheeks, floor of the mouth, hard and soft palate, sinuses, and pharynx (throat), can be life threatening if not diagnosed and treated early. Cancers that are known collectively as head and neck cancers usually begin in the squamous cells that line the moist, mucosal surfaces inside the head and neck (for example, inside the mouth, the nose, and the throat). Head and neck cancers can also begin in the salivary glands, but salivary gland cancers are relatively uncommon. A Cystostomy is a dental treatment that consists in removing a dental cyst. A cyst is a pouch of tissue filled with fluid or pulp that can form next to or around teeth. 'Cystostomy' means 'opening the cyst', whereby the volume of the cyst is reduced. In dentistry, a Cystostomy is only one way to medicate a cyst. Dental cysts are commonly a result of infections in the pulp or inner portion of the tooth, or caused by faulty root treatment. Regardless of the cause, a dental cyst should always be removed and tested in a dental laboratory to ensure that it is of benign nature and doesn't contain cancer cells. SUBSCRIBE: http://bit.ly/Oc61Hj Donnie Fritts lying in bed in 2003 after surgery to remove part of his face due to aggressive face cancer. WHEN Sharon Fritts married her husband Donnie she promised to stick by him through thick and thin. But within a few years those vows were to be tested as Donnie developed an aggressive form of cancer on his face. In early 2003 Donnie was told by doctors to make a choice: his face or his life. He decided to have an operation that would remove the ultra-rare cancer—called ameloblastic carcinoma -- along with much of his face. The 12-hour surgery was a success but left Donnie, a former carpenter, with a gaping hole in the middle of his face. He no longer had a nose, upper lip and palate and part of his forehead was missing. There was some joy in 2011 as a top team of surgeons gave Donnie a prosthetic to make it look like he had a normal face. But sadly their joy was short-lived as Donnie developed a life-threatening infection. He battled back to health but was forced to wait for a new prosthetic. Sharon is determined to stand by the man she loves until a solution can be found. She said: "Donnie is an amazing man. He's so funny and has kept his sense of humor throughout. I still see the man I saw when we walked down the aisle." Donnie and Sharon's story will feature in an episode of new series Body Bizarre, titled Born With An Extra Body: Body Bizarre at 10pm on Monday November 25th. http://www.uk.tlc.com/shows/body-bizarre/ For more amazing footage of the amazing side of life, visit the Like Barcroft TV on Facebook: http://goo.gl/7ayYgY Follow Barcroft TV on Twitter: http://bit.ly/10vFLY9 Barcroft Media website: http://bit.ly/19OYwp

I received an email from a Rescue Fan that had a severe soar throat and wasn't sure if she should go to the doctor. I suggested they do, and she was diagnosed with throat ulcers. I thought I'd do a RoyOnRescue to help explain what they are and why you should see a doctor. I hope it helps.
