Government programs, like Medicare and Medicaid, can ask for a lower price from health service providers because they have the numbers: the hospital has to comply or else risk losing the business of millions of Americans. There are dozens of private health insurance providers in the United States and they each need to bargain for prices with hospitals and doctors. The numbers of people private insurances represent are much less than the government programs. That means a higher price when you go to the doctor or fill a prescription. Well, we already know the answer. Just ask America’s veterans—they’ve had government-run health care for decades. The U.S. Department of Veterans Affairs (known as the VA) runs the largest hospital and health care system in America. The VA employs over 340,000 people—twice the size of the Marine Corps. And it has a $180 billion annual budget, making it the second largest department in the Federal Government. Only the Department of Defense budget is bigger. The VA is a true single-payer health care system. It runs over 150 hospitals and 1,400 community-based clinics across all 50 states. The doctors, nurses, administrators – everyone that works for the VA – is a government employee. The system actively serves some 7 million patients—one-third of the 21 million veterans alive in the U.S. today. Sounds impressive, right? But for the past few decades—and especially for veterans of the war in Vietnam, as well as the wars in Iraq and Afghanistan, where I served—the VA has been an abysmal failure: inefficient, bureaucratic and sometimes deadly. Among veterans, horror stories about the VA abound. These stories were tragically brought to light in 2014, when whistleblowers in Phoenix revealed that 1,700 veterans there had waited an average of 115 days just to receive an initial appointment. According to the VA’s official policy, that wait time should have been no more than 14 days. As if that wasn’t bad enough, the Phoenix VA then lied about it, releasing falsified waiting lists to the public to cover its tracks. Phoenix turned out to be the norm, not the exception. The VA’s inspector general found systemic problems across the country. In Fort Collins, Colorado, for example, clerks were instructed to falsify records to show that doctors were seeing more patients than they actually were. In Columbia, South Carolina, delays in diagnosis and treatment directly led to the deaths of multiple patients. The VA program there had nearly 4,000 backlogged appointments despite a $1 million grant earmarked to reduce delays. And in the VA’s hospital in Pittsburgh, in 2011 and 2012, there was an outbreak of Legionnaires’ Disease that officials knew about for more than a year before informing patients. At least six veterans died as a result. The Obama Administration’s own Deputy Chief of Staff, Rob Nabors, revealed that VA health care has a “corrosive culture” with “significant” and “systemic failures.” The politicians’ response to this debacle? Spend more money -- a lot more money. The VA’s budget has almost doubled since 2009. They’ve hired 100,000 new people in the past decade. Wait times have actually gone up, yet not one administrator was fired for the wait-list scandal. National Health Insurance Debate Under Nixon
In 1971, President Nixon proposed a national health insurance plan built on heavily employer private coverage. Senator Ted Kennedy proposed what would today be called a single-payer plan. In 1974, the debate had morphed into Nixon vs. Kennedy-Mills vs. Organized labor. Despite the prediction in the second clip shown, the result was stalemate rather than passage in 1974 or 1975.

The Fallacy of Single-Payer Health Care
Chris Pope joins John Stossel to talk about the debate over single-payer health care—a system in which the federal government would assume all health-care costs currently borne by private insurers, employers, and individuals. Momentum for single-payer is growing among Democratic politicians. Senator Bernie Sanders’s “Medicare for All” proposal would impose $32 trillion in new costs over ten years and a likely annual tax increase of $26,000 per American household. It’s hard to imagine that Congress would adopt massive federal tax increases and rationing of health-care services, but incremental proposals, such as Medicaid buy-in and strict price controls, could be on the horizon if Democrats capture the White House in 2020 and do well in the congressional elections. Advocates for single-payer health care point to European countries with generous social-welfare systems as evidence of its success. They don’t acknowledge the fact that Germany and France, like the United States, have private and employer-funded health insurance, in addition to government programs for the poor. In the meantime, U.S. lawmakers continue to ignore reforms—like eliminating barriers to hospital competition—that would probably be more effective in improving America’s health-care system. According to the Affordable Care Act or Obamacare, greater levels of funding will be extended to those without the necessary medical coverage and ability to cover the costs. Government will encourage seeking private care and subsidize policy premiums. While the intention for Obamacare is to make health assistance accessible for all, the introduction of single payment systems may relieve the burden placed on the hospital systems, doctors and state tax. Unfortunately, the initiatives of Obamacare have not adequately addressed the real issues at hand. Millions of people will remain without cover or underinsured, but required to continue to pay excessive deductibles and premiums. Insurers are required to provide minimum and cos- effective forms of coverage, but the complexity of the procedure and continuous expenses charged by insurers make medical protection an impossibility for many individuals. With the option of enrolling for a single pay system or national care plan, the exorbitant and complex costs of exclusive private insurance are eliminated and the entire family can benefit from comprehensive coverage. This modern national plan aims to make therapies and clinical assistance readily available from inpatient to outpatient solutions. A wide range of therapies will be delivered for all family members excluding cosmetic and select procedures. Major benefits of Medicare for All includes the lowered costs and the ability to receive a better quality of care more efficiently. A concern with current systems is an inability for patients to obtain timely treatment and specialist attention. A national plan will help decrease the overall costs for prescription pills and therapies while removing the need to make co-payments or settle deductibles for medication, doctor visits or surgery. Investing in an improved standard of Medicare can simplify the healthcare industry and help more people without adequate cover or no cover to receive the appropriate levels of protection. Jeffrey M. Borkan, MD, PhD is a family physician educator, researcher, clinician, and advocate whose career has bridged two fields (family medicine and medical anthropology), both in the US and abroad. He joined the Department of Family Medicine at Brown University as Professor and Chair in 2001, after having been the ViceChair of Behavioral Science at Tel Aviv University, and the coordinator of a national Family Medicine Practice Based Research Network. Dr. Borkan received his BA from the University of Michigan and his MD and PhD at Case Western Reserve University. He completed his residency in Family Medicine at the University of Washington and was a fellow in Social Medicine at Harvard. At Brown, Dr. Borkan has worked with a broad coalition to spearhead the growth and development of the Department of Family Medicine, the sole family medicine department in the State (and the site of training for two thirds of the family physicians). He has been active in Rhode Island and national healthcare policy, including chairing the Rhode Island Primary Care Physician Advisory Board, serving as a board member of the national Academic Family Medicine Advocacy Association (AFMAA) and the Patient Centered Primary Care Collaborative (PCPCC), president and chair of the Association of Departments of Family Medicine, and, currently, the chair of the Council of Academic Family Medicine (the national coordinating body for all academic Family Medicine). Dr. Borkan has taken several leadership roles in medical education innovation and currently chairs the Brown MD Curriculum Committee during a period of curriculum redesign. Dr. Borkan's association with the principles of the patient centered medical home (PCMH) is long standing, based in experiences in a variety of urban and rural settings as a family physician, administrator, and researcher. He is the Principle Investigator on a Patient-Centered Medical Home Initiative (AAU Grant, HRSA, 2010 to 2015) and is on the Executive Committee of the CSI Rhode Island multipayer PCMH initiative. In the spirit of ideas worth spreading, TEDx is a program of local, self-organized events that bring people together to share a TED-like experience.
