Knee Arthritis Stretches & Exercises - Ask Doctor Jo

These knee arthritis stretches and exercises are simple to do and should provide some relief from knee arthritis pain. They are also great for keeping your knees nice and fit. Read Doctor Jo's full blog post about this at http://www.askdoctorjo.com/content/knee-arthritis-stretches-exercises The first stretch is for your calf muscles. Start off with your legs out in front of you. Keep the leg you want to stretch out in front of you. Take a stretch strap, dog leash, belt, or towel and wrap it around the ball of your foot. Relax your foot, and pull the strap towards you stretching your calf muscle. You should feel the stretch under your leg. Hold the stretch for 30 seconds, and do three of them. The next stretch is for the hamstrings. Keep your leg that you want to stretch out in front of you. Try to keep your back straight and just bend at your hips. Keep your leg straight and pull your toes towards you to get an extra stretch. Hold this stretch for 30 seconds, and repeat three times. The last stretch is for your quad muscles. This time you will lie on your stomach to stretch. Wrap the strap or dog leash around your ankle, and then lie on your stomach. Bring the strap up and around your shoulder so you are comfortable. Then pull the strap towards you bringing the lower leg towards your bottom. Hold this stretch for 30 seconds doing three of them. Now we will move on to the exercises. The first exercise is going to be a quad set. Put your leg straight out in front of you again. You can use a towel roll underneath your knee as a target. Activate your quad muscle by squeezing it and pull your toes toward you at the same time. This will push your knee into the ground. Try to hold this for 5-10 seconds and do 10 of them. Next you will do a short arc quad (SAQ). For this, you will use a foam roll or a rolled up beach towel. You want something big so you have a good bend in your knee. Prop your leg over the roll with your knee on the roll. Try to keep your knee on the roll the whole time, and then lift your foot in the air until your leg is straight. If you pull your toes towards you at the end, this will help activate all the muscles in the leg. Try not to go fast because then you are only using momentum. Go slow and controlled with your movements. Start of with 10 of these. If you get to 20-25 and it is easy, then you can add ankle weights. For the next exercise, you will sit in a chair. This one is called a long arc quad (LAQ). This is similar to the SAQ where you want to go slow and controlled, and then bring your leg up straight out in front of you. Slowly come back down. Start of with 10 of these. If you get to 20-25 and it is easy, then you can add ankle weights. Now you are going to stand up for the last exercises. For the hamstring exercise standing up, you want to keep the top part of your leg as straight as possible in line with your body. It might try to come forward if your muscles are tight, but try to keep it down. Slowly bring your foot back behind you, like you are trying to kick your bottom. Make sure you are going slow and controlled with these. Start of with 10 of these. If you get to 20-25 and it is easy, then you can add ankle weights. The last one is for calf strengthening. With your feet about shoulder width apart, and holding onto something for balance if you need to, come up on your toes bringing your heels off the ground, and slowly come back down. If that becomes easy, you can do this one foot at a time. Start off with ten and work your way up to 20-25. Perform all exercises on both legs – this will help prevent imbalance between the rehab and non-rehab knee. Pain-relief modalities are essential throughout this program to help limit discomfort and allow you to progress with the exercises. These include: Anti-inflammatory medications [NSAIDs] Topical NSAIDs or analgesic ointments Moist heat prior to exercise to loosen up stiffness Icing for 5-10 minutes after exercise to limit inflammation Knee compression sleeves If you develop any questions or concerns regarding your condition while using this video, contact your physician Stretching The goals of these exercises are to improve knee motion and reduce tension in the joint. Please remember the following as you engage in stretching exercises for knee arthritis: Perform STRETCHING exercises prior to doing STRENGTHINING exercises. Hold stretch in each position 30 seconds; then relax for 15 seconds; and repeat Do 5 repetitions for each stretch The following stretching exercises, which are demonstrated in this video, should be conducted during your knee arthritis conditioning program: STANDING CALF STRETCH Standing quad stretch Standing hamstring stretch Early Strengthening Exercises The goal of these exercises is to build strength and stability in the knee. Please remember the following as you engage in early strengthening exercises for knee arthritis: Let pain be your guide; if you find a certain exercise too painful to complete, then reduce the intensity of the exercise or abandon it altogether Start with no weight and progress to light dumbbells or ankle weights to increase resistance Perform 1-2 sets for 12-15 repetitions each exercise When performing leg extension, keep resistance light to avoid irritation The following early strengthening exercises, which are demonstrated in this video, should be conducted during your knee arthritis conditioning program: Half squats Standing hamstring curls Calf raises Leg extensions Straight leg raises Prone hip extensions Hip abduction Hip adduction Advanced Strengthening & Conditioning Exercises The goal of these exercises is to advance muscle strength, coordination and endurance for long-term pain relief. Please remember the following as you engage in advanced strengthening and conditioning exercises for knee arthritis: Continue EARLY STRENGTHENING exercises. As strength and flexibility improve, add these exercises to improve knee stability and pain-free function Cross-training exercises limit repetitive stress on certain body parts and reduce overuse injuries All exercises are low impact; start with a single exercise for 10-15 minutes and progress as tolerated Perform 1-2 sets of strengthening exercises for 12-15 repetitions The following advanced strengthening and conditioning exercises, which are demonstrated in this video, should be conducted during your knee arthritis conditioning program: Leg press – machine Dumbbell squaTS Cross training – stationary bike Cross training – elliptical machine Cross training – walking Cross training – incline walking This video should not be used as a substitute for regular physical therapy visits and guidance from your physician. Often, people are told that something in an X-ray or MRI is the reason they have pain. This could be a labral tear, arthritis, or cam or pincer bone shapes called femoroacetabular impingement (FAI/hip impingement). It's important to remember that none of these things have been shown to be definitively linked to hip pain. In fact, in study after study, there is no relationship between the severity of labral tears, arthritis, or FAI bone shapes and pain. The second mistake is stopping all movement. Often doctors will tell you stop moving because it will accelerate damage to your hips. If you're familiar with the history of back pain and spinal fusion surgery, this may sound familiar. For years, if you had chronic back pain, doctors would look at X-rays and MRIs for signs of arthritis, degenerative discs, and other pathologies. Surgery was often considered the best option when a few months of rest and/or physical therapy failed. Over the decades, high quality research has shown that long periods of bed rest actually make things worse, that spinal fusion is not better than long term exercise based, non-invasive treatment, and that spinal fusion itself doesn't reliably relieve back pain! This is a familiar pattern that happened with shoulder impingement, knee meniscus tears, and seems to now be happening with hip surgeries. The third most common mistake with hip pain is giving up too soon on non-invasive approaches. A spine surgeon, Dr. David Hanscom, wrote a book about how to get out of back pain without surgery. After years of performing spinal fusions and seeing their disappointing results, he strongly advises that people dedicate themselves to lengthy, detailed exercise regimens before ever considering surgery. And if first attempts at therapy or training fail, he strongly suggests that people be patient and diligently seek out other help and other ways to help their muscles. You need to learn about your unique body and what it uniquely needs. Shoulder Arthritis Stretches & Exercises - Ask Doctor Jo

These shoulder arthritis stretches and exercises should help relieve the pain caused by shoulder arthritis. See Doctor Jo’s blog post about this at: http://www.askdoctorjo.com/shoulder-arthritis The first stretches are called pendulums. Use a chair or counter top for balance, and lean over so your arm hangs down towards the ground. Move your body, not your arm in circles so your arm swings around like a pendulum. You can also rock front to back and side to side. Start off with 10 of each and work your way up to a minute of each. Then you are going to be in standing using a weight or resistive band. Try to keep your arm straight the whole time and don't bend your elbows. Your thumb is going to be pointed upwards. The first two are front and back which are flexion and extension. You don't need to raise the band way above your head. You can bring it to a 90 degree angle which is where your arm is parallel to the floor for flexion. Now you are going do put your arm out at a 45 degree angle which we call scaption. Now you will do shoulder shrugs. Lift your shoulders up towards the ceiling, but try to keep your head and neck in one spot. Don’t bring your ears down towards your shoulders, lift your shoulders up. Do 10 of these. Now for shoulder circles. You can roll them clockwise, and then counter clockwise. This will stretch out the shoulder and neck muscles. Start with10 each way, and work your way up from there. The next exercise is going to be rows or scapular retraction. You can use your feet as an anchor. You want to keep your elbows in close to your body and make a rowing motion. Try to squeeze your shoulder blades together when you pull back. Make sure you are using a slow controlled motion. For the final stretches you can use a Swiss/therapy ball, or if you don’t have one, you can use a table or counter top. This is an active assisted stretch, which means you are moving the arm now, but the ball will support the weight. You will slide your arms forward with your thumbs facing upward towards the ceiling and lean your body forward until you feel a stretch. You can stabilize the ball with your other hand, or stretch one shoulder at a time. You can also go out at an angle like the scaption or completely out to the side to stretch your pec muscles as well. Osteoarthritis Back Pain Stretches & Exercises - Ask Doctor Jo

Osteoarthritis (OA) of the spine (or back) is caused when the joints and discs in the neck and lower back degenerate. It can cause stiffness and pain in the back and is usually worse in the morning and at night. See Doctor Jo’s blog post about this at: http://www.askdoctorjo.com/osteoarthritis-spine These stretches and exercises should help with the osteoarthritis pain in your back. The first stretch is called the pelvic tilt. You will roll back onto your back with your knees bent. You want to imagine pushing your belly button into the ground, or push your back flat onto the ground like you are squishing something. You can put your hand under the curve of your back, and try to push your back into your hand. Hold these for about 3-5 seconds. Now you will do a figure four stretch. Cross your foot over onto the thigh of your other leg into a figure four position. Grab the leg on the ground underneath the thigh, and pull your leg toward your chest. Hold this stretch for 30 seconds, and do three on each side. Then you will do a single knee to chest stretch, bend one knee and bring your leg up to your chest as far as you comfortably can. Hold this for 30 seconds and do it three times on each side. The next stretch is a child’s pose stretch. Turn over onto your knees and sit back onto your feet. Put your arms out in front of you on the ground and stretch them forward reaching in front of you. Hold for 30 seconds and do them three times. If it is comfortable, you can hold them longer than 30 seconds. Get on your stomach in prone. This will help you stretch your spine into extension. You can prop up on your elbows and hold it for a few minutes. Now go up on your hands and knees in a quadruped position or all fours. Try to keep your back straight like a tabletop, and try to keep your neck in a neutral position so you don't strain anything. Now lift your opposite arm and leg and the same time. Some people call this the bird dog exercise. Start off with 10 on each side. The final set of exercises are going to be planks. Start with a plank on your elbows. You can do this modified with your knees bent and on the ground, or you can get into a full plank position. Make sure your elbow lines up with your shoulder, and that your body is in a straight line. Hold this for about 20-30 seconds, and work your way up to a minute. Finally you are going to lie on your side with the side you want to strength on the ground. Try to get your body in a straight line, and place your elbow on the ground just below your shoulder. Come up into a side plank. Hold it for 15-20 seconds, and work your way up to a minute. If this is too hard, you can start the side plank on your knees. Knee osteoarthritis (OA), or as it's sometimes called wear-and-tear arthritis, is a chronic condition of the knee joint. It's caused when the cartilage between the joint breaks down leading to pain, stiffness and swelling. See Doctor Jo’s blog post about knee osteoarthritis at: http://www.askdoctorjo.com/knee-osteoarthritis The first exercises for knee osteoarthritis is a quad set. Straighten out your leg with your knee straight . You are going to try to push your knee down into the ground as hard as you can. To do this, you are activating your quad muscle. If you need a target for your knee, roll up a towel and place it under your knee. While you are squeezing your quad muscle to push the knee down, you can also pull your toes up towards you to activate the calf muscle. Try to told these for 3 - 5 seconds and do 10 of them. The next exercise is a heel slide lying down. Prop up your good knee and put your surgery knee straight out. Still keeping your heel down when you slide, pull it towards your body as far as you can, and then slide back down. You can do about 10 of these. If you want to get a better stretch, then you can use a strap, dog leash, or belt to get some extra bend in the knee. Place the strap on your shin and make sure it won’t slide off. Then use the strap to pull your knee towards you while you are doing the heel slide. After that you will do a bridge. Push your bottom up off the ground, but try to do this slowly, and do one segment of your back at a time curling upward. Then slowly go down the same way, one segment at a time. Now you will do a straight leg raise lying on your back. For all of these you want to try to keep your leg straight by locking out your knee, and pull your toes towards your nose to activate the muscles in your leg. Slowly raise your leg to about the other side where the knee is bent, and then slowly come back down. Then you will roll onto your stomach. Now you will do a hamstring curl. This time, keep your upper leg or thigh on the ground, and bend at your knee. Bring your foot towards your buttocks as far as you comfortably can, and then slowly come back down. Start off with 10, and work your way up to 20-25. If that becomes easy, then you can add light ankle weights. Finally on the ground, take a ball or pillow folded in half, and put it between your knees. Squeeze into the ball and hold for 3 seconds, repeat 10 times. The last exercise is standing up. This is on a tall step, but you should start off with something shorter like 2-3 inches. You can also use a step at home where there is a rail to hold onto. Go very slowly and controlled with the step up and step down. Count to three for both up and down with the movement. Hip Arthritis Stretches & Exercises - Ask Doctor Jo

These hip arthritis stretches and exercises are easy to do in the comfort of your home and should provide some pain relief. See Doctor Jo's blog post about this at http://www.askdoctorjo.com/content/hip-arthritis-stretches-exercises The first exercise will be sitting down. Make sure your back is against the chair, and your knees and hips are at about a 90 degree angle. Then you will bring your knee straight up towards the ceiling, and this is called a hip flexion exercise. Make sure you do slow and controlled movements. Start off with 10 and work your way up to 20-25. If those become easy, then you can add ankle weights. The next exercise will be a sit to stand. Again, you want this to be controlled movements. Try not to just pop up and plop down. Put your feet about shoulder width apart and lean forward as you stand up, then stick your bottom back and have your chest forward while you sit back down. Try to also keep your feet flat and your knees behind your toes when doing this. If that is easy, you can just tap your bottom on the chair and not sit all the way back down. Start off with 10 of these, and then work your way up to 20-25. So now get down on the ground, and you will stretch out those hip flexor muscles. Keep the knee of the leg you want to stretch on the ground, and then bring the other leg up into a lunge like position. Try to keep your back upright and straight. If you lean forward with your upper body, you won’t get a good stretch. Shift your hips forward until you feel a stretch in front of the thigh. Hold this for 30 seconds and do three of them. Lying down on the ground in supine or on your back, you will do a hip abduction exercise. With your legs straight out, try to keep your toes pointing up toward the ceiling through the whole movement. Then slowly slide your leg out to the side, and then slide it back in. Start off with 10 of these. If you get to 20-25 and they are easy, then you don’t need to do them in this position anymore. Now roll onto your side for the clamshell exercise. This one is harder than it looks. You want to keep your hips straight up and down, perpendicular to the floor. Your hips will want to roll back, but keep them up as much as you can. Then keeping your knees together with one on top of the other, bend your knees slightly towards your body. Then lift the top leg up slightly, like a clamshell opening up. It doesn’t have to be a big movement because you want to keep your hips from moving. Start off with 10 of these, and then work your way up to 20-25. If they become easy, you can add a weight or use a resistive band. The last exercise is a bridge. Even though this is a simple movement, it really works the hamstrings and glut muscles. Lie on your back with your knees propped up. Slowly lift your butt off the ground until your hips are in a straight line with your chest and knees. You can go one segment of the spine at a time to really loosen up your back. Start off with 10 of these, and then work your way up to 20-25. Hip arthritis can cause pain all around the hip and in the groin area. It can also be painful when you walk. These hip arthritis stretches and exercises should help make the hip area stronger to relieve the pain. See Doctor Jo’s blog post about this at: http://www.askdoctorjo.com/hip-arthritis-stretches-exercises The first stretch is called the pelvic tilt. You will roll back onto your back with your knees bent. You want to imagine pushing your belly button into the ground, or push your back flat onto the ground like you are squishing something. You can put your hand under the curve of your back, and try to push your back into your hand. Make sure you are breathing and not holding your breath while doing these exercises. Next you will do a bridge. Push your bottom up off the ground, but try to do this slowly, and do one segment of your back at a time curling upward. Then slowly go down the same way, one segment at a time. Now you will do a four way hip lying down. Start off lying on your back. For all of these you want to try to keep your leg straight by locking out your knee, and pull your toes towards your nose to activate the muscles in your leg. Slowly raise your leg to about the other side where the knee is bent, and then slowly come back down. Then roll onto your side with the leg you want to work on the bottom for hip adduction. You can place your bent knee on top in front of or behind the leg on the ground, which ever is more comfortable. You won’t be able to lift your leg high without rolling your hips. Then you will roll to the other side for hip abduction. Keep your leg straight, and try to lead with your heel and take your leg slightly behind you to engage the glut muscles. Then you will roll over onto your stomach for hip extension. You won’t be able to lift your leg very high with this one either. Do all of these ten times, and work your way up to 20-25. The next exercise will be a plank on your elbows. You can do this modified with your knees bent and on the ground, or you can get into a full plank position. Make sure your elbow lines up with your shoulder, and that your body is in a straight line. Hold this for about 20-30 seconds, and work your way up to a minute. Now you are going to get onto all fours or in the quadruped position. Try to keep your hands directly underneath your shoulders, and try to keep your back in a straight line, no arching up or down. Kick one leg straight out behind you, and raise the opposite arm at the same time. Some people call this the bird dog. Do ten on each side. There are types of psoriatic arthritis like 1. Symmetric arthritis – affecting same joints on both sides of body e.g. right and left knee 2. Asymmetric arthritis – affecting any joint 3. Spondylitis – inflammation of spinal column and the last is 4. Arthritis mutilans – severe and destructive inflammation of fingers and toes, affecting less than 5% of PsA patients. Psoriatic arthritis occurs when body’s immune system attacks healthy cells. However, scientists aren’t quite sure why it happens. There are factors that can increase your risk of getting psoriatic arthritis. For example: Having psoriasis Family history of psoriatic arthritis and Age. The condition is characterized by symptoms like, 1. Inflammation of the joints 2. Stiffness and 3. Limited mobility. Unfortunately, psoriatic arthritis cannot be completely cured, but you can control the symptoms with physical therapy, adding Vitamin D to your diet. Surgeries and joint replacements are performed in cases when medications and low-impact exercises don’t help. Preventing PsA isn’t easy, but chances can be minimized with exercises and healthy diet. So start eating healthy because a good dietary modification can help you to prevent psoriatic arthritis. Cervical Spondylosis Stretches & Exercises - Ask Doctor Jo

Cervical spondylosis is a general term for age-related wear and tear of the spinal discs in your neck. As the discs dehydrate and shrink, signs of osteoarthritis develop and degeneration happens. This can cause achiness and soreness as well as decreased motion in the neck. See Doctor Jo’s blog post about this at: http://www.askdoctorjo.com/cervical-spondylosis-stretches-exercises First start off with some gentle neck stretches by turning your head from side to side like you are looking over your shoulder. This is neck rotation. Hold it for about 10-15 seconds, and go back and forth about 5 times. Next you are going to take your ear to your shoulder on each side. Make sure you are not shrugging your shoulders up. This is side bending. Hold it for about 10-15 seconds, and go back and forth about 5 times. Now bring your chin to your chest and then looking up towards the ceiling. This is neck flexion and extension. Hold it for about 10-15 seconds, and go back and forth about 5 times. Then you are going to do isometric exercises in each direction. Basically this is when you activate the muscle, but you are not moving it. You can do this by gently pressing into your hand in each direction. Hold each one of these for 3-5 seconds. Now place your hand under your thigh to keep your shoulder down, then side bend your head to the opposite side and gently put pressure with your other hand to get a stretch through your trapezius muscles. Hold these for 30 seconds and perform 3 times on each side. The final stretch is going to be for your levator scapulae muscle. This is the muscle that is used when you shrug your shoulders, and they can get overworked when you are stressed out. Take your arm on the side of the pain and place it above and behind your shoulder. Then take your opposite arm and place on top and slightly behind your head. Look down towards the opposite knee of the pain and apply gentle pressure for a good stretch. Hold these for 30 seconds and perform 3 times on each side. There is about 2cc of the synovial fluid inside the knee that helps in the motion and lubrication of the joint. Between the hyaline cartilage which is called the articular cartilage you can see the meniscus, the lateral and the medial, the meniscus is a shock absorbing cartilage or cushion between the articular cartilages. The hyaline cartilage has 4 layers: 1- The superficial layer. 2- The middle layer. 3- The deep layer. 4- The calcified layer After the calcified layer you find the bone. These cartilage cells are supposed to live forever. Good cartilage cells are sterile; they can’t make more cartilage if these cartilage cells are destroyed. If the cartilage is subjected to excessive wear, trauma, injury, overuse, excessive weight or improper alignment, then the cartilage will wear away leaving the bone to rub against bone. The cartilage doesn’t have the ability to heal itself by hyaline cartilage, but sometimes it can heal itself by inferior type of cartilage called fibrocartilage, especially if the area that needs to be repaired is small. What is the treatment of the arthritis of the knee? 1- Medication: people respond differently to medications, the doctor select the type, dosage and duration of the treatment, the medication has to be safe and effective. Ask the doctor about the side effect of the medication. There are several medications for example: NSAIDS. 2- Losing Weight: it’s advisable to lose about 5% of the body weight, especially if the BMI was above 25. 3- Low Impact Physical Therapy: such activities like swimming or cycling, adds less stress on the knees, life style modification to protect the knee will slow the progress of the arthritis, there is a strong evidence that physical therapy will help the patient. The physiotherapy will decrease the pain, will improve the function, increase the strength, the range of motion; physiotherapy should be individualized with program that meets the patient’s needs, the life style, and expectations. 4- The physician will also use intra-articular injection of steroids; viscosupplementation, or hyaluronic acid, injection will relieve the patient’s pain and disability. 5- Other injection methods: - Gene therapy - PRP - Growth factor - Stem cells 6- Other treatment options: - Acupuncture - Massage - Glucosamine - Chondroitin sulfate - Valgus directing brace - Wedges in the foot - Arthroscopic debridement and lavage 7- Assistance devices: - Cane - Shock absorbing shoe - Shoe inserts - Knee sleeves - Support brace 8- Arthroscopy shouldn’t be done in arthritis unless there is loose body or meniscal tear causing mechanical symptoms of recurring lacking, catching, swelling and pain. There are some guide lines from the American Academy of Orthopaedic Surgeon AAOS: these guidelines are suggestive, and the treatment of the arthritis patient should be individualized and based on: - The doctor’s clinical judgment. - The patient’s clinical situation. - The evidence tested and published. It is a combination of all of these factors. If it works for you, it may not work for your neighbor. So basically arthritis can be minimal, and anything can help the arthritis ion this situation. But if the arthritis is moderate then the physician has many options for treatment and none of those options are predictable in the result. When the arthritis is severe, total knee is the most predictable option for treatment of arthritis. You have to reduce the pain and improve the function of the knee.
