What Are the Symptoms of Oral Cancer? The most common symptoms of oral cancer include: A sore that doesn't heal A sore that bleeds A growth, lump or thickening of the skin or lining of your mouth Loose teeth Poorly fitting dentures Tongue pain Jaw pain or stiffness Difficult or painful chewing Difficult or painful swallowing Sore throat Causes of mouth cancer Cancer results when a genetic mutation instructs cells to grow without control. Untreated, oral cancer will start in one part of the mouth, then spread to other parts of the mouth, to the head and neck, and the rest of the body. Mouth cancers typically start in the squamous cells that line the lips and the inside of the mouth. This is called squamous cell carcinoma. The exact cause of the mutations is unknown, but there is evidence that certain risk factors increase the chance of mouth cancer developing. Risk factors for mouth cancer Tobacco and alcohol use are prominent risk factors for mouth cancer. Smokers are three times more likely than nonsmokers to develop mouth cancer, and people who smoke and drink alcohol have up to 30 times higher risk than those who do not smoke and drink. A torus is a donut (doughnut) shaped structure. But in medicine, torus refers to a ridge or linear elevation. 0:24 – ANATOMY: The middle part of our face is made up of the maxillary bones. The maxilla forms the front anterior three-quarter of the hard palate (palatine process of maxilla). The palatine bones form the back posterior one-quarter of the hard palate. The incisive foramen is where the incisive nerve and blood vessels pass through. Posterior nasal spine. The inter-maxillary suture line stretches from the incisive foramen to the posterior nasal spine. 0:58 – A torus palatinus arises from the line of union that forms the inter-maxillary suture line. It is a downward extension of the bones that make up the hard palate. A torus palatinus has to satisfy three criteria: 1) A bony growth, 2) that lies on the midline, 3) of the hard palate. We have to distinguish a torus palatinus from other growths on the palate, e.g. necrotizing sialometaplasia (an inflammatory condition of the salivary glands on the palate), cysts of the incisive canal, minor salivary gland cancers (e.g. adenoid cystic carcinoma, mucoepidermoid carcinoma), and other types of cancers (e.g. lymphoma, Kaposi’s sarcoma). 1:12 – INDICATIONS FOR SURGERY: A torus palatinus only needs to be treated when: 1) there is frequent ulceration and pain on the torus, 2) when food gets stuck in the grooves on the sides of the torus, and 3) when it interferes with the fitting of dentures. 1:23 – Case Presentation: A 42 year old woman presents with a large mass on her palate. She wanted surgery because food was getting stuck in the grooves at the sides of the torus, giving rise to bad breath and poor oral hygiene. 1:26 – Surgery video demo: Lignocaine and adrenaline injection to reduce bleeding during surgery. Midline palatal incision made. Palatal mucoperiosteal flaps are elevated to expose the bony torus palatinus. The torus is removed with a drill, using an out-to-in approach. The outer part of the torus is made up of very hard cortical bone. More effort is needed to drill away the hard cortical bone. The good thing is that cortical bone is less vascular (fewer blood vessels). So there is hardly any bleeding in this stage of the surgery. As I get deeper, the torus is made up of soft medullary bone. Soft medullary bone is more vascular (more blood vessels). Surgery at this stage results in more bleeding. Now that I have an idea of the depth of bone removal from doing the left side, the torus removal on the right side should be easier and quicker. Instead of an out-to-in approach as in the left side, I can now do an in-to-out approach in the right side. This means that I drill the soft medullary bone first, and egg-shell the hard cortical bone. The egg-shelled hard cortical bone can now be fractured and removed en-masse. I now flush the area thoroughly to remove all bone chips and fragments. I trim away the excess mucoperiosteal tissue. Finally I close the wound with absorbable sutures. The wound looks ugly now, but in six weeks, the healing will be very impressive. 5:21 – Let’s compare what the torus looks like before surgery, and how the wound looks like six weeks after surgery.

Oral cancer appears as a growth or sore in the mouth that does not go away. Oral cancer, which includes cancers of the lips, tongue, cheeks, floor of the mouth, hard and soft palate, sinuses, and pharynx (throat), can be life threatening if not diagnosed and treated early. Cancers that are known collectively as head and neck cancers usually begin in the squamous cells that line the moist, mucosal surfaces inside the head and neck (for example, inside the mouth, the nose, and the throat).  Head and neck cancers can also begin in the salivary glands, but salivary gland cancers are relatively uncommon. Generally red sores appears on the palate, on the tip of the tongue, on the retromolar area and the floor of the mouth. These are mainly because of the bacterial, viral, or fungal infections and also ill fitting orthodontic braces and also dentures and also broken teeth and also in case of chronic smokers and alcohol users. The lesions generally are associated with candidiasis, canker sore, cold sore, broken teeth, periodontal abscess, leukoplakia, and and in case of other lesions of the oral cavity. These can be managed by proper visiting of the dentist, management of the broken tooth by going for extraction of the broken teeth and repair of the overhanging restorations. Repair of the ill- fitting dentures repair of the orthodontic appliances, treating the general conditions like leukoplakia with the balanced diet using the Vitamin B12 and application of the anti-fungal agents like Nystatin, Amphotericin, itraconazole, cotrimazole like anti-fungal agents. The sores can also be treated by using of the warm water along with the salt gargling and also using an anti-fungal or analgesic or anaesthetic gels and also oral gauze.

A soft tissue lesion on the buccal gingiva of #22 has been present for several years. Retreatment on #22 was done because the tooth will be used for an over denture abutment. Periapical lesion appears to be shrinking radiographically. The Soft tissue lesion does not appear to be changing clinically. Etiology unknown. Firm, Movable, Not fixed to underlying tissues. Appears white in the mouth. Patient feels an annoying feeling when touching it. Otherwise asymptomatic. Does not appear to be growing. After excision appears to be bluish, cauliflower like appearance. Send for biopsy.

Upper Jaw Tumour Excision and Bone Grafting

This young man came to our hospital complaining of difficulty in swallowing due to a swelling in the left side of the roof of his mouth (palate). He said that the swelling was initially very small and gradually grew to the present size however it was just occasionally mildly painful. CT scan and biopsy were done to evaluate the lesion and extent. Found to be pleomorphic adenoma, it was planned to excise this upper jaw tumor and reconstruct the defect with bone graft. After administering anesthesia, the upper jaw tumor was approached intraorally. A crevicular incision was placed in the left palatal region and a mucoperiosteal flap was raised exposing the tumor. The entire tumor was excised completely and the surgical site was thoroughly debrided of any remnants. An iliac (hip) bone graft was taken using trapdoor technique. The outer cortical bone was opened like a trapdoor, the inner cancellous bone was scooped out and the cortical bone trapdoor was placed back. In human anatomy, the "mouth" is the first portion of the alimentary canal that receives food and saliva. The oral mucosa is the mucous membrane epithelium lining the inside of the mouth. In addition to its primary role as the beginning of the digestive system, in humans the mouth also plays a significant role in communication. While primary aspects of the voice are produced in the throat, the tongue, lips, and jaw are also needed to produce the range of sounds included in human language. The mouth consists of two regions, the "vestibule" and the "oral cavity proper". The mouth, normally moist, is lined with a mucous membrane, and contains the teeth. In facial expression, this "mouth line" is iconically shaped like an up-open parabola in a smile, and like a down-open parabola in a frown. A "down-turned mouth" means a mouth line forming a down-turned parabola, and when permanent can be normal. Also, a "down-turned mouth" can be part of the presentation of Prader-Willi syndrome. Upon examination of the throat, the uvula is swollen, pale, and somewhat translucent (uvular hydrops). If greatly enlarged, the uvula might rest on the tongue and move in and out with respiration. There might be an associated rash or a history of exposure to phsical stimuli, allergens, or a recurrent seasonal indicence. What to do: Because of the known association of uvular with hypopharyngeal edema, watch for signs of airway compromise. If a patient complains of resiratory difficulty or breathes with stridor, commence treatment with intravenous lines and intubation and cricothyrotomy equipment at the bedside, and a crosstable lateral soft tissue neck x ray to rule out epiglottic swelling. If there is no acute respiraory difficulty, ask about precipitating events. Consider foods, drugs, physical agents, inhalants, insect bites and hereditary angioedema. When fever, sore throat and pharyngeal injection are present, culture the throat with a rapid strep screen and give an antibiotic that covers Haemophilus influenzae (e.g., Biaxin, Augmentin, Bactrim). It is reasonable to obtain a complete blood count with a manual differential to demonstrate eosinophilia to support the possibility of an allergic reaction or a high leukocyte count with increased granulocytes and bands to support a bacterial infection. Initially the patient should receive parenteral H1 and H2 antihistamines like hydroxazine 50-100mg im or diphenhydramine 25-50mg iv along with cimetidine 300mg iv or po or ranitidine 50mg iv or 150mg po. More severe cases should receive repeated doses of epinephrine 0.3ml of 1:1000 sq every 20 minutes x3. Nebulized isomeric or racemic epinephrine or albuterol are also effective. Parenteral corticosteroids like SoluMedrol 125mg iv are also typically used, although efficacy remains umproven. If there is a history of recurrent episodes of edema and there is a family history of the same, consider ordering a C4 complement level or C'1 esterase inhibitor levels as a screening test for hereditary angioedema. In this condition, the edema often involves the uvula and soft palate together. Uvular decompression may be useful in patients that are resistant to medical therapy or whose symptoms progress rapidly. This procedure consists of grasping the uvula with forceps and either making several lacerations with a sterile needle or snippint the distal centimeter as a patial uvulectomy. All patient s should be observed for an adequate period of time to insure that there is either improvement or no further worsening of the swelling before being discharged home. Upon discharge, they should receive 4-5 days of H1 and H2 blockers and steroids if required.

Using special X-ray shots like biting on a thin sheet of paper or rectangular X-ray, doctors can successfully carry out an assessment of the dental roots, jaws and caries. The dental X-ray is no different from any normal X-ray and thereby involves the same principles. It takes skill and experience for a dentist to correctly recognize the structures in the X-ray and to interpret them with exactness. The panoramic X-ray was initially conceived in the dental field so that dental practitioner can get a view of the complete jaw structure. Corrective jaw surgery, or orthognathic surgery, is a group of procedures performed to correct dentofacial irregularities, most commonly manifested as misalignments of the jaws. These deformities not only cause malocclusion or bad bite, but also create problems in the temporomandibular joint - the TMJ- and the airway, resulting in difficulties chewing, swallowing, speaking and breathing. While the surgery is performed to correct functional problems, patient's appearance may be dramatically improved as a result. Open bite is a condition where the upper and lower front teeth do not touch when the mouth is closed or at rest. An open bite can lead to a number of oral health conditions including tooth wear, tooth breakage and TMJ disorders. It may also cause speech problems known as ‘lisping’ in some individuals. Open bite surgery involves removing some of the bone of the upper jaw to move it to a new position. Once the jaws are aligned, plates and screws are used to secure the bones in place. Protruding lower jaw is corrected in a procedure called mandibular setback surgery. The tooth-bearing portion of the lower jaw is separated from its base and moved backward for proper alignment. In a similar way, receding lower jaw, or “weak chin”, is corrected with mandibular ADVANCEMENT surgery. In this case, the tooth-bearing portion of the lower jaw is repositioned FORWARD. Orthognathic surgeries are commonly performed in combination with orthodontic treatments and may take several years to complete.

A snapshot of HPV is not a pretty picture. The first nationwide accounting found at least 50-percent of the sexually active population will have genital HPV in their lifetime. "It's really an asymptomatic infection. Most women who have HPV have absolutely no symptoms. Whereas other STDs, like herpes, certainly has symptoms if there's an outbreak," says Dr. Kevin Fleishman, gynecologist on Lee Memorial Health System's medical staff. Many of them clear without ever being detected. Some are more likely to turn bad. The human papilloma virus has more than 100 strains. "Some are low risk, some are high risk. The high risk types are known to cause almost all cervical cancer," says Dr. Fleishman. This new study revealed a rising number of people with oral HPV. It's estimated 16-million Americans between 14 and 69 are infected. This raises their risk for cancers of the mouth. "Many, many of these cancers are caused by HPV and these are largely cancers of the tonsils or the base of the tongue. Way back in the back of the throat where you can't see with a tongue blade," says Dr. Phillip Andrews, otolaryngologist on Lee Memorial Health System's medical staff. The FDA approved an HPV vaccine for boys and girls. First aimed at preventing cervical cancer in girls, doctors now think it could also prevent 10,000 new cases of oral cancer each year. But fewer than half the people who could get the vaccine take even the first of 3 doses. "The best time to do a vaccine is before somebody's been exposed to what you're vaccinating against. And therefore, when it comes to sexually transmitted diseases, the best time to vaccinate is before there's been any sexual activity," says Dr. Fleishman. The findings concluded the rise in HPV cancer rates combined with low vaccination rates require an intervention of public education. It also left him with large gaps in his teeth that made him feel self-conscious about his appearance. But he is now able to use his mouth with greater ease after a team of scientists used a 3D printer to model and build the section of his jaw that was removed. The patient had to undergo the surgery in order to remove the cancer in the roof of his mouth. He experienced further complications after developing radiation-induced lockjaw following six weeks of radiotherapy treatment.

Lower jaw Reconstruction with Dental Implants after Cancer Surgery - Bilateral Neck Dissection

This middle-aged man was previously operated by us for carcinoma of the floor of the mouth and alveolus (oral cancer/mouth cancer). He had undergone resection of the tumor with affected portion of the lower law along with bilateral neck dissection, which is the removal of lymph nodes in the neck. He had lost multiple teeth in his upper and lower jaw due to which he was finding it very difficult to eat and speak. Because of the several missing teeth his cheeks and mouth appeared sunken, his face appeared small giving him an aged appearance. He wanted to have complete dental rehabilitation with fixed teeth. The surgical plan was to reconstruct the mandible with rib graft and place implants to replace the missing teeth. Rib graft was harvested and valsalva maneuver was done to ensure that there was no pleural damage. Crestal incision was placed and the mandibular bone was accessed. The graft was used to reconstruct the front teeth region of the lower jaw which was deficient following tumor removal. Implants were placed in the mandible. A loose upper tooth was removed and immediate implant was placed in the socket. Further implants were placed in the upper jaw to replace the lost teeth. Surgical sites were sutured. After complete osseointegration of the implants, permanent prosthesis resembling natural teeth crowns will be fixed to complete the rehabilitation. It can be scarlet fever. Where you see a strawberry tongue or a red coloured tongue and the most commonly asked question for a red patch on the tongue is when there is immune system like a fever or malaise , there is a velvety red patch and that is erythroplakia, which is a precancerous condition and this particular condition progresses into cancer very fast and has very high incidence of developing into a cancer and that condition should be consulted with a dentist or an oral and maxillofacial surgeon. This particular patch if it exists more than 2 weeks, then you have to visit a dentist. There are other anatomical variations called as the geographic tongue or there are certain other conditions called as canker sore, you have a yellow white spot and a red ring around it and that also goes away once the systemic condition goes away. So there are several reasons and several conditions that must be understood, diagnosed properly and given the right treatment for.

Life after cancer surgery to the upper jaw.

Let me introduce my dear patient, Mrs. Ola Sawitsky. Mrs. Sawitsky underwent maxillectomy surgery for cancer of the hard palate. She had the left side of her hard palate and oll of the teeth on the left side of her upper jaw removed making a communication between her mouth and her nose. That means that every time she tries to speak she sounds like she's muffled. Every time she tries to eat everything comes out her left nostril. What I did for her was I made her a surgical obturator which closes the defect immediately at the time of surgery. I also replaced all of her teeth removed with the tumor back in her mouth at the time of surgery. When Mrs. Sawitsky awoke from surgery she was able to speak and swallow as though nothing had had occurred. In her own words, Mrs. Switzky will now tell you about her journey and how she has come from prior to surgery, receiving the diagnosis of cancer, being terrified that she would never again be able to live her life as a functioning participating member of society, to post surgery, where she awoke speaking and eating right away. Mrs. Sawitsky is now out having a wonderful time with her friends and family since the day she came home from the hospital. Canker Sore or Aphthous ulcer : Canker sores (Aphthous ulcer) are small, painful ulcers on the inside of the mouth, tongue, lips, or throat.Canker sores are white or yellow and surrounded by a bright red area. They are not cancerous. Causes : Canker sores are a common form of mouth ulcer. They may occur with viral infections. In some cases, the cause is unknown. A number of factors causes outbreaks Lack of certain vitamins and minerals in the diet (iron, folic acid, or vitamin B-12) Oral trauma Hormonal changes Anxiety or emotional stress Heredity Drugs (including anti-inflammatory drugs and beta-blockers) Food allergies or sensitivities (chocolate, tomatoes, nuts, and acidic foods such as pineapple, and preservatives ) Toothpastes containing sodium sulfate Symptoms : small white or yellow oval-shaped ulcer painful red area in your mouth tingling sensation in your mouth Fever General weakness lymph nodes swelling Treatment : In most cases, the canker sores go away without treatment. Swallowing, or deglutition, is the process by which food passes from the mouth, through the pharynx and into the esophagus. As simple as it might seem to healthy people, swallowing is actually a very complex action that requires an extremely precise coordination with breathing since both of these processes share the same entrance - the pharynx. Failure to coordinate would result in choking or pulmonary aspiration. Swallowing involves over twenty muscles of the mouth, throat and esophagus which are controlled by several cortical areas and by the swallowing centers in the brainstem. The brain communicates with the muscles through several cranial nerves. Swallowing consists of three phases: 1. Oral or buccal phase: this is the voluntary part of swallowing, the food is moistened with saliva and chewed, food bolus is formed and the tongue pushes it to the back of the throat (the pharynx). This process is under neural control of several areas of cerebral cortex including the motor cortex. 2. Pharyngeal phase starts with stimulation of tactile receptors in the oropharynx by the food bolus. The swallow reflex is initiated and is under involuntary neuromuscular control. The following actions are taken to ensure the passage of food or drink into the esophagus: - The tongue blocks the oral cavity to prevent going back to the mouth. - The soft palate blocks entry to the nasal cavity. - The vocal folds close to protect the airway to the lungs. The larynx is pulled up with the epiglottis flipping over covering the entry to the trachea. This is the most important step since entry of food or drink into the lungs may potentially be life threatening. - The upper esophageal sphincter opens to allow passage to the esophagus. 3. Esophageal phase: food bolus is propelled down the esophagus by peristalsis - a wave of muscular contraction that pushes the bolus ahead of it. The larynx moves down back to original position.

Please 'like', 'share' and 'comment' on this video!!! So yes...this lesion on the tonsillar pillar (not the tonsil) is on MY throat. And believe me...I am not one to whine but this hurt a LOT. Every time I swallowed it felt like a needle rubbing my throat on that side. Eating dinner almost made me break into tears. So finally I asked around to the other doctors in my Emergency Department to see what they would suggest I do. One said to take an Aspirin an place it right against the sore until it dissolved. But how can I hold an aspirin all the way back there? Then they said gargles with salt water, sure that eases it...but it does NOT cure it. Then one suggested burning it with a silver nitrate stick. OUCH! burning my throat on purpose? Well...the one doctor went back to his office and got me 20% lidocaine spray and a few silver nitrate sticks. So...having these 'tools' at hand I recorded this video. And it has worked! TODAY...they day after the procedure I have only mild soreness at the side but I can swallow ok. For those of you who were asking and commenting let me clear some things up: 1. No...I am not a smoker. 2. No...the white stuff on the tongue is NOT candida. But it is caused by the chronic sinus drainage I have. You can appreciate it on the video. YES I know it looks disgusting so don't insult me I beg you. 3. Yes...there is ALSO a tonsillolith above the area of the canker sore in this video. 4. Yes...my molars are 'yellow'...that I blame on my thirst for CocaCola and chronic staining of the enamel. "Feeling of lump of actual small small occurring in the throat is a very common thing nowadays, these are actually, harmless unless they are progressively enlarging in size, becoming more symptomatic like pain, redness, ulcerating, burning sensation, unable to eat, then only these need to be looked at. Normally, if you look at a person’s throat, the surface of it has small small bulges, which are sometimes bigger, may be they grow up to 5mm – 7mm in size, they are actively infected. If they are remaining in the same size and not painful, they can safely be left alone.
