Cancer and Pernicious Anemia Cancer has two kinds of manifestation: Organ dependent, tumor Organ independent and systemic, paraneoplasia, and cachexia. All three start in the pre-clinical stage of cancer and progress side by side. Cancer is a triad: 1. Tumor 2. Paraneoplasia (neuro-endocrine manifestations) 3. Cachexia (weight loss). Pernicious anemia is also a triad an its common denominator is known. Comparison between the two triads has interesting implications on the understanding of cancer. Pernicious anemia is a triad: 1. Anemia 2. Para-anemia (Neural manifestations) 3. Pancytopenia (Weight loss) Vitamin B-12 is required for DNA synthesis. Its depletion will affect all proliferating tissues. It is most pronounced in rapidly proliferating, e. g. Bone marrow and gastrointestinal tract. All tissues are composed of Tissue Proliferative Units (TPU) and display the same cell kinetic structure: A two compartment cell renewal system with a feedback from Q to P. Vitamin B12 and the intrinsic factor of parietal cells form a complex which is carried to the terminal ileum, where it binds to the intrinsic-factor receptors on the luminal membranes of ileal cells. Gastritis destroys parietal cells. Intrinsic factor production is abolished which initiates a malabsorption of vitamin B-12 at terminal ileum. Depletion of vitamin B-12 in the organism causes pernicious anemia. The analogy between the two diseased raises the following question: Might cancer be cured like pernicious anemia, with one essential ingredient?

PERNICIOUS ANEMIA (ADDISON ANEMIA) Symptoms, Causes & Treatments OVERVIEW Anemia is a medical condition in which the blood is low in normal red blood cells. Pernicious anemia is one of the vitamin B-12 deficiency anemias. It’s caused by an inability to absorb the vitamin B-12 needed for your body to make enough healthy red blood cells. Pernicious anemia is a rare condition, with a prevalence of .1 percent in the general population and 1.9 percent in people who are older than 60 years, according to the Journal of Blood Medicine. This type of anemia is called “pernicious” because it was once considered a deadly disease. This was due to the lack of available treatment. Today, though, the disease is relatively easy to treat with B-12 injections or supplements. However, if left untreated, vitamin B-12 deficiency can lead to severe complications. SYMPTOMS The progression of pernicious anemia is slow. It may be difficult to recognize the symptoms because you may have become used to not feeling well. Commonly overlooked symptoms include: • Weakness. • Headaches. • Chest pain. • Weight loss. In rare cases of pernicious anemia, people may have neurological symptoms. These can include: • An unsteady gait. • Spasticity, which is stiffness and tightness in the muscles. • Peripheral neuropathy, which is numbness in the arms and legs. • Progressive lesions of the spinal cord. • Memory loss. Other symptoms of a B-12 deficiency, which can overlap with pernicious anemia, include: • Nausea and vomiting. • Confusion. • Depression. • Constipation. • Loss of appetite. • Heartburn. CAUSES Lack of vitamin B-12 People with anemia have low levels of normal red blood cells (RBCs). Vitamin B-12 plays a role in creating RBCs, so the body requires an adequate intake of vitamin B-12. Vitamin B-12 is found in: • Meat. • Poultry. • Shellfish. • Eggs. • Dairy products. • Fortified soy, nut, and rice milks. • Nutritional supplements. Lack of IF Your body also needs a type of protein called intrinsic factor (IF) to absorb vitamin B-12. IF is a protein produced by cells in the stomach. After you consume vitamin B-12, it travels to your stomach where it binds with IF. The two are then absorbed in the last part of your small intestine. In most cases of pernicious anemia, the body’s immune system attacks and destroys the cells that produce IF in the stomach. If these cells are destroyed, the body can’t make IF and can’t absorb vitamin B-12. TREATMENTS The treatment for pernicious anemia is a two-part process. Your doctor will treat any existing vitamin B-12 deficiency and check for iron deficiency. Treatment begins with: • Vitamin B-12 injections that are slowly decreased over time. • Complete blood counts to measure vitamin B-12 and iron levels in blood serum. • Blood tests to monitor replacement treatments. Vitamin B-12 injections can be given daily or weekly until the B-12 levels return to normal (or close to normal). During the first few weeks of treatment, your doctor may recommend limiting physical activity. After your vitamin B-12 levels are normal, you’ll only need to get the shot once per month. You can administer the shots yourself or have someone else give them to you at home to save you trips to the doctor. After your B-12 levels are normal, your doctor may recommend you take regular doses of B-12 supplements instead of the injection. These come in pills, nasal gels, and sprays.

Pernicious Anemia is a B12 deficiency. i found out I had it 5 or so years ago. I was experiencing a lot of fatigue, migraines, weird rashes and bizarre symptoms. After trying to figure it out on my own I went to the doctor and got tested for all sorts of things and everything was normal. I was frustrated because I didn't feel normal. After talkijng with my grannie she mentioned Pernicious Anemia a B12 deficiency that my great grandma had. She encouraged me to see if the doctor checked my B12 levels. They hadn't chaecked them so I had more blood drawn and BOOM my B12 were crazy low! Then the doctor checked to see if I had the intrinsic factor that absorbs B12 into the blood stream. I did not have it!! So I went of B12 shots (every 2-3) weeks and had my gut check for damage. I have been so much healthier, so much so I forget I have Pernicious Anemia!

Vitamin B12, vitamin B12 or vitamin B-12, also called cobalamin, is a water-soluble vitamin with a key role in the normal functioning of the brain and nervous system, and for the formation of blood. It is one of the eight B vitamins. It is normally involved in the metabolism of every cell of the human body, especially affecting DNA synthesis and regulation, but also fatty acid synthesis and energy production. Neither fungi, plants, nor animals are capable of producing vitamin B12. Only bacteria and archaea have the enzymes required for its synthesis, although many foods are a natural source of B12 because of bacterial symbiosis. The vitamin is the largest and most structurally complicated vitamin and can be produced industrially only through bacterial fermentation-synthesis. Vitamin B12 consists of a class of chemically related compounds (vitamers), all of which have vitamin activity. It contains the biochemically rare element cobalt. Biosynthesis of the basic structure of the vitamin is accomplished only by bacteria (which usually produce hydroxocobalamin), but conversion between different forms of the vitamin can be accomplished in the human body. A common semi-synthetic form of the vitamin, cyanocobalamin, does not occur in nature, but is produced from bacterial hydroxocobalamin and then used in many pharmaceuticals and supplements, and as a food additive, because of its stability and lower production cost. In the body it is converted to the human physiological forms methylcobalamin and adenosylcobalamin, leaving behind the cyanide, albeit in minimal concentration. More recently, hydroxocobalamin, methylcobalamin, and adenosylcobalamin can be found in more expensive pharmacological products and food supplements. The extra utility of these is currently debated. Vitamin B12 was discovered from its relationship to the disease pernicious anemia, which is an autoimmune disease in which parietal cells of the stomach responsible for secreting intrinsic factor are destroyed. Intrinsic factor is crucial for the normal absorption of B12, so a lack of intrinsic factor, as seen in pernicious anemia, causes a vitamin B12 deficiency. Today, we're going to talk about B12 deficiency, and B12 deficiency is actually more prevalent than we originally thought. Folks over the age of 60, young women on birth control, folks that have had weight loss surgery in the past are all more at risk for B12 deficiency. The older population loses the ability to produce enough gastric acids and enzymes in their stomach to break down foods properly. One of the factors in this gastric acid is a product called intrinsic factor, which is actually a protein. And the role of intrinsic factor is that it cleaves on to the B12, whether it's from food or from the supplement, and helps that B12 travel through your digestive system to the end of your small intestine, where it's actually absorbed and metabolized. Without intrinsic factor, you won't be able to absorb the B12 your body needs. Another population that's at risk from B12 malabsorption would be folks that have had bariatric surgery procedures. Again, whether you've had gastric bypass, the band, or the sleeve, you've limited your ability to have enough gastric acid and enzymes in your new stomach--or your new pouch--to properly have enough intrinsic factor. One way to side-step this is to take the more active version of B12, which is Methylcobalamin. Methylcobalamin has the active component in it that, if you have less intrinsic factor or none at all, you're still guaranteed to absorb, hopefully, all or some of it in your small intestine. Some symptoms of B12 deficiency are low energy, fatigue, anemia, mental fatigue, mood changes, and sleep difficulties. The best ways to take your B12 is to make sure that your B12 supplement is indeed Methylcobalamin. That's the more active version that we spoke about earlier. Pernicious anemia is a chronic illness caused by impaired absorption of vitamin B-12 because of a lack of intrinsic factor (IF) in gastric secretions. It occurs as a relatively common adult form of anemia that is associated with gastric atrophy and a loss of IF production and as a rare congenital autosomal recessive form in which IF production is lacking without gastric atrophy.

For those of us who have been kicking the topic of vitamin D & B12 down the road like me or for those who have never heard of this! Studies have been showing for decades that B12 and Vitamin D deficiency is no vegan issue - pretty much everyone is affected. If it was a vegan issue, only vegans would be affected (but that's by far not the case). But studies show meat-eaters to be just as likely or even more likely to develop B12 deficiency. And no B12 deficiency was ever reversed by just eating meat again. We are talking about A LOT of B12 we lack, not a couple of decimal places... And Vitamin D3 can only be consumed from diet in minimal amounts. So it's a "sun deficiency" issue. The extra cells build up and form red, scaly patches called scales or plaques on the skin. About 30 percent of people with psoriasis also develop swelling, stiffness, and pain in their joints. This form of the disease is called psoriatic arthritis. 4. Multiple sclerosis Multiple sclerosis (MS) damages the myelin sheath — the protective coating that surrounds nerve cells. Damage to the myelin sheath affects the transmission of messages between your brain and body. This damage can lead to symptoms like numbness, weakness, balance issues, and trouble walking. The disease comes in several forms, which progress at different rates. About 50 percent of people with MS need help walking within 15 years after getting the disease. 5. Systemic lupus erythematosus (lupus) Although doctors in the 1800s first described lupus as a skin disease because of the rash it produces, it actually affects many organs, including the joints, kidneys, brain, and heart. The hallmark symptoms of Sjögren’s syndrome are joint pain, dry eyes, and dry mouth. 10. Hashimoto’s thyroiditis In Hashimoto’s thyroiditis, thyroid hormone production slows. Celiac disease People with celiac disease can’t eat foods containing gluten — a protein found in wheat, rye, and other grain products. When gluten is in the intestine, the immune system attacks it and causes inflammation. Celiac disease affects about 1 percent of people in the United States. A larger number of people have gluten sensitivity, which isn’t an autoimmune disease, but can have similar symptoms like diarrhea and abdominal pain.

And it's always confused me as a doctor. Do you really think there's an insurance policy for not eating well or not exercising or moving your body or not living in a compassionate and peaceful way? There's no insurance policy for that. When you don't eat well you get sick. And that's one of the reasons that I really promote a food first philosophy that certainly if people have a deficiency, for example, if you have an illness like pernicious anemia where your body doesn't allow you to absorb B12, of course you have to take a B12 supplement. Or if you're severely iron deficient, take an iron supplement. But over the long term what we really want to see is that people are getting their nutrients from food. So some of the reasons for this aren't about nutrition, they're about community, that when you're engaged when your food and your food supply, when, for example, you go to a farmers market or if you have kids if you take them to a farm and teach them about where our food comes from that really creates a different philosophy or a different mindset about nutrition and about how we nourish ourselves. Certainly you can live on a multivitamin and sipping on some coconut oil, but that to me doesn't create a great meal. And that's really where I think about my favorite healthy delivery system, it's the dinner table where as people sit around there's a lot more going on than just the food. My family we're talking and we're processing the day and we're trying something new. Even just engaging with your food the creative process of cooking, you take some of these wonderful food I just sort of picked up at the deli and I thought well we can just whip up a little quick salad, add in lots of fats and proteins with these nuts and seeds, s little leafy green with the watercress. These functions are particularly important in tissues that undergo frequent cellular division (like hematopoietic cells) and during periods of rapid cellular division (such as infancy and pregnancy). Folate is found naturally in leafy vegetables and Folium is the Latin word for leaf. Today many grains are also fortified with Folate. With so many items having Folate added a deficiency from decreased intake of Folate is not very common. Usually a deficiency is caused by an increased demand for folate through pregnancy or hemolytic anemias such as Sickle Cell Disease. Folate deficiencies in pregnant women are linked with the occurrence of Neural Tube Defects in the fetus. A deficiency can also be caused by drugs that inhibit the pathways that Folate is involved in such as Trimethoprim (antibiotic) and Methotrexate (Chemotherapy). A Folate deficiency primarily causes anemia (decreased red blood cells). Anemias can present with a wide range of symptoms including weakness, fatigue, pale skin and shortness of breath. Folate Deficiency causes a specific type of Anemia called Macrocytic/Megaloblstic Anemia where the size (Mean Corpuscular Volume) of the red blood cells increased. This is because as the creation of DNA is inhibited the cell cycle in the hematopoietic cells is stalled. Cellular division is stopped but the cell continues to grow resulting in a smaller number of cells which are larger than normal cells. Patients with this type of anemia also often present with Hypersegmented Neutrophils on a blood smear, which is a neutrophil with 5 or more "segments" in the nucleus. You are also going to see an elevation of Homocysteine levels in Folate Deficiency. This is because B12 needs to receive a methyl group from Folate so it can pass it on to Homocysteine to create Methionine. Less Folate means B12 doesn't have a methyl group to pass on and Homocysteine builds up. Unlike B12 deficiencies, Folate deficiency does not result in a buildup of Methylmalonic Acid or neurological symptoms. Vitamin B12 (Cobalamin) is a vitamin, like Folate, that is important for 1 carbon chemical reactions. B12 is found in many animal products and Vegans are at the highest risk of deficiency. B12 taken in through the diet is bound to proteins. Stomach acid and digestive enzymes must first separate B12 from the proteins. If there is a deficiency of stomach acid due to disease or medication (like proton pump inhibitors or antacids) it can cause decreased absorption and B12 deficiency. Once it is no longer protein bound, B12 binds Intrinsic Factor which is released by the Parietal Cells in the stomach. Intrinsic Factor then chaperones the B12 to the terminal ileum where it is absorbed. Pernicious Anemia is the autoimmune destruction of parietal cells which leads to less Intrinsic Factor production. Less intrinsic factor means less B12 absorption and B12 deficiency. Damage to the terminal ileum, such as in Crohn's Disease, can also inhibit B12 absorption. Folate and B12 work closely together handing off methyl groups to each other. It is sort of like a game of hot potato. Folate doesn't "want" the methyl group so it passes it on to B12. B12 doesn't "want" it either so it passes it off to methionine. In effect, B12 helps recycle methyltetrahydrofolate back into tetrohydrofolate which can be used to create DNA. This means that a deficiency of B12 can cause a deficiency of Folate as less Folate is being recycled into its "active" form. Therefore, B12 deficiency present very similarly to Folate deficiency. You get Macrocytic/Megaloblastic Anemia with Hypersegmented Neutrophils and increased Homocysteine levels. If you mistakenly diagnose Folate Deficiency when it is really B12 Deficiency the patient will get better with Folate supplementation, but permanent neurological damage will be done overtime. Odd chain fatty acids are broken down to eventually give Methylmalonyl CoA. B12 is a cofactor in the process that converts methylmalonyl CoA into Succinyl CoA which can then be used in the TCA cycle to generate energy. If there is not enough B12 this reaction is slowed and Methylmalonic Acid builds up. This Methylmalonic Acid build up is toxic to neurons and leads to demyelination in the posterior and lateral columns of the spinal cord. This is called Subacute Combined Degeneration and presents with peripheral numbness/tingling, spasticity, and loss of vibration and proprioception.

The Importance of B12 | Post Surgery Nutrition | Blossom Bariatrics | Las Vegas Hi guys, Michelle Phelan here, nutritionist at Blossom Bariatrics and today I'm going to share with you some amazing tips and some awesome finds at the grocery store. Today I'd really like to talk a little bit about vitamin B12, the signs of B12 deficiency, and excellent sources of B12. Signs of B12 deficiency are fatigue, constipation, depression, low sperm count, and low libido, weakness, anemia, and asthma. So all of these can mimic other things going on in your body, so it's important to get your levels tested. So something to also keep in mind post surgery is that the nature of perhaps a sleeve surgery, or gastric bypass is that there's a protein in your stomach, produced in your stomach by the cells in your stomach called intrinsic factor and intrinsic factor, after surgery, particularly with sleeve patients, is available much less so. So you have less intrinsic factor available and this is the little guy who likes to hang on to that B12 whether from supplements or food and help that B12 travel safely through the different PH's in your digestive system where it can be disrobed and methylated, or become active at the bottom of your small intestine. So it's important again to make sure you have enough digestive juices, hydrochloric acid, and intrinsic factor in your stomach. Now if this is not the case, which happens in many patients post surgery with the sleeve procedure, it's important to take a high quality B12 supplement. Now many of you know that I really promote methylcobalamin B12 post surgery because it's methylated. It's already active, it's ready to go. Whether you have enough intrinsic factor or not. So play it safe. Make sure that you're B complex has the methylcobalamin B12 version of B12 in it. It's super important. Don't get anything else if you're a post-surgery bariatric patient. Something else I'd like to add is that there's some things that can deplete your body of B12 much more quickly. So if you are on things like Metformin, diabetes medication, birth control, depression medication, or Nexium, like acid reflux medication, these things can inhibit the absorption of B12 as well. In fact, although Nexium and these antacids help short term with the pain of acid reflux and acid indigestion long term, they inhibit your stomach's ability to create enough hydrochloric acid to properly break down food and it also inhibits your stomach's ability to make intrinsic factors. So you could be at a huge disadvantage to getting B12 if you're taking these medications. So it's always beneficial to supplement with methylcobalamin B12. Make sure that the B12 is in addition to your B complex. B12 is one of the eight B vitamins. They all work together best as a family. Okay, top B12 containing foods, so beef liver, I know right? But that is the top way to get B12 naturally. Sardines, I know, another ugh right? But again, the second best way to get B12 from food. Grass-fed organic beef, so there's another option as well. Tuna has B12 in it, cottage cheese, lamb, salmon, all of these things have high amounts of B12. But also keep in mind that although these foods are on the very much eat these foods list, as a post-surgery bariatric patient, if you like them, as a patient post surgery you're not able to eat a lot of them, maybe like one or two ounces. So you still need to supplement. If you really like beef liver, I still recommend you supplement with methylcobalamin B12. Thanks for watching.
