Over 100,000 new cases of colon cancer are diagnosed in the United States alone annually. For patients who are surgically receptable, meaning they can undergo an operation for their colon cancer, we might not catch small clusters of cancer cells called micrometastases that might be around at the time of surgery, but are not detectable by current methods. For patients who have high risk tumor features or positive lymph nodes, the goal of postoperative systemic therapy which is delivered via the bloodstream is to try and get rid of some of these small clusters of cells, to really increase the cure rate. For patients who have non operable, metastatic colon cancer systemic therapies like chemotherapy and targeted therapies are used.

Mesothelioma Survival Rates
Mesothelioma Survival Rate Mesothelioma Survival Rate “How long can I live with this disease?” is a common query for those who have been diagnosed with mesothelioma. Indeed, many patients will turn to their doctor with this question as will their concerned friends and family members. As with most diseases, however, there is no one answer as to the life expectancy of a mesothelioma patient. In general, the mesothelioma survival rate is based on a number of different factors. Every case is a little bit different and, although the survival rate for mesothelioma victims has traditionally been grim, more and more patients are living longer with the disease as scientists discover new and more effective ways to detect the disease and to treat it. Survival rates for mesothelioma patients are dependent upon a variety of factors that may include any or all of the following: Mesothelioma Latency Period Mesothelioma is an unusual form of cancer in that it has a very long latency period. Essentially, this means that the disease often “hides” in the body for many years, sometimes for up to four or five decades. Hence, 40-50 years may have passed since initial exposure to asbestos first occurred. This extended latency period also means that the disease is not detected until symptoms arise. Often, by that time, the cancer has spread from the primary area to other parts of the body. Stage of Disease at the Time of Diagnosis Individuals whose mesothelioma is discovered in Stage 1 or 2 have a greater chance of surviving the disease for an extended period of time due to the fact that more treatment options will be available to them. Individuals diagnosed with Stage 3 or 4 mesothelioma will have fewer treatment options, and thus, a less favorable prognosis. The worst mesothelioma prognosis is seen in patients with advanced cancer that present or are symptomatic with distant metastases of the primary tumor, i.e., the cancer has spread from the primary area to other parts of the body. Operability of the Tumor Due to the aforementioned latency period, surgery to remove the primary mesothelioma tumor is often not an option. However, if the disease is discovered in an early stage, a surgical procedure to remove the tumor may be a possibility, especially when the patient is otherwise healthy. Age of the Patient The younger the patient, the better the chance for survival. Studies have shown that age does indeed play a factor in survival rate and young patients certainly fair better. Co-morbidity with Other Illnesses Patients, especially older ones, who have many other medical issues are less likely to survive for any length of time with mesothelioma. This is especially true with older patients, who account for large numbers of mesothelioma victims. Heart disease, kidney problems, diabetes, hypertension, and a host of other illnesses or disorders can severely impact treatment options and, hence, shorten a patient’s life span. Mesothelioma Treatment Plan The length of time a mesothelioma victim survives may depend on the treatment(s) he receives. Top mesothelioma doctors will be as aggressive as possible, understanding that this is a tough cancer to treat. Area and Type of the Disease Mesothelioma comes in various forms. About 75 percent of cases appear in the pleura, 15 to 20 percent in the peritoneum, and just 5 percent in the pericardium. Pleural mesothelioma is easier to treat and patients will generally have a longer mesothelioma survival rate than those whose disease is centered elsewhere. In addition, those with epithelial mesothelioma (most cases are of this type) have a better prognosis than those with other types of mesothelioma.

The N Engl J Med image of the week shows the plantar surfaces of a patient’s feet. She received a heart transplant recently. Clonal hematopoiesis of indeterminate potential (CHIP), which is defined as the presence of an expanded somatic blood-cell clone in persons without other hematologic abnormalities, is common among older persons and is associated with an increased risk of hematologic cancer. We inspect a four-cohort study showing that CHIP increases the risk of coronary disease substantially and that deletion of Tet2 in mice accelerates atherosclerosis. Pertuzumab increases the rate of pathological complete response in the preoperative context and increases overall survival among patients with metastatic disease when it is added to trastuzumab and chemotherapy for the treatment of human epidermal growth factor receptor 2 (HER2)–positive breast cancer. In the next trial, oncologists investigated whether pertuzumab, when added to adjuvant trastuzumab and chemotherapy, improves outcomes among patients with HER2-positive early breast cancer. They randomly assigned patients with node-positive or high-risk node-negative HER2-positive, operable breast cancer to receive either pertuzumab or placebo added to standard adjuvant chemotherapy plus 1 year of treatment with trastuzumab. The outcome was positive but barely so. Twenty years ago, men with localized prostate cancer were randomized to radical prostatectomy or watchful waiting. At 12 years, outcomes were no different. After nearly 20 years, the absolute difference in all-cause mortality between men assigned to surgery and those assigned to observation was less than 6 percentage points, and the absolute difference in prostate-cancer mortality was 4 percentage points. The extended follow-up yielded slightly greater differences in mortality favoring surgery than those described earlier, although the differences remained nonsignificant. Few studies have evaluated the relationship between changes in diet quality over time and the risk of death. Epidemiologists used Cox proportional-hazards models to calculate adjusted hazard ratios for total and cause-specific mortality among 47,994 women in the Nurses’ Health Study and 25,745 men in the Health Professionals Follow-up Study from 1998 through 2010. Changes in diet quality over the preceding 12 years (1986–1998) were assessed with the use of the Alternate Healthy Eating Index–2010 score, the Alternate Mediterranean Diet score, and the Dietary Approaches to Stop Hypertension (DASH) diet score. Colorado Experience: Colorado's Cold War (Rocky Flats)
In 1951, in the early days of the Cold War, the U.S. Atomic Energy Commission announced the construction of a new facility in Arvada, Colorado, 16 miles from Metro Denver. Although the general public was largely unaware of what went on at the site, the Rocky Flats Nuclear Weapons Plant manufactured the plutonium “trigger” used in every nuclear weapon created in the United States. For the next several decades, Rocky Flats was a major stimulus to the local economy, providing thousands of jobs to area residents. But it was also a source of increasing controversy, as reports of radioactive contamination affecting the local environment and the truth about what was really going on at Rocky Flats became public. Individualization of Primary Surgical Therapy for Breast Cancer (USPHS, 1969)
Jerome A. Urban, M.D. lectures, using diagrams, graphs, and photographs. Three surgical approaches to breast neoplasms are discussed: modified radical mastectomy, radical mastectomy, and extensive radical mastectomy. Modified radical is indicated when: tumor is in situ and confined to the breast, tumor is low-grade, non-aggressive. Photographs shown of Paget's disease, nipple lesions, eczema of nipple with crusting. Radiation therapy should follow surgery. Modified radicallooks better and allows better function than the radical or extensive radical. Radical mastectomies followed by radiation therapy were performed on patients with early infiltrating tumors (survival rate 50%). In patients in which the disease was confined to the breast and who were treated with post-operative radiation, the rate was 54.4%. The extended radical mastectomy was performed on patients with a high risk of internal mammary spread, recurring cancers after radical mastectomy and radiation therapy, especially parasternal recurrence. Outer quadrant recurrence was much less frequent. Survival rates were 84% in cases where the axilla was not involved and 19% where the axilla was involved. Charts by Julian Blum from Middlesex Hospital, London, show the outcome of untreated breast cancer 1805-1933 and 1902-1933. Urban gives directions for performing extended radical when tumors are in other than the outer quadrants. Slides illustrate fascia inserted internally and externally, the latter preferred. Urban repeatedly emphasized that good primary therapy produces good results, and urges mammography, attendance at cancer clinics, and self-examination. Definitive diagnosis should be made from surgically removed tissue. Physicians should be alert for breast abnormalities in clinic and office. Produced by the United States Public Health Service.
