[PubMed] Abstract INTRODUCTION: The objective of this systematic review is to determine if the treatment of streptococcal pharyngitis with a daily dose of amoxicillin is similar in effectiveness to other dosing schedules (every 6, 8 or 12 hours) of the same antibiotic or penicillin V. MATERIAL AND METHODS: Randomised clinical trials (RCT) comparing amoxicillin (one dose per day) compared to other dosages of amoxicillin (every 8-12 hours) or penicillin V (every 6, 8 or 12 hours). Search databases consulted: Medline, Central, EMBASE and Google Scholar. The results were combined using the risk difference (RD). We measured the effectiveness of each treatment with a negative throat culture on the 14-21th day, being previously positive to group A Streptococcus (under a non-inferiority hypothesis, where the upper limit of the 95% confidence interval [95% CI] of the DR does not exceed 10%) and clinical failure on days 10-21. The results were combined according to a fixed effects model or random depending on whether or not there was heterogeneity. RESULTS: Four RCT met the selection criteria with 1,314 participants (657 received amoxicillin once per day, and 657 received other antibiotics or dosages): a) any positive culture for Streptococcus (14-21st day, 4 RCTs): DR: -0.5% (95% CI: -5.1% to 4.2%; b) persistence of the same serotype (14-21st day, 3 RCT): DR: 0.32% (95% CI: -3.1% to 3.7%; c) clinical failure (2 RCT): DR: 1.7% (95% CI: -1.9% to 5.4%; d) adverse effects (4 RCT): DR: -0.39% (95% CI: -1.5% to 6.8%). There were no statistically significant differences in any comparisons. CONCLUSIONS: Amoxicillin, administered once daily is not inferior to other dosages of the same antibiotic or penicillin V. This will make a thick paste she can put it inside her baby’s cheek a little at a time. Always label the medicine with: the name of the baby, the name of the medicine, how much medicine to give the baby, how often to give it, and for how many days. Explain the label to the mother. Have her give the first dose there at the clinic. Encourage the mother to breastfeed the baby afterward so that the baby will swallow all the medicine. The mother should keep all medicine in a cool place, out of the sun, and out of reach of children. Let her know that all the tablets or syrup must be used to complete the treatment even if her baby gets better. Check that the mother understands before she leaves the clinic. Remember: Make the preparation and dosing easy for the mother. Guide her by giving the first dose of amoxicillin at the clinic. Peptidoglycan, the most important component of the cell wall, is a polymer made of N-acetyl muramic acid alternating with N-acetyl glucosamine which are cross-linked by chains of four amino acids. The function of the bacterial cell wall is to maintain the characteristic shape of the organism and to prevent the bacterium from bursting when fluid flows into the organism by osmosis. Synthesis of peptidoglycan and ultimately the bacterial cell wall occurs in a number of stages. One of the first stages is the addition of 5 amino acids to N-acetyl muramic acid. Next, N-acetyl glucosamine is added to the N-acetyl muramic acid to form a precursor of peptidoglycan. This peptidoglycan precursor is then transported across the cell membrane to a cell wall acceptor in the periplasm. Once in the periplasm, the peptidoglycan precursors bind to cell wall acceptors, and undergo extensive crosslinking. Two major enzymes are involved in crosslinking: transpeptidase and D-alanyl carboxypeptidase. These enzymes are also known as penicillin binding proteins because of their ability to bind penicillins and cephalosporins. Eventually, several layers of peptidoglycan are formed all of which are crosslinked to create the cell wall. Gram positive bacteria have many more layers than gram negative bacteria and thus have a much thicker cell wall. Beta-lactam antibiotics include all penicillins and cephalosporins that contain a chemical structure called a beta-lactam ring. This structure is capable of binding to the enzymes that cross-link peptidoglycans. Beta-lactams interfere with cross-linking by binding to transpeptidase and D-alanyl carboxypeptidase enzymes, thus preventing bacterial cell wall synthesis. By inhibiting cell wall synthesis, the bacterial cell is damaged. Gram positive bacteria have a high internal osmotic pressure. Without a normal, rigid cell wall, these cells burst when subjected to the low osmotic pressure of their surrounding environment. As well, the antibiotic-penicillin binding protein complex stimulates the release of autolysins that are capable of digesting the existing cell wall. Beta-lactam antibiotics are therefore considered bactericidal agents. Bacterial resistance to beta-lactam antibiotics may be acquired by several routes. One of the most important mechanisms is through a process known as transformation. During transformation, chromosomal genes are transferred from one bacterium to another. When a bacterium containing a resistance gene dies, naked DNA is released into the surrounding environment. If a bacterium of sufficient similarity to the dead one is in the vicinity, it will be able to uptake the naked DNA containing the resistance gene. Once inside the bacterium, the resistance gene may be transferred from the naked DNA to the chromosome of the host bacteria by a process known as homologous transformation. Over time, the bacterium may acquire enough of these resistance genes to result in a remodelling of the segment of the host DNA. If this remodelled DNA segment codes for cross-linking enzymes (i.e. penicillin binding proteins), the result is the production of altered penicillin binding proteins. These altered penicillin binding proteins can still cross-link the peptidoglycan layers of the cell wall but have a reduced affinity for beta-lactam antibiotics thus rendering the bacterium resistant to the effects of penicillin and other beta-lactam agents. This transfer process has resulted in penicillin-resistant S. pneumoniae through the acquisition of genes from other naturally occurring penicillin-resistant Streptococcus species. A second important mechanism by which bacteria become resistant to beta-lactam antibiotics is by the production of enzymes capable of inactivating or modifying the drug before it has a chance to exert its effect on the bacteria. Penicillin was formerly the antibiotic of choice in treating this infection. With the advent and widespread use of broader-spectrum antibiotics, significant drug resistance has developed. Penicillin may still be effective in treatment of pneumococcal pneumonia, but it should only be used after cultures of the bacteria confirm their sensitivity to this antibiotic. The baby who is not feeding well sucks less or poorly, or not at all. He may not open his mouth when offered the breast, or he doesn’t act hungry. He may not wake up to feed or he sucks weakly and doesn’t stay awake long enough to empty the breast. Here we compare a healthy baby. She is alert and suckles eagerly at the breast. Movement only when stimulated is the second danger sign. This baby appears sluggish and drowsy. Try to wake the baby by clapping or rubbing a foot. Notice that the baby doesn’t wake up easily. Now gently jiggle the baby; he wakens only briefly with this stimulation. Here we compare a normal sleeping baby. When we try to wake him -- even just by clapping, he reacts normally, by fussing and crying. And then he stays awake. This baby only moves when stimulated. Lethargy and poor feeding often overlap. A cold body temperature is the third danger sign. A body temperature measuring less than 35.5 degrees is dangerously low. If there is no thermometer, compare the baby’s body temperature to your own. If the baby’s feet and belly are cold the baby is too cold and may be very sick. Warm the baby quickly through skin-to-skin contact on the mother’s chest, and cover them with blankets. If, after the baby is warmer, he still looks unwell, this is a danger sign. A hot body temperature is the fourth danger sign. A body temperature measuring greater than or equal to 38 degrees is too high. The baby’s body will feel hot to touch compared to your own body. The mother may report that the baby’s mouth is hot on her breast and the baby’s face and body may look flushed. Cool the baby off by removing his clothes briefly. If the baby is still too hot or looks unwell, this is a danger sign. Severe chest indrawing is the 5th danger sign. Watch the baby’s chest and abdomen closely: notice as the baby takes a breath, his lower chest sucks in, and the belly appears to move out. Babies with breathing problems often have chest indrawing and fast breathing occurring together. Compare a baby with normal breathing. His chest moves out when he takes a breath, in contrast, when the baby with chest indrawing takes a breath, his lower chest draws in and his belly appears to moves out. A baby with any of the 5 signs of severe illness needs to be referred right away to a hospital. Give the first doses of antibiotics before the baby leaves. These babies will need intramuscular injections of ampicillin and gentamicin. Notify the facility, write a referral note and arrange transportation. Make sure the baby is fed often and kept warm through skin-to-skin contact throughout the trip. Every effort should be made to refer the baby to a hospital. If the family does not accept to go or cannot get there, do your best to care for the baby in the community though realize this care is not the same. Treat the baby according to your national guidelines with one of 2 regimens. The preferred option is amoxicillin by mouth twice daily for 7 days plus IM gentamicin once daily for 7 days. Check the baby at each daily visit to be sure he’s gradually improving. The second option is giving amoxicillin by mouth twice daily for 7 days plus IM gentamicin once daily for just 2 days.
