Today, the estimated worldwide prevalence of type 1 and 2 diabetes is close to 300 million. This number is predicted to rise to 439 million by 2030, with type 2 diabetes accounting for about 90% of these cases. Individuals with type 2 diabetes are often told by their doctors that their disease can be effectively managed with healthy eating, regular exercise and blood sugar monitoring. These students have created a series of three videos that simplify the complex disease process of type 2 diabetes in the hopes that better understanding will motivate patients to better manage their disease. In this video, the students uses animation to show how obesity can increase a person's risk of getting diabetes by inducing insulin resistance. Approximately 85% of type 2 diabetics are obese. Diabetes is a disease of sugar metabolism so your body actually, in type 2 diabetes has a resistance to insulin. Insulin is the hormone that helps to control the sugar levels in your body. Obesity is a major risk factor for developing diabetes. The diet that is necessary to help keep your sugars regulated, if you are a diabetic many times will also help you lose weight if you are obese. So you want to try to avoid simple sugars, things like white bread, obviously anything high in sugar. You want to try to eat things with higher protein content perhaps and things that have a lower glycemic index. A low glycemic index would be a type of food that your sugar does not increase substantially after you eat it. So for example, if you were to eat beans as opposed to a piece of white bread, your sugar would be better controlled after eating the beans, and those are also better food choices, if you're trying to lose weight. The good news is that if you do lose weight, most likely your diabetes will improve and your doctor will be better able to control your sugars. Sometimes diabetes can actually resolve, if you are able to lose a substantial amount of weight. So that's just a little bit about how obesity and diabetes are related.

Diet soda is linked with diabetes? According to recent research, artificial sweeteners alters our gut microflora/bacteria which worsens our ability to regulate glucose. Yet another reason to avoid diet soda. The bigger issue, however, is the worsening of our sweet tooth. Like sugar, our consumption of artificial sweeteners like Splenda (sucralose) and Nutrasweet (aspartame) is going up. Our love of sweeteners - natural ones too (Stevia) - shows our addiction to sweetness is only worsening. So if things aren't as sweet anymore, it doesn't mean your taste buds don't work. According to the October 2014 issue of the journal, Nature, artificial sweeteners such as saccharine, Splenda, and Nutrasweet (aspartame) cause changes in the human microbiome (intestinal microflora) that lead to glucose intolerance (insulin resistance) within one week in more than half of the subjects of a small study. When stool from these people was tranplanted to mice, the mice developed insulin resistance. However, when stool from those people who did not develop glucose intolerance was transpanted to mice, it did not cause insulin resistance. Thus, the abnormalities causing the early changes of type 2 diabetes and of obesity were transmitted by the intestinal microflora! It is ironic that artificial sweeteners that have low or no calories is causing the reverse effect of why they were introduced! Now we need to inform the American Dietetic Association, the American Diabetes Association, and MDs that articifial sweeteners should never be recommended. Let's see how long it takes for these changes to come into clinical practice. Once again, follow the dollars if you want to understand how artificial sweeteners were approved by the FDA and endorsed by organizations with conflicts of interest.

In fact about 85% of type 2 diabetics are obese. One option to help lose weight is weight loss surgery and weight loss surgery or bariatric surgery has been shown to positively affect diabetes and in many cases to actually cure diabetes in some people. When we talk about weight loss surgery, we're talking about things like the gastric band, the gastric sleeve or the gastric bypass. So with all of these surgeries you lose weight, and after you lose weight, your sugars are usually better controlled. The exciting thing about the sleeve gastrectomy or gastric sleeve and the gastric bypass surgeries, in particular, are that after you have these surgeries the hormonal environment in your body is changed, such that your body is better able to handle sugars. For example, I have patients that are type 2 diabetic that are on insulin and then have gastric bypass, say. Some of these patients actually leave the hospital and never need their insulin again. So clearly, the weight loss surgery is curing their diabetes, not just from weight loss because they're not losing 100 pounds overnight, but it's because of the hormonal changes that happen in their body. Studies have been published by the Cleveland Clinic, as well as Cornell comparing the difference between weight loss surgery and medication alone for the treatment of diabetes. And I believe the results were actually published on the front page of the New York Times, and these studies showed that weight loss surgery, actually had better results in controlling diabetes than medication alone. So if you are obese and you do have type 2 diabetes, speak with your doctor because maybe weight loss surgery is an option for you.

Liz Cromwell always puts her family first: She’s currently caring for her eight children as well as her husband, who suffers from spinal stenosis. After a painful nine months of pregnancy followed by the stillbirth of her ninth child, Cromwell says she hit rock bottom. She discovered she was a Type 2 diabetic, gained weight and suffered from severe depression. “I have to always be fighting to stay on top for my family because I know if I’m gone then it’s gonna be a struggle. That is why I just decided that I need to do something for me,” says Cromwell. After trying months of boot-camp-style workouts yet failing to see much progress, Liz finally found Eton CrossFit and instantly knew it was the thing she needed to change her entire life. “We’ve got an amazing, amazing, amazing community—very supportive. … Once I walk into that box, it’s a great release,” says Cromwell, whose fitness has improved dramatically since she started training. The community at Eton CrossFit also helped Cromwell overcome additional obstacles including shoulder surgery and fibroids. In just two years, Cromwell—who “feels like a whole new person”—has greatly decreased her body-fat percentage and cut her insulin usage down from an entire pen to just 20 mL per day. A compound that plays a role in the regulation of the body's day/night rhythm has also been found to be implicated in how the liver produces glucose, offering a biological link between the body's biological clock and metabolic disorders such as diabetes. The research is published online ahead of print in Science. Here is some information on diabetes: • It is a condition in which glucose levels in the blood rise • Chronically high levels of glucose in the blood can lead to health problems such as heart disease, stroke, and blindness • Laboratory studies have linked disrupted biological clocks with an increased risk for diabetes Researchers from the University of California at San Diego identified a small molecule they called KL001 while screening for compounds that affect the body's day/night internal clock, known as the circadian rhythm. They demonstrated that KL001 affects the circadian rhythm by preventing the breakdown of a protein known as cryptochrome. This protein regulates the circadian rhythm of both plants and animals, but it is also involved in the production of glucose by the liver. The investigators used a complex mathematical model to help clarify the role that KL001 plays in regulating the circadian rhythm. This model helped them zero in on its effect on the liver. They placed KL001 into the liver cells of mice. Normally, in the presence of the hormone glucagon these cells will produce glucose. Adding KL001 to the cells prevented this process. Today's research provides a biological basis for the observed link between disrupted circadian rhythms and the development of metabolic disorders such as obesity and diabetes. It may be the first step toward developing new treatments for these disorders.

Also a couple of other slips I noticed since posting: * I said Mexico went from effectively zero obesity to 70%, it was effectively zero obesity to a total of 70% Overweight and Obese I believe. * I also referred to small dense pattern B LDL as being the pathogenic component, but didn't clarify that the latest science suggests that it is the presence of these that indicates other issues (e.g. higher number of ApoB particles, more oxidative stress etc) There's probably a few more, but the message doesn't change....
