With childhood obesity on the rise in the United States, physicians are encountering greater challenges in treating common conditions such as asthma. In recent years, research has shown that a possible connection between obesity and asthma, metabolic syndrome, and overall lung function. Erick Forno, MD, of the Division of Pulmonary Medicine, Allergy, and Immunology at Children’s Hospital of Pittsburgh of UPMC, explains how several recent studies have shed light on this connection, and that pediatric pulmonologists should be increasingly aware of these considerations.

In a recent study, scientists have found that asthma could give rise to childhood obesity epidemic. The University of Southern California study showed that beyond wheezing and shortness of breath, asthma can lead to bodies that make young people more susceptible to other health problems later in life. Senior author Lida Chatzi said asthma and obesity pack a one-two punch against childrenís health, which raises concerns about a public health crisis due to their prevalence. For two decades, scientists have documented the parallel epidemics of childhood asthma and obesity, with focus on how obesity is a risk factor for asthma. In adults, obesity is an important risk factor for new asthma, especially among women, but the relationships appear to differ in children. Few studies look at the problem the other way around to understand how asthma contributes to obesity in kids, which prompted scientists to undertake this research. On average, the scientists found that children diagnosed with asthma had a 66 percent higher risk of becoming obese than those without an asthma diagnosis. For children with persistent wheezing symptoms, their risk of developing obesity was 50 percent greater compared to children without such symptoms. Children with active asthma were nearly twice as likely to develop obesity than those without asthma and wheezing, according to the study. The study appears in the European Respiratory Journal. A GENOMICS OBESITY EPIDEMIC OUR PERSPECTIVE Abstract GENOMICS is the study of the entire human genome and involves not only studying the actions of single GENES but also the interactions of multiple GENES with each other and with the environment. This article emphasizes the multifactorial nature of common OBESITY, which is caused by the interaction of GENES, environment, and lifestyle. Individual variation in GENES that influence behavior, satiety, and taste suggests that a one-size-fits-all approach to reducing or preventing OBESITY may be ineffective. Data are not yet available to allow for personalized OBESITY interventions based on genetic predisposition. However, a GENOMICS approach may provide a useful framework for addressing the OBESITY epidemic. More research is needed before specific targeted public health interventions that include GENOMIC strategies can be effectively integrated into addressing OBESITY in public health practice. Introduction OBESITY is a multifactorial disorder that reflects complex interactions of GENES, environment, and lifestyle. Considering the OBESITY epidemic from a GENOMICS perspective, which takes into account the effect of all GENES in the genome as well as the interactions of those GENES with each other and with the environment, has the potential to improve the effectiveness of OBESITY prevention and intervention strategies. Evidence of Genetic Role in Weight Regulation Numerous family studies indicate a genetic component to weight regulation. Stunkard et al (1) show that body mass index (BMI) is highly correlated in identical twins, even if each twin is raised in a different environment. Studies of eating behavior in fraternal and identical twin pairs suggest that a significant portion (45%–60%) of eating behaviors (e.g., binge eating) was due to genetic factors (2). Studies of children and their parents also point to a genetic role for OBESITY. Whitaker et al (3) reported that, although not all OBESE children have OBESE parents, parental OBESITY is a significant predictor of children’s OBESITY. Parental OBESITY more than doubled the risk of OBESITY for children younger than 10 years. ONE PROSPECTIVE LONGITUDINAL STUDY (4) FOUND THAT THE STRONGEST PREDICTOR OF OBESITY WAS PARENTAL OBESITY, INDEPENDENT OF DIET OR ACTIVITY. Another study (5) found that, after controlling for environmental factors, children of OBESE parents had a higher preference for fatty foods, a lower liking for vegetables, a greater tendency to overeat, and a stronger preference for sedentary activities than did children of normal-weight parents. How Much of OBESITY is Genetic? Separating genetic factors from other factors is difficult because GENES are part of a dynamic system that is constantly in flux in response to environmental cues. Lifestyle issues too little activity and too many calories from food and drinks are the main contributors to childhood obesity. But genetic and hormonal factors might play a role as well. For example, recent research has found that changes in digestive hormones can affect the signals that let you know you're full. Risk factors of Childhood Obesity! increase your child's risk of becoming overweight: Diet: Regularly eating high-calorie foods, such as fast foods, baked goods and vending machine snacks, can easily cause your child to gain weight. Family factors: If your child comes from a family of overweight people, he or she may be more likely to put on weight. This is especially true in an environment where high-calorie foods are always available and physical activity isn't encouraged. Psychological factors: Personal, parental and family stress can increase a child's risk of obesity. Some children overeat to cope with problems or to deal with emotions, such as stress, or to fight boredom. Their parents may have similar tendencies. Socioeconomic factors: People in some communities have limited resources and limited access to supermarkets. As a result, they may opt for convenience foods that don't spoil quickly, such as frozen meals, crackers and cookies. Eight-year-old Sohn Soo-min is heavier than most of his classmates. After a body check up, his parents were told he has a body fat ratio twice the average for a boy his age. Like many kids in Korea, Soo-min likes to tuck into calorie and carbohydrate-rich food like white rice, red meat and fast food, and doesn′t get enough exercise. "I heard people say that fat children usually eat up being taller as adults." The problem has worsened in recent years. In Korea, one in four boys aged 5 to 17 are obese, which is slightly higher than the OECD average. What′s more concerning is that well over half of those boys remain obese into adulthood and are at higher risk of becoming morbidly obese. Obesity can lead to a host of other health issues, like diabetes, high blood pressure and it can also increase the chance of cerebrovascular disease at a young age. "Obesity is known to cause cancer, including colorectal cancer, breast cancer, as well as asthma, fatty liver and other liver disorders." It also incurs great social costs in terms of medical expenses and loss of production. According to recent data, the socioeconomic costs stemming from child obesity in Korea amounts to 1.3 billion U.S. dollars each year. Child and teenage obesity not only affects the individual, but their family, school and the community as a whole. Experts advise parents to create a friendly, but firm atmosphere for their children, and encourage them to exercise and eat fresh fruit and vegetables to maintain a healthier lifestyle. Children who are obese can suffer social discrimination. The stress caused by this can lead to low self-esteem. This in turn can slow down intellectual and social performance. These debilities may carry over into adulthood. Help your child maintain a healthy weight. The following steps will help you bring about the changes your child needs to defeat obesity. Develop Healthy Eating Habits Limit the eating of sugar and saturated fats, and especially trans fat. Make available a generous supply of vegetables, fruits, and whole-grain products. Advocate drinking more water and less sugar-sweetened drinks. Serve appropriately-sized portions. Remember how small your child's stomach really is. Make healthy changes to old recipes. Replace high-sugar high-fat foods with healthy alternatives. Remove unhealthy temptations. Increase your child's physical activity. It is recommended that children and teens get an hour of moderate intensity physical activity daily. A few examples include: A brisk walk. Jump rope. Swimming. Playing soccer. Dancing. A game of tag. Limit the time your child spends watching TV, playing video games, or surfing the Web. A two hour limit of sedentary time is a good goal. If your child can’t control his or her time on the Internet on his own, consider buying a software-based computer timer such as Ez Internet Timer. With this program, parents can easily enforce time limits. Help your child develop a good attitude to food and nutrition. Reward your child with things other than 'treat' foods so they grow up feeling chocolate, cake, cookies, chips, fizzy drinks etc are not something they deserve or a form of consolation. Avoid them forming a habit of comforting eating, if possible. It's easy for parents to feel they are depriving their children if they deny them what their peers are enjoying. You're being a good parent if your child has a healthy, balanced diet, however much the child may whinge and whine, that, "Everyone else is allowed candy for breakfast". It is less certain whether obesity is a completely independent risk factor or whether associations with obesity are there because of other risk factors linked to obesity. Using multi-modal state-of-the-art medical measurements, personalized computational models and advanced image processing pipelines, we aim to identify highly detailed obesity phenotypes in children and teenagers. To exploit this high dimensional and multi-modal data, we propose a deep learning approach to build compact patient signatures that can be efficiently used to created predictors based on similarity computation and case-based retrieval.

Childhood Obesity Welcome to Dr. Nandi’s health update. Today, we’re talking about a crisis in our country, childhood obesity. Nearly 20% of children ages 611 are obese in the US. In the past 30 years, childhood obesity has tripled. How is childhood obesity defined? It is a condition when the excess body fat affects the child’s health, usually based on a standard body mass index or BMI. When the BMI is over the 95th percentile, obesity occurs. When this occurs, a child has increased risk of Diabetes, high cholesterol, high blood pressure, asthma and sleep disorders. It can also result in early puberty or menstruation. Just as important, childhood obesity can result in low self esteem and bullying. Depression and learning problems are also seen more commonly. What causes childhood obesity? Genetics contributes to 525% of childhood obesity. Hormonal causes exist, but most cases are due to increased intake and less exercise. Children from lower income homes are at greater risk as well. A diagnosis is made by a use a growth chart and determining your child’s BMI percentile. A blood test for cholesterol, diabetes and other hormone imbalance also is used. After determining your child suffers from childhood obesity, treatment is initiated. For kids age 7 or less with no health problems, weight maintenance is encouraged. Over 7 yrs of age, controlled weight loss is the goal. Physical activity is encouraged and sedentary activities such as watching TV or computer use is limited. Medicines may be used for weight loss in adolescents over 12 if diet and exercise is not effective. Weight loss surgery can be an option for some adolescents with severe obesity but is uncommonly performed. Thank you for watching. Remember; make healthy living part of every day!
