Care of mother after delivery After the very exhausting phase of giving birth, the newborn is not the only one who needs to be taken care of. It is also essential to make sure that the woman is in a stable condition because the immediate postpartum is a critical stage for both the woman and her baby. Care within the First 24 Hours Providing nursing care to a postpartum woman during the first 24 hours entails the following: Assess the woman’s family profile to determine the impact that the newborn would give to the family and to the woman. Assess the woman’s pregnancy history, especially if the pregnancy was planned or unplanned as it will determine the ability of the woman to bond with the newborn. Assess the labor and birth history such as the length of labor and if any analgesia or anesthesia was used to determine any necessary procedures to be done. Determine the infant’s data and profile to help with planning the care of the newborn and promote bonding between the parents. The woman would also need a postpartum course such as her activity level after birth, any difficulties or pain felt, and if she is successful with infant feeding to determine any need for anticipatory guidance in home care. Assess any laboratory data of the woman to be certain that she is recovering well and if any procedures or additional diagnostic tests need to be performed. Assess the woman’s general appearance because it is a reflection of how well the woman is moving into the taking hold phase of recovery. Assure the woman that losing a quantity of her hair is not a sign of illness but because she is returning to her nonpregnant state, as hair grows rapidly during pregnancy because of increased metabolism. Assess for facial edema, especially for a woman with pregnancy-induced hypertension. Advise the woman to purchase a nursing bra that is one to two sizes larger than her pregnancy size to allow for increase. Assess the woman’s breast for any cracks or fissures, and avoid squeezing the nipple. Also, assess for signs of mastitis such as inflammation of a certain part of the breast. Assess the location, consistency, and height of the fundus through palpation. If the uterus is not firm upon palpation, massage it gently. Placing the infant on the mother’s breast also aids in stimulating contractions. Lochia is expected in a postpartum woman for 2 to 6 weeks, so assessment of its characteristics is necessary to determine if it is the normal lochia or not. Observe the perineum for ecchymosis, hematoma. Edema or any drainage and bleeding from the stitches. Care in Preparation for Discharge Before the woman is discharged, she must be educated properly regarding the care of the newborn and herself at home. Assess first the ability of the mother to absorb new instructions and to listen. Conducting group classes regarding newborn care could greatly help mothers learn not only what the instructors teach but also from the experiences that some mothers could share to the group. It is also recommended for fathers to attend such classes so the mother would have someone she can rely on with the newborn care. Individual instruction is also sought after postpartum, as the family will need to know how to care for the woman and the newborn after discharge. Teaching should not always be formal; it may come in the form of comments during classes or procedures. Instruct the woman to avoid lifting heavy objects for the first three weeks after birth. Make sure that the woman and the family understood the discharge instructions amidst all the frenzy of the new baby; review instructions with parents before they leave. Calling or visiting 24 hours after discharge is the best way to evaluate whether the family has been able to grasp all instructions and integrate the newborn into the family. Care after Discharge Discharge from the healthcare facility usually occurs after 2 to 3 days after woman can rest better at home and may eat better if she has cultural preferences regarding food. The newborn can also be exposed earlier to the routines of the family, and make it easier for her to adjust to extrauterine environment. A home visit after the discharge is usually recommended to check on how the family is doing now that they have a newborn in the house. High-risk newborns, newborns born to adolescent mothers, and newborns with mothers who have abused drugs during pregnancy need to have a specially planned discharge and home visit. Pregnancy history is assessed during the postpartum visit and if there are any difficulty with the bonding between the mother and the baby, and allow the woman to relate her labor and birth experiences.

APGAR Newborn Assessment Score Scale Test Made Easy w/ NCLEX Practice Questions

APGAR newborn assessment score scale (made easy) is a quick test used to assess the status of a newborn after birth. APGAR is used to assess a newborn at 1 minute and 5 minutes after birth. If the score is 6 or less the newborn will be assessed again at 10 minutes after birth. The word APGAR is a mnemonic for the categories assessed during the APGAR score assessment. The letters of APGAR stand for the following: Appearance (skin color), Pulse (heart rate), Grimace (reflex irritability), Activity (muscle tone), Respiratory (effort). Each category is scored 0-2 and the higher the score the better the baby is doing. A score of ten is the highest score a newborn can score. For the NCLEX exam and lecture exams, the nursing student should be familiar with the nursing interventions for a newborn based on an APGAR score. APGAR scores 7-10 require normal post-delivery care, scores 4-6 require some resuscitation measures such as oxygen/suction and stimulating the newborn, and scores 0-3 require full resuscitation measures. Stages of labor nursing OB review for nursing students who are studying for the NCLEX exam and maternity nursing lecture exams. This lecture explains the various stages of labour. There are four stages of labor and they include Stage 1, Stage 2, Stage 3, and Stage 4. In nursing school, you must know that the Stage 1 of Labor has three phases. These phases are the Latent (early labor), Active, and Transition phases. Stage 1 of labor will be where the cervix will dilate (open) from 0 to 10 cm and will have hundred percent effacement (thinning). During the Latent phase of Stage 1, the cervix will dilate 1-4 cm, contractions will occur every 5 to 30 minutes and be 30-45 seconds in length. During the active phase of stage 1, the cervix will dilate to 4 to 7 cm and thin (dilation will be 1 cm per hour), and contractions will be noticeably stronger and longer (45 to 60 seconds) every 3 to 5 minutes. The next phase, which is the transition phase, will be the shortest but most intense in regards to the strength and duration of contractions. During the transition phase, the cervix dilates from 8 to 10 cm and thins completely. This phase will lead to Stage 2 of Labor. Stage 2 of labor begins when the cervix has fully dilated and ends when the baby is fully delivered. Stage 3 of labor begins when the baby is fully delivered and ends when the placenta is delivered. Stage 4 of Labor is the first 1-4 hours after the delivery of the placenta and is the time when the mother is at most risk for hemorrhage, infection, and other problems. Fundal Height Measurement by Weeks Nursing Maternity Lecture NCLEX

Fundal height measurement is an important part of maternity nursing. By measuring the fundal height during pregnancy, we can determine how well the baby is growing and gestational age. To measure the fundal height, the patient will need to lie down on their back. You will then use a tape measure to measure from the symphysis pubis to the fundus. Fundal height is an important part of maternity nursing, and this is something nursing students might see on exams, such as the NCLEX-RN. Key Concepts about Fundal Height: The fundal height is above the symphysis pubis at 12 weeks. It is at the belly button (navel or umbilicus) at 20 weeks. At 36 weeks it measures at the xiphoid process, and then at 37-40 weeks the fundus regresses downward about 4 cm from the xiphoid process. After pregnancy (1 hour after delivery), the fundus is at the umbilicus. It will decrease 1 cm per day and be non-palpable by 10-14 days. By day 7, it will be at the symphysis pubis. Placenta previa nursing NCLEX review lecture on the types (partial, total, marginal previa), causes, symptoms, and nursing interventions. Placenta previa is the abnormal attachment of the placenta in the uterus near or over the cervical opening. The placenta is a very important structure for maintaining the pregnancy and it helps deliver nutrients and oxygen to the baby along with removing waste via the umbilical cord. Where should the placenta normally attach? The placenta should attach either at the top or the side of the uterus, NOT in the lower parts of the uterus, near or over the cervical opening. Placenta previa causes: Maternal age greater than 35 years old Multiples Already had a baby Drug use: cocaine or smoking Surgery to the uterus that causes scarring like fibroid removal or c-section etc. There are different types of placenta previa. These types include: Total placenta previa: the placenta completely covers the cervical opening Partial placenta previa: the placenta partially covers the cervical opening (not fully covered) Marginal placenta previa: the placenta is near the edge of the cervical opening Cases of placenta previa vary along with treatment. Placenta previa can be detected at the 20 week ultrasound. Sometimes if the placenta is found to be low lying (partial or marginal previa) the placenta will move upward away from the cervix as the uterus grows throughout the rest of the pregnancy. The position of the placenta will be reassessed at the 32 week ultrasound. Therefore, in some cases of placenta previa, it will correct itself. Placenta previa symptoms include: bright red painless bleeding, relaxed or soft uterus, and episodes of bleeding that are visible, bleeding after intercourse, abnormal baby position (breech or transverse lie). At Smile Ranch Dentistry, we require an IBCLC assessment before we perform a frenectomy on any nursing baby. Nursing dyads struggle for a multiplicity of reasons which may not be attributed to a tethered lip or tongue. For example, a mother with insufficient glandular tissue or some hormonal dysfunction may result in suboptimal nursing and performing a frenectomy on an infant in this scenario could be unnecessary. An IBCLC is highly trained and able to identify how babies function, and provide the best care plan for pre, during, and post-procedure. In particular, IBCLC's that have advanced training such as the Jennifer Tow’s class are able to create the most effective care plans around the release. In addition, IBCLC's are able to diagnose many other issues that may be affecting the babies oral functions and breastfeeding success that are outside our scope as dentists.
