Kidney stones (also called renal calculi, nursing lecture on kidney stones symptoms, treatment, pathophysiology, and causes for the NCLEX exam. What are kidney stones, which are also called renal stones or renal calculi? They are hard insoluble crystallized minerals and salts that have formed out of the filtrate produced by the nephron. Renal stones can vary in size. They can be very hard to see, while others are large like a walnut. In addition, renal calculi are composed of various materials, therefore there are different types of renal stones. Renal stone types include: calcium oxalate, uric acid, calcium phosphate, struvite, or cystine. In order for kidney stones to form, there has to be a concentration of minerals and salts in the filtrate. The concentration of minerals can form together to create a crystal (which can grow over time, break off and travel through the urinary system, or cause a blockage). Many predisposing factors can increase the risk of renal calculi formation. Renal stone causes include: consuming high amounts of oxalate, purine, protein foods or calcium supplements, recurrent urinary tract infections, hyperparathyroidism, hypercalcemia or hypercaluria, high levels of uric acid as with gout, gastrointestinal disorders such as ulcerative colitis, Crohn's Disease, genetic factors, or immobility. Kidney stone symptoms include: extreme pain (renal colic or ureteral colic), nausea/vomiting, urinary retention, infection. Treatment for renal stones include extracorporeal shock wave lithrotripsy (ESWL). It is an noninvasive procedure where shockwaves are created to penetrate though the skin and body tissue. Shockwaves will hit the stone and break it down into grains of sand like particles which can be passed out via the urinary system. Another procedure is called a percutaneous nephrolithotomy (invasive and require a surgical incision). This is where the stone is removed by a urologist and doesn't have to be passed as with EWSL. In addition, a ureteroscopy can be performed and this is where a scope is inserted through the urinary system and the stone can be removed or broken. Acute kidney injury (also called acute renal failure) nursing NCLEX review lecture on the nursing management, stages, pathophysiology, and causes (prerenal, intrarenal, postrenal). What is Acute Kidney Injury? It is the SUDDEN decrease in renal function that leads to the build up of waste in the blood, fluid overload, and electrolyte imbalances. What are the causes of Acute Kidney Injury? There are three causes, which are based on location. The first is known as prerenal injury and this is an issue with the perfusion to the kidneys that leads to decreased renal function. A second cause is known as intrarenal injury, and this is due to damage to the nephrons of the kidney. Lastly, postrenal injury is due to a blockage located in the urinary tract after the kidney that can extend to the urethra. This is causing the back flow of urine, which increases the pressure and waste in the kidneys. Stages of Acute Kidney Injury: There are four stage of acute kidney injury, which include initiation, oliguric, diuresis, and recovery stage. The initiation stage starts when a cause creates an injury to the kidney and then signs and symptoms start to appear. This leads to the oliguric stages. The patient will void less than 400 mL/day of urine during this stage and will experience increased BUN/creatinine levels, azotemia, hyperkalemia, hypervolemia, increase phosphate and decreased calcium levels along with metabolic acidosis. After this stage the patient can progress to the diuresis stage and this is where the patient will void 3-6 Liters of urine per day due to osmotic diuresis. The patient is at risk for hypokalemia, dehydration, and hypovolemia. The last stage is recovery and the patient's glomerular filtration rate has returned to normal. Therefore, the kidneys are able to maintain normal BUN and creatinine levels, electrolyte, and water levels. This video is part of a weekly NCLEX review series where I will be going over NCLEX style questions with you. I will be helping you analyze each question, and walk you through how to select the correct option. NCLEX questions require critical thinking and you must know how to use your nursing knowledge to gather the facts and analyze what the question is asking. This specific question is a type of question that wants to know based on your morning assessment findings which finding is abnormal and requires immediate nursing action. NCLEX Question: You have just received nursing report from the previous shift and are performing your morning patient assessments. You have a total of 4 patients that are either post-opt or pre-opt for surgery. Which assessment finding requires further nursing action? A. Standard & Isolation Precautions Nursing | Infection Control Contact, Droplet, Airborne PPE NCLEX

Nursing standard precautions and isolation precautions (nursing) review for the NCLEX exam. This review will cover standard precautions for infection control and transmission-based precautions (also called isolation precautions) such as contact, droplet, and airborne precautions. In addition, I will give you 3 isolation precaution mnemonics on how to remember droplet, contact, and airborne precautions. Standard precautions are precautions we follow with every patient at all times because we do not know if the person has an infectious disease or not. This is to protect ourselves and other patients. Standard precautions include: hand hygiene, wearing appropriate PPE (personal protective equipment), injection safety, respiratory/cough etiquette, patient room placement, handling of patient laundry and equipment. Isolation precautions are precautions we follow when we know or suspect a patient has an infectious disease. Depending on the type of infectious disease the patient has we will place the patient in the appropriate precautions. For example, if a patient has Pertussis (whooping cough) we will place the patient in droplet precautions. As the nurse, we would continue to follow standard precautions along with always wearing a surgical mask during patient contact. For a patient in airborne precautions, we would wear an N95 mask along with following standard precautions and place the patient in a special AIIR room (which is a negative pressure room). For a patient in contact precautions, we would always wear gloves and a gown during patient contact and follow standard precautions. NCLEX drag and drop practice question on the steps for mixing insulin (clear to cloudy). This NCLEX-style question requires you to correctly put in order the steps on how to correctly mix a short-acting and intermediate insulin in one syringe for administration. On the NCLEX exam, you may receive drag and drop type questions where you will be required to put the options in the correct order. These questions can be about nursing procedures or nursing priorities. This particular NCLEX practice question will require you to determine which insulin in drawn up in the syringe first and how you would perform these steps correctly. This video is part of a weekly NCLEX review series where I will be going over NCLEX-style questions with you. I will be helping you analyze and breakdown each question, and walk you through how to select the correct option. NCLEX questions require critical thinking and you must know how to use your nursing knowledge to gather the facts and analyze what the question is asking. NCLEX Drag and Drop Practice Question on Nursing Procedure: The physician orders 10 units of Humulin-R and 12 units of Humulin-N subcutaneously before breakfast daily. You are required to mix these two medications in one syringe for administration. Correctly put in order the steps for how you will perform this procedure: Withdraw 10 units of Humulin-R Clean off each vial top with alcohol prep Don gloves Inject 10 units of air into the vial of Humulin-R Withdraw 12 units of Humulin-N to equal a total of 22 units in the syringe Roll the vial of Humulin-N in between the palms of the hands Inject 12 units of air into the vial of Humulin-N Wash hands Watch the video for the correct answer and rationale. Pneumonia Treatment, Nursing Interventions, Antibiotics Medication | NCLEX Respiratory Part 2

Pneumonia nursing interventions lecture and medication treatment which includes antibiotics. Pneumonia is a lower respiratory disorder that affects the lung parenchyma which includes the alevolar ducts, bronchioles, and aveoli. This respiratory disorder is due to an infection (caused by either bacteria, virus, or fungi) that causes inflammation of the alevoli sacs. Patients with pneumonia can experience respiratory acidosis and other complications. The nurse's role is very important when providing care to the patient experiencing pneumonia. Some nursing interventions for pneumonia include: monitoring respiratory system (lung sounds, respiratory rate, oxygen saturation), skin color for cyanosis, especially the lips, nasotracheal suction as needed, maintaining patient hydration with 2-3 L of fluid orally (if not contraindicated), educating about incentive spirometer usage, collecting sputum cultures if ordered, encourage education about up-to-date vaccinations (especially Pneumovax and annual flu shot) and administering medications per doctor's order. This lecture concentrates on the antibiotics used to treat pneumonia. As the nurse, it is important to know the major side effects of these medications, if they are narrow or broad spectrum, and which drugs are included in each category. These medications include: vancomycin, macrolides, tetracyclines, fluoroquinolones, penicillins, cephalosporins. Asthma treatment (asthma attack), symptoms, pathophysiology nursing NCLEX review lecture. This lecture will detail the pathophysiology of asthma along with the early warning signs vs active signs of an asthma attack. In addition to a sample asthma action plan along with using a peak flow meter to monitor asthma, asthma triggers, nursing interventions for asthma, and all the medications used to treat asthma, which are bronchodilators and anti-inflammatory medications. What is asthma? Asthma is a chronic lung disease (no cure) that causes narrowing and inflammation of the airways (bronchi and bronchioles) that leads to difficulty breathing. How does it happen? Normally, when you breathe in air, it travels down through your upper airway to your lower airway, which includes the trachea, bronchi, bronchioles, and alveoli (where gas exchanges happens). The oxygen you breathe in crosses over into your blood stream, and the carbon dioxide in your blood crosses over into the airway to be exhaled. In patients with asthma, the bronchi and bronchioles are chronically inflamed and can become so inflamed that it leads to an asthma attack (wheezing, chest tightness, shortness of breath, coughing), which is usually due to a trigger of some type. Asthma triggers include: -Environment: smoke, pollen, pollution, perfumes, dander, dust mites, pests (cockroaches), cold and dry air, mold -Body Issue: respiratory infection, GERD, hormonal shifts, exercise-induced -Intake of Certain Substances: drugs (beta adrenergic blockers that are nonselective), NSAIDS, aspirin, preservatives (sulfites) Signs and Symptoms of an asthma attack include: chest tightness, wheezing (auscultate…expiratory wheezing and can progress to inspiratory wheezes in severe cases), coughing, difficulty breathing (***especially exhaling), increased respiratory rate Asthma treatment includes: bronchodilators (beta-agonists, anticholinergics, theophylline along with anti-inflammatories (corticosteroids, leukotriene modifiers, immunomodulators, cromolyn). How much does nursing school cost? How much RN school, LPN school, or nurse practitioner school? Those are great questions. Your total nursing school tuition costs will depend on many different factors. In this video, I'll break down some of the aspects of nursing school tuition. Some of the factors that can impact nursing school tuition costs/fees include the following: -In state vs out-of-state tuition (out-of-state tuition often costs more) -Private vs public school (public schools often charge less tuition) -Full time vs part time status (you can sometimes save money by taking more credit hours) -The length of the program (shorter programs often cost less) -Location (small rural areas cost less than big city areas) -Opportunity costs...and more http://www.registerednursern.com/nursing-school-costs-tuition/ LPN programs will typically have a tuition cost of around $4,000-30,000, with a common range of about $8-10,000. ADN-RN programs typically cost around $12,000-35,000 or more. BSN-RN programs cost around $25,000-120,000, depending on the program. If you want to become an advanced practice registered nurse (APRN), such as a nurse practitioner or CRNA, you'll generally need a BSN first, followed by 2-4 years of additional schooling. The total costs for an APRN degree will typically start at around $63,000 and go up to as high as $200,000 or more, depending on many factors. Nevertheless, nursing school tuition costs should be looked at as an investment, not an expense. The average RN salary is $71,000. That's not the average starting salary, but it gives you an idea of how much money nurses can make over their career. Congestive heart failure (congestive heart failure CHF) pathophysiology, treatments, and symptoms nursing lecture on heart failure for nursing students. This congestive heart failure NCLEX review with give you mnemonics on how to remember the causes of heart failure and the different types of heart failure, such as left-sided or right-side heart failure. I also discuss the pathophysiology of both left ventricular systolic and diastolic dysfunction along with right-sided dysfunction. Signs and symptoms of heart failure affecting the left-side are pulmonary in nature with the patient experience, dyspnea, crackles, orthopnea, and paroxysmal nocturnal dyspnea. Whereas, the signs and symptoms of right-sided heart failure are peripheral in nature with the patient experiencing swelling of the liver, hands, feet, abdomen nocturia, and jugular venous distention. Tests used to diagnose heart failure include a blood test called BNP, echo-cardiogram, heart catheterization, chest x-ray, or nuclear stress test. Do NOT crush these medications when crushing pills for medication administration. This is a mnemonic to help you remember which category of drugs you can NOT crush. This list will help you remember the group of medications that you should NOT crush for a tube feeding and oral administration. Why do we crush medications? If a patient has a feeding tube or can't swallow medications whole due to swallowing difficulties, we must crush the medication. Tips on crushing medications: Not all medications can be crushed! Always consult with pharmacy if you are not 100% sure if a medication can be crushed. Also, look at the name of the drug for certain letters. For example, Detrol LA or Toprol XL are both long-acting medications that can’t be crushed. These are the general categories of drugs that cannot be crushed: Enteric coated (EC) Long acting (LA. XL) Extended release (ER, XR) Controlled release/delivery (CR, CD) Sustained release/action (SR, SA) Always assess swallowing status before administering oral medications, and placement of feeding tube per hospital protocol. Acute glomerulonephritis nursing (poststreptococcal) NCLEX review. Poststreptococcal glomerulonephritis is a type of nephritic syndrome which means there is the leakage of both red blood cells and proteins into the urine due to an inflamed glomerulus. This type of acute glomerulonephritis tends to present 14 days after a strep infection of the throat or skin and mainly affects the pediatric population from ages 2-10. Signs and symptoms of glomerulonephritis includes hematuria (tea or cola-colored urine), mild proteinuria, mild edema (specifically in the face and around the eyes), decreased glomerular filtration rate, increased BUN and creatinine, oliguria (low urine output), hypertension. Myocardial infarction (heart attack or MI) ischemia lecture on the pathophysiology, ECG, nursing role, complications, signs and symptoms. This video on myocardial infarction will help students prepare for the NCLEX exam. Myocardial infarction is when blood supplied to the heart muscle is limited which causes injury to the heart tissue. Injury from an MI causes complications such as pericarditis, cardiac rupture, cardiogenic shock, arrhythmias, ventricular aneurysm, heart failure, and depression. Signs of symptoms of an MI include: chest pain that radiates, is heavy/intense, and is not relieved by nitroglycerin or rest, nausea/vomiting, cold sweat, anxiety, increased heart rate or blood pressure, irregular heart rate. Tools used to diagnose a myocardial infarction include: cardiac markers (troponins, myoglobin, CK or CK-MB levels), EKG, echocardiogram, stress test with myocardial perfusion imaging, or heart catheterization. As the nurse it is important you know the basics about how to read an EKG during a possible myocardial infarction. You must know what areas of the leads reflect which heart wall and what type of EKG changes you are looking for: ST-segment depression or elevation, T-wave inversion or hyperacute, or pathological Q-wave. Crohn's disease nursing review regarding diet, symptoms, pathophysiology, nursing interventions and treatment for NCLEX exam. Crohn's disease is a type of inflammatory bowel disease that causes inflammation and ulcers formation in the GI tract. Crohn's can be found in both the small and large intestine; however, it seems to be most common in the terminal of the ileum and beginning of the colon. Crohn's disease affects the whole bowel lining which is why its complications are unique when comparing crohn's disease vs ulcerative colitis. Crohn's disease complications include fistulas, fissures, abscesses, malnourishment, strictures which can lead to bowel obstructions, and other changes such as mouth ulcers, skin changes, gall and liver problems etc. Crohn's disease symptoms include abdominal pain (which tends to be located in the right lower abdomen), ulcers in the mouth and GI lining, anal fissures, diarrhea which can contain pus, mucous, and blood, and undernourishment. Crohn's disease tends to present in scattered patches throughout the GI tract where healthy areas will be noted next to diseased areas. This will give a cobble-stone appearance during a scope of the GI lining. This is different than ulcerative colitis which tends to start in the rectum and migrate in a continuous fashion through the large intestine without any skipped areas. Nursing interventions for Crohn's Disease include: educating patient about this condition along with medication and diet regime and smoking cessation, monitoring nutrition status, assessing GI system by monitoring bowel sounds, bowel movements, monitoring administration of TPN (total parenteral nutrition) if ordered, providing ostomy education (if the patient has one). GERD | Gastroesophageal Reflux Disease Nursing NCLEX Lecture | Symptoms and Treatment

Gastroesophageal reflux disease (GERD) nursing NCLEX lecture on the symptoms, GERD diet, nursing interventions, and treatment. GERD is a chronic condition where stomach contents is flowing back up into the esophagus. What causes gastroesophageal reflux disease? GERD is mainly due to a weak or damaged lower esophageal sphincter. The LES is a collection of circular muscles at the end of the esophagus that closes and prevent toxic acids and GI contents from flowing back into the esophagus once it enters the stomach. What causes the LES to become weak? Many things such as pressure on the LES from pregnancy, obesity, hiatal hernia, or overeating along with medications such as antihistamines, calcium channel blockers, sedatives, and even smoking. In addition, GERD can be caused by delayed gastric emptying or impaired motility of the esophagus. Signs and symptoms of GERD include: heartburn, regurgitation (bitter taste in the mouth), dry chronic cough especially at night, upper GI pain, voice changes, nausea, problems swallowing etc. Complications of gastroesophageal reflux disease include: narrowing of the esophagus (strictures), Barret's esophagus, bleeding, increased risk of esophageal cancer, aspiration, lungs infections, and ear infections. What is the treatment for GERD? It is treated with lifestyle changes (smoking cessation, weight loss, diet changes etc.), medications, and in the worst cases surgery (fundoplication). How is gastroesophageal reflux disease diagnosed? An endoscopy is performed to assess the esophagus for changes such as erosion or strictures etc. Esophageal manometry assesses the function of the esophagus' ability to squeeze food down into the stomach and how the lower esophageal sphincter closes. PH monitoring measures the acid amounts in the esophagus for a 24 hour period as the patient perform activities of daily living (a small tube stays in the esophagus to help measure the acid amounts) Medications used to treat GERD include: Antacids, histamine receptor blockers, proton-pump inhibitors, and prokinetics. Nursing interventions for GERD include: diet teaching (avoids foods that relax the lower esophageal spincter: peppermint/spearmint, alcohol, coffee, fatty/greasy foods), smoking cessation, limiting acids foods such as citrus and tomatoes, avoid eating right before bed, sitting up an hour after a meal, and eating small meals rather than large ones. Monitor the patient for complications, assessing quality/frequency of pain, medication history, and administering medications per MD order. Gout attack (acute or chronic) nursing lecture on the symptoms, treatment, diet, and pharmacology review for the NCLEX exam. Gout is a type of arthritis due to the accumulation of uric acid in the blood. Symptoms of gout are due to uric acid crystals that form within the joint that leads to severe pain, redness, and limited mobility. Acute gout attacks happen suddenly (often waking the patient in the middle of the night) and tends to affect the big toe (knee, elbow, fingers, toe can also be affected). The affected joint will present with swelling, inflammation, and severe pain. The site will also be warm to the touch, and the patient will be unable to tolerate even the slightest pressure on the site. Chronic gout is due to constant elevated uric acid levels that leads to repeated acute gout attacks. In chronic gout, tophi tend to develop. Tophi are large masses of uric acid crystals that have formed into large clumps. They are white/yellowish nodules that can found under the skin (helix of the ear, fingers, elbow etc.), within the joints, or bones. Tophi can lead to bone deformity and joint damage. Patients with gout are educated to follow a diet that is low in purines, alcohol, and high fructose corn syrup drinks. Food high in purines include seafood, organ meats, red meat, and alcohol (especially beer). This is an NCLEX hotspot practice question example about a nursing assessment for the drug Digoxin (Lanoxin). For this particular NCLEX practice question, you will have to identify what area on the diagram you will assess prior to administering Digoxin. Before a nurse administers Lanoxin, it is important to assess the apical pulse for 1 full minute. The apical pulse must be 60 beats per minute or greater. Therefore, to answer this question correctly you must identify on the diagram where the apical pulse is located on an adult patient. The apical pulse is located at the apex of the heart and is found at the 5th5th intercostal space midclavicular line. This location is also the point of maximum impulse. Nephrotic syndrome nursing lecture on signs and symptoms, pathophysiology, and nursing interventions for preparation of NCLEX exam. Nephrotic syndrome is a set of symptoms experienced by a patient with damage to the filtering structure (glomerulus) of the kidneys, which causes a massive amount of proteins to be leaked into the urine. Why is nephrotic syndrome happening? There is damage to the glomerulus of the nephron, which allows a massive amounts of proteins to leak into the urine (more than 3 grams per day). The patient mainly loses the protein albumin via the urine along with other types of protein that help fight infection and prevent blood clot development. Causes of nephrotic syndrome include primary and secondary causes. Primary causes can be due to the most common cause of nephrotic syndrome, which is minimal change disease along with focal segmental glomerulosclerosis, or membranoproliferative glomerulonephritis. Secondary causes include diseases that are causing damage to the kidney such as Lupus, Diabetes, Hepatitis, heart failure etc. Signs and symptoms of Nephrotic syndrome include massive proteinuria, which causes the urine to be dark and frothy/foamy, hypoalbuminemia, edema (weight gain), hyperlipidemia. Peptic ulcer disease pharmacology and nursing care NCLEX lecture review. Peptic ulcer disease is the formation of ulcers in the lining of the upper GI tract that affects mainly the mucosal lining of the stomach, duodenum or esophagus. There are various types of peptic ulcers such as gastric ulcers which are located in the stomach and duodenal ulcers which are located in the duodenum. Peptic ulcer disease is treated with various medications such as proton-pump inhibitors, histamine receptor blockers, antacids, antibiotics, muscosal healing agents, and bismuth subsalicylates. Treatment of ulcers due to h. pylori infection includes a various drug regime of antibiotics, PPIs, H2 blockers, and bismuth subsaliylates per physician order. Peptic ulcer disease nursing care involves assessing, monitoring, educating, and administering medications to the patient per physician's order. The nurse will want to assess and monitor the patient for any signs and symptoms of complications from a peptic ulcer, such as gastrointestinal bleeding. GI bleeding could present as vomiting "coffee ground" emesis or bright blood or dark, tarry stools along with increased heart, low blood pressure, abdominal mass or tenderness. The patient should be monitored and educated on dumping syndrome if a gastric resection is performed to cure the ulcer. In addition, the patient should follow a soft, bland until the ulcer is healed. It is also important the patient avoids alcohol usage, smoking, or consuming caffeine products because these products can irritate the ulcer. Signs and symptoms of PUD include main indigestion and epigastric pain that can be felt from the breastbone to the belly button. It is typically described as a gnawing, aching, or dull pain. Complications of peptic ulcer disease include: gastrointestinal bleeding, perforation which can lead to peritonitis, and obstruction of the pylorus which is a muscular section that connects the stomach and duodenum together and allows food to flow between these parts of the GI tract. Coronary artery disease (also called coronary heart disease) NCLEX lecture video on the nursing interventions, treatment, and medications for CAD. CAD is the build up of fatty plaques due to atherosclerosis that causes the arteries of the heart to become narrow. This leads to restrictive blood supply to the heart muscle which can lead to chest pain (stable or unstable angina) to myocardial infraction (MI). Nursing interventions for coronary heart disease include education about modifying lifestyle such as smoking cessation, diet (low fat and calorie), and exercise. Medication management plays another role in coronary artery disease management. As the nurse, it is important to provide education to patients about being compliant with medications, how to take them, and possible side effects. Medications for CAD include nitrates (Nitroglycerin), antiplatelets (Aspirin or Plavix), statins (Lipitor), beta blockers (Metoprolol), or ACE inhibitors (Lisinopril). The main coronary arteries that supply the heart with blood include the left and right coronary artery. The left coronary artery branches off into the left circumflex artery and left anterior descending artery. The right coronary artery branches off into the right marginal artery and posterior descending artery. If ischemia has been present for a long time collateral circulation can develop as well. This is where tiny blood vessel re-route blood supply to the heart to pass the blockage. Signs and symptoms of heart disease include: chest pain (especially with activity and is relieved with rest which is known as stable angina), shortness of breath, feeling very tired.
