Coronary Artery Disease (CAD) Anatomy, Nursing, Heart Disease, Pathophysiology, Treatment Part 1

Coronary artery disease (also called coronary heart disease) NCLEX lecture video on the nursing, anatomy, pathophysiology, treatment, and signs and symptoms of coronary artery disease. CAD is the build up of fatty plaques due to atherosclerosis that causes the arteries of the heart to become narrow. This leads to restrictive blood supply to the heart muscle which leads to chest pain (stable or unstable angina) to myocardial infraction (MI). The main coronary arteries that supply the heart with blood include the left and right coronary artery. The left coronary artery branches off into the left circumflex artery and left anterior descending artery. The right coronary artery branches off into the right marginal artery and posterior descending artery. If ischemia has been present for a long time collateral circulation can develop as well. This is where tiny blood vessels re-route the blood supply to the heart to pass the blockage. Signs and symptoms of heart disease include: chest pain (especially with activity and is relieved with rest which is known as stable angina), shortness of breath, and feeling very tired. Coronary artery disease is diagnosed in various ways: lipid profile which measures the cholesterol levels, EKG, stress test, and heart catheterization. CAD is treated with medications (discussed in part 2 of this series), heart catheterization (most commonly with stent placement) or coronary artery bypass graft surgery. Congestive heart failure (congestive heart failure CHF) pathophysiology, treatments, and symptoms nursing lecture on heart failure for nursing students. This congestive heart failure NCLEX review with give you mnemonics on how to remember the causes of heart failure and the different types of heart failure, such as left-sided or right-side heart failure. I also discuss the pathophysiology of both left ventricular systolic and diastolic dysfunction along with right-sided dysfunction. Signs and symptoms of heart failure affecting the left-side are pulmonary in nature with the patient experience, dyspnea, crackles, orthopnea, and paroxysmal nocturnal dyspnea. Whereas, the signs and symptoms of right-sided heart failure are peripheral in nature with the patient experiencing swelling of the liver, hands, feet, abdomen nocturia, and jugular venous distention. Tests used to diagnose heart failure include a blood test called BNP, echo-cardiogram, heart catheterization, chest x-ray, or nuclear stress test. Left-sided heart failure is the most common type of heart failure and can lead to right-sided heart failure. Patients with left ventricular systolic dysfunction have low ejection fractions (EFs) compared to patients with left ventricular diastolic dysfunction who may have a normal ejection fraction. Tetralogy of Fallot is a congenital heart defect that is characterized by FOUR structural defects. The structural defects of tetralogy of fallot include: right ventricular hypertrophy, aorta displacement, pulmonic stenosis, and ventricular septal defect (VSD) Signs and symptoms of Tetralogy of Fallot include: activity intolerance, tet spells (cyanosis), shortness of breath, increase respiratory rate, fatigue and faints easily, failure to thrive (small for age, not meeting developmental milestone, weight loss), trouble feeding, cardiac murmur (harsh systolic murmur and systolic thrill) Nursing care: treating tet spells, which includes placing the patient in a knee-to-chest position WHY? Infective endocarditis and non-infective endocarditis lecture on the nursing management, pathophysiology, treatment, signs and symptoms. In this video, I will discuss in detail the different types of endocarditis (infective endocarditis with the sub-types acute and subacute and non-infective). In addition, I detail the pathogenesis of how a vegetation grows on the heart valve and the complications of endocarditis, such as embolism, heart failure, valve regurgitation/stenosis. Signs and symptoms of IE include petechiae, anorexia, high fever, heart failure, cardiac murmur, Osler's nodes, Janeway lesions, Roth Spots, and splenomegaly. Infective endocarditis can be diagnosed with a TEE (transesophageal echocardiogram) which allows the physician to see the backside of the heart and the heart valves. Patients who are at risk for infective endocarditis include patients with a history if IV drug use, rheumatic heart disease, mitral valve prolapse, or other valve defects. Pathogens can enter into the blood stream through invasive procedures (dental work, implanted devices, central line placement) and infect the valve. The next part of the word: Aneurysm - describes a disease process where there is abnormal widening or stretching of a portion of an artery. This is due to weakening of the blood vessel. This can be caused by a number of different reasons including: AtheroSclerosis - This is the most common cause. ArtheroSclerosis means: hardening of the arteries (normally the aortic wall is very elastic) There are many different types of Aortic Aneurysms, depending on the location and also the shape of the aneurysm. with different signs and symptoms associated with each: One is: Ascending Aneurysm which is also known as Thoracic Aneurysm - Signs and symptoms of this type of aneurysm include: - Bradycardia - blood pressure differences in the right & left arm - jugular vein distention -Murmur AND -PeriCardial friction rub Another type is a Abdominal Aneurysm. - There are usually no symptoms associated with this type of aneurysm however if you listen to the heart you may be able to hear a bruit. Diagnostic test include: - Echocardiography or Cardiac Echo - which is an ultrasound that can show the aneurysm and also it’s size - A dissecting Aortic Aneurysm requires emergency surgery.

Cardiac arrhythmias classified by site of origin: Sinus rhythms, from the SA node; Atrial rhythms the atria; Ventricular rhythms from the ventricles. Sinus bradycardia and sinus tachycardia may be normal or clinical depending on the underlying cause. For example, sinus bradycardia is considered normal during sleep and sinus tachycardia may be normal during physical exercises. Cardiac arrhythmias that originate from other parts of the atria are always clinical. The most common include: atrial flutter, atrial fibrillation and AV nodal re-entrant tachycardia. These are forms of supraventricular tachycardia or SVT. Atrial flutter or A-flutter is caused by an electrical impulse that travels around in a localized self-perpetuating loop, most commonly located in the right atrium. This is called a re-entrant pathway. For each cycle around the loop, there is one contraction of the atria. The atrial rate is regular and rapid - between 250 and 400 beats per minute. Ventricular rate, or heart rate, however, is slower, thanks to the refractory properties of the AV node. The AV node blocks part of atrial impulses from reaching the ventricles. In this example, only one out of every three atrial impulses makes its way to the ventricles. The ventricular rate is therefore 3 times slower than the atrial rate. This is an example of a “3 to 1 heart block”. Ventricular rate in A-flutter is usually regular, but it can also be irregular. On an ECG atrial flutter is characterized by absence of normal P wave. Instead, flutter waves, or f-waves are present in saw-tooth patterns. Atrial fibrillation is caused by multiple electrical impulses that are initiated randomly from many ectopic sites in and around the atria, commonly near the roots of pulmonary veins. These un-synchronized, chaotic electrical signals cause the atria to quiver or fibrillate rather than contract. The atrial rate during atrial fibrillation can be extremely high, but most of the electrical impulses do not pass through the AV node to the ventricles, again, thanks to the refractory properties of the cells of the AV node. Those do come through are irregular. Ventricular rate or heart rate is therefore irregular and can range from slow - less than 60 - to rapid -more than 100 - beats per minute. On an ECG, atrial fibrillation is characterized by absence of P-waves and irregular narrow QRS complexes. The baseline may appear undulating or totally flat depending on the number of ectopic sites in the atria. In general, larger number of ectopic sites results in flatter baseline. AV nodal re-entrant tachycardia or AVNRT is caused by a small re-entrant pathway that involves directly the AV node. Every time the impulse passes through the AV node, it is transmitted down to the ventricles. The atrial rate and ventricular rate are therefore identical. Heart rate is regular and fast, ranging from 150 to 250 beats per minute. Ventricular rhythms are the most dangerous. In fact, they are called lethal rhythms. Ventricular tachycardia or V-tach is most commonly caused by a single strong firing site or circuit in one of the ventricles. It usually occurs in people with structural heart problems such as scarring from a previous heart attack or abnormalities in heart muscles. Impulses starting in the ventricles produce ventricular premature beats that are regular and fast, ranging from 100 to 250 beats per minute. On an ECG V-tach is characterized by wide and bizarre looking QRS complexes. P wave is absent. V-tach may occur in short episodes of less than 30 seconds and cause no or few symptoms. Sustained v-tach lasting for more than 30 seconds requires immediate treatment to prevent cardiac arrest. Ventricular tachycardia may also progress into ventricular fibrillation. Ventricular fibrillation or v-fib is caused by multiple weak ectopic sites in the ventricles. These un-synchronized, chaotic electrical signals cause the ventricles to quiver or fibrillate rather than contract. The heart pumps little or no blood. V-fib can quickly lead to cardiac arrest. V-fib ECG is characterized by irregular random waveforms of varying amplitude, with no identifiable P wave, QRS complex or T wave. Amplitude decreases with time, from initial coarse v-fib to fine v-fib and ultimately to flatline.

Coronary artery disease (also called coronary heart disease) NCLEX lecture video on the nursing interventions, treatment, and medications for CAD. CAD is the build up of fatty plaques due to atherosclerosis that causes the arteries of the heart to become narrow. This leads to restrictive blood supply to the heart muscle which can lead to chest pain (stable or unstable angina) to myocardial infraction (MI). Nursing interventions for coronary heart disease include education about modifying lifestyle such as smoking cessation, diet (low fat and calorie), and exercise. Medication management plays another role in coronary artery disease management. As the nurse, it is important to provide education to patients about being compliant with medications, how to take them, and possible side effects. Medications for CAD include nitrates (Nitroglycerin), antiplatelets (Aspirin or Plavix), statins (Lipitor), beta blockers (Metoprolol), or ACE inhibitors (Lisinopril). The main coronary arteries that supply the heart with blood include the left and right coronary artery. The left coronary artery branches off into the left circumflex artery and left anterior descending artery. The right coronary artery branches off into the right marginal artery and posterior descending artery. If ischemia has been present for a long time collateral circulation can develop as well. This is where tiny blood vessel re-route blood supply to the heart to pass the blockage. Signs and symptoms of heart disease include: chest pain (especially with activity and is relieved with rest which is known as stable angina), shortness of breath, feeling very tired. Coronary artery disease is diagnosed in various ways: lipid profile which measures the cholesterol levels, EKG, stress test, heart catheterization. Coarctation of aorta nursing pediatrics NCLEX review (congenital heart disease defect). What is coarctation of the aorta? It's a congenital heart defect where narrowing is present in the aorta. What does the aorta do? It’s the largest artery is the body! It’s a very important structure that comes off of the left ventricle. It carries rich oxygenated blood that has just left the lungs to go to the body via a network of arteries that supply the brain, upper/lower extremities, organs, and tissues. Quick Facts about CoA: In coarctation of the aorta, the narrowing of the aorta tends to occurs AFTER the left subclavian artery, either before or after the ductus arteriosus. There are different types of CoA: Preductal and postductal According to CDC.gov, 1,600 babies in the U.S. are born with this condition each year. It can be diagnosed during a prenatal visit with fetal ultrasound or after birth with an echocardiogram. It’s thought to be caused by the tissue that makes up the ductus arteriosus, which has extended into a section of the aorta. Therefore, when the ductus arteriosus NORMALLY closes a section of the aorta narrows too. Auscultating (how to listen to heart sounds) heart sounds for assessing S1, S2, S3, and S4 along with heart murmurs. Listening to the heart with a stethoscope is the best way to conduct the heart assessment during a nursing assessment of the cardiovascular system. Auscultation of heart sounds allows the nurse to assess valve closure of the aortic, pulmonic, tricuspid, and mitral (bicuspid) valves. S1 is the closure of the AV valves which are the tricuspid and mitral valves, and S2 is the closure of the SL valves aortic and pulmonic. During the assessment of heart sounds it is important to position the patient correctly to increase sound quality and to be familiar with the heart auscultation sites. Ventricular Septal Defect Nursing | NCLEX Pediatric Congenital Heart Defects

Ventricular septal defect (VSD) nursing NCLEX review over the pathophysiology, signs and symptoms, complications, nursing interventions, and treatments. What is a ventricular septal defect (VSD)? It’s a congenital heart defect that occurs when a “hole” forms in the ventricular septum. This causes an increase in blood flow to the lungs. There are four types of ventricular septal defects: -Membranous: a hole is found at the upper section of the ventricular septum and is very close to the tricuspid and aortic valves (Notes: if the hole is found too close to the valves, the valves can become damaged and lead to regurgitation…example aortic valve etc.) -Muscular: found at the lower part of the septum in the muscle layer. Small muscular VSDs have a high chance of closing on their own. -Outlet (conal or subarterial): found at structures where blood is leaving the heart around the pulmonic and aortic valve location...hence the name “outlet” (not as common) -Inlet (atrioventricular canal): found at structures where blood is entering the heart around the tricuspid and bicuspid valve (mitral) Pathophysiology of a ventricular septal defect includes heart failure and pulmonary hypertension due to a left to right shunt of blood. Please watch the video for an in-depth review of this. If a VSD is left untreated, it can lead to Eisenmenger Syndrome, which is the reversal of blood shunting in the heart right to left. This will lead to cyanosis and clubbing of the nails. Myocardial infarction (heart attack or MI) ischemia lecture on the pathophysiology, ECG, nursing role, complications, signs and symptoms. This video on myocardial infarction will help students prepare for the NCLEX exam. Myocardial infarction is when blood supplied to the heart muscle is limited which causes injury to the heart tissue. Injury from an MI causes complications such as pericarditis, cardiac rupture, cardiogenic shock, arrhythmias, ventricular aneurysm, heart failure, and depression. Signs of symptoms of an MI include: chest pain that radiates, is heavy/intense, and is not relieved by nitroglycerin or rest, nausea/vomiting, cold sweat, anxiety, increased heart rate or blood pressure, irregular heart rate. Tools used to diagnose a myocardial infarction include: cardiac markers (troponins, myoglobin, CK or CK-MB levels), EKG, echocardiogram, stress test with myocardial perfusion imaging, or heart catheterization. As the nurse it is important you know the basics about how to read an EKG during a possible myocardial infarction. You must know what areas of the leads reflect which heart wall and what type of EKG changes you are looking for: ST-segment depression or elevation, T-wave inversion or hyperacute, or pathological Q-wave. Stroke nursing (CVA) cerebrovascular accident lecture on ischemic and hemorrhagic strokes along with nursing care, tPA, symptoms, treatments, and pathophysiology. A stroke occurs when blood rich in oxygen can NOT reach brain cells (due to either a blockage or bleeding) and the cells begin to die. Types of stroke: Ischemic (most common) is due to a blood clot within the blood vessel or stenosis of an artery feeding the brain tissue. This limits the blood that can reach the brain cells. This can happen due to: -Embolism: where a clot has left a part of the body (example the clot has left the heart due to a heart valve problem or atrial fibrillation) and has travelled to the brain, which stops blood flow. -Thrombosis: a clot forms within the artery wall within the neck or brain. This is seen in patients with hyperlipidemia and atherosclerosis Hemorrhagic: there is bleeding in the brain due to a break in a blood vessel. There will be limited blood flow to the brain cells and this can cause excessive swelling in the brain from the leakage of blood in the brain tissue. A ruptured brain aneurysm or uncontrolled hypertension can cause this. In addition, a patient can experience a TIA (transient ischemic attack) also called a mini-stroke. This is where signs and symptoms of a stroke last only a few minutes to hours and resolve. It is a warning signs an impending stroke may occur. Stroke symptoms include: facial drooping, arm numbness or weakness, slurred speech or difficulty talking etc. Treatment for stroke (specifically an ischemic type) is tPA (tissue plasminogen activator). This medication works to dissolve the clot that has caused th decreased blood flow. However, tPA must be administered within 3 hours of the onset of stroke symptoms. Nursing care for stroke includes: monitoring vitals (especially blood pressure, respiratory status), watching and educating the patient with neglect syndrome (this most likely occurs with patients who've had a right brain injury), diet and swallowing techniques, hemianopsia, and how to communicate with a patient with aphasia (watch the video for more in depth nursing interventions for stroke). Heart sounds (S1, S2, S3, S4, murmurs) for nursing assessment examination. This video details the anatomy of the heart, heart sound auscultation points (sites), blood flow, diastole, systole, tips for distinguishing S1 from S2, and how to use the diaphragm and bell of the stethoscope when listening the atrioventricular (AV) and semilunar (SL) valve closure, such as the aortic, pulmonic (S2 sounds), tricuspid, and mitral valves (S1 sounds) and the extra heart sounds such as S3, S4, and murmurs. In addition, I talk about patient positioning to obtain the best acoustic sounds for heart sounds that includes sitting-up/supine, left side placement, and sitting-up, leaning forward. In the next video, I will perform a cardiac assessment of the heart by demonstrating how to place the stethoscope's chest piece to listen to heart sounds.
