Ahhhh....the feeling you get while spending a few days on the beach. Is it the sand in your toes...the warm water and salty breeze...or is it the chemical reaction that takes place under your skin when your Vitamin D is activated into a powerhouse biologically influential hormone called D3? It is the production of Vitamin D made in your skin with direct sun exposure. Here is a list of signs and symptoms of Vitamin D deficiency to reveal why it is so vital to your health and optimal body function. Signs of low Vitamin D levels Symptoms of low Vitamin D levels Fatigue Joint and Bone pain Muscle Spasm Weakness Uncontrolled weight gain High blood pressure Restless Sleep Poor concentration and Memory Depressed mood Headaches Bowel problem(diarrhea, constipation or both) Bladder problems(urgency, frequency or both Depression and S.A.D. Fibromyalgia Parkinson's Disease Alzheimer's Disease Arthritis Gout Tendonitis and Bursitis Osteoporosis and Osteoarthritis Gum Disease and tooth loss Obesity Diabetes Heart Disease Metabolic Syndrome Autoimmune Disease(MS, SLE) Cancer (Breast, Prostate, Colon, Skin) So, why all the recent hype with this Vitamin? First of all, it is actually not a Vitamin, it is really a hormone. This means it has a direct influence on the function of almost every cell in our bodies. Its chemical structure is very similar to other hormones like testosterone or estrogen as they are produced from the same starting molecule. The scary thing is that most of us living above the 30th parallel have levels that are actually not optimized to prevent and treat disease states. You'd have to spend most of your day in a speedo laying in direct sunlight in your front yard and unfortunately your levels would still not be optimized. Studies show that the optimal level in your blood to prevent chronic disease and optimize function is around 60-70. Many of my patients see me with pain, fatigue and arthritis problems and their levels have never been checked, or they say they were told by their doctor that their results are within the "normal" range. The problem with this is that a blood level is normal if between 30 and 100. Normal is NOT OPTIMAL! If your level is 35 then you may be told you don't need any Vitamin D. By the way, my level mid-summer level was 38 last year and I spent a lot of time in a swim suite (not a speedo) at the lake. That level may be in the "normal" range, but according to the latest research it is a long way from being OPTIMAL. Keep in mind that too much of a good thing is not always better. It is very important to keep your levels in an OPTMAL range. Not too low and not too high. Once you are started on Vitamin D it is important recheck your blood levels over a few months to target the range needed for top performance. If you knew that you could do something that would decrease your weight, blood pressure and diabetes risk, decrease your risk of cancer (prostate, breast and colon), decrease pain, arthritis and risk of osteoporosis, increase your energy and mood and increase your quality of sleep, it would be silly not to take advantage of that fountain of youth. Yet only a small fraction of us take full advantage of the powers of Vitamin D. Mary, a patient of ours at Rejuv, came to see me for treatment of her arthritic and painful knees. Over the years of dealing with pain and orthopedic concerns we have discovered that we do our patients a disservice if we don't address the other important issues that contribute to the pain and arthritis we are only treating a fraction of what is ailing us. We checked a Vitamin D level on her and it was 32. It was in the "normal" range, but not optimized to deal with her decreasing bone density, arthritis and overall lack of energy and fatigue. We treated her knee arthritis with Prolotherapy and started her on the appropriate dose of Vitamin D based on her lab value. Two months later, her levels where 56 and she had a returned energy and zeal and was enjoying life again without pain or fatigue. It is important that we treat our bodies as well or better than we would our car. If your tank is running low or your joints and body don't seem to be performing optimally, get your "fluid" levels checked and optimized to restore your vitality and prolong your engines life. You can always exchange your car for a younger model, but you only have one body. 6 Side Effects of Too Much Vitamin D 1. Elevated Blood Levels Achieving adequate levels of vitamin D in your blood may help boost immunity and protect you from diseases like osteoporosis and cancer (5). However, there isn't universal agreement on the optimal range for these levels. Although a vitamin D level of 30 ng/ml (75 nmol/l) is typically considered adequate, the Vitamin D Council recommends maintaining levels of 40–80 ng/ml (100–200 nmol/l), and states that anything over 100 ng/ml (250 nmol/l) may be harmful (6, 7). Despite the fact that more people are now taking vitamin D supplements, it's rare to find someone with very high blood levels of this vitamin. One recent study looked at data from more than 20,000 people over a 10-year period. It found that only 37 people had levels above 100 ng/ml (250 nmol/l). Only one person had true toxicity, at 364 ng/ml (899 nmol/l) (8). 2. Elevated Blood Calcium Levels Vitamin D helps your body absorb calcium from the food you eat. In fact, this is one of its most important roles. However, if vitamin D intake is excessive, blood calcium may reach levels that cause symptoms that are not only unpleasant, but dangerous. Symptoms of hypercalcemia, or high blood calcium levels, include: Digestive distress, such as vomiting, nausea and stomach pain Fatigue, dizziness and confusion Excessive thirst Frequent urination The normal range of blood calcium is 8.5–10.2 mg/dl (2.1–2.5 mmol/l). In one case study, an older man with dementia who received 50,000 IU of vitamin D daily for six months was repeatedly hospitalized with symptoms related to high calcium levels (10). In another, two men took improperly labeled vitamin D supplements, leading to blood calcium levels of 13.2–15 mg/dl (3.3–3.7 mmol/l). What's more, it took a year for their levels to normalize after they stopped taking the supplements (11). 3. Nausea, Vomiting and Poor Appetite Many side effects of too much vitamin D are related to excessive calcium in the blood. These include nausea, vomiting and poor appetite. However, these symptoms don't occur in everyone with elevated calcium levels. One study followed 10 people who had developed excessive calcium levels after they had taken high-dose vitamin D to correct deficiency. Four experienced nausea and vomiting and three had a loss of appetite (12). 4. Stomach Pain, Constipation or Diarrhea Stomach pain, constipation and diarrhea are common digestive complaints that are often related to food intolerances or irritable bowel syndrome. However, they can also be a sign of elevated calcium levels caused by vitamin D intoxication (15). These symptoms may occur in those receiving high doses of vitamin D to correct deficiency. As with other symptoms, response appears to be individualized even when vitamin D blood levels are similarly elevated. In one case study, a boy developed stomach pain and constipation after taking improperly labeled vitamin D supplements, whereas his brother experienced elevated blood levels without any other symptoms (16). 5. Bone Loss Because vitamin D plays an important role in calcium absorption and bone metabolism, getting enough is crucial for maintaining strong bones. However, too much vitamin D can also be detrimental to bone health. Although many symptoms of excessive vitamin D are attributed to high blood calcium levels, some researchers suggest that megadoses may lead to low levels of vitamin K2 in the blood (18). One of vitamin K2's most important functions is to keep calcium in the bones and out of the blood. It's believed that very high vitamin D levels may reduce vitamin K2 activity (18, 19). 6. Kidney Failure Excessive vitamin D intake frequently results in kidney injury. In one case study, a man was hospitalized for kidney failure, elevated blood calcium levels and other symptoms that occurred after he received vitamin D injections prescribed by his doctor (20). Indeed, most studies have reported moderate-to-severe kidney injury in people who develop vitamin D toxicity (9, 12, 13, 14, 16, 17, 21). In one study of 62 people who received excessively high-dose vitamin D injections, each person experienced kidney failure — whether they had healthy kidneys or existing kidney disease (21). The Bottom Line Vitamin D is extremely important for overall health. Even if you follow a healthy diet, you may require supplements in order to achieve optimal blood levels. However, it is also possible to have too much of a good thing. Make sure to avoid excessive doses of vitamin D. Generally speaking, 4,000 IU or less per day is considered safe as long as your blood values are being monitored. In addition, make sure you purchase supplements from reputable manufacturers to reduce the risk of accidental overdose due to improper labeling. Dosing obese teens with vitamin D shows no benefits for their heart health or diabetes risk, and could have the unintended consequences of increasing cholesterol and fat-storing triglycerides. These are the latest findings in a series of Mayo Clinic studies in childhood obesity. Seema Kumar, M.D., a pediatric endocrinologist in the Mayo Clinic Children’s Center, has been studying the effects of vitamin D supplementation in children for 10 years, through four clinical trials and six published studies. To date, Dr. Kumar’s team has found limited benefit from vitamin D supplements in adolescents. The latest study, Effect of Vitamin D3 Treatment on Endothelial Function in Obese Adolescents, appears online in Pediatric Obesity. “After three months of having vitamin D boosted into the normal range with supplements, these teenagers showed no changes in body weight, body mass index, waistline, blood pressure or blood flow,” says Dr. Kumar. “We’re not saying the links between vitamin D deficiency and chronic diseases don’t exist for children—we just haven’t found any yet.”

Vitamin D supplementation is also popular, since getting enough of this vitamin is difficult from diet alone. Unfortunately, there are quite a few individuals who are deficient in this vitamin without even knowing it as the signs and symptoms are either subtle or not well known. Over time this could have a negative effect on an individuals quality of life. Lets discuss some signs and symptoms of vitamin D deficiency. 1. Those who are vitamin D deficient may feel more fatigued and tired. Since there are so many reasons why we can feel tired and fatigued during the day, such as lack of sleep or obesity, vitamin D deficiency is often overlooked. Vitamin D deficiency can cause daytime fatigue or even make it harder to get out of bed in the mornings, even with quality sleep. 2. Vitamin D plays an important role in keeping our immune system strong and free from different bacteria and illness. This is because vitamin D directly effects the cells that are responsible for fighting against infection. Therefore, another sign and symptom of vitamin D deficiency is getting sick more often. Low levels of vitamin D can be a contributing factor to becoming infected with cold and flus more often. As well, it has been found that taking vitamin D supplements may reduce the risk of certain respiratory tract infections. 3. Another possible symptom of vitamin D deficiency is bone and lower back pain. Vitamin D is an important vitamin for maintaining overall bone health. It supports bone health and helps with the body’s absorption of calcium. It has been found that those individuals with a vitamin D deficiency were more like to have back pain which often limited their daily activities. As well, they were also more likely to have bone pain through out their bodies, such as in their legs and joints. A deficiency in this vitamin will not only cause bone pain, but it can also cause bone loss since it not only helps with calcium absorption, but also bone metabolism. This can lead to lower bone mineral density. 4. Vitamin D deficiency may also negatively effect our mood. Feelings of depression may be a sign of vitamin D deficiency. This has often been linked to older adults. By being deficient in this vitamin, it may also cause seasonal depression during the colder months, when exposure to sunlight is limited. Depression and all mental health symptoms are complex and should be discussed with your medical practitioner to determine its exact cause and remedy. 5. Lastly, another sign and symptom of vitamin D deficiency may be hair loss. When hair loss is severe, this can be the result of nutrient deficiency. Hair loss may be a sign of vitamin D deficiency in female-patten hair loss. As well, there have been links found between low levels of vitamin D and alopecia areata, and low levels of this vitamin may be a risk factor for developing this autoimmune disease which can effect hair loss. Everyone seems to be talking about vitamin D lately. But what's the big deal? Why should you care about vitamin D? Well, I'll tell you why, because there are many, many consequences of vitamin D deficiency. One consequence, having a poor immune system or being sickly. You can also have muscle weakness with vitamin D deficiency. And, of course, you can have weak bones if your vitamin D is deficient. There are some observational studies that show that vitamin D deficiency may be associated with fatigue, poor wound healing, hair loss. What is vitamin D? Vitamin D is a fat-soluble vitamin that is important in your body's absorption of calcium and phosphorus, which are both essential for bone health. But beyond being essential for bone health, vitamin D is also important for your immune system and for muscle strength. How do you get vitamin D in your system? Well, one way is through sun exposure. Yes, on a beautiful sunny day like today your skin can actually synthesize vitamin D from the sunlight. You can also get vitamin D through your diet, through foods like fatty fish, cod liver oil, cow's milk, and some vitamin D can be found in eggs, and then, of course, you can take vitamin D supplements. Who is at risk for vitamin D deficiency? People who don't have enough sun exposure are at risk. So if you are a person who wears a lot of sunscreens, or if you always carry an umbrella to prevent sun exposure, then you could be at risk for vitamin D deficiency, or people with darker skin with a lot of melanin in the skin are also at risk for vitamin D deficiency. Also, if you are a person who is homebound, or bed bound like a nursing home patient, you are at risk, and people who live in cold climates where they are essentially indoors during all the winter months, and not out getting sun exposure. These people are also at risk for vitamin D deficiency. You are also at risk if you have an inadequate dietary intake of vitamin D. What happens if you don't eat fatty fish, if you don't eat eggs, if you don't drink a cow's milk, and if you don't supplement your diet with vitamin D? Then certainly you can have vitamin D deficiency. You are also at risk for vitamin D deficiency if you are unable to absorb the vitamin D in your intestines properly. If you have Crohn's disease, then you are at risk, and if you have had a gastric bypass surgery, you can also be at risk for vitamin D deficiency. Patients with kidney or liver disease are at risk for vitamin D deficiency as well. Here's why. Both the liver and the kidney have enzymes which help to transform vitamin D into a biologically active form, so if you have kidney disease or liver disease, then make sure you check with your doctor for your vitamin D level, because you are definitely at risk for vitamin D deficiency. So how can you be diagnosed with vitamin D deficiency? Well, you simply go to your physician and ask for a blood test, your vitamin D level. A normal vitamin D level is at very least 30 nanograms per milliliter, but we actually prefer your vitamin D level to be greater than 50. You have vitamin D insufficiency if your vitamin D level is less than 30, and you have frank vitamin D deficiency if your vitamin D level is less than 20. So what is the treatment for vitamin D deficiency? Well, in addition to sun exposure, and to increasing vitamin D in your diet, you can get vitamin D supplementation. The two most commonly used vitamin D supplements are vitamin D2, or ergocalciferol, and vitamin D3, or cholecalciferol. I actually use both of these types of vitamin D when I treat my patients in my practice with vitamin D deficiency. This ends my overview of vitamin D deficiency symptoms, effects, and treatment. In certain studies, Americans has been found to be the #1 population in vitamin D deficiency. Some of the main symptoms of vitamin D deficiency are a weakened immune system, candida or bacterial overgrowth, emotional ups and downs, hormone imbalance, trouble putting on muscle or losing weight, weakened bones or teeth, cancer, heart disease, or diabetes. Vitamin D supports having a healthy immune response, digestive health, hormone balance, and more. The biggest causes of vitamin D deficiency are not getting enough sunlight, certain chemicals from plastic bottles (bpa), and toxicity. Try to get outside everyday for about 20-30 minutes of natural sunlight. Certain foods that are sources of vitamin D are wild caught fish, raw fermented milk, mushrooms, and eggs. However, there are small amounts in the food you eat. For a good supplement, I would recommend a high quality vitamin D supplement. You don’t want to take vitamin D2, but you do want to take vitamin D3. For supplementation, take about 2000-5000 IUs a day. However, if you have a severe deficiency, then for 1-2 months, you may take 5000 IUs twice daily. Lastly, you want to take your vitamin D3 with some form of fat like coconut oil or avocado to help absorption in your body. You mentioned that you went to the doctor and they told you that your vitamin D levels were low, and you're wondering what you can do to bring them back up. Now if your doctor recommended a supplement, then that would be best. And generally speaking, if your vitamin D levels are normal, most people need about 600 International Units of vitamin D a day. But if you're deficient, you might need up to 1000 International Units a day. So your doctor can tell you what dosage is most appropriate for you based on your lab results. But there are also other things you can do. Now sun is actually the best way to give us vitamin D. We are exposed to the sun, and our body synthesizes this vitamin in our body. But in Utah, we don't get much sun exposure during the winter, so it's important to get it from other sources. Now during the spring, summer, and early fall, spending 15 minutes in the sun each day will help boost our vitamin D levels. But during the winter you can drink 3 cups of vitamin D fortified milk or eat salmon. Salmon and other fatty fish are actually very high in vitamin D. A piece of salmon has about 265% of your daily allowance of vitamin D. So if you just eat 2 servings of salmon a week, then you're actually getting quite a bit of vitamin D. What we need are trials showing the difference in health outcomes when levels are brought up to 100nmol plus There is also a restriction on the number of tests that an NHS patient can receive in the UK. That is restricted to one test a year.

Researchers at the University of California, San Diego School of Medicine have found that persons with lower blood levels of vitamin D were twice as likely to die prematurely as people with higher blood levels of vitamin D. The finding, published in the June 12 issue of American Journal of Public Health, was based on a systematic review of 32 previous studies that included analyses of vitamin D, blood levels and human mortality rates. The specific variant of vitamin D assessed was 25-hydroxyvitamin D, the primary form found in blood. Garland said the blood level amount of vitamin D associated with about half of the death rate was 30 ng/ml. He noted that two-thirds of the U.S. population has an estimated blood vitamin D level below 30 ng/ml.
