Congestive Heart Failure Congestive heart failure is sometimes simply called “heart failure”. It is the leading cause of hospitalization in people over 65 affecting about 670,000 Americans annually. Currently nearly 6 million persons in the United States have a diagnosis of heart failure. Heart failure doesn’t mean that the heart has “stopped” working, but, that its pumping ability is weaker than normal. With heart failure, the pressure in the heart increases because the blood moves through the heart and body at a slower rate. As this happens the chambers of the heart may slowly change by stretching to hold more blood to pump through the body or by becoming stiff and thickened. This helps to keep the blood moving for a while, but eventually the heart muscle walls become weakened and then unable to pump efficiently. The next thing that usually happens is the kidneys respond by causing the body to retain fluid (water) and salt. The body becomes congested as this fluid builds up in the arms, legs, ankles, feet, lungs, or other organs. This condition is described now as “congestive heart failure”. Congestive Heart Failure (CHF), is a usually a chronic illness that affects the chambers of the heart. There are four chambers in the heart. The upper half of the heart is known as the atria which is divided into 2 sides, the left and the right side, and the other lower half is the left and right ventricles. A healthy normal heart is a strong, muscular pump somewhat bigger than a clenched hand. It pumps blood continuously through the circulatory system. The right atria takes in deoxygenated blood from the systemic circulation and sends it back out to the lungs through the right ventricles where the blood gets to be oxygenated. Oxygenated blood leaves the lungs, travels to the left atrium, then on to the left ventricles, which pumps it to the systemic circulation to provide oxygen and nutrient rich blood to the different organs and tissues. The heart pumps blood to the lungs and to all the body’s different organs and tissues by a sequence of highly organized contractions of the four chambers. For the heart to function properly, the four chambers must beat in an organized way. With heart failure, the heart muscles becomes weak for various reasons, making the heart slowly lose the capacity to pump enough blood throughout the body to meet the body’s demand of oxygen and nutrients. As the heart’s pumping turns out to be less effective, blood may back up into different zones of the body. Fluid may build up in the lungs, liver, gastrointestinal tract, and the arms and legs. Thus, it is called Congestive Heart Failure (CHF). There are two fundamental issues in congestive heart failure: Systolic dysfunction – when the heart can’t pump enough blood supply to meet the body’s demand. Diastolic dysfunction – when the heart cannot accept all the blood being sent in. Sadly, many people suffer with both systolic and diastolic heart failure. Causes: CHF is often a chronic condition, yet it may come on all of a sudden. It can be brought about by various heart problems. The condition may affect just the right side or just the left half of the heart. But more often, both sides of the heart are involved. The most common causes of CHF are: Coronary artery disease (CAD) – a condition wherein cholesterol and other types of fatty substances blocks or narrows the arteries that supply the heart with blood. This can weaken the heart muscle over time or suddenly. Materials and Methods: Patients who required hospitalization for congestive heart failure were randomly assigned to a group treated for 7 days with furosemide alone (FRO group) and a group treated with furosemide plus tolvaptan (TLV group) for 7 days and examined for symptoms. Physical examinations were performed every day, and blood testing, including N-terminal pro-brain natriuretic peptide (NT-proBNP) level, plasma renin activity (PRA), plasma aldosterone concentration (PAC), and noradrenaline (NAD) level, was performed on days 1, 3, and 7. Results: FRO and TLV groups consisted of 51 (age, 66.4 ± 11.8 years, 62% of males) and 47 patients (67.9 ± 14.5 years, 64% of males), respectively. During the study, the TLV group had higher urine volume and decreased blood pressure due to the suppressed diuretic effect. The two groups showed significant differences in the degree of improvement of the jugular venous pressure (FRO vs. TLV groups: 6.3 ± 1.6 vs. 7.6 ± 2.5 cmH2O, P 0.001, on day 3) and other physical findings. Although no significant differences in NT-proBNP and NAD levels were found, there were significant differences in PRA (19.8 ± 12.9 vs. 11.8 ± 8.0 ng/[mL . h], P 0.001, on day 3) and PAC (FRO vs. TLV groups: 180.4 ± 148.4 vs. 124.7 ± 95.5 ng/mL, P 0.01 on day 3 and 79.4 ± 73.9 vs. 56.8 ± 38.2 ng/mL, P 0.05 on day 7). Unlike some other diuretics, it does not cause your body to lose potassium. Diuretics are sometimes referred to as 'water tablets'. Spironolactone is often used alongside other diuretics. When it is used like this, it may be prescribed as a combination product, such as in Lasilactone® (spironolactone with furosemide) and Aldactide® (spironolactone with hydroflumethiazide). Combinations like these help to reduce the number of tablets you need to take each day. Before taking spironolactone Some medicines are not suitable for people with certain conditions, and sometimes a medicine may only be used if extra care is taken. Sulfonamides are used to treat allergies and cough, as well as antifungal and antimalarial functions. The moiety is also present in other medications that are not antimicrobials, including thiazide diuretics (including hydrochlorothiazide, metolazone, and indapamide, among others), loop diuretics (including furosemide, bumetanide, and torsemide), acetazolamide, sulfonylureas (including glipizide, glyburide, among others), and some COX-2 inhibitors (e.g., celecoxib). Sulfasalazine, in addition to its use as an antibiotic, is also used in the treatment of inflammatory bowel disease. Sulfonamide drugs were the first antibiotics to be used systemically, and paved the way for the antibiotic revolution in medicine. The first sulfonamide, trade-named Prontosil, was a prodrug. Experiments with Prontosil began in 1932 in the laboratories of Bayer AG, at that time a component of the huge German chemical trust IG Farben. The Bayer team believed that coal-tar dyes which are able to bind preferentially to bacteria and parasites might be used to attack harmful organisms in the body. After years of fruitless trial-and-error work on hundreds of dyes, a team led by physician/researcher Gerhard Domagk (working under the general direction of Farben executive Heinrich Hörlein) finally found one that worked: a red dye synthesized by Bayer chemist Josef Klarer that had remarkable effects on stopping some bacterial infections in mice. The first official communication about the breakthrough discovery was not published until 1935, more than two years after the drug was patented by Klarer and his research partner Fritz Mietzsch. Prontosil, as Bayer named the new drug, was the first medicine ever discovered that could effectively treat a range of bacterial infections inside the body. It had a strong protective action against infections caused by streptococci, including blood infections, childbed fever, and erysipelas, and a lesser effect on infections caused by other cocci. However, it had no effect at all in the test tube, exerting its antibacterial action only in live animals. Later, it was discovered by Bovet,[7] Federico Nitti and J. and Th. Jacques Tréfouël, a French research team led by Ernest Fourneau at the Pasteur Institute, that the drug was metabolized into two pieces inside the body, releasing from the inactive dye portion a smaller, colorless, active compound called sulfanilamide.[8] The discovery helped establish the concept of "bioactivation" and dashed the German corporation's dreams of enormous profit; the active molecule sulfanilamide (or sulfa) had first been synthesized in 1906 and was widely used in the dye-making industry; its patent had since expired and the drug was available to anyone.[9] The result was a sulfa craze.[10] For several years in the late 1930s, hundreds of manufacturers produced tens of thousands of tons of myriad forms of sulfa. This and nonexistent testing requirements led to the elixir sulfanilamide disaster in the fall of 1937, during which at least 100 people were poisoned with diethylene glycol. This led to the passage of the Federal Food, Drug, and Cosmetic Act in 1938 in the United States. As the first and only effective antibiotic available in the years before penicillin, sulfa drugs continued to thrive through the early years of World War II.[11] They are credited with saving the lives of tens of thousands of patients, including Franklin Delano Roosevelt, Jr. (son of US President Franklin Delano Roosevelt) and Winston Churchill. Sulfa had a central role in preventing wound infections during the war. American soldiers were issued a first-aid kit containing sulfa pills and powder, and were told to sprinkle it on any open wound. The sulfanilamide compound is more active in the protonated form. The drug has very low solubility and sometimes can crystallize in the kidneys, due to its first pKa of around 10. This is a very painful experience, so patients are told to take the medication with copious amounts of water. Newer analogous compounds prevent this complication because they have a lower pKa, around 5–6, making them more likely to remain in a soluble form. Many thousands of molecules containing the sulfanilamide structure have been created since its discovery (by one account, over 5,400 permutations by 1945), yielding improved formulations with greater effectiveness and less toxicity. My Lasik Surgery Video - Medical Videos LASIK, or "laser-assisted in situ keratomileusis," is the most commonly performed laser eye surgery to treat myopia (nearsightedness), hyperopia (farsightedness) and astigmatism. Like other types of refractive surgery, the LASIK procedure reshapes the cornea to enable light entering the eye to be properly focused onto the retina for clearer vision. In most cases, laser eye surgery is pain-free and completed within 15 minutes for both eyes. The results — improved vision without eyeglasses or contact lenses — can usually be seen in as little as 24 hours. If you're not a good LASIK candidate, a number of other vision correction surgeries are available, such as PRK and LASEK laser eye surgery and phakic IOL surgery. Your eye doctor will determine if one of these procedures is suitable for your condition and, if so, which technique is best. How Is LASIK Surgery Performed? First, your eye surgeon uses either a mechanical surgical tool called a microkeratome or a femtosecond laser to create a thin, circular "flap" in the cornea. The surgeon then folds back the hinged flap to access the underlying cornea (called the stroma) and removes some corneal tissue using an excimer laser. This highly specialized laser uses a cool ultraviolet light beam to remove ("ablate") microscopic amounts of tissue from the cornea to reshape it so it more accurately focuses light on the retina for improved vision. For nearsighted people, the goal is to flatten the cornea; with farsighted people, a steeper cornea is desired. Excimer lasers also can correct astigmatism by smoothing an irregular cornea into a more normal shape. It is a misconception that LASIK cannot treat astigmatism. After the laser reshapes the cornea, the flap is then laid back in place, covering the area where the corneal tissue was removed. Then the cornea is allowed to heal naturally. Laser eye surgery requires only topical anesthetic drops, and no bandages or stitches are required.

Research and clinical studies prove it to be a very effective drug class for many, as it works by causing the blood vessels to become enlarged or dilated, and thus causing a reduction in the blood pressure. This lowering of the blood pressure makes it easier for the heart to pump blood and it can improve the function of a failing heart. In the United States, 10 ACE inhibitors are in use: Ramipril (Altace) Enalapril (Vasotec, Epaned) Quinapril (Accupril) Captopril (Capoten) Benazepril (Lotensin) Lisinopril (Prinivil, Zestril) Fosinopril (Monopril) Moexipril (Univasc) Trandolapril (Mavik) Perindopril (Aceon) NOTE...that the brand names of all of these above end with the suffix “pril”. NOW.. let’s take a closer look at this drug class, angiotensin-converting enzyme (ACE) inhibitors, these drugs are helpful in treating heart, blood vessel, and kidney problems... Mechanism of Action: The human body is an amazing organism…In order to regulate water and blood pressure, our body follows special mechanisms. For example when the blood pressure drops too low, the kidneys release renin into the bloodstream. Renin then reacts with a protein called angiotensinogen that is produced by the liver. As a result, they form angiotensin I. Then another reaction converts angiotensin I to angiotensin II, with the help of angiotensin-converting enzyme found in the respiratory system. This whole mechanism is known as Renin-angiotensin system (RAS). Angiotensin II is a powerful substance. It circulates around the body in the bloodstream and induces several changes on the cardiovascular system. Such as stimulating adrenal cortex to release adolsterone and Constricting or narrowing down the blood vessels around the body. Aldosterone increases the reabsorption of water and sodium in the tubules of the kidneys. As a result, The volume of the blood and the concentration of salt in the blood increase leading to high blood pressure. The constriction of blood vessels results in high pressure inside the vessels, which also means an increase in blood pressure. Both of these events cause the heart to exert more pressure while pumping blood into the main arteries. Hence, it reverses the low blood pressure but it can also lead to other cardiovascular disorders if it persists for a long time. ACE inhibitors reduce the impact of renin-angiotensin-aldosterone system (RAAS) on the blood pressure by inhibiting the angiotensin converting enzyme. As a result, The conversion of angiotensin I to angiotensin II is slowed down which brings about several changes in the cardiovascular system. Just the physical swelling will cause that.DO THESE THINGS RIGHT NOW (TODAY) BEFORE GOING TO THE HOSPITAL FIRST THING: Laxative-If you even think you may have a thrombosis-Start immediately on either All-Bran cereal or Mira-Lax with each meal SECOND THING: Plan ahead-Transport (911=$900)/meals, grocery shopping, business now SECOND THING: Buy a pair of thigh high compression stockings 20-30mm Hg-no Rx nec. WHAT TO EXPECT: At the ER they will put an IV line in you to keep fluids going into your body. Your leg diameter will continue to swell for the next few days. Be sure you ask for a doppler to confirm thrombosis. Your pain levels will increase...you won't be able to walk but a few meters without passing out from orthostatic hypotension. You will only use these until about two or three weeks after you get home then order some stockings at 30-40mm for the next 4-6weeks and then go to 40-50mm. As soon as you can (when the 30-40mm began to stretch and lose compression.The hospital may give you some short knee high compression socks which are useless and just slow your progress or they may use a machine with a pressure cuff to stimulate circulation---they usually don't get the cuff adjusted tight enough to do any good....take some control here and do it yourself. This will also make going to the toilet a lot more difficult. Until you can get the fluid in your body down (ask doc to use lasix for that) you will be in great pain and pretty immobile and severely constipated and unable to pee. You will be put on Coumadin or Warfarin accompanied with a few shots of Lovenox injections for about ten days. The Coumadin you may be on forever. You will also be given medication for pain.

ACCUMULATION OF FLUID IN THE ABDOMINAL CAVITY (ASCITES) Symptoms, Causes & Treatments OVERVIEW Ascites is the buildup of an abnormal amount of fluid inside the abdomen (belly). This is a common problem in patients with cirrhosis (scarring) of the liver. Approximately 80% of patients with cirrhosis of the liver develop ascites. CAUSES Ascites is the end result of a series of events. Cirrhosis of the liver is the most common cause of ascites. When cirrhosis occurs, blood flow through the liver is blocked. This blockage causes an increase in the pressure in the main vein (the portal vein) that delivers blood from the digestive organs to the liver. This condition is called portal hypertension. Ascites occurs when portal hypertension develops. The kidneys cannot rid the body of enough sodium (salt) through urine. Not being able to rid the body of salt causes fluids to build up in the abdomen, resulting in ascites. SYMPTOMS Most people who develop ascites develop a large belly and experience a rapid gain in weight. Some people also develop swelling of the ankles and shortness of breath. TREATMENTS Limit the amount of salt in your diet. The most important step to treating ascites is to drastically reduce your salt intake. Recommended limits are 2,000 mg or less a day. Seeing a nutritional specialist (dietitian) is helpful especially because the salt content in foods is difficult to determine. Salt substitutes – that do not contain potassium -- can be used. Often, patients will require diuretics ("water pills") to treat ascites. Take these pills as prescribed. Common diuretics are spironolactone (Aldactone®) and/or furosemide (Lasix®). These water pills can cause problems with your electrolytes (sodium, potassium) and kidney function (creatinine). Taking water pills is not a substitute for reducing your salt intake. Both are needed to treat ascites. Other, increasingly more aggressive treatments include: Paracentesis as a treatment: Sometimes fluids continue to build up in the abdomen despite use of diuretics and a restricted salt diet. In these cases, patients may need paracentesis to remove this large amount of excess fluid. Surgery: Surgical placement of a shunt (tube) between the main vein (portal vein) and smaller veins is sometimes used as a treatment. A radiologist can place a shunt directly through the liver, thereby relieving portal hypertension and diminishing ascites. By increasing blood flow, all organs of the body are better able to perform their function. For example, in patients with ascites, improved kidney function helps rid the body of excess sodium (salt) and prevent the buildup of fluids. Liver transplant: This approach is reserved for patients with very severe cirrhosis whose livers are failing.
