When I consider purchasing an individual health insurance plan for myself or my family, do I have any financial obligations beyond the monthly premium and annual deductible? Answers: It depends on the plan, but some plans have the following cost-sharing elements that you should be aware of. Co-Payments: Some plans include a co-payment, which is typically a specific flat fee you pay for each medical service, such as $30 for an office visit. After the co-payment is made, the insurance company typically pays the remainder of the covered medical charges. Deductibles: Some plans include a deductible, which typically refers to the amount of money you must pay each year before your health insurance plan starts to pay for covered medical expenses. Coinsurance: Some plans include coinsurance. Coinsurance is a cost sharing requirement that makes you responsible for paying a certain percentage of any costs. The insurance company pays the remaining percentage of the covered medical expenses after your insurance deductible is met. Out-of-pocket limit: Some plans include an out-of-pocket limit. Typically, the out-of-pocket limit is the maximum amount you will pay out of your own pocket for covered medical expenses in a given year. The out-of-pocket limit typically includes deductibles and coinsurance. But, out-of-pocket limits don't typically apply to co-payments. Lifetime maximum: Most plans include a lifetime maximum. Typically the lifetime maximum is the amount your insurance plan will pay for covered medical expenses in the course of your lifetime. Exclusions & Limitations: Most health insurance carriers disclose exclusions & limitations of their plans. It is always a good idea to know what benefits are limited and which services are excluded on your plan. You will be obligated to pay for 100% of services that are excluded on your policy. Beginning September 23, 2010, the Patient Protection and Affordable Care Act (health care reform) begins to phase out annual dollar limits. Starting on September 23, 2012, annual limits on health insurance plans must be at least $2 million. By 2014 no new health insurance plan will be permitted to have an annual dollar limit on most covered benefits. Some health insurance plans purchased before March 23, 2010 have what is called "grandfathered status." Health Insurance Plans with Grandfathered status are exempt from several changes required by health care reform including this phase out of annual limits on health coverage. If you purchased your health insurance policy after March 23, 2010 and you're due for a routine preventive care screening like a mammogram or colonoscopy, you may be able to receive that preventive care screening without making a co-payment. You can talk to your insurer or your licensed eHealthInsurance agent if you need help determining whether or not you qualify for a screening without a co-payment. There are five important changes that occurred with individual and family health insurance policies on September 23, 2010. Those changes are: 1. Added protection from rate increases: Insurance companies will need to publically disclose any rate increases and provide justification before raising your monthly premiums. 2. Added protection from having insurance canceled: An insurance company cannot cancel your policy except in cases of intentional misrepresentations or fraud. 3. Coverage for preventive care: Certain recommended preventive services, immunizations, and screenings will be covered with no cost sharing requirement. 4. No lifetime maximums on health coverage: No lifetime limits on the dollar value of those health benefits deemed to be essential by the Department of Health and Human Services. 5. No pre-existing condition exclusions for children: If you have children under the age of 19 with pre-existing medical conditions, their application for health insurance cannot be declined due to a pre-existing medical condition. In some states a child may need to wait for the state's open-enrollment period before their application will be approved.

HMO - an organization of doctors and other professionals that by contract, provide in-network services from self-funded (premium-paid) health care benefit plans. PPO - a network of physicians, hospitals, etc that provide a substantial in-network discount below regularly-charged rates. POS - basically combines characteristics of both the HMO and a PPO. Servie is sometimes in and out of network. COBRA - A law that protects coverage for a limited time when there is job loss, transition, death, divorce. Those are basic definitions of health care plans and organizations so when you're thinking of cheap, affordable health care plans, keep this in mind. But before we consider the merits of the government managing your health care—and that’s what this all boils down to—maybe we should ask a more basic question: What do we mean by “health care”? Because if you get sick—and here, we’re talking major illness—or you’re in serious pain, you don’t just want health care; you want quality health care. And where is your best chance of finding that? The answer is right here in America. For skilled doctors, cutting-edge medical treatments, and care without long delays, no other country rivals the United States. Not even close. Nobody from Texas is going to Canada for medical treatment. It’s almost always the other way around. Sure, our health care system has lots of issues—and we should address them—but do we really want to upend all the advantages that we do have and start from scratch? Because that’s what would have to happen if we completely turn health care over to the government. So, let’s imagine we make the change. We hear a lot about how great free health care would be, but it’s only fair we look at the downside. The first is that government-run health care takes medical decisions away from patients—that means you—and puts them in the hands of bureaucrats. They decide, for example, how many MRI machines are going to be available, or under what conditions you can get back surgery or a bypass, or even whether you qualify for cancer treatment. That’s how it works in the United Kingdom under its single-payer system. Because it has finite resources, the National Health Service, or NHS, sharply restricts access to treatments like hip and knee replacements, cataract surgery, and even prescription drugs to deal with common conditions like arthritis and diabetes. If you suffer from any of these ailments and many others in the UK, you may just have to live with the pain. And let’s hope you don’t have a medical emergency. In a January 2018 article in the New York Times, patients in emergency rooms around London are described as having “to wait 12 hours before they are tended to. Corridors are jammed with beds carrying [the] frail and elderly.” To deal with the situation, “hospitals [were] ordered to postpone non-urgent surgeries until the end of the month.” That hardly seems like an improvement over what we have in the US. UnitedHealthcare dental plans can offer a great way to meet your needs. More dentists in our network means you are more likely to keep the same dentist you use today and also have coverage available when traveling. You can add vision benefits, which are available in most states, to your dental plan. Our dental plans are family-friendly. And since there’s no maximum age, these plans can also benefit those over age 65. Also, our Dental Plans do not have any medical underwriting. The minimum issue age for these new dental plans is 18. As long as the primary insured is at least 18 years of age, then younger dependents can be on the plan. There are four dental plans available for you to choose from. Dental Primary is the lowest priced of the four plans, and doesn’t include coverage for major services. Dental Primary Preferred offers coverage for major services for a slightly higher premium. Dental Premier Choice is a benefit-rich plan. UnitedHealthcare Dental Premier Elite is a benefit-rich plan with more out-of-network coverage. Dental Primary and Dental Primary Preferred have a deductible for basic services of $50, and Dental Primary Preferred has a deductible for major services of $50. Neither plan has a waiting period for preventive services, but both have a six-month waiting period for basic services. Also, there is a 12-month waiting period for major services on Dental Primary Preferred. Both plans have a $25 copay for preventive care, and the same annual maximum of $1,000 per covered person. Basic services are covered at 70% coinsurance after the deductible, and major services are covered at 50% on Dental Primary Preferred. Major services are not covered on Dental Primary. Both plans cover up to 4 bitewing x-rays once per calendar year and provide out-of-network care based on the network rates. Dental Premier Choice and Dental Premier Elite have a deductible of $50 combined for basic and major services. Neither plan has a waiting period for preventive services, but both have a six-month waiting period for basic services, and a 12-month waiting period for major services. Preventive care is covered at 100%, no copays or deductibles. Both plans start with an annual maximum of $1,200 per covered person. Over time, your annual maximum can increase to $1,500 per covered person. Basic services are covered at 80% coinsurance after the deductible. Major services are covered at 50%. Also, both plans cover up to 4 bitewing x-rays once per calendar year. Dental Premier Choice provides out-of-network care based on the network rate. Dental Premier Elite offers more out-of-network coverage. These dental plans also utilize the UnitedHealthcare Dental network, with access to providers across the United States. Also, if you have an existing UnitedHealthcare dental plan, you can switch to one of the plans! This change requires a new application, and new waiting periods will apply on the new coverage. For an application sent online, the effective date will be the later of the requested effective date or the day after receipt by Golden Rule Insurance Company. For a mailed paper application, the effective date will be the later of the requested effective date or the day after the postmark date. The vision coverage you can add to a dental plan features an extremely simple plan design. There are no waiting periods, and covered services have a simple copay. Utilizing a network provider is very important. After the copay of $10 or $25, depending on the service, the vision plan pays 100% of your covered expenses. If you should choose to utilize an out-of-network provider, UnitedHealthcare still provides coverage, just on a more limited basis. "so i just want to take care of my family and save a buck." while he and his wife were accepted--his daughter was denied not for being sick--but for being a kid. the letter from presbyterian health plan says it in plain black and white. "denial reason: children are not eligible for coverage at this time." "you know surprised. cuz i went online to do it. i filled out all the paperwork but nowhere was there a disclaimer...right now we can't cover children or there's issues." turns out presbyterian and other health insurance are still working on individual coverage plans for people 19 and under that would be legal under new health care reform laws. "it could be as early as this week that we get approval, and when we do we are going to be reaching out to everybody." presbyterian health plan president dr. dennis batey says the new rules are so complex, it's taken a while for his company to draft a new children's plan. "with healthcare reform, the products that we've been offering in the past we could no longer offer. we are working with superintendent franchini of the department of insurance. we are awaiting approval of the product. " state insurance superintende john franchini says in the meantime, parents who need coverage for kids can find it through the state. In fact, a 20071 study found that people diagnosed with serious mental illness (SMI) died approximately 25 years earlier than the general population from largely preventable conditions, including cardiovascular disease, infectious disease, and diabetes. Medicaid Redesign, coupled with provisions of the Affordable Care Act, is creating the push to integrate behavioral health and physical health, improve coordination of care for clients and move the full Medicaid behavioral healthcare benefit into managed care. Simultaneously, New York State will be shifting from an institutional model of care to a community-based model, thus adding a level of complexity to an already demanding transition. For community-based service providers, this shift in both federal and State policies will have an impact on virtually every aspect of care in New York State, This webcast will provide an overview of the history of mental health treatment in New York State, discuss challenges in addressing mental health in the current system, illustrate how the shift in federal and state policy will impact community health providers and workers, and highlight successful approaches to addressing mental health at the community level.

I think believing in yourself and trusting yourself even in the face of failure is a really important quality for med school. Med school can be really challenging and there’s gonna be times when you doubt yourself and feel like you’re just not good enough. But you really have to fight through those moments of insecurity- No one is going to do that for you. Only you can build yourself back up after making mistakes. And trust me, there will be lots of mistakes but getting back up and learning from them is what’s going to make you a great doctor. A health savings account (HSA) is a savings vehicle established to set aside funds tax free to pay for health care expenses. HSAs allow individuals who have high-deductible health plans (HDHPs) to save money for health-care expenses tax free. One of the advantages of HSAs is that unlike FSAs, funds remaining in your account at the end of the year are not forfeited. There is no "use it or lose it" provision. Moreover, the funds are portable and can be used through your retirement years. Generally, if you are covered under an HDHP, you are eligible to establish an HSA. You will not be eligible to open an HSA, even if you are covered under an HDHP, if any of the following apply: 1. You are already covered under a non-HDHP, including a comprehensive major medical plan, a plan sponsored by your employer or your spouse's employer, or a prescription drug plan or rider with a low deductible or no deductible. (Some plans such as dental or vision care insurance, long-term care insurance, disability insurance, and accident insurance are exempted.) 2. You can be claimed as a dependent on another person's income tax return. 3. You have enrolled in Medicare. If your spouse has non-HDHP family coverage, but that plan does not cover you, you may still contribute to an HSA if you are otherwise eligible to do so. However, your spouse will not be eligible to contribute to an HSA. How do you establish an HSA and make contributions? An HSA is a tax-exempt trust or custodial account. You can open an HSA on your own or through your employer if they offer HSAs as part of a cafeteria plan. Contributions may be made by you or your employer directly or through salary reduction under a cafeteria plan (if available). However, no contributions can be made to your HSA once you retire. For tax year 2014, you can contribute up to $3,300 for individual coverage or $6,550 for family coverage, indexed annually for inflation. You and your spouse may also be eligible to make additional "catch-up contributions" of $1,000/each to your HSA if you are 55 or older. However, no regular or catch-up contributions can be made once you reach age 65 and are enrolled in Medicare. Contributions may be made monthly or as a lump-sum any time before your tax return becomes due. What are the tax benefits? In effect, an HSA provides a triple-tax advantage… (1) Individual contributions you make to your HSA that do not exceed the maximum contribution limit are tax deductible on your federal income tax return; even if you don't itemize. (2) You can direct your contributions to a savings or investment option offered by the qualified trustee or custodian and any interest or investment earnings on those contributions grow tax deferred until withdrawn. (3) Distributions will be tax free when withdrawn to pay qualified medical expenses for yourself, spouse or qualified dependents. Distributions for non-qualified expenses are considered taxable income and are subject to an additional 20 percent penalty tax. No penalty applies if a non-qualified distribution occurs after age 65 or the death or disability of the beneficiary. What are qualified medical expenses? Qualified medical expenses are health-care expenses, as defined by Internal Revenue Code 213(d), which are paid by you, your spouse, or your dependents. These include laboratory fees, prescription and nonprescription drugs, dental treatment, ambulance service, eyeglasses, and hearing aids, as well as many other health care expenses. Health insurance premiums are not covered except for the following: (1) COBRA coverage, (2) Qualified long-term care insurance, (3) Health coverage maintained while receiving unemployment compensation and (4) Retiree health insurance other than a Medicare supplemental policy (Medigap). Over-the-counter (OTC) medications are no longer considered a qualified medical expense however, OTC medicines and insulin prescribed by a physician will still be considered qualifying expenses. Last thing… Funds remaining in your HSA upon your death become the property of your designated beneficiary.
