Metformin has been known to increase the lifespan of roundworms, and more recently mice. A diabetes trial in England, the UKPDS looked at diabetics and cardiovascular events related to how well their diabetes was controlled. It was noticed that those treated with metformin had less events and lived longer than those not treated with metformin. The FDA just approved the first human trials looking at metformin and longevity. There are several proposed mechanisms. One is that it is a diabetes drug, however no other diabetes drug has shown benefits. It lowers insulin levels and insulin and IGF (Insulin like growth factor) may cause inflammation or turn on cancer cells (gene expression). Another btheory is that it effects oxidative stress in the cells, also increasing gene expression of cancer or aging cells. Normally we tke about antioxidants as being healthy, this may be a controlled form of oxidative stress with a beneficial effect. If true, then taking antioxidaants may counteract the effect. Metformin is a safe drug, inexpensive and may have multiple beneficial effects, in both the diabetic, and non diabetic. It is extremely early in this investigation, and I cannot recommend it just yet in non diabetics. However, if you are a pre-diabetic, or diabetic, you should have a conversation with your doctor if you are not on it already.

But the problems is, if you are type 1, they are giving you insulin, your insulin is going to be high now because you are going to be taking insulin to control this out of control blood sugar. So in both of this conditions you have high sugar and high insulin at the same time. Now it’s more dangerous to have high insulin than it is to have high sugar, there is more problems. Sustain high levels of insulin destroys the retina, kidney, arteries and the brain. Of course high sugars will do it too but high insulin creates even more destruction. The goal is to take the least amount of insulin possible. If you are type 1 and maybe your cells are completely destroyed of any chance of recovery but we still want you to take the least amount of insulin possible so it would make sense to avoid the triggers of insulin. Unfortunately, the American Diabetes Association does not recommend what i'm gonna recommend but the needs to be more focus on the diet and the frequency of eating because reducing your carbohydrates and doing the intermittent fasting are the two things they are gonna allow the person to take the least amount of insulin. The study, published in April of 2015in the journal Annals of Internal Medicine, found that only 3.7% of people with prediabetes were prescribed metformin over a three-year period, based on data from a large national sample of adults ages 19 to 58. According to a Medscape article on the study, 7.8% of people with prediabetes with a BMI of 35 or higher or a history of gestational diabetes were prescribed metformin — still a very low rate for the highest-risk groups, in which evidence for the benefits of metformin is strongest. It appears that most doctors simply aren’t following the ADA’s guidelines or aren’t aware of them, as they relate to prediabetes. This despite a 2011 study that showed a reduced rate of 31% of those with prediabetes who took Metformin. Now, I know you didn’t ask about all this, you wanted to know the cost of medication. For some, this so-called diet consists of avoiding sugar, while others believe it to be a strict way of eating that controls glucose. Unfortunately, neither are quite right. The "diabetes diet" is not something that people with type 1 or type 2 diabetes should be following. "That just simply isn't how meal planning works today for patients with diabetes," says Amy Campbell, MS, RD, LDN, CDE, a nutritionist at Joslin and co-author of 16 Myths of a Diabetic Diet. "The important message is that with proper education and within the context of healthy eating, a person with diabetes can eat anything a person without diabetes eats," Campbell states. What's the truth about diabetes and diet? We know now that it is okay for people with diabetes to substitute sugar-containing food for other carbohydrates as part of a balanced meal plan. Prevailing beliefs up to the mid-1990s were that people with diabetes should avoid foods that contain so-called "simple" sugars and replace them with "complex" carbohydrates, such as those found in potatoes and cereals. A review of the research at that time revealed that there was relatively little scientific evidence to support the theory that simple sugars are more rapidly digested and absorbed than starches, and therefore more apt to produce high blood glucose levels. Now many patients are being taught to focus on how many total grams of carbohydrate they can eat throughout the day at each meal and snack, and still keep their blood glucose under good control. Well-controlled blood glucose is a top priority because other research studies have concluded that all people with diabetes can cut their risk of developing diabetes complications such as heart disease, stroke, kidney and eye disease, nerve damage, and more, by keeping their blood glucose as closely controlled as possible. What does this mean for people with diabetes? This means that a person who has worked with a dietitian and a diabetes treatment team to figure out how many grams of carbohydrate they can eat throughout the day can decide at any given meal what they will eat. Those with diabetes who are not on insulin need to focus on keeping the amount of carbohydrate they eat consistent throughout the day. Those on insulin can decide both what and how much to eat at a given meal (as long as it doesn't exceed their daily allotment), and can then adjust their insulin accordingly. "There aren't any foods that are 'off-limits,'" says Campbell. "Rather , one just needs to learn how to spend his or her grams of carbohydrate wisely over the course of the day." Frequent home blood glucose monitoring is then used to keep track of the effects of meals and activity levels on their blood glucose. They work with their healthcare team to make adjustments in their food intake, physical activity, and medication to keep their blood glucose as close to normal as possible. How does carbohydrate counting work? Most foods--except meat and fat--contain some carbohydrate, and carbohydrate increases blood glucose faster than any other food. The number of grams of carbohydrate that a person can eat each day or at each meal is determined by: - Weight and weight loss goals - How physically active an individual is (because physical activity will lower their blood glucose) - What diabetes medication or insulin they are taking, and when - Other factors such as age or the presence of high blood fats (or any other medical issue, for that matter) For example, a 6' 2" tall man with diabetes who weighs 180 pounds and wants to maintain his current weight might be told he could eat 350 grams of carbohydrate spread out over the day. His goal would be to spread those grams out over the course of the day so that he doesn't send his blood glucose too high at any one time. If he is taking insulin or oral diabetes medication, he might also have to manage when he eats his carbohydrate in such a way that there is enough sugar from his meals in his bloodstream when his medication is working its hardest.

In another video I discovered that even though the American Diabetes Association recommends Metformin for those with prediabetes, only about 10% or less was actually prescribed the drug. Some of you may not be taking any diabetes meds while others may be taking Metformin and even insulin of some sort. So I will try to give you a brief overview thanks to my friends from the United Kingdom Diabetes Association who has done extensive studies on the topic. Metformin contains the active ingredient metformin hydrochloride (or metformin hcl). Metformin is available both in combination with other drugs, or as a single treatment (a monotherapy). Metformin was approved in 1994 in the United States. Metformin helps those with diabetes to respond normally to insulin. Like most diabetic drugs, the ultimate goals of Metformin are to lower blood sugar to a normal level and maintain this level. Metformin can be used in conjunction with other diabetic drugs, and diabetics should also use diet and exercise to help control their condition. Metformin helps the body to control blood sugar in several ways. The drug helps type 2 diabetics respond better to their own insulin, lower the amount of sugar created by the liver, and decreasing the amount of sugar absorbed by the intestines. There are other diabetic drugs, but for those of you with prediabetes or even Type 2, I suspect you take Metformin. A quick word about insulin. Even those very few with prediabetes take insulin, I think it is worth giving a brief overview. And those of you with Type 2 who take insulin, this may give you some insight as to what it does and how it works. Insulin is a hormone made in your pancreas, a gland located behind your stomach. It allows your body to use glucose for energy. Glucose is a type of sugar found in many carbohydrates. After a meal or snack, the digestive tract breaks down carbohydrates and changes them into glucose. Glucose is then absorbed into your bloodstream through the lining in your small intestine. Once glucose is in your bloodstream, insulin causes cells throughout your body to absorb the sugar and use it for energy. Insulin also helps balance your blood glucose levels. When there’s too much glucose in your bloodstream, insulin signals your body to store the excess in your liver. The stored glucose isn’t released until your blood glucose levels decrease, such as between meals or when your body is stressed or needs an extra boost of energy. People with diabetes either don’t make enough insulin or your body doesn’t use it properly. That is where insulin injections come into play. Insulin injections simply replace or supplement your body’s supply of insulin. All types of insulin produce the same effect. The makeup of different types of insulin affects how fast and how long they work to help mimic the natural increases and decreases of insulin levels throughout the body during the day. Rapid-acting insulin: This type of insulin begins working approximately 15 minutes after injection. Its effects can last between two and four hours. It's often used before a meal. Short-acting insulin: You inject this insulin before a meal. It starts working 30 to 60 minutes after you inject it and lasts five to eight hours. Intermediate-acting insulin: This type of insulin starts working in 30 minutes to an hour after injection, and its effects may last up to eight hours. Long-acting insulin: This insulin may not start working until about two hours after you inject it. However, it can last up to 24 hours. According to the American Journal of the College of Cardiology The only way to delay cardiovascular disease in patients with pre-diabetes is to prevent (or delay) the development of diabetes. Unfortunately, there is no proven way to prevent the decline in beta cell function in persons destined to have diabetes. Therefore, priority must be given to reducing insulin resistance. This is best achieved by lifestyle intervention—weight reduction and increased physical activity. Since your body isn’t using insulin well when you have prediabetes, a lower insulin resistance is a very good thing. And of course, there are all the traditional benefits of exercise: it can help you lose weight, keep your heart healthy, make you sleep better, and even improve your mood. The American Diabetes Association recommends at least 150 minutes of moderate activity a week—that’s 30 minutes five days a week. You can get that through activities such as walking, bike riding, or swimming. Lose weight: If you’re overweight, you should get started on a weight loss program as soon as you’re diagnosed with prediabetes. Losing just 5 to 10% of your weight can significantly reduce your risk of developing type 2 diabetes. The combination of eating well and exercising more is a great way to lose weight—and then maintain your new, healthy weight. In most cases you are written a prescription for a machine called a glucometer (some call a glucose meter). Sometimes your healthcare provider will have one to give you. Generally you can get a glucometer kit for free, with enough supplies to get started. You will then pay for the test strips and lancets each month or however often you will need them. Before we get into how to use the glucometer, I think it is important for us to explore some other areas concerning the task of taking blood sugar readings. WHY DO I NEED TO MONITOR MY BLOOD GLUCOSE? Monitoring your glucose daily is simply a vital part of managing diabetes. It also is important because it empowers you to be in charge of your disease. No matter what your treatment plan consists of, blood sugar monitoring gives you immediate feedback into how well your plan is doing. Is it working? “Checking your blood glucose gives you the freedom to make choices without worry, the confidence to learn from your actions, and the motivation to keep striving to do better,” says Linda Dale, Clinical Nurse Specialist in the Outpatient Diabetes Education Program at the University of Michigan Comprehensive Diabetes Center in Ann Arbor, Michigan. “Monitoring tells you that what you’re doing either is working or isn’t, and it serves as motivation to keep up actions that are working or to make changes.” WHAT DO THE NUMBERS MEAN? Knowing what the numbers mean is very important in regards to taking blood glucose readings. If you know everything about getting the numbers, but don’t know what they mean, you are wasting time and effort. Knowing if your blood sugar is too high or too low or just right is vital in treating diabetes. You were diagnosed with diabetes because of these numbers. Your fasting blood glucose level was greater than 126 mg/dl on 2 separate readings. This is the amount of glucose that was present in your blood after a time of not eating. Normal blood glucose levels are less than 100. Blood glucose readings of 100-125 are considered pre-diabetes. You are shooting for a fasting blood glucose level of 80-130 before each meal which is called pre-prandial. A post-prandial reading (which is 1-2 hours after eating) should be less than 180. A reading of less than 80 is considered hypoglycemic. This means your blood glucose level is getting too low. In this case you will need to get some carbs in your system. The recommended amount of carbs is 15 grams and then check your blood sugar levels again. If your readings are extremely high, especially before eating, say in the 180 range, you may need to take a fast acting insulin to help bring it down. This should be discussed with your doctor and is one of the reasons taking blood sugar readings are so important. “Regular monitoring is especially helpful for showing the positive effects of exercise,” says Dale. “Say your readings have regularly been around 140 mg/dl, but you start taking a walk every day and you start getting more readings around 120 mg/dl. The lancet device is spring loaded, most devices require you to set the spring by pulling back on the back of the device. You should use a fresh lancet every time. Look for instructions on how to change your lancet. 5. You will put the lancet device firmly on your finger and then depress the button that will allow the lancet to make a very small hole in the finger. 6. Squeeze around the hole and get a drop of blood to come to the surface of the skin. 7. Then place that drop of blood on the test strip that is in your glucometer. 8. Wait for the results. 9. Write the results down in a blood sugar journal. (There are several phone apps that are great for this. Some glucometers even have apps that will document this result for you on your phone.) And there you have it. You have successfully taken a blood glucose reading and hopefully know why you did so.

Short, intense physical activity can help you manage blood sugar levels. It's the end of the day, and you don't have time to hit the gym for your regular workout. Should you leave it until tomorrow or carve out time for a little exercise today? If you have diabetes, go for the second option. Any physical activity is better than none, and adding short, intense bursts of activity to a mini workout may help your body do a better job of metabolizing glucose to regulate your blood sugar level. Short, Fast, and Furious In a small study, volunteers worked out on stationary bikes three times a week for a minimum of 17 minutes per session. That's pretty manageable. Then, during the sessions, they threw in a handful of 30-second bursts of high-intensity cycling. Again, not too taxing. After the bursts, they rested or cycled slowly for 4 minutes. The result? When the volunteers were given the equivalent of a meal's worth of glucose at the end of the study, their bodies metabolized it better than before the study. Just a Little Helps a Lot Researchers suspect that bursts of intensity during workouts elicit stronger muscle contractions and therefore more glucose uptake in the large muscles attached to bones. The lesson: If your busy day only leaves time for a short workout, be sure to add a few bouts of higher-intensity effort. Your blood sugar will be better for it. Does Everyone With Prediabetes Eventually Get Diabetes?

BUT, whether or not you develop Type 2 is totally up to you. You have the power to prevent it or you have the power to let your diagnosis progress. The American Diabetes Association says that serious lifestyle changes are effective in preventing type 2 diabetes after you’ve been diagnosed with pre-diabetes. Your doctor will walk you through what you need to change, but typical recommendations are: Eat well: A registered dietitian (RD) or certified diabetes educator (CDE) can help you create a meal plan that’s full of good-for-you and good-for-your-blood-glucose-level food. The goal of the meal plan is to control your blood glucose level and keep it in the healthy, normal range. Your meal plan will be made just for you, taking into account your overall health, physical activity, and what you like to eat. Exercise: When you exercise, your body uses more glucose, so exercising can lower your blood glucose level. Also when you exercise, your body doesn’t need as much insulin to transport the glucose; your body becomes less insulin resistant. Since your body isn’t using insulin well when you have prediabetes, a lower insulin resistance is a very good thing. And of course, there are all the traditional benefits of exercise: it can help you lose weight, keep your heart healthy, make you sleep better, and even improve your mood. The American Diabetes Association recommends at least 150 minutes of moderate activity a week—that’s 30 minutes five days a week. You can get that through activities such as walking, bike riding, or swimming. Lose weight: If you’re overweight, you should get started on a weight loss program as soon as you’re diagnosed with prediabetes. Losing just 5 to 10% of your weight can significantly reduce your risk of developing type 2 diabetes. The combination of eating well and exercising more is a great way to lose weight—and then maintain your new, healthy weight. Metformin: For people who are at a very high risk of developing type 2 diabetes after being diagnosed with prediabetes, the doctor may recommend a medication. The American Diabetes Association says that metformin should be the only medication used to prevent type 2. It works by keeping the liver from making more glucose when you don’t need it, thereby keeping your blood glucose level in a better range. Your doctor will keep a close watch on your blood glucose levels, monitoring them to make sure that your pre-diabetes doesn’t become type 2 diabetes. If needed, he or she may suggest adjustments (e.g., different diet or more exercise) to better control your blood glucose levels.

Approximately 1 in 3 Americans have prediabetes, but only about 1 in 10 knows it. What works better at preventing it from turning into full-blown diabetes—drugs or diet and exercise? But on an individual level, if you want to know what are the chances you won’t get diabetes if you change your lifestyle, then the 100% answer is more accurate. Lifestyle interventions only work when we do them. Kale is only healthy if it actually gets into our mouth. It’s not healthy just sitting on the shelf.
