Donald I. Abrams, MD. is chief of the Hematology-Oncology Division at San Francisco General Hospital and a Professor of Clinical Medicine at the University of California San Francisco. He has an Integrative Oncology consultation practice at the UCSF Osher Center for Integrative Medicine. He received an A.B. in Molecular Biology from Brown University in 1972 and graduated from the Stanford University School of Medicine in 1977. After completing an Internal Medicine residency at the Kaiser Foundation Hospital in San Francisco, he became a fellow in Hematology/Oncology at the Cancer Research Institute of the University of California, San Francisco in 1980. He was one the original clinician/investigators to recognize and define many early AIDS-related conditions. He has long been interested in clinical trials of complementary and alternative medicine interventions for HIV/AIDS and cancer, including evaluations of medicinal Cannabis. In 1997 he received funding from the National Institute on Drug Abuse to conduct clinical trials of the short-term safety of cannabinoids in HIV infection. Subsequently he was granted funds by the University of California Center for Medicinal Cannabis Research to continue studies of the effectiveness of cannabis in a number of clinical conditions. He completed a placebo-controlled study of smoked cannabis in patients with painful HIV-related peripheral neuropathy as well as a study evaluating vaporization as a smokeless delivery system for medicinal. He conducted an NIDA-funded trial investigating the possible pharmacokinetic interaction between vaporized cannabis and opioid analgesics in patients with chronic pain. He is now conducting an NIH-funded trial investigating vaporized cannabis in patients with Sickle Cell disease. He co-authored the chapter on “Cannabinoids and Cancer” in the Oxford University Press Integrative Oncology text that he co-edited with Andrew Weil. San Francisco Medical Marijuana Stops Spread of Breast Cancer - NBC NEWS Marianne Favro/KNTV Reporting EXCERPTED FROM PUBMED: Cannabidiol enhances the inhibitory effects of Delta9-tetrahydrocannabinol on human glioblastoma cell proliferation and survival: The cannabinoid 1 (CB(1)) and cannabinoid 2 (CB(2)) receptor agonist Delta(9)-tetrahydrocannabinol (THC) has been shown to be a broad-range inhibitor of cancer in culture and in vivo, and is currently being used in a clinical trial for the treatment of glioblastoma. It has been suggested that other plant-derived cannabinoids, which do not interact efficiently with CB(1) and CB(2) receptors, can modulate the actions of Delta(9)-THC. There are conflicting reports, however, as to what extent other cannabinoids can modulate Delta(9)-THC activity, and most importantly, it is not clear whether other cannabinoid compounds can either potentiate or inhibit the actions of Delta(9)-THC. We therefore tested cannabidiol, the second most abundant plant-derived cannabinoid, in combination with Delta(9)-THC. In the U251 and SF126 glioblastoma cell lines, Delta(9)-THC and cannabidiol acted synergistically to inhibit cell proliferation. The treatment of glioblastoma cells with both compounds led to significant modulations of the cell cycle and induction of reactive oxygen species and apoptosis as well as specific modulations of extracellular signal-regulated kinase and caspase activities. These specific changes were not observed with either compound individually, indicating that the signal transduction pathways affected by the combination treatment were unique. Our results suggest that the addition of cannabidiol to Delta(9)-THC may improve the overall effectiveness of Delta(9)-THC in the treatment of glioblastoma in cancer patients. PMID: 20053780

I hold the department's highest metal of honor for bravery—that used to mean a lot more to me than it does now. What I'd like to talk to you about today is—my now 7 year old daughter. This is an identical twin, her sister is now dead. Her sister, when she was 4 years old, Crystin—developed a highly malignant brain tumor that had spread throughout her spine and her brain. The doctors told us that we had really two options—take her home and let her die, or bring her in for massive dosages of chemo and radiation simultaneously. In either event she was going to die, they were quite certain of that—and very quickly. Believing her only chance to be the standard route, we gave her the chemo and radiation. It burnt her skull so bad she had second degree burns and her hair never came back. To change her diapers we had to wear rubber gloves because her urine was so toxic and it burned her. At the end of 6 months, miraculously she survived the standard treatment, although there was a high expectation that she wouldn't. She still had cancer. We were told "sorry, we've done everything we can, now she's going to die, probably within a couple of months." My wife and I choosing not to except that, started reading—the first book I picked up, the third chapter, discussed Dr. Burzynski. As you may guess, I have some expertise in fraud, in fact I'm quite certain there are enough attorneys in the room that I could be ordered as an expert in fraud—and, I conducted my own investigation. I have no doubt the man is not a fraud. I have no doubt that he does what he does out of earnest belief that his medicine works. Now, you are in a position to judge for yourselves whether is works or not—but it's well established by the FDA, that it's non-toxic. Eighteen months later, we took my daughter off the Antineoplaston—she had not died. She had no signs of tumor, she remained free for eighteen months of cancer. Within a month, her cancer was wide-spread in her brain. We put her back on Burzynski's—by the way at the objections of our doctors who for some reason felt that it had failed her. We put her back on—within nine weeks the tumor was completely gone. She died last July, of neurological necrosis—her brain fell apart from the radiation. The autopsy showed that she was completely cancer-free. Out of fifty-two cases of that disease ever, no one died cancer-free, just Cryssie. So she didn't die of a terminal illness—she died of my inability to care for her properly and she died from bad advice. She died because there is a government institution, that disseminates false information, and is not looking out for the welfare of the people. Mice treated for 20 consecutive days with THC and CBD had reduced primary tumor size." In 1998, a research team at Madrid's Complutense University Led by Dr. Manuel Guzman discovered that THC can selectively induce programmed cell death in brain tumor cells without negatively impacting surrounding healthy cells. They reported in the March 2002 issue of "Nature Medicine" they had destroyed incurable brain cancer tumors in rats by injecting them with THC. And in 2007 even Harvard Researchers found that compounds in cannabis cut the growth of lung cancer. There is also an organization called The SETH Group that showed compounds in cannabis can stop the growth of human glioblastoma multiforma (GBM) brain cancer cells. The SETH Group says "No chemotherapy can match this nontoxic anti-cancer action." It is Dr. Robert Melamede, Professor of Biology at the University of Colorado and explains how the body's Endo-Cannabinoid system (and plant based Cannabinoids as suppliment) kills cancer cells and inhibits tumor growth. Donald Abrams, M.D. is chief of Hematology and Oncology at San Francisco General Hospital and the co-author—with Andrew Weil—of Integrative Oncology (Oxford University Press). Abrams has extensive experience working with cancer and HIV/AIDS patients and is a pioneer in the field of medical cannabis research. The U.S. government classifies cannabis—along with heroin and LSD—as a Schedule I drug, the most tightly restricted category of drugs in the United States. According to the federal government, Schedule I drugs are unsafe and have "no currently accepted medical use in treatment in the United States." However, as medical cannabis proponents have pointed out since the Controlled Substances Act was passed by Congress in 1970, cannabis has been used medicinally for thousands of years, and there has never been a reported case of a marijuana overdose. Moreover, in recent years clinical researchers around the world have demonstrated the medicinal value of cannabis. If it were not for the gay people who had this disease. If it were not for the people who supposed these people, we would still have cannabis,” he explained. “But we would not have the respect that medical marijuana joined together, because San Francisco is a small area. We got a million people but we’re all stacked together.” Justmann was diagnosed with HIV in 1988 and says HIV medication made him feel worse. So he started using cannabis to help ease the symptoms. He then met Dennis Peron in 1992, who helped write the first medical cannabis legislation in the U.S after he saw what it did for AIDS patients. The two advocated together and helped form the early medical cannabis scene in California. “There were no dispensaries, before Dennis opened up [194 Church Street],” said Justmann. “I think that the dispensaries were your gateway area, you non-judgmental are. How the hell can I judge you, because you look sick, when the rest of us look sick. You follow that point, there was no judgement.” At 73, Justmann is still very much an activist and plans to work with the San Francisco mayor to make launching a cannabis business in San Francisco less cumbersome. Abrams, MD. is chief of the Hematology-Oncology Division at San Francisco General Hospital and a Professor of Clinical Medicine at the University of California San Francisco. He has an Integrative Oncology consultation practice at the UCSF Osher Center for Integrative Medicine. He received an A.B. in Molecular Biology from Brown University in 1972 and graduated from the Stanford University School of Medicine in 1977. After completing an Internal Medicine residency at the Kaiser Foundation Hospital in San Francisco, he became a fellow in Hematology/Oncology at the Cancer Research Institute of the University of California, San Francisco in 1980. He was one the original clinician/investigators to recognize and define many early AIDS-related conditions. He has long been interested in clinical trials of complementary and alternative medicine interventions for HIV/AIDS and cancer, including evaluations of medicinal marijuana. In 1997 he received funding from the National Institute on Drug Abuse to conduct clinical trials of the short-term safety of cannabinoids in HIV infection. Subsequently he was granted funds by the University of California Center for Medicinal Cannabis Research to continue studies of the effectiveness of cannabis in a number of clinical conditions. He completed a placebo-controlled study of smoked cannabis in patients with painful HIV-related peripheral neuropathy as well as a study evaluating vaporization as a smokeless delivery system for medicinal. His last NIDA-funded trial investigated the possible pharmacokinetic interaction between vaporized cannabis and opioid analgesics in patients with chronic pain. He is now conducting an NIH-funded trial investigating vaporized cannabis in patients with Sickle Cell disease. He co-authored the chapter on “Cannabinoids and Cancer” in the Oxford University Press Integrative Oncology text that he co-edited with Andrew Weil. Those are just a few of the studies done that show the effects of cannabis on cancer. Even after all of the research behind this it seemed that this treatment was being ignored. So over the next year and a half I began talking about this to pretty much anyone that would listen and found that like myself, most people had not heard anything about cannabis oil as a treatment for cancer. In November 2011 all of that talking finally reached the right person. I was contacted by a man who was looking for help. This man wishes to remain anonymous so I'll refer to him as Bob. Bob had stage 4 cancer in his stomach, throat and pancreas. Over the course of about 6-8 weeks from when Bob was diagnosed, he became so sick he could no longer eat and he lost over 50lbs. He was actually supposed to be dead before thanksgiving and when i first talked to him he was given about 2 weeks to live. Bob had been given radiation the month before he contacted me and he and his doctor were hopeful that would help. But instead of helping it seemed to make his cancer worse. So instead of trying to continue to help Bob, his doctor told him he was sorry but the cancer had spread too much and there was nothing left to do for him. Watch as the active ingredient in marijuana, THC, kills cancer cells. Since 1974 studies have shown that cannabis has anti-tumor effects. The results of the 1974 study, reported in an Aug. 18, 1974, Washington Post newspaper feature, were that cannabis's component, THC, "slowed the growth of lung cancers, breast cancers and a virus-induced leukemia in laboratory mice, and prolonged their lives by as much as 36 percent." They reported in the March 2002 issue of "Nature Medicine" they had destroyed incurable brain cancer tumors in rats by injecting them with THC. And in 2007 even Harvard Researchers found that compounds in cannabis cut the growth of lung cancer. There is also an organization called The SETH Group that showed compounds in cannabis can stop the growth of human glioblastoma multiforma (GBM) brain cancer cells. The SETH Group says "No chemotherapy can match this nontoxic anti-cancer action." One study that they did at Columbia, they had AIDS patients in a hospital, inpatient hospital, and they found out that if they used marijuana, that they got from a government source, they were able to eat more calories and gain more weight at the end of the study, compared to people who were using a placebo marijuana. And that's very important for people who have AIDS. They often suffer from wasting syndromes. Secondly, AIDS patients have to take varied medicines that upset the stomach quite a bit, and highly active antiretrovirals, or heart medicines. There was a study at Stanford, by Dr. Israelsky's group, that showed that if you used cannabis or marijuana, you had a much higher adherence rate to those antiretroviral drugs. Two to four times higher rate to keep taking those drugs on a routine basis. Which is very important if you are trying to suppress a virus which is really prone to become resistant and adapt. So you have to keep the regiment on. So that's very useful. Dr. Donald Abrams in U.C. San Francisco has shown that AIDS patients, HIV/AIDS patients developed this painful nerve, burning sensation in their legs, called HIV Sensory Neuropathy. It's like a burning sensation. And we don't have any good drugs to treat it. It turns out the drug that we have the best evidence for in medicine to treat it with is marijuana, hands down. And it's a growing problem as more and more people are living with longer lives. Because the drugs are becoming more available and HIV/AIDS turns into a chronic disease. So treatment of burning pain sensations, boosting your appetite, and helping gain weight, helping maintain your adherence to your antiretroviral drugs are all benefits that marijuana can afford to the HIV/AIDS patient. And Dr. Abrams, the cancer and AIDS specialist at San Francisco, says that it also helps with their mood. Living with a terminal in some cases or chronic illness can be something that causes quite a bit of adjustment and depression problems. And marijuana has been shown to be helpful with boosting moods. So that's the story behind marijuana and AIDS. Without AIDS activists medical marijuana wouldn't have really been a reality. Because they really pushed in California in the 1990's to help get laws passed that allowed patients with conditions like AIDS to use it for compassionate purposes. And the federal government actually stopped their program supplying marijuana to patients on a case by case basis when they were flooded with AIDS applications. So that's why part of why we have medical marijuana today.

What are the potential therapeutic benefits of Cannabis to ameliorate physical and psychological illnesses? Because of the constraints on conducting medical research on Cannabis and related products, much available information is empirical and has not been subjected to the rigors of the scientific method. What are the outstanding questions, and how might they be most usefully addressed through research and clinical practice? Donald I. Abrams, MD is the chief of the Hematology-Oncology Division at Zuckerberg San Francisco General Hospital and a Professor of Clinical Medicine at the University of California San Francisco. He is a graduate of Brown University and the Stanford University School of Medicine. He did his residency in internal medicine at Kaiser San Francisco and his fellowship at the University of California San Francisco. He was an early pioneer in HIV/AIDS medicine when he was challenged to conduct a clinical trial of cannabis for the AIDS wasting syndrome. He received his first NIH grant to study inhaled cannabis in 1997 and has also been funded by the University of California Center for Medicinal Cannabis Research to conduct subsequent clinical trials. He has studied the effects of cannabis in peripheral neuropathy, in association with sustained release opioids and in patients with sickle cell disease. He was a member of the committee that wrote the 2017 publication from the National Academies of Sciences, Engineering and Medicine on The Health Effect of Cannabis and Cannabinoids. ______ The UC Botanical Garden is a non-profit research garden and museum for the University of California at Berkeley, having a notably diverse plant collection including many rare and endangered plants. Transcription to follow: I think another very exciting field about cannabis medicines, is the potential effect that cannabiniods and maybe some of the other components of the marijuana plant may have against cancer cells. Interestingly, although this work was initially being done the National Cancer Institute in the 1970s. It disappeared in the United States and most of this work is done in Spain and Italy. And many of my colleagues in those countries are very enthusiastic about the anti cancer activities of cannabiniods and how they work in many different places in the cancer cell cycle. To lead to arrest of cell division, cell migration and metastasis and cell invasiveness. So I think that there is a lot to be studied in this field too that is very exciting.
