How To Take Synthroid Properly | Best Way To Take Levothyroxine Correctly | When To Take Thyroid Medication After Eating Hey guys! This weeks video is all about how to take your thyroid medication properly to get the most out of it. Levothyroxine is a medicine used to treat an underactive thyroid gland (hypothyroidism) which affects 1 in 70 women and 1 in 1,000 men in the UK. The thyroid gland makes thyroid hormone which helps to control energy levels and growth. Levothyroxine is taken to replace the missing thyroid hormone. KEY FACTS: • Levothyroxine starts working straight away, but it may be several weeks before your symptoms start to improve. • The most common side effects of levothyroxine are caused by taking a bigger dose than you need. Your doctor can lower your dose to help reduce any side effects. • Before you start taking levothyroxine, your healthcare professional will do a blood test to see what dose you need. Once you start taking the medicine you'll have regular blood tests to see how well it’s working. HOW AND WHEN TO TAKE: • Unless told otherwise by your prescriber or healthcare professional take levothyroxine once a day in the morning, ideally at least 30-60 minutes before having breakfast or a drink containing caffeine like tea or coffee. • Both breakfast and caffeinated drinks can stop your body taking in levothyroxine properly so it doesn't work as well. • If you can't take it first thing in the morning on an empty stomach take levothyroxine at least 2 hours after eating food. • If you buy any medicines, check with a pharmacist that they are safe to take with levothyroxine. Levothyroxine should not be taken at the same time as indigestion remedies or preparations containing calcium or iron (which are contained in some vitamin products). These types of medicines reduce the amount of levothyroxine absorbed by your body. Leave at least 2-4 hours between taking your dose of levothyroxine and any such preparation. • Soya interferes with thyroxine absorption, therefore if you are taking thyroxine you should try to avoid soya. If you wish to take soya, there should be as long a time interval as possible between eating the soya and taking the thyroxine. • Avoid products such as kelp, as they may interfere with thyroid function & wellbeing. Kelp is derived from seaweed and is naturally high in iodine. Because of this it is sometimes marketed as a "thyroid booster" and can be purchased in dry preparations and tablets. These will allow your doctor to adjust your dose to suit you. At the start of treatment you can expect to have blood tests often. Once your hormone levels are stable, you’ll usually have a blood test after 4 to 6 months, and after that once a year. You may need blood tests more often if you: • Are pregnant • Start or stop a medicine that can interfere with levothyroxine • Have any symptoms that could mean your dose is not quite right MISSED DOSES: If you forget to take a dose, take it as soon as you remember, unless it's almost time for your next dose. In which case just skip the forgotten dose. Do not take 2 doses together to make up for a missed dose. If you forget doses often, it may help to set an alarm to remind you. In this video, I go over the complete list of Levothyroxine side effects ranging from weight gain to hair loss and everything in between. Yes, Levothyroxine can cause hair loss and it can also cause weight GAIN. How can that be if levothyroxine is supposed to boost your metabolism? In this video I explain how the metabolism of thyroid hormone in your body from T4 to reverse T3 may help explain why many people actually gain weight on this particular medication. I also go over the recommendations I give to patients if this happens to them. I also go over how to determine which thyroid medication is right for YOUR Body and how to dose your levothyroxine. When patients have reactions to levothyroxine it's from 1 of 3 reasons typically: 1) they are reacting to a filler ingredient or a dye within the medication itself. 2) they are converting or metabolizing the thyroid hormone into by products that don't promote proper activity 3) they are getting too much thyroid hormone for THEIR body (this one is less common) The side effects when patients react to fillers or dyes include reactions like facial swelling, edema, hives, itching sensation, etc. The side effects from improper metabolism include reactions like weight gain, hair loss, slower than normal metabolism, cold hands/extremities, etc. The side effects from TOO much medication include hot flashes, tachycardia or rapid heart rate, palpitations, hair loss, intolerance to heat. Are you taking too much Synthroid? Synthroid is the most commonly prescribed medication in the US which means that millions of people are taking this medicine. Of those millions, there will always be a small percentage who are either taking too much or too little. We are going to focus on those people who are taking too much. Symptoms of taking too much Synthroid include... Common but less serious side effects: Increased hair loss Weight loss (sometimes but not always) Tremors in the extremities Jittery sensation Heart palpitations Anxiety Flushing of the skin Heat intolerance (not able to stand hot environments) Rare, but serious side effects of Synthroid overdose include: Osteoporosis or decreased bone density Heart enlargement Atrial fibrillation Chest pain Shortness of breath *If you experience any of these symptoms you should seek medical care as soon as possible. Does Levothyroxine Cause Weight Gain? What to do if it is slowing down your metabolism

Does Levothyroxine cause weight gain? That is the question I am going to answer in this video today. Here is a common problem: patients with hypothyroidism present to their doctor, get put on T4 medication like Levothyroxine or Synthroid and they CONTINUE to gain weight - despite no change in their diet or exercise routine. How can this be? In this video, I answer what I think is going on in many patients and it has to do with T4 to reverse T3 conversion when certain patients are given medications that contain T4 only. In the setting of inflammation, trauma, infections, liver/kidney issues, low-calorie diets, and nutrient deficiencies the body can take T4 and convert it into the thyroid blocking metabolite reverse T3. High levels of reverse T3 then compete for binding at the T3 cellular receptor site and put the brakes on thyroid and metabolic function. So how do you treat this condition? Most patients will need medication containing T3 like natural desiccated thyroid or liothyronine/Cytomel to "flush" the system. I also give several tips that I use in my office to help boost thyroid function if your doctor isn't willing to give you medication with T3 in it: 1. Optimize T4 to T3 conversion - I recommend the combination of zinc plus selenium. 2. Fix your adrenals - Consider using adrenal adaptogens or adrenal glandulars both of which can enhance cortisol and improve your stress response. 3. Add high-intensity exercise - To help lower insulin and leptin levels. 4. Reduce inflammation and find hidden sources of inflammation - the easiest first place to look is at your diet. 5. Lower leptin and insulin levels - It will be impossible to lose weight with leptin resistance and/or insulin resistance and both conditions reduce T4 to T3 conversion. Today is all about Synthroid. If you take this medication then you'll want to learn more about these 10 tips and tricks that you can use to help feel better. Synthroid is one of the most common thyroid medications used to treat hypothyroidism but it's not without problems. These tips are designed to help you find your optimal dose, help you understand if you are absorbing Synthroid, help you figure out if you need a new medication and more. 10 Tips to know when taking Synthroid: #1. Peripheral T4 to T3 conversion First off, you need to understand that Synthroid contains the inactive thyroid hormone T4. This means that in order for thyroid hormone to be utilized in your body it must first be activated through peripheral thyroid conversion. Several factors including genetics, inflammation, nutrient deficiencies and so on can impact this process. There are situations in which you may be taking enough Synthroid but your body is not able to activate it. #2. Absorption of Synthroid In order for your body to use Synthroid, it must also be absorbed in your intestinal tract! Several medications, supplements, and even foods can impact how much you absorb. Do your best to take Synthroid away from supplements such as Calcium and Iron which can limit absorption. #3. Are supplements helping or hurting you? Some supplements can decrease absorption but there are others that can help. The use of Selenium and Zinc can help your body activate Synthroid after it is absorbed. #4. The time of day that you take Synthroid Synthroid can be safely taken in the morning but it can also be taken at night. Studies have shown that patients who take it in the evening actually have higher free T4 and free T3! #5. Levothyroxine vs Synthroid: Are they the same? Levothyroxine and Synthroid both contain the same active ingredient but they are not considered bio-equivalents. Some patients do better on Synthroid over Levothyroxine so switching which one you take can impact how you feel. #6. Tirosint Tirosint is another T4 only medication but it is unique because it has the fewest inactive ingredients. Tirosint is more expensive than Synthroid but it can be used in cases where you may be reacting to fillers or dyes in Synthroid. #7. Dosage In order for Synthroid to work properly, you must be getting an adequate dose. You can ensure that you get a high enough dose by following all of the thyroid lab tests (more info below). #8. Switching to NDT or T3 medication Your thyroid naturally produces both T4 and T3 but Synthroid only has T4. In some cases, you may need to switch medications or add T3 in order to feel better. #9. The full thyroid lab panel While monitoring your dose you need to be looking at more than just the TSH. If you take Synthroid you should be looking at the following tests: - TSH - Free T3 - Total T3 - Free T4 - Reverse T3 These tests will give you an idea as to how your body is utilizing Synthroid. #10. Synthroid side effects Lastly, make sure that you watch our for the side effects of Synthroid which include: - Weight gain - Hair loss - Heart palpitations - Rashes - Headaches - Insomnia - Hot flashes - Changes in menstrual cycle Any of these side effects may indicate you need to switch medications! This video will discuss the side effects of stopping thyroid medication abruptly. Is it safe to stop taking your thyroid medication? The answer is no for several reasons which I will explain here. But first, we need to talk about why you are taking thyroid medication, to begin with. Thyroid medication is prescribed to those people who are deficient in thyroid hormone (meaning their body isn't producing enough). So if you were to stop taking your thyroid medication then your body would revert back to whatever it was capable of producing before you started taking thyroid medication. This process usually results in certain symptoms including: - Hair loss - Weight gain - Depression - Increased pain - Increased fatigue - And other symptoms of hypothyroidism But why do you experience these symptoms? These 4 reasons will help you understand why you will most likely feel worse if you do: #1. Your body may be reliant upon thyroid medication. If you've had your thyroid removed or if it's been destroyed by radioactive iodine then you may not be able to produce thyroid hormone on your own. In this case, you will be required to take thyroid hormone. #2. Your HPT axis may be blunted. Taking thyroid medication blunts the normal HPT axis in your body and it can take weeks to months for this system to recover. During this period you will most likely feel very poorly. #3. Your body may not be able to produce thyroid hormone by itself. Conditions such as Hashimoto's damage the thyroid gland and even though you may be able to produce some thyroid hormone you may not be able to produce enough to sustain yourself. #4. A drug manufacturer is recalling bottles of thyroid medication because the drug was made with active ingredients that were not sourced to proper FDA standards. According to the FDA, Westminster Pharmaceuticals issued the voluntary recall last week for all lots of Levothyroxine and Liothyronine. The company says it has not received any reports of any injury or illness related to the recall. The recall includes tablets of Levothyroxine and Liothyronine with the following dosage: 15 mg, 30 mg, 60 mg, 90 mg, and 120 mg. “It’s safe to take, the exact precise milligrams might not be perfect,” said Dr. Paul Sack, an endocrinologist at MedStar Union Memorial Hospital.

If you are taking supplements and if you take thyroid medication then this information is important for you. Certain medications and over the counter supplements can act to block or bind up thyroid medication like Levothyroxine or Synthroid. If you take supplements together with your thyroid medication then you may be limiting how much thyroid medication can actually enter into your body. It takes over 13 nutrients to create thyroid hormone, convert T4 to T3 and sensitize the cellular receptors to T3. Many thyroid supplements cram all of these nutrients into a supplement and call it "good". This is the WRONG approach to take. Instead basing your supplementation off of the needs of your body is far superior. That means using lab work and other markers to find what nutrients you need in what doses. 9 Thyroid supplements that I find most hypothyroid patients are deficient in: 1. Vitamin B12 - Thyroid hormone is involved in the proper absorption of thyroid hormone. See the video for a complete list of symptoms. Repleting low vitamin B12 is best done through sublingual or intramuscular​ injections. 2. Adrenal support - Most thyroid patients also have issues with cortisol dysregulation. If you have advanced adrenal fatigue most patients do better on adrenal glandulars. For more mild disease consider using adrenal adaptogens. If your cortisol is elevated consider using phosphatidylserine. 3. Zinc - Low levels of zinc can cause issues with your skin and issue with hair growth. Adding zinc boosts immune function, T4 to T3 conversion and acts as an anti-inflammatory. Make sure to supplement with zinc bound to picolinic acid. 4. Iron - This is what I consider a "Goldilocks" nutrient in that you don't want TOO much or TOO little. Unfortunately, most doctors look for iron deficiency anemia and don't necessarily jus treat iron deficiency unless anemia accompanies it. Use these optimal levels to supplement with iron. 5. Magnesium - Hypothyroidism alters magnesium and low levels of magnesium can lead to issues like depression, anxiety and other changes like muscle cramps or restless leg syndrome. Replace low levels with magnesium glycinate, magnesium citrate if you have constipation or magnsium threonate if you have neurological issues. 6. Selenium - This nutrient is involved in the thyroid conversion process. Not only that but supplementing has been shown to reduce TPO antibodies and boost the immune system. In addition, using iodine with selenium can help reduce negative side effects. 7. Probiotics - Up to 20% of thyroid conversion happens in the gut and inflammation can reduce this number. Hypothyroidism sets up the gut for imbalances that lead to inflammation. Using probiotics can help improve your GI function. 8. Digestive enzymes - Hypothyroidism leads to low stomach acid which can predispose patients to malabsorption of certain nutrients. Using enzymes can help digest your food, increase nutrient absorption and if taken in between meals may promote the breakdown of immune complexes. 9. Iodine - If used correctly iodine can be helpful in boosting thyroid function. The thyroid hormone is made up of iodine moieties and low levels of iodine can reduce the amount of functioning thyroid hormone in the body. (TBS #9) Is NDT (Natural Desiccated Thyroid) the Best Thyroid Medication?

This is lesson #9 in my thyroid beginner series and today it's all about NDT or natural desiccated thyroid! So what is NDT? Stands for Natural Desiccated Thyroid. This thyroid medication is porcine-derived which means it comes from the thyroid gland of pigs. It contains both the active T3 thyroid hormone and the inactive thyroid hormone T4. The ratio of T4 to T3 is around 76% T4 to 23% T3 which is roughly similar to the same amount that your thyroid gland produces naturally of 80:20. NDT is felt to be safer and more natural than other synthetic hormones such as Synthroid or Levothyroxine and other people believe it's the best thyroid medication. But is that actually true? There may be some truth to it, but your main goal should be to always find the best thyroid medication for YOU. Some of the problems with NDT include: - Many people take too much thyroid hormone which may cause issues - Dosing is static which means you can’t tweak the T4 and T3 concentrations. - May lead to variations in free T3 & Free T4 levels. - Symptoms arise from the use of T3 which can be difficult to manage in some patients. - May be hard to break down for some patients in the intestines due to fillers or dyes. - May lead to immunogenic reaction (may not be ideal for patients with Hashimoto’s) So with that in mind, who should use NDT? I've come up with a quick list below: - Those who have failed T4 medication. - Those with low free T3 levels (usually need a T3 medication). - Those who are post-thyroidectomy. Everybody knows that if you can’t lose weight and keep it off, you need thyroid meds, right? Wrong! Thyroid drugs are most certainly not the answer. Common thyroid drugs like levothyroxine, but known by many other names too have long been used with little success in the treatment of underactive thyroid, Hashimoto's disease, enlarged thyroid gland, thyroid cancer, and myxedema. They are also prescribed on the false belief that they help weight loss in hypothyroid people who can’t shed excess fat. But it doesn’t work. Only small amounts of fat, if any, can be lost. Once thyroid hormones are stopped, fat is regained. And this is true even when thyroid medication is continued for years. By the way, Synthroid is now the fourth most prescribed drug in the United States. There can also be all sorts of negative consequences in allowing your body to be treated with these synthetic chemicals that can seriously interfere with your quality of life. For one, they compete with natural hormones that your thyroid gland is still producing. You see, it’s not feasible for any human body filled with synthetic chemicals to be tricked into responding to them as though they were bio-equivalent, despite the claims of drug companies and the medical establishment, who tell you that drugs used to treat your body are an identical equivalent. The standard medical model for the treatment of thyroid disorders is badly flawed. There are huge misunderstandings about causes and effective treatments for thyroid deficiencies, which, by the way, may now affect as many as 90% of women in the Western world. A great place to begin getting an understanding of all of this is by reading the work of Dr. David Brownstein, author of Overcoming Thyroid Disorders and Iodine: Why You Need It, Why You Can’t Live Without It (third edition). Now, back to your thyroid. The thyroid has an enormous need for natural iodine. Today is lesson #17 and it's all about the best way to take thyroid medication. Should you take your thyroid medication in the morning? Should you take it in the evening? Should you split your dose? Should you take it on an empty stomach? All of these questions and more will be answered. First off... why does it matter when or how you take your thyroid medication? The first thing you should know is that in order for thyroid hormone to be activated it MUST be absorbed by your body. If it isn't absorbed then you won't feel better. When you take it or how you take it may alter how much is absorbed which may impact how you feel. What time of day that you take your medication matters: Newer studies have shown that taking your thyroid medication in the evening may actually help increase free thyroid hormone levels. This is felt to be because the GI tract is more active during the morning which means that you may not absorb as much of it at that time. In addition, many people drink coffee and caffeine in the morning which activates your GI tract. Should you take it with or without meals? Taking thyroid hormone with meals may result in decreased absorption of your medication as your body competes with food and energy for absorption. To avoid this you should take your thyroid medication on an empty stomach. You can pretty much avoid this issue entirely by taking your thyroid medication in the evening because most people don't eat right before bed! What about supplements? There are a range of supplements which also interfere with how well your body can absorb your medication. These supplements include: - Iron - Calcium - Sucralfate - Acid blockers - Caffeine - Iodine - Fiber If you take your thyroid medication with these supplements then the amount that you absorb may be reduced! And, lastly, should you split your dose throughout the day? You don't necessarily have to do this but you may find an improvement in your symptoms by doing so. Splitting your dose may look like this: Instead of taking 100mcg of T4 in the morning you may want to take 50mcg in the morning and 50 in the evening. Some people feel better by doing this. Hey guys! Tirosint vs Levothyroxine - The case for Switching Thyroid Medication

Tirosint is a T4 only thyroid medication but with some twists. Many patients report significant improvement in hypothyroid symptoms when switching from T4 only medications like Levothyroxine and Synthroid over to Tirosint. But how can that be? Especially when Tirosint is a T4 only medication? This occurs for several reasons: 1) Inactive ingredients - + Levothyroxine and Synthroid contain MANY inactive ingredients (complete list in the video) which your body may be reacting to and limiting absorption. These inactive ingredients may be causing negative side effects that mimic hypothyroidism or limit your ability to reach the optimal dose. Tirosint has 3 inactive ingredients and 1 active ingredient making it the "cleanest" thyroid medication (T4 only). 2) Decreased absorption - + Due to these inactive ingredients the absorption of Levothyroxine and Synthroid may be inferior to that of Tirosint. The fewer inactive ingredients means the more thyroid hormone that will be absorbed which means the better you symptoms will be. This is also very important for hypothyroid patients who have GI related conditions such as SIBO/SIFO, GERD and low stomach acid. Tirosint is more easily absorbed and utilized than other forms of thyroid hormone due to these reasons. In this video, I break down how various thyroid medications differ from one another and what thyroid hormones that they contain. There are two main types of thyroid hormones: T4 and T3. Thyroid medications contain both or just one or the other of these hormones which impact how they work in your body. Video #2 in a weight loss with hypothyroidism video series. This video represents an actual hypothyroid patient from my practice who lost 40 pounds over 6 months with before/after pictures. You will immediately notice her changes in the picture above and I walk you through in detail how YOU can get these types of changes as well. Hypothyroid patients almost always have difficulty with weight loss due to a variety of reasons. Most patients wrongly believe that their weight loss problem is due primarily to a lack of thyroid hormone and that's where they are wrong. Low thyroid hormone by itself causes a slow metabolism but it also causes and encourages other hormone imbalances. Specifically, it leads to insulin resistance, leptin resistance, and even estrogen/progesterone imbalances. It is these changes that make future weight loss impossible. So that means focusing on thyroid hormone alone generally won't result in significant weight loss. Even adding on NDT or T3 to your regimen will likely result in 5-10 pounds of weight loss at MOST. Compare that to focusing on the other hormone imbalances and you can see the advantage in this case study above. I show you exactly how to test for the following hormone imbalances: 1. Adrenal related issues (checking cortisol levels) 2. Thyroid resistance (by checking free T3 and reverse T3) 3. Low testosterone (by checking free and total testosterone) 4. Estrogen dominance (by checking estradiol + progesterone) 5. Checking for leptin resistance (by checking fasting serum leptin levels) 6. Checking for insulin resistance (by checking fasting insulin and Hgb A1c) I also go over the treatment for each of these conditions and show you how the patient above was able to get these results. This particular patient has Hashimoto's thyroiditis, but this will also work for patients who have hypothyroidism. How long does it take thyroid medication to work? That's the question we will be exploring in today's video. The short answer? About 60% of people will experience some improvement within 4-6 weeks of taking their medication. Another 20% will experience symptomatic relief even sooner and the last 20% may experience symptomatic improvement up to 8+ weeks after starting their medication. Why does it take so long? It has to do with how thyroid hormone works once it's in your body. Thyroid hormone has to interact with your cells and change genetic transcription and this process takes time. NHS Drs hardly dare go against the guidelines put in place by they RCS and the BTA despite over whelming scientific and historical evidence that they are wrong about thyroxine only being best treatment, and the TSH blood test is also not as gold standard as they would have you believe. This lady had to go to TEN specialists before getting any help, the type of which she will mostly have got ideas about from talking to others on forums. There are other treatments and other reasons for not getting well on thyroxine. Other viable treatments which the Drs either avoid or are ignorant about. As well as Thyroid UK there is Thyroid Patient Advocacy with great advice and Drs who advise them, or in the US google Stop the Thyroid Madness (also for excellent treatment advice ie what to look for in a Dr too) and Real Thyroid Help. Don't put up with them fobbing you off with antidepressants and weight loss advice this is serious stuff. Lack of T3 is implicated in fibromyalgia and thyroid disfunction in chronic fatigue syndrome. Many serious health risks are associated with thyroid disorders, but if NHS Drs step outside the ''guidelines'' they can find themselves up in front of the GMC, so they don't, they let you suffer. They could give you some triiodithyroidine or Armor natural or its generics, if you don't feel well on other treatments. Just some of the disgusting policies in modern medicine. Dr Chris doesn't tell you the alternative here either does he? Neither does he debunk it but shhh we may get into trouble.
