It is the third leading cause of cancer death for men in the U.S. More than 160,000 new cases of prostate cancer are expected to be diagnosed in 2017. "The reality is that prostate cancer is the most commonly diagnosed solid malignancy in men," says Dr. Eugene Kwon, a Mayo Clinic urologist. Dr. Kwon says, unfortunately, far too many patients fail their initial treatment for prostate cancer. "The fact is that, after you treat it, it doesn’t necessarily go away. It has a tendency to come back, and, for that very reason, a lot of men get into significant trouble." A big part of the challenge is locating the cancer when it returns to surgically remove it or zap it with radiation therapy. But technology is giving health care providers something even better than X-ray vision to see where it's hiding.

Dr Neal Shore speaks to Prof Karim Fizazi about the changing landscape of treatments for prostate cancer, discussing immunotherapies, PARP inhibitors and PSMA testing as future targets and treatments. Dr Fizazi highlights that anti-PD1 treatment (pembrolizumab) may provide benefit for metastatic castration resistant patients following antiandrogen treatment (5 of the 28 patients in the trial had a PSA decline of 50%). Trials building on immune checkpoint therapies with novel combinations to improve response rates have shown promise. Among these, the combination of pembrolizumab and enzalutamide is being explored as a doublet or a triplet with the addition of ADXS-PSA in KEYNOTE-046. Dr Shore raises PARP inhibitors as an avenue of research, with the recent TOPARP trial confirming the activity of olaparib, and Prof Fizazi considers how different genetic abnormalities will inform selection in upcoming trials and eventual care. PSMA testing, already providing new insights when used in combination with PET imaging, may also offer new inroads in developing treatments, and Prof Fizazi summarises current clinical interest in targeted or radiologic medicine.

Current treatment for prostate cancer is expensive, has numerous side-effects, and has a high mortality rate. It is astonishing how long this ailment has affected humanity without providing a reliable solution. Consequently, our team will examine how gold charged nanoparticles are used to aid the treatment of prostate cancer, specifically via drug delivery. These particles have a gold inner core and are coated with polyethylene glycol. Polyethylene glycol and gold are used due to their relatively inert state and non-toxicity. Gold is also used because, at a nanoscale, it is a catalyst for reactions. The surface chemistry of gold nanoparticles can be altered so that they can be coated with small polymers and other therapeutic agents such as cancer-treating drugs (specifically Docetaxel and Doxorubicin). This also gives the patient's DNA the ability to bond to the nanoparticle’s surface, which allows these nanoparticles to travel through the body without being rejected. These nanoparticles are spherical shaped, and while studies are beginning to show that non-spherical particles are more effective than spherical particles, researchers do not yet fully understand the shape effect non-spherical particles have on cancerous cells. The diameter of these nanoparticles is less than 170nm so that they can effectively fit through and diffuse inside cells and capillaries. Additionally, these nanoparticles are charged. Due to their charge, the nanoparticles repel each other, which prevents them from aggregating. Furthermore, it simply helps nanoparticles interact with cells and diffuse through concentration barriers more efficiently. This method of prostate cancer treatment is still being researched and has yet to be approved for medical use; however, it has the potential to replace current treatment methods, as it could be more effective and safer in practice. An enlarged prostate, or benign prostatic hyperplasia (BPH), is the most common noncancerous prostate problem, occurring in most men by the time they reach their 60s. Symptoms of BPH can include slow, interrupted, or weak urinary stream; urgency with leaking or dribbling; and frequent urination, especially at night. Prostatic artery embolization (PAE) is a cutting-edge procedure performed to help improve urinary symptoms caused by an enlarged prostate without the risk of sexual side effects. Originally developed in Europe and South America, this procedure is now being studied in the United States. Physicians at Mayo Clinic in Arizona have developed a new surgical procedure for the treatment of prostate cancer using natural orifices - signaling the next step in the evolution of minimally invasive surgery. The latest advances in surgical treatment of the disease involve using the body's own natural orifices as access points instead of making incisions through the skin. These types of procedures, Natural Orifice Transluminal Endoscopic Surgical or NOTES have advanced over the past decade and now a NOTES procedure has been perfected to remove the prostate called Natural Orifice Transluminal Endoscopic Surgical Radical Prostatectomy or NOTES RP. The SELECT trial looked at supplementing men with either selenium or vitamin E to decrease the risk of prostate cancer. The trial was stopped early when it appeared that those men that took extra vitamin E actually had MORE prostate cancer. After stopping the trial, the men were followed for 3 more years, and the men that developed prostate cancer were added to the numbers, and a growing number of men in the vitamin E arm developed prostate cancer. At that point the data appeared to confirm a link between taking extra vitamin E and developing high grade prostate cancer. Where it becomes interesting is that they monitored blood levels of omega 3 fatty acids as well. They were not given fish oil as a supplement. The men who had the more aggressive types of cancer tended to have higher blood levels of Omega 3 in their blood. The blood levels did not correlate with their intake. The huge jump was made that omega 3 fish oil caused prostate cancer. Asians and Pacific Islanders tend to as an ethnicity, to have the lowest rate of prostate cancer, and have the highest fish intake. How could dietary fish oil then, increase the risk of cancer. If, indeed, there is a link between prostate cancer and omega 3 blood levels, then perhaps, high omega 3 levels may be a marker for, not a cause of prostate cancer. Elevated PSA blood levels is a marker for prostate cancer. No one has ever supposed if you ingested PSA it would cause cancer. It is something that rises in the presence of cancer, and is used as a screening tool. Perhaps, one day we will do blood levels of omega 3s, and if there is a rise, not related to dietary intake changes, then we might look for prostate cancer???

Proton beam therapy is making treatment of complicated cancer tumors safer and more efficient. "What we’re finding is we can be more precise with our radiation," says Dr. Nadia Laack, radiation oncologist at Mayo Clinic. Traditional radiation uses X-rays, which go all the way through the body. Proton therapy delivers radiation to a tumor using beams of proton particles that can be stopped at a specified depth after delivering energy to a target. Dr. Laack says the precision of proton therapy and its ability to avoid harming healthy tissue is especially important in treating young patients and adults with tumors near vital structures. "For example, women with left-sided breast cancer," Dr. Laack explains. “With traditional radiation, these women had a higher risk for heart issues over their lifetime, because of exposure to the radiation. With protons, we can nearly-eliminate that dose to the heart and make left-sided breast cancer treatment safer. Recurrent prostate cancer is defined as cancer that has recurred following initial therapy, which could include surgery, radiation therapy, hormone therapy or chemotherapy. While much concern is directed at the prostate, most of it focuses on cancer, but many times the enlarged prostate gland has a far less sinister explanation. "We're talking benign prostatic hyperplasia. Essentially as men age, the prostate, which is the gland that sits underneath your bladder and surrounds the urethra, enlarges. Forty-percent of 40 year old men have enlarged prostate, 50% of 50 year old men have enlarged prostate and 60% of 60 year old men have enlarged prostate," says Dr. Meir Daller, a urologist on the medical staff of Lee Memorial Health System. At age 20 the prostate gland is about the size of a walnut. By the time a man is 40 it may have grown to the size of an apricot. By 60 it could be as big as a lemon. The growth has implications even when it's not cancerous. Not considered life threatening, benign prostatic hyperplasia or BPH can greatly impact quality of life. The enlarged prostate constricts the urethra, which affects the ability to urinate. Symptoms usually start after age 50. "Waking up at night several times, two or three times at night. You have difficulty falling asleep again, you have dribbling, you have a weaker stream, and you have a hesitancy," says Dr. Daller. The condition is not linked to cancer and doesn't increase the risk of cancer. At its worst, it can lead to a weak bladder, kidney infections and kidney failure. The condition is treatable by first managing activities, including limiting beverages at night, especially those with alcohol and caffeine. A doctor may also recommend drug therapies to relax or shrink the prostate and finally surgery if the symptoms become too severe. A new procedure called the plasma button dissolves excess tissue without the pains of traditional surgery. "In this new procedure, you're using a special gas that essentially vaporizes the insides of the prostate and the end result is the same. The advantage is the faster surgery and the fact that it's bloodless," says Dr. Daller. With an aging population, the enlarged prostate will most likely be a growing problem.
