Today we're talking about preeclampsia. So preeclampsia is the pregnancy induced hypertension. Okay, so pregnancy induced hypertension defined as a 30 point increase in systolic pressure and a, or a 15 point increase in diastolic pressure. This is from baseline vital signs of a pregnant patient. Now, that being said, a lot of time we won't have the base line vitals so that being said, what we're going to identify here is any reading over 140 over 90 is considered hypertensive in a patient. Keep that in mind, is that a lot of pregnancy patients, we see a drop in systolic pressure and lower than normal blood pressures during pregnancy, so you're seeing a 140 on 90, now that's not considered super hypertensive in normal states, but in someone that's pregnant, it's certainly is because we are going to see a natural drop in blood pressure during pregnancy. That increase to 140 over 90 is considered significant. Most commonly seen in the last 10 weeks or first 48 hours after labor. So yes, you can see it in the first 48 hours after labor. That's important, I've seen this a couple of times where it's kind of taken people by shock that it's actually happening after labor. It is the case. They're not exactly sure why it happens, there's a couple studies saying out there that a hypermnesic state or a metabolic state is the case of this. So, and also vasospasmic states. That's kind of something they know is happening is some sort of vasospasm is occurring. They're not really sure why. It's kind of the key here. Signs and symptoms, headaches, visual problems, epigastric pain, peripheral edema and the peripheral edema is and the headaches is simply because of that huge increase in pressures. So headaches are happening because they have a lot of pressure in their head, obviously, and then peripheral edema is just because of the, basically hypostatic pressures that are occurring here is that we have a lot of push back which is leaving fluid, leaving the vascular, leaving the vessels and going to the spaces, right? So that's why causes the edema. Treatment, keep the patient calm and keep them in dim lights because they can get quite a bit of visual problems and like spots in their eyes. Also, pain behind their eyes. Place in left lateral position, meaning the arm that their trimester members, so we want to keep them off their back or supine off their backs, that way the fetus isn't putting pressure on the inferior [venicava 00:02:40] that's returning blood back to the heart. Start an IV, very important, and also if you're an [inaudible 00:02:47], starting to use magnesium sulfate with the direction of your medical director. The reason being is that magnesium sulfate is known to be a smooth muscle relaxant, and if you look back at the cause, we're looking at abnormal vasospasm, so the theory behind using magnesium sulfate in this case is to try to reduce some of those vasospasm, that way we can hopefully reduce some of that hypertension that we're seeing with preeclampsia and limit the chances of going into a full eclampsia which is a seizure. Controlling Blood Pressure in Preeclampsia Overview Preeclampsia is a serious condition that can occur during pregnancy. The condition causes your blood pressure to become very high and can be life-threateningly. Preeclampsia can occur early in pregnancy or even postpartum, but often occurs after 20 weeks gestational age. An estimated 10 percent of women experience preeclampsia. Doctors don’t know exactly what causes preeclampsia. They think it’s possibly related to blood vessels in the placenta developing improperly. This can be due to family history, blood vessel damage, immune system disorders, or other unknown causes. Regardless of the cause, preeclampsia requires fast action to control blood pressure. What Are the Symptoms of Preeclampsia? If you have two blood pressure measurements equal to or greater than 140/90 mm Hg four hours apart and you do not have a history of chronic high blood pressure, you may have preeclampsia. This increase in blood pressure can take place suddenly and with no warning. Other symptoms associated with preeclampsia include: difficulty breathing nausea severe headache shortness of breath sudden weight gain swelling in the face and hands too much protein in the urine, which can indicate kidney problems vision changes, such as sensitivity to light, blurred vision, or temporary vision loss vomiting It’s important to seek immediate medical treatment if you experience any of these symptoms. Women may initially pass off their symptoms as those of a normal pregnancy. If you suspect you have preeclampsia, it’s better to be safe than to experience more serious complications. How Do Doctors Control Your Blood Pressure? Your doctor will consider how far along you are in your pregnancy and your baby’s development when deciding how to control your blood pressure. If you’re 37 weeks pregnant or further along, delivery of the baby and placenta are recommended to stop progression of the disease. If your baby hasn’t yet developed enough, your doctor may prescribe medicines designed to help your baby grow while also keeping your blood pressure low. Examples of steps you could take in an attempt to keep your blood pressure low and minimize side effects include: decreasing your salt intake drinking plenty of water throughout the day increasing the amount of protein in your diet, if your diet has previously lacked enough protein resting on the left side of your body to reduce pressure to major blood vessels Keep in mind that taking these steps may not effectively prevent your preeclampsia from worsening. Your doctor will likely recommend you come to their office regularly for checkups to test your baby’s health. What Are the Complications of Preeclampsia? The most serious complication of preeclampsia is death, both to the mother and baby. Doctors also know that women who experience preeclampsia during pregnancy are at greater risk for cardiovascular and kidney disease in the future. Women with preeclampsia can also have seizures (known as eclampsia) or they’re at risk for HELLP syndrome. This serious condition stands for hemolysis, elevated liver enzymes, and low platelet counts. This condition can cause blood clotting disorders, severe pain, and can be life-threatening. It’s important to call your doctor right away if you’re experiencing any of the symptoms of preeclampsia to help prevent any of these possible complications. What Is the Outlook for People with Preeclampsia? If you’re far enough along with your pregnancy to deliver your baby, your blood pressure will usually return to normal levels after giving birth. Sometimes this can take up to three months. In most instances, your doctor will do everything possible to help your baby develop enough to be safely delivered.

TREATMENT AND PREVENTION Treating and preventing varicose veins In general, doctors are conservative when treating varicose veins. You’ll probably be advised to make changes to your lifestyle, instead of trying more aggressive treatments. Lifestyle changes The following changes may help prevent varicose veins from forming or becoming worse: Avoid standing for extended periods of time. Lose weight or maintain a healthy weight. The exact recurrence risk depends on several factors that are related both to the mother's medical condition, when in pregnancy it was diagnosed, and the circumstances of her prior preeclampsia. The risk of recurrence increases if preeclampsia in the previous pregnancy was diagnosed early in pregnancy and if the woman has had preeclampsia in two previous pregnancies. Women with chronic hypertension, for example, have a 70% risk of recurring preeclampsia in the next pregnancy. And women with severe preeclampsia in their first pregnancy have a 45% risk of recurrence. Treatment for preeclampsia may include: Medications to lower blood pressure These medications, called antihypertensives, are used to lower your blood pressure if it's dangerously high. Blood pressure in the 140/90 millimeters of mercury range generally isn't treated. In the hospital, your doctor may perform regular nonstress tests or biophysical profiles to monitor your baby's well-being and measure the volume of amniotic fluid. A lack of amniotic fluid is a sign of poor blood supply to the baby. Delivery If you're diagnosed with preeclampsia near the end of your pregnancy, your doctor may recommend inducing labor right away. The readiness of your cervix whether it's beginning to open (dilate), thin (efface) and soften (ripen) also may be a factor in determining whether or when labor will be induced. In severe cases, it may not be possible to consider your baby's gestational age or the readiness of your cervix. What is HELLPS, and how is it related to preeclampsia?

Preeclampsia is a serious medical condition associated with pregnancy, and it can start to develop anytime after the 20th week, and it's associated with elevated blood pressure. A normal blood pressure for a person is 120 over 80, and if a pregnant woman has blood pressures that are 140 over 90 or greater, then a doctor is going to - that's going to get their attention and they'll want to look into it. And that involves drawing a woman's blood, and looking at a urine sample, and determining if there's protein in the urine. The combination of elevated blood pressure and protein in the urine is what diagnoses preeclampsia. And then it can be severe or not severe. And severe features would include changing labs and also symptoms such as a headache that the woman can't just kick. It's not just due to the fact that she's hungry, or tired, or hasn't gotten enough sleep, or is stressed out. It's a persistent headache that she just can't kick. So if a pregnant woman has elevated blood pressure and has a headache, or has had a sudden increase in swelling, or is having pain underneath her ribs (especially off to the right side) - these are all things that are associated with preeclampsia. And ultimately, the cause of preeclampsia is pregnancy, so the only cure is delivery. And if this starts to happen earlier on in the pregnancy, it's difficult, because of course you want to buy as much time during the pregnancy as you can, but sometimes it's so severe that delivery is necessary, because the process won't start to reverse itself until after the placenta is delivered, and of course, the placenta can't be delivered until after the baby is born. And so a doctor will determine that it's time for delivery when the risk to the woman and her baby is higher if she is to stay pregnant than it is for the baby to be born at an early gestational age. I referred to labs earlier that are done. And the blood work that's done helps a doctor to determine if a woman is going into HELLPS. It's a syndrome associated with preeclampsia, and it's an acronym, HELLPS. So the H stands for Hemolysis of red blood cells. EL stands for Elevated Liver enzymes. And then the last portion, LP, stands for Low Platelets. And so these are all changing labs that a doctor would see in a severe case of preeclampsia. And again, it would warrant delivery, but the doctor would determine the exact timing of delivery based on their knowledge of the woman's circumstances. Now you had a specific question about magnesium sulphate. And this is a medication used for a lot of different reasons, but as it relates to preeclampsia, it helps to reduce a woman's chances of having a seizure related to this condition, and so it's important to be on it if your doctor has said that you need it. HELLP Syndrome is a series of symptoms that make up a syndrome that can affect pregnant women. HELLP syndrome is thought to be a variant of preeclampsia, but it may be an entity all on its own. There are still many questions about the serious condition of HELLP syndrome. The cause is still unclear to many doctors and often HELLP syndrome is misdiagnosed. It is believed that HELLP syndrome affects about 0.2 to 0.6 percent of all pregnancies. The name HELLP denotes for: *H- hemolysis *EL- elevated liver enzymes LP- low platelets counts. It is often assumed that HELLP Syndrome will always occur in connection with preeclampsia, but there are times when the symptoms of HELLP will occur without a diagnosis of preeclampsia being made. About 4-12% of women with diagnosed preeclampsia will develop HELLP syndrome. Unfortunately since the symptoms of HELLP syndrome may be the first sign of preeclampsia, this is what can often lead to a misdiagnoses. The symptoms of HELLP may cause misdiagnoses of other conditions such as hepatitis, gallbladder disease, or idiopathic,thrombotic thrombocytopenic purpura , which is a bleeding disorder. The most common symptoms of HELLP syndrome include: Headaches Nausea and vomiting that continue to get worse–This may also feel like a serious case of the flu. Upper right abdominal pain or tenderness Fatigue or malaise A woman with HELLP may experience other symptoms that often can be attributed to other things such as normal pregnancy concerns or other pregnancy conditions. These symptoms may include: Visual disturbances High blood pressure Protein in urine Edema Severe headaches Bleeding Because the symptoms of HELP can mimic many other conditions or complications, it is encouraged that physicians run a series of blood tests, including liver function, on any woman experiencing symptoms during the third trimester of pregnancy. HELLP syndrome may occur before the third trimester but it is rare. It also may occur within 48 hours of delivery, although symptoms may take up to 7 days to be evident. Blood pressure measurements and urine tests to check for protein are often monitored when diagnosing HELLP syndrome. But the following tests and results are what help a physician to make an accurate diagnosis of HELLP Syndrome: Hemolysis -Red blood bells Abnormal peripheral smear Lacatate dehydrogenase more than 600 U per L Bilirubin more than 1.2 mg per dl Elevated liver Enzyme levels Serum aspartate amniotransferase more than 70 U per L Lacatate dehydrogenase more than 600 U per L Low Platelets Platelet count Treatment’s that may be used to manage HELLP until baby is delivered include: Bed rest and admission into a medical facility to be monitored closely Corticosteroid Magnesium Sulfate Blood transfusion if platelet count gets too low Blood pressure medication Fetal monitoring and tests including biophysical tests, sonograms, non stress tests and fetal movement evaluation If the pregnancy is more than 34 weeks gestation or the symptoms of HELLP begin to worsen, delivery is the recommended course of treatment. In the past, Cesarean delivery was the most common way for delivery of babies whose moms were dealing with HELLP syndrome . HELLP syndrome does not cause reason for an automatic cesarean, and in some situations, operative surgery may cause more complications due to the possibility of blood clotting problems related to low platelet counts.

They were also giving Her liquids(Potassium Chloride 1000mL) & a little Magnesium Sulfate 2 grams in the other P.I.C.C. Line. They discharged Her on Thursday but we came back to the hospital on Tuesday because Dr. Ferreira said Her blood is only circulating ~40% of the Oxygen that She breaths in. That makes Her struggle to breathe and feel so uncomfortable to where She cannot relax enough to sleep. She took Oxycodone and finally She could sleep. Last night She took Ativan by IV to relax Her breathing enough to sleep, but maybe so much relaxing medicine has made Her blood pressure drop to 88 over 40? Update after Dr. Visit Blood count is 6.6 (8-31-16@9:40am) & normal should be 12 or 13 (when She was admitted 8-29-16 Her blood count was 9. Her platelet count is 60 & normal platelet count would be 130. They would do something if Her platelet count was to drop below 40. Vancomycin is an IV antibiotic that they are giving Her to fight the UTI they found on Tuesday when she was reAdmitted to the hospital. My Wife is A positive and had a blood transfusion 8-31-16@1pm.
