If a family member or friend experiences these changes, suggest that they visit their doctor. Breathing changes Shortness of breath or becoming easily winded can also be possible symptoms of lung cancer. Changes in breathing can occur if lung cancer blocks or narrows an airway, or if fluid from a lung tumor builds up in the chest. Make a point of noticing when you feel winded or short of breath. If you find it's difficult to breath after climbing stairs or performing tasks you could do before without having a hard time breathing, don't ignore it. Pain in the chest area Lung cancer may produce pain in the chest, shoulders, or back. An aching feeling may not be associated with coughing. Tell your doctor if you notice any type of chest pain, whether it is sharp, dull, constant, or comes and goes. You should also note whether it is confined to a specific area or is occurring throughout your chest. When lung cancer causes chest pain, the discomfort may result from enlarged lymph nodes or metastasis to the chest wall, the lining around the lungs called pleura, or the ribs. Wheezing When airways become constricted, blocked, or inflamed, the lungs produce a wheezing or whistling sound when you breathe. Wheezing can be associated with multiple causes, some of which are benign and easily treatable. However, wheezing is also a symptom of lung cancer, which is why it merits your doctor's attention. Don't assume that wheezing is caused by asthma or allergies. Have your doctor confirm the cause. Raspy, hoarse voice If you hear a significant change in your voice, or if someone else points out that your voice sounds deeper, hoarse, or raspier, get checked out by your doctor. Hoarseness can be caused by a simple cold, but this symptom may point to something more serious when it persists for more than two weeks. Hoarseness related to lung cancer can occur when the tumor affects the nerve that controls the larynx, or voice box. Drop in weight An unexplained weight loss of 10 pounds or more may be associated with lung cancer or another type of cancer. When cancer is present, this drop in weight may result from cancer cells using energy. It could also result from shifts in the way the body uses energy from food. Don't write off a change in your weight if you haven't been trying to shed pounds. It may be a clue to a change in your health. Cigarette smoking is the main preventable cause of lung cancer, accounting for more than 80% of cases; other causes include, exposure to radon, second-hand smoke and other air pollutants, occupational exposures to smoke and asbestos, family history and prior radiation to the chest. What does screening mean? When doctor’s screen for lung cancer, it means that they are looking for lung cancer in people who don’t show any signs or symptoms of having the disease. The idea behind screening is that if doctors can detect lung cancer when the disease is at an early stage, they may be able to treat the cancer earlier and thereby lengthen the person’s life. Screening involves casting a wide net, with an imperfect test, to identify people who may have cancer. People with abnormal results will have to undergo extra, more invasive tests to determine if they actually have cancer. The test may also identify those with abnormal results who do not have cancer. The Task Force recommends screening for lung cancer only in people aged 55-74 years who have a history of heavy smoking and who are either current smokers or who quit within the last 15 years. Screening does not apply to people with a history of lung cancer or who are suspected of having lung cancer. That is because screening is done to detect people who have no indication that they may have cancer. I’ll just give you three examples. In 1930s there was a colossal increase in lung cancer death rates among men that was recorded from the routine statistics in the United Kingdom and in Canada, and as well as in the U.S. That led to an investigation as to why there was this colossal increase in routinely registered lung cancer deaths. That led to the identification of smoking as one of the major hazards, and eventually led to a concerted effort to reduce smoking. By some measures, that saved something like nine million lives in the U.S. alone. The second example is a more immediate one. In 1980s, there was an outbreak of strange deaths among young men in San Francisco. This was picked up through the routine death registration system that had information on causes of death. That led to the identification of HIV/AIDS, first in African, or first in American men, but then also in Africa, and heralded the onset of the global epidemic. It identified an infectious cause by studying routine deaths that, again, were registered. The third example is a negative one, but worth bearing in mind. In the swine flu scare that occurred just a few years ago, Mexico did not have appropriate routine statistics to be able to judge that deaths that were occurring were – in fact, in people infected – not because of that infection. This led to a worldwide panic, basically, about a big pandemic. Had the data been of better quality, the world would have actually avoided a lot of these concerns. So, absolutely, routine death statistics have identified the important conditions. They’ve identified new conditions like AIDS, or in other cases, they have been useful in tracking successes like we’ve had for tuberculosis control or for reductions in childhood mortality. Jonathan Davies backs new campaign to improve lung cancer survival rates in Wales Lung cancer is one of the four most common cancers in Wales and the commonest cancer worldwide. Each year in Wales, lung cancer is responsible for more deaths than bowel and breast cancers combined. Survival from lung cancer is poor compared to most other common cancers. Around half of all people with lung cancer in Wales die from the disease within six months of diagnosis, almost three-quarters within a year. Almost a third of women in Wales survive at least one year, only a quarter of men do so – a mere 6.5 per cent of people survive for five years or more. Survival in Wales is almost the lowest in Europe – 28th out of 29 countries in the Eurocare study. If the best lung cancer survival in the Eurocare study applied in Wales during 2012, an approximate estimate of well over 340 more people might have survived at least a year, and over 190 more people might survive at least five years. One-year lung cancer survival in Wales is consistently the lowest of all UK countries for men and women – the slight increases since 1999 are the smallest in the UK, although there was a decrease for men in Northern Ireland. Survival dramatically decreases with increasing age. One-year survival in the 45-54 year-old group is almost twice that in the 75+ age group in Wales, although most cases occur between ages 65 to 79. Lung cancer stage at diagnosis is strongly related to one-year survival. Lung cancer can be cured or survival improved if it is diagnosed early, although many other factors play a part in addition to early diagnosis. Overall, survival varies little by area deprivation, unlike the wide inequalities in incidence, but the survival by stage at diagnosis does vary by area deprivation. For potentially treatable stage 1 lung cancer one-year survival is 91 per cent in the least deprived areas of Wales but is only 74 per cent in the most deprived areas. There are other inequalities. There is a seven percentage point difference in survival between health board residents with the highest (Betsi Cadwaladr) and lowest (Aneurin Bevan) one-year lung cancer survival for 2010-2012. And residents of Cwm Taf have the highest stage 1 one-year survival (86.2 per cent) - over eight percentage points higher than stage 1 survival for Wales, and almost 15 percentage points higher than the lowest health board population (Cardiff and Vale). In conclusion, poor lung cancer survival in Wales appears to be due to a number of factors that need to be better understood, mapped and quantified. There are inequalities and variations within Wales and compared with the rest of the UK and many other European countries. Earlier diagnosis across the population is needed and may be possible, but this is limited by the lack or type of lung cancer symptoms. Once someone presents to the NHS in Wales with a suspicion of lung cancer there may be key issues that lie along the patient’s pathway, including: - a low surgical resection rate of early stage patients - too many patients presenting as emergencies - variations in lung cancer staging practices - GPs and patients delaying referral - GP access to specialist advice - waiting times for x-rays Our ageing population and higher prevalence of other illnesses may also play a part. We will be examining some of these issues later in our series of reports on lung cancer in Wales. Finally, lung cancer can be prevented. Smoking accounts for around 36 cases per week in Wales, mainly in smokers but also in non-smokers. Around 9 cases per week can be linked to other factors such as radon, asbestos, particulate air pollution and occupational exposure, and silicosis, again in smokers and in non-smokers. The risk from radon and asbestos is greatly multiplied by the addition of smoking. Effective population-level population-level measures exist to control tobacco and most of the other risk factors.

The War on Cancer refers to the effort to find a cure for cancer by increased research to improve the understanding of cancer biology and the development of more effective cancer treatments, such as targeted drug therapies. The aim of such efforts is to eradicate cancer as a major cause of death. The signing of the National Cancer Act of 1971 by then U.S. President Richard Nixon is generally viewed as the beginning of the war on cancer, though it was not described as a "war" in the legislation itself. Despite significant progress in the treatment of certain forms of cancer (such as childhood leukemia), cancer in general remains a major cause of death 40+ years after this war on cancer began, leading to a perceived lack of progress and to new legislation aimed at augmenting the original National Cancer Act of 1971. New research directions, in part based on the results of the Human Genome Project, hold promise for a better understanding of the genetic factors underlying cancer, and the development of new diagnostics, therapies, preventive measures, and early detection ability. However, targeting cancer proteins can be difficult, as a protein can be undruggable. Though there has been significant progress in the understanding of cancer biology, risk factors, treatments, and prognosis of some types of cancer (such as childhood leukemia) since the inception of the National Cancer Act of 1971, progress in reducing the overall cancer mortality rate has been disappointing. Many types of cancer remain incurable (such as pancreatic cancer) and the overall death rate from cancer has not decreased appreciably since the 1970s. The death rate for cancer in the U.S., adjusted for population size and age, dropped only 5 percent from 1950 to 2005. Cancer is expected to surpass cardiovascular disease as the leading cause of death in the world by 2010, according to the World Health Organization's World Cancer Report 2008. There is evidence for progress in reducing cancer mortality. Age-specific analysis of cancer mortality rates has had progress in reducing cancer mortality in the United States since 1955. An August 2009 study found that age-specific cancer mortality rates have been steadily declining since the early 1950s for individuals born since 1925, with the youngest age groups experiencing the steepest decline in mortality rate at 25.9 percent per decade, and the oldest age groups experiencing a 6.8 percent per decade decline. Dr. Eric Kort, the lead author of this study, claims that public reports often focus on cancer incidence rates and underappreciate the progress that has been achieved in reduced cancer mortality rates. The effectiveness and expansion of available therapies has seen significant improvements since the 1970s. For example, lumpectomy replaced more invasive mastectomy surgery for the treatment of breast cancer.[42] Treatment of childhood leukemia[2] and chronic myeloid leukemia (CML) have undergone major advances since the war on cancer began. The drug Gleevec now cures most CML patients, compared to previous therapy with interferon, which extended life for approximately 1 year in only 20-30 percent of patients. Dr. Steven Rosenberg, chief of surgery at the NCI has said that as of the year 2000, 50% of all diagnosed cases of cancer are curable through a combination of surgery, radiation, and chemotherapy. Cancer surveillance experts have reported a 15.8 percent decrease in the age-standardized death rate from all cancers combined between 1991 and 2006 along with an approximately 1 percent annual decrease in the rate of new diagnoses between 1999 and 2006. A large portion of this decreased mortality for men was attributable to smoking cessation efforts in the U.S. A 2010 report from the American Cancer Society found that death rates for all cancers combined decreased 1.3% per year from 2001 to 2006 in males and 0.5% per year from 1998 to 2006 in females, largely due to decreases in the 3 major cancer sites in men (lung, prostate, and colorectum) and 2 major cancer sites in women (breast and colorectum). Cancer death rates between 1990 and 2006 for all races combined decreased by 21.0% among men and by 12.3% among women. This reduction in the overall cancer death rates translates to the avoidance of approximately 767,000 deaths from cancer over the 16-year period.
