Low Blood Pressure caused by Adrenal Fatigue

Blood pressure is an automatically regulated mechanism in our body. It is designed to provide oxygen through blood, through our brain, as well as various parts of our body. https://www.drlam.com/blog/signs-symptoms-low-blood-pressure-causes-correlation-adrenal-fatigue/19097/ This is Dr. Lam, founder of DrLam.com and today we are going to talk about low blood pressure consistently. The regulation of blood pressure falls within many factors. Blood volume can effect the blood pressure, the endothelial wall of the blood vessel itself constricting and dilating through what we call baroreceptors or blood pressure sensitive receptors can help modulate the blood pressure, and of course electrolytes and the sodium balance can also play an important factor. The organs responsible including the kidneys, the adrenals, as well as the brain, can also make a big factor in regulating the blood pressure on a 24/7 basis without us knowing. Of course we can not discount the role that the heart plays as well. So maintaining normal blood pressure is a very important part of survival. Just going from a sitting to a standing position requires that it activates all the systems in various degrees so that we can stand up, not feel dizzy, and are able to walk and not feel dizzy and then have normal activity. Any time the blood pressure is not regular the brain is not getting enough profusion or blood flow, and then the body will suffer. In the case of adrenal fatigue, what happens is, because the adrenal glands regulate the sodium and potassium balance, there is a tendancy, as the adrenals get weaker, that the body is not able to regulate this as much as it should. As a result, the sodium is usually lost through the deficieny or the subclinical effectiveness of aldosterone: the hormone that is regulating the blood flow and the water flow. Now this hormone is critical because in adrenal fatigue this hormone aldosterone tends to be low. As a result the sodium is not going to be easily maintained in the body. At this time the labratory values may be the same, or when I say the same I mean normal. People come in a lot of them with low blood pressure because they are losing sodium, and without sodium your water does not flow. So in other words the water stays with the sodium, wherever the sodium goes the water goes. So as long as the sodium is high the water will be retained in the body. When the sodium is low then the body will tend to lose water, and if you lose water, then your blood pressure will go down; it will stay down consistently. That is why many people with low blood pressure tend to do better when they eat more salt they can be perked up and feel much better. But of course, you have to be careful because too much salt can lead to another set of problems including hypertension, as well as swelling in the legs, what we call edema. So first we have to understand that low blood pressure is a symptom and not a disease. Number two you have to look at the big picture of how everything else comes together. In an adrenal fatigue situation, oftentimes low blood pressure does not stand alone; it comes along with other clinical signs and symptoms that you have to put the pieces together. It is one of many symptoms that presents itself. Usually the more advanced the adrenal fatigue the more prominent this is. A very interesting thing that you also have to understand is that in the early stages of adrenal fatigue, blood pressure tends to rise because of the vessel constriction of blood vessels narrowing as a result of hormones; and then in later stages it tends to go down. So if you look at the blood pressure, it can be high and still have adrenal fatigue in early stages, if it stays low consistently then it could mean the body is in a state of advanced adrenal fatigue if all other medical workup is normal. So, low blood pressure constistently is an important symptom to look for in adrenal fatigue and it points us to the underlying dysfunction that is maybe happening in the adrenal glands. Most people worry about high blood pressure, and with good reason as it portends numerous health risks, such as the possibility of an increased risk of heart attacks and strokes. But… Low blood pressure brings a different set of problems, such as reduced brain function and increased mortality risk. If the upper or lower number deviates by more than 10 from 120/80, it pays to be aware low blood pressure may be affecting your health. Think of it like this. The function of your blood pressure is to push the blood out of your blood vessels by permeating out of them to the rest of the body. High blood pressure is often a compensatory response to something that is going on in the body. If you cover up the end response of your body you can get other symptoms. Such as, a very common side effect of blood pressure medications is brain fog or mental “unclarity”. Which really makes perfect sense. Your brain is now not getting the blood (or oxygen) it needs to function, which equals……uhhhhhhhh? What? As a side note I often find that in the elderly, if they have not taken care of themselves and their arteries are already starting to harden, this will lead to decreased permeability of the blood vessels. As a result, blood pressure goes up to compensate and get blood to the brain. If you lower blood pressure too much, you no think too much! ;-) Blood pressure pushes blood through about 100,000 miles of veins, arteries, and capillaries in the body, carrying oxygen, nutrients, immune cells, hormones, neurotransmitters, and other vital compounds. High blood pressure strains blood vessels. However, low blood pressure means not enough blood is getting to capillaries and tissues, particularly in your hands, feet, and brain. This deprives those tissues of sufficient oxygen and nutrients. You may have chronic nail fungal infections and cold hands and feet if so. These can also be signs of an immune system that is deficient as well. So it pays to check both. Adrenal fatigue as a cause of low blood pressure The most common cause of low blood pressure (other than taking blood pressure medications) in a functional medicine model is poor adrenal function. The adrenals are two walnut-sized glands that sit atop the kidneys. They produce stress hormones and help regulate blood pressure. Many people today suffer from adrenal fatigue due to chronic stress. Other causes of adrenal fatigue are poor diets, low blood sugar, chronic infections, gut problems, inflammation, and unmanaged autoimmunity — all stressors. Aldosterone helps control blood pressure by holding onto salt and losing potassium from the blood. The increased salt increases the blood pressure. Hyperaldosteronism is a disease in which the adrenal gland(s) make too much aldosterone which leads to hypertension (high blood pressure) and low blood potassium levels. Primary hyperaldosteronism can be caused by either hyperactivity in one adrenal gland (unilateral disease) or both (bilateral disease). Unilateral disease is usually caused by an aldosterone producing adenoma (benign tumor) and less commonly by adrenal cancer or hyperplasia (when the whole gland is hyperactive). Tips for low cortisol, most people are treating adrenals as if they have high cortisol when the problem may be low cortisol. Low cortisol happens when the stressor sucks up all the raw materials to make cortisol. All of the pregnenolone and progesterone have been stolen and there is nothing left to make cortisol. What happens here is strange. These are a few things you may see with low cortisol: 1. Weird / tremor anxiety 2. Feelings of "tension" 3. Feel like blood sugar is always low. Cortisol helps blood sugar get into the cell. Feeling of hunger and low blood sugar even after meeting. The best test is 4x Cortisol plus DHEA. Can find through a practitioner or go to http://canaryclub.org and order the test. Most people talk about high cortisol and the herbs to treat that. But if you have low cortisol you need to do different things, not lower it even more. My favorite things for low cortisol: 1. Licorice root extract. Use as a tea or tincture. Do in morning shot. Can do before bed if waking during the night. Take during the wake ups too. Keep next to the bed. 2. Vitamin C. Adrenal cortext is where vitamin C is stored. Although 80% of cases occur when the body tries to defend itself against its own tissues and organs, about 20% of cases occur due to infection and eventual breakdown of the adrenal glands which can be caused by tuberculosis. 2) Teeth appear whiter Addison’s disease can make your teeth appear whiter. What’s really happening, however, is the gradual darkening of the skin, which can make your teeth appear to be whitening even though nothing is actually happening to them. This darkening of the skin is one of the most common symptoms of Addison’s disease, so apparent whitening of the teeth occurs in the majority of cases. 3) ‘Brain fog’ is a common symptom ‘Brain fog’ is a common symptom of Addison’s disease. ‘Brain fog’ is an unexplained phenomenon where a patient feels like he or she is mentally in a ‘fog.’ Those who have experienced it have described ‘brain fog’ as confusing or disorienting. Interestingly, there is no evidence that it exists, but some patients hypothesize that it’s caused by low levels of KORT-I-SAL cortisol, the hormone that Addison’s disease patients are deficient of. 4) Treatments exist Treatments exist. Hydrocortisone tablets can be taken twice daily to make up for a lack of cortisone produced by the adrenal glands. When taken, the disease becomes manageable, and most patients are able to lead a normal life. Other treatment options include antibiotics if the cause of the disease was tuberculosis and a medication called fludrocortisone acetate if the aldosterone hormone is also deficient. Despite the availability of treatments, Addison’s disease cannot be cured, and must be treated for life. 5) Craving salt is a common symptom Craving salt is a common symptom of Addison’s disease. Other symptoms include fatigue, abnormal coloring of the skin, weight loss, low blood pressure, nausea, and painful joints and muscles. 6) Named after Thomas Addison Addison’s disease is named after Thomas Addison, a doctor from England who first discovered the condition. He wrote about it prior to it’s first being identified in 1855. 7) Without medication, it is life-threatening Without proper medication, Addison’s disease is a life-threatening condition. If hormone-replacement medication is not taken, the hormone levels in body become dangerously unbalanced, and it eventually leads to death. 8) Adrenal crisis can occur An adrenal crisis can occur. We learned earlier that when someone has Addison’s disease, it means that their adrenal glands produce an insufficient amount of cortisol. When cortisol levels get too low, an adrenal crisis can occur. Symptoms of an adrenal crisis include vomiting, muscle weakness, dizziness, fatigue, and low blood pressure. If an adrenal crisis occurs, the victim should be injected with hydrocortisone and seek medical care immediately. 9) Vomit is a red flag Vomit is a red flag. If someone with Addison’s disease throws up due to an adrenal crisis, they need to be injected immediately, and hospital treatment may be required. If a patient experiences unrelated but prolonged vomiting due to any number of other conditions, they similarly will need to be injected and treated at a hospital. 10) Four diagnosis options There are four diagnosis options. Usually, the process of diagnosis for Addison’s disease involves blood tests. The point of the blood tests are to see if the symptoms you’re experiencing could be caused by adrenal insufficiency. Other diagnosis options include a CT scan, to examine your adrenal glands; an ACTH stimulation test, to look for differences before and after stimulating your adrenal glands; and insulin tests, to see if your condition is the result of pituitary disease rather than Addison’s disease.

Key Signs and Symptoms of Low Blood Pressure People frequently talk about high blood pressure, as it can lead to heart attacks, strokes and kidney failure. But low blood pressure, also known as hypotension, can also cause damage to your body. While a blood pressure reading of 140/90 mm Hg or above is considered high, a reading of less than 90/60 mm Hg is considered low. Everyone’s blood pressure drops at one time or another and doesn’t cause any noticeable symptoms. For instance, the transition from sitting or lying down to standing can cause a drop in blood pressure. Some people may experience low blood pressure right after eating or from prolonged standing. However, certain conditions can cause prolonged periods of hypotension that can be bad for your health. Such conditions include pregnancy, significant blood loss due to an injury, poor circulation caused by a heart condition, weakness that accompanies dehydration, dieting to lose weight, anaphylactic shock, infections of the bloodstream and endocrine disorders, such as diabetes, adrenal insufficiency and thyroid disease. Certain medications like beta-blockers, diuretics, tricyclic antidepressants and erectile dysfunction drugs can also cause hypotension. A deficiency of the essential vitamins B12 and folic acid can cause anemia, which in turn can lead to low blood pressure. Having persistent low blood pressure is not good. If the blood pressure gets severely low, your body will not receive enough oxygen to carry out its normal functions. A decreased oxygen level can lead to impaired functioning of the heart and brain as well as cause difficulty breathing. In certain cases, people with low blood pressure can lose consciousness or go into shock (when the organs shut down). This makes it more important to be aware of the signs and symptoms of low blood pressure, so that timely treatment can be provided. Here are the key signs and symptoms of low blood pressure. 1. Dizziness or Light-Headedness. One of the key symptoms of low blood pressure is dizziness and light-headedness. Low blood pressure causes poor blood flow to the brain. This reduces the oxygen supply to the brain and makes you feel dizzy and light-headed. Fatigue. Low blood pressure can affect your energy level and cause fatigue. The flow of blood plays a key role in the production of energy, as it provides the body with oxygen and necessary nutrients. The mitochondria within the cells need nutrients for energy production. When there is a disruption in the blood flow due to a drop in blood pressure, it disrupts the energy creation process, which in turn causes fatigue and tiredness. Even sleep disruptions caused by this change in blood volume can add to your fatigue. Having super low energy is never a good sign, so see your doctor. 3. Rapid Heart Rate. If you experience a rapid heart rate or palpitation, it may be due to a drop in your blood pressure level. Poor blood flow to the heart causes it to contract irregularly. If the heart is not getting filled with the right amount of blood, it compensates by beating faster. It can also cause your pulse and breathing to speed up. But a 2015 study by Chloe Eldridge found that “Current evidence shows hypoglycaemia is considerably prevalent amongst people with type 2 diabetes, particularly for those on insulin, yet still fairly common for other treatment regimens. This highlights the subsequent need for educational interventions and individualisation of therapies to reduce the risk of hypoglycaemia.” First let’s define what we mean by hypoglycemia. Hypoglycemia happens when the amount of blood glucose (sugar in the blood) drops to a level that's too low to sustain normal functioning; in most people, this is defined as a blood-sugar level below 70 mg/dl. Left untreated, hypoglycemia can lead to a coma and even death. It is very important that those with diabetes monitor sugar levels and treat hypoglycemia when it is present and right away. It happens more often than we think. So what does it feel like to have hypoglycemia? Let’s look at the most common symptoms. There are two classes of hypoglycemic symptoms - those due to lack of sugar in the brain and those due to the adrenalin response of your body. When blood sugar levels get too low, the body will signal for the adrenal glands to kick in. This signals the liver for more sugar. Symptoms from the adrenalin response: One of the first signs you might notice is an insatiable hunger. You’ve eaten but nothing you eat can seem to fill you up. If you have this feeling and you just can’t get enough to eat, check your blood sugar immediately. If it is lower than 70, get 15 grams of fast acting carbs in your body. This can come from a variety of sources: 4 ounces fruit juice 6 ounces regular soda (not diet) 4 or 5 saltine crackers 1 tablespoon of jelly or honey 8 pieces of hard candy Inability to rest at night. This could be due to nocturnal hypoglycemia. Signs of Nocturnal Hypoglycemia are: Restlessness. Making unusual noises. Attempting to get out of bed or accidentally rolling out of bed. Sleepwalking. Nightmares. Sweating. You may wake up with a headache in the morning if your blood sugar was low during the night. Sometimes a snack before bed can reduce the frequency and severity of these sleep disturbances. Some people have a strong feeling of anxiety. This is caused by that rush of adrenalin. This overload of adrenalin can cause some very anxious moments and can also make you feel panicked. You may have uncontrollable sweating even if you aren’t hot. This can be one of the very first signs of hypoglycemia. The central nervous system kicks in after the adrenalin has been pushed from the adrenal glands and this causes the autonomic nervous system to issue a “sweat order” to the skin. Shakes and tremors. Because of the adrenalin rush, the central nervous system starts to malfunction. As a result, it releases chemicals that encourage glucose production and also produce the tremors. Symptoms from lack of sugar in the brain: Fatigue can actually fall in both categories. There is a fatigue that can set in after an adrenalin rush, but also a fatigue from the lack of sugar in the brain. Either can be very serious. Emotional instability. Mood swings and sudden emotional episodes not typical of your normal behavior are among the neurological symptoms of hypoglycemia. These could include irrational outbursts, random crying for no reason which may become uncontrollable, extreme anger without cause, and a strong desire to be left alone. Mild mood changes that may not be as severe, such as general irritability or becoming easily annoyed, can also be a signal that your blood sugar may be dropping. Dizziness and light-headedness. If you experience these common symptoms of hypoglycemia, heed them and treat the hypoglycemia quickly. Dropping blood sugar levels can result in fainting. If you feel dizzy or light headed, sit or lie down immediately to avoid injuring yourself. Because the brain is especially sensitive to a drop in sugar, you may experience a sense of confusion and an inability to concentrate on one thing at a time. Your mind seems to wander all over the place. If your vision suddenly becomes blurry or you see double, a drop in blood sugar may be to blame. This is also a result of the brain’s lack of sugar affecting the optic nerve. Your sugar-starved brain may cause you to slur your words. To others this may resemble someone who has been drinking. But you may not even notice. These lists are not exhaustive, but are the more common symptoms of hypoglycemia. According to WebMD, Some people have no symptoms of low blood sugar. Darkening of the skin in certain areas may also occur. Under certain circumstances, an adrenal crisis may occur with low blood pressure, vomiting, lower back pain, and loss of consciousness. An adrenal crisis can be triggered by stress, such as from an injury, surgery, or infection.[1] Addison's disease arises from problems with the adrenal gland such that not enough of the steroid hormone cortisol and possibly aldosterone are produced,[1] most often due to damage by the body's own immune system in the developed world and tuberculosis in the developing world.[2] Other causes include certain medications, sepsis, and bleeding into both adrenal glands.[1][2] Secondary adrenal insufficiency is caused by not enough adrenocorticotropic hormone (ACTH) (produced by the pituitary gland) or CRH (produced by the hypothalamus). Despite this distinction, adrenal crises can happen in all forms of adrenal insufficiency. Addison's disease is generally diagnosed by blood tests, urine tests, and medical imaging.[1] Treatment involves replacing the absent hormones.[1] This involves taking a corticosteroid such as hydrocortisone and fludrocortisone.[1][3] These medications are usually taken by mouth.[1] Lifelong, continuous steroid replacement therapy is required, with regular follow-up treatment and monitoring for other health problems.[4] A high-salt diet may also be useful in some people. If symptoms worsen, an injection of corticosteroid is recommended and people should carry a dose with them. Often, large amounts of intravenous fluids with the sugar dextrose are also required. Without treatment, an adrenal crisis can result in death The signs and symptoms include fatigue; lightheadedness upon standing or difficulty standing, muscle weakness, fever, weight loss, anxiety, nausea, vomiting, diarrhea, headache, sweating, changes in mood or personality, and joint and muscle pains. Some patients have cravings for salt or salty foods due to the loss of sodium through their urine.[8] Hyperpigmentation of the skin may be seen, particularly when the patient lives in a sunny area, as well as darkening of the palmar crease, sites of friction, recent scars, the vermilion border of the lips, and genital skin.[9] These skin changes are not encountered in secondary and tertiary hypoadrenalism.[10] On physical examination, these clinical signs may be noticed:[8] Low blood pressure with or without orthostatic hypotension (blood pressure that decreases with standing) Darkening (hyperpigmentation) of the skin, including areas not exposed to the sun. Characteristic sites of darkening are skin creases (e.g. of the hands), nipple, and the inside of the cheek (buccal mucosa); also, old scars may darken. This occurs because melanocyte-stimulating hormone (MSH) and ACTH share the same precursor molecule, pro-opiomelanocortin (POMC). After production in the anterior pituitary gland, POMC gets cleaved into gamma-MSH, ACTH, and beta-lipotropin. The subunit ACTH undergoes further cleavage to produce alpha-MSH, the most important MSH for skin pigmentation. In secondary and tertiary forms of adrenal insufficiency, skin darkening does not occur, as ACTH is not overproduced. While this can be overcome by sugar fixes initially, in mild Adrenal Fatigue – and that’s why people take Coca-Cola, or Pepsi Cola, or pop drinks, as well as donuts or juices to give themselves that little boost, this is only a quick fix, and usually the symptoms will come back after only a short time, usually within one or two hours. - Hypoglycemia is also related to the autonomic nervous system dysfunction. Secondly, we have a musculoskeletal system breakdown. Now, collagen and protein are broken down in a catabolic state of function during Adrenal Fatigue. This can lead to chronic pain syndromes, joint pain, chronic fatigue, and fibromyalgia. - As cortisol goes up, one of the side effects of this hormone is the breakdown of protein, resulting in an overall net catabolic state, meaning the amount of buildup of the muscle is less than the amount of breakdown, so as a result, broken down muscles are not adequately replaced. Collagen is broken down without significant replenishment. Outwardly, wrinkles start to develop as premature aging sets in. Internally, organs and muscle breakdowns tends to lead to chronic muscle and joint pains of unknown origin, especially after strenuous exercise or heavy lifting. - Adrenal Fatigue therefore is often associated with poor ability to digest protein, as well as the gastrointestinal systems tend to slow down. This will only add to the musculoskeletal pain and syndromes. It should come, therefore, as no surprise that that secondary fibromyalgia and chronic fatigue are commonly associated with adrenal exhaustion. - Because Adrenal Fatigue can lead to insomnia, sleep disturbances, brain fog, anxiety and depression if the central nervous system is involved. Peripheral nervous system dysfunction is often associated with orthostatic hypertension, dizziness, lightheadedness, temperature intolerance, fainting spells, tingling and numbness sensation of extremities, as well as sweating disregulation. - Chronic lack of sleep is strongly associated with decreased immunity, as well, and impaired glucose intolerance, as well as the decreased alertness and concentration. Cortisol, DHEA, testosterone, and estrogen are key hormones produced by the adrenals, - Those with advanced Adrenal Fatigue are strongly associate with an increase in fear, anxiety, depression, as well as difficulty concentrating. - A lot of people also complain of brain fog, which is a mental state where the memory becomes clouded or unclear. Adrenal fatigue occurs when your adrenal glands aren’t able to produce enough of certain hormones, such as norepinephrine, cortisol and DHEA, which allow you to respond to the conditions of your daily life in healthy and flexible ways. Simply put, the theory behind adrenal fatigue is that your adrenal glands are unable to keep pace with the demands of perpetual fight-or-flight arousal. As a result, they can't produce quite enough of the hormones you need to feel good (1) So, What are Your Adrenal Glands? Located at the top of each kidney, the adrenal glands are two, triangular-shaped organs that measure about 1.5 inches in height and 3 inches in length They are also known as your body’s shock absorbers - produce hormones that help the body control blood sugar, react to stressors like a major illness or injury, and regulate blood pressure. Cause(s) of Adrenal Fatigue Adrenal fatigue is caused when your adrenal glands cannot cope due to you being overly stressed – basically, the main cause is stress The adrenal glands are an essential part of your body's responses to any kind of physical, emotional or psychological stress. Many different types of stress can cause adrenal fatigue. Examples of physical stress include any severe or recurrent infection or illness, having an operation, poor diet or a severe injury. An emotional or psychological stress may be caused by many situations, including relationship, family or work difficulties. (3) However, there are 3 specific hormones that can be a cause of adrenal fatigue: Norepinephrine Also called adrenaline is commonly thought of as the fight-or-flight hormone. It’s produced when something is (or you think it is) threatening. This hormone makes your heart pound, your blood rush to your heart and large muscle groups, your pupils widen, your brain sharpens, and your tolerance for pain increase—basically, it prepares you for battle. Cortisol Cortisol increases your appetite and energy level while toning down your immune system’s allergic and inflammatory responses. This hormone stimulates the storage and release of energy in the body, helps the body resist the stressful effects of infections, trauma, and temperature extremes, and helps you maintain stable emotions. Ideally, cortisol is released into the system only on an occasional basis, rather than in response to chronic stress. If cortisol levels become too high for too long, they may have undesirable side effects, including muscle wasting, kidney damage, fluid retention, spiking blood sugar levels, weight gain, and increased vulnerability to bacteria, etc. Solutions Rid the diet of excess sugar and processed grains These are very hard on the adrenal glands. Eating excess sugar/starches will cause blood sugar to spike and insulin to come to the rescue due to excess glucose in the bloodstream. Blood sugar then drops or plummets, causing the adrenals to work to bring levels back up. When you are suffering from fatigued adrenal glands, cortisol levels drop and make it difficult to maintain normal blood sugar levels. People with adrenal fatigue tend to have low blood sugar. This video discusses the role of adrenal glands and how the stress hormone, cortisol, can be managed. One thing I failed to mention above is that if you tend to have more of the hypoadrenal (low adrenal) symptoms such as fatigue, weight gain, low blood pressure, drops in blood sugar (hypoglycemia), can't maintain sleep, etc. you will need to eat like a hypoglycemic. This means eating food every 3 hours regardless of your level of hunger. Each of your meals should include some form of protein such as meat, eggs, legumes, nuts and seeds in them. However, if you tend to have more of the hypercortisol picture where you are more anxious, gain weight, feel like you have climbed a mountain by the end of the day, can't fall asleep at nights, etc, you will need to only eat when you feel hungry and in moderation. Some people have both hypo- and hyper-adrenal (cortisol) symptoms in which case having an Adrenal Stress Index (ASI) done would be invaluable. Also in all cases, you have to evaluate your life and eliminate or reduce all the energy robbers out of your life.

PHEOCHROMOCYTOMA Symptoms, Causes & Treatments OVERVIEW A pheochromocytoma (fee-o-kroe-moe-sy-TOE-muh) is a rare, usually noncancerous (benign) tumor that develops in an adrenal gland. Usually, this type of tumor affects one of your two adrenal glands, but it can affect both. If you have a pheochromocytoma, the tumor releases hormones that cause either episodic or persistent high blood pressure. Untreated, a pheochromocytoma can result in severe or life-threatening damage to other body systems, especially the cardiovascular system. SYMPTOMS Signs and symptoms of pheochromocytomas often include: • High blood pressure • Heavy sweating • Headache • Rapid heartbeat (tachycardia) • Tremors • Paleness in the face (pallor) • Shortness of breath (dyspnea) Less common signs or symptoms may include: • Anxiety or sense of doom • Constipation • Weight loss CAUSES Researchers don't know what causes a pheochromocytoma. The tumor develops in specialized cells, called chromaffin cells, situated in the center of an adrenal gland. These cells release certain hormones, primarily adrenaline (epinephrine) and noradrenaline (norepinephrine), that help control many body functions, such as heart rate, blood pressure and blood sugar. The role of hormones Adrenaline and noradrenaline are hormones that trigger your body's fight-or-flight response to a perceived threat. The hormones prompt higher blood pressure, a faster heart rate and a boost in other body systems that enable you to react quickly. A pheochromocytoma results in the irregular and excessive release of these hormones. Related tumors While most chromaffin cells reside in the adrenal glands, small clusters of these cells are also in the heart, head, neck, bladder, back wall of the abdomen and along the spine. Tumors in these chromaffin cells, called paragangliomas, may result in the same effects on the body. TREATMENTS The primary treatment for a pheochromocytoma is surgery to remove the tumor. Before you undergo surgery, your doctor will prescribe specific blood pressure medications that block the actions of the high-adrenaline hormones to lower the risk of developing dangerously high blood pressure during surgery. Pre-operative medications You will likely take two drugs for seven to 10 days that help lower blood pressure before surgery. These drugs will either replace or be added to other blood pressure drugs you take. • Alpha blockers keep smaller arteries and veins open and relaxed, improving blood flow and decreasing blood pressure. Alpha blockers include phenoxybenzamine (Dibenzyline), doxazosin (Cardura), and prazosin (Minipress). Side effects might include irregular heartbeat, dizziness, fatigue, vision problems, sexual dysfunction in men and swelling in your limbs. • Beta blockers cause your heart to beat more slowly and with less force. They also help keep blood vessels open and relaxed. In preparing for surgery a beta blocker is added several days after starting the alpha blocker. • Beta blockers include atenolol (Tenormin), metoprolol (Lopressor, Toprol-XL) and propranolol (Inderal, Innopran XL). Possible side effects include fatigue, upset stomach, headache, dizziness, constipation, diarrhea, irregular heartbeat, difficulty breathing and swelling in the limbs. • High-salt diet Alpha and beta blockers widen (dilate) the blood vessels, causing the amount of fluid within the blood vessels to be low. This can cause dangerous drops in blood pressure with standing. A high-salt diet will draw more fluid inside the blood vessels, preventing the development of low blood pressure during and after surgery. Surgery In most cases, the entire adrenal gland with a pheochromocytoma is removed with laparoscopic, or minimally invasive, surgery. Your surgeon will make a few small openings through which he or she inserts wandlike devices equipped with video cameras and small tools. The remaining healthy adrenal gland carries out the functions normally performed by two, and blood pressure usually returns to normal. In some unusual situations, such as when the other adrenal gland has been removed, surgery might remove only the tumor, sparing some healthy tissue. If a tumor is cancerous (malignant), the effectiveness of surgery might rely on removing the tumor and all cancerous tissue. However, even if all of the cancerous tissue isn't removed, surgery might limit hormone production and provide some control of blood pressure.

10 Warning Signs of Low Blood Sugar Hypoglycemia can cause both short- and long-term complications. Know the signs so that you can treat the condition as soon as you're aware of it. Hypoglycemia, or low blood sugar, is common among people with diabetes and can occur even when you're carefully managing the condition. To prevent hypoglycemia and its dangerous side effects, it's crucial to monitor your glucose levels and treat low blood sugar as soon as you become aware of it. Pay attention to these telltale signs of dipping blood sugar levels to make sure yours stays under control: 1. Ravenous Hunger If you've already eaten but still aren't satisfied, or if you suddenly, inexplicably feel as if you're starving, your body is signaling that it needs more glucose. Work with your healthcare team to determine the exact amount of sugar your body needs. A good starting point is the American Diabetes Association's recommendation to eat between 15 and 20 grams (g) of sugar or carbohydrates with each snack, and between 40 and 65 g at each meal. Some good options include 2 tablespoons of raisins, 4 ounces of fruit juice, or some hard candy — check with the package to determine how many to have. 2. Feelings of Anxiety When glucose levels fall too low, your body tells the adrenal glands to release the hormone epinephrine (also called adrenaline), which signals the liver to make more sugar. The excess epinephrine creates an "adrenaline rush," which can cause feelings of anxiety. 3. Restless Nights Nocturnal hypoglycemia, which is very common, can cause a number of sleep disturbances. "Symptoms include night sweats, nightmares, episodes of waking suddenly and crying out, and feelings of unrest and confusion upon waking," says Palinski-Wade. "A snack before bed can reduce the frequency and severity of sleep disturbances." 4. Shakes and Tremors The central nervous system starts to malfunction when glucose levels are off balance. As a result, it releases catecholamines, chemicals that encourage glucose production and also produce these symptoms. 5. Emotional Instability Mood swings and sudden emotional episodes not typical of your normal behavior are among the neurological symptoms of hypoglycemia, including irrational outbursts, random or hysterical crying, uncontrollable anger, and a strong desire to be left alone. Mild mood changes that may not be as severe, such as general irritability or becoming easily annoyed, can also be a signal that your blood sugar may be dropping. 6. Sweating This symptom is controlled by the autonomic nervous system — the part of the central nervous system that governs the skin, among other things — and is usually one of the first signs of hypoglycemia. The excessive perspiration comes on without warning, regardless of how warm or cold the external temperature may be. 7. Dizziness and Light-headedness If you experience these common symptoms of hypoglycemia, heed them and treat the hypoglycemia quickly. Dropping blood sugar levels can also cause you to faint, so if you feel yourself start to swoon, sit or lie down immediately to avoid injuring yourself. 8. Wandering Thoughts Because the brain is especially sensitive to a drop in glucose, you may experience a sense of confusion and an inability to concentrate on one thing at a time. 9. Vision Problems If your vision suddenly becomes blurry or you see double, a drop in blood sugar may be to blame. 10. Slurred Speech Your sugar-starved brain may not allow you to detect a change in how you sound, but other people will likely notice a difference.
