Welcome, to this edition of the First Aid Show. With first aid, we often look at someone's pulse. So, what we're going to do now is have a look at the different places on the body that we can detect a pulse. So now looking at the pulse points within the body, where can you detect a pulse on somebody? So, your primary pulse point would be your radial pulse, which is found in your wrist, just below your thumb. You should be able to feel your own pulse. So, that would be your starting point. Then working up your arm, you have a brachial pulse and then you also have one into the neck as well, which is your carotid pulse. Other pulses that you are able to detect would be your femoral pulses, which is just to where your leg and groin bend. You also have another pulse behind the knee, which is your popliteal pulse and then on top of the foot is your pedal pulse. And you should be able to feel all of your own pulses. When you take your pulses, obviously, you are going to use one hand. The only pulse you would ever use... You would never use two hands is the carotid. You would only ever feel one side at one time. And what sort of things are you feeling for? You actually feel the pulse pass through underneath your fingers and you will feel it just bounce slowly under your fingers. If you cannot find it, just have a fiddle around, you will eventually find it, just slight pressure on the wrist. If you press too deep, obviously, it will just feel as though you cannot feel anything, just come off very slightly. You will find it, you just have to have a dig around and find it in there. And will the pulse points, can you... Will that indicate blood pressure at all, if you lost the pulse point in a certain part of the body? Yes, normally the radial pulses, when somebody becomes unwell, the body is incredibly clever, and it will protect the vital organs. So, it will protect the brain, the heart, and all the internal organs. So, when you become unwell, you will lose what is called your peripheral pulses. So, you will lose your radial pulse, you will lose your pedal pulse and then you will slowly lose the pulses and it is to protect the main area of the body. Your carotid pulse, you should never lose, unless, of course, you have a cardiac arrest, or you die.

Learn to Measure Blood Pressure and Pulse - Medical Videos Step-by-Step Blood Pressure Check If you purchase a manual or digital blood pressure monitor (sphygmomanometer), follow the instruction booklet carefully. The following steps provide an overview of how to take your left arm blood pressure on either a manual or digital blood pressure monitor. Simply reverse the sides to take a blood pressure in your right arm. 1. Locate your pulse Locate your pulse by lightly pressing your index and middle fingers slightly to the inside center of the bend of your elbow (where the brachial artery is). If you cannot locate your pulse, place the head of the stethoscope (on a manual monitor) or the arm cuff (on a digital monitor) in the same general area.2. Secure the cuff Thread the cuff end through the metal loop and slide the cuff onto your arm, making sure that the stethoscope head is over the artery (when using a manual monitor.) The cuff may be marked with an arrow to show the location of the stethoscope head. The lower edge of the cuff should be about 1 inch above the bend of your elbow. Use the fabric fastener to make the cuff snug, but not too tight. Place the stethoscope in your ears. Tilt the ear pieces slightly forward to get the best sound. 3. Inflate and deflate the cuff If you are using a manual monitor: Hold the pressure gauge in your left hand and the bulb in your right. Close the airflow valve on the bulb by turning the screw clockwise. Inflate the cuff by squeezing the bulb with your right hand. You may hear your pulse in the stethoscope. Watch the gauge. Keep inflating the cuff until the gauge reads about 30 points (mm Hg) above your expected systolic pressure. At this point, you should not hear your pulse in the stethoscope. Keeping your eyes on the gauge, slowly release the pressure in the cuff by opening the airflow valve counterclockwise. The gauge should fall only 2 to 3 points with each heartbeat. (You may need to practice turning the valve slowly.) Listen carefully for the first pulse beat. As soon as you hear it, note the reading on the gauge. This reading is your systolic pressure (the force of the blood against the artery walls as your heart beats). Continue to slowly deflate the cuff. Listen carefully until the sound disappears. As soon as you can no longer hear your pulse, note the reading on the gauge. This reading is your diastolic pressure (the blood pressure between heartbeats). Allow the cuff to completely deflate.

The first heart sound “lub”, also known as S1, is caused by the closing of the AV valves after the atria have pumped blood into the ventricles. The second heart sound “dub”, or S2, originates from the closing of the aortic and pulmonary valves, right after the ventricles have ejected the blood. The time interval between S1 and S2 is when the ventricles contract, called SYSTOLE. The interval between S2 and the NEXT S1 is when the ventricles relax and are filled with blood, called DIASTOLE. Diastole is longer than systole, hence the lub-dub, lub-dub, lub-dub… Heart sounds are auscultated at 4 different sites on the chest wall which correspond to the location of blood flow as it passes through the aortic, pulmonic, tricuspid, and mitral valves, respectively. This is how SIMILAR defects associated with DIFFERENT valves are differentiated. Heart murmurs are whooshing sounds produced by turbulent flow of blood. Murmurs are diagnosed based on the TIME they occur in the cardiac cycle, their changes in INTENSITY over time, and the auscultation SITE where they are best heard. Examples of conditions associated with common systolic murmurs include: - MITRAL valve regurgitation, when the mitral valve does NOT CLOSE properly and blood surges back to the left atrium during systole. The murmur starts at S1, when the AV valves close, and maintains the same intensity for the entire duration of systole. This holosystolic murmur is best heard at the mitral region -the apex, with radiation to the left axilla. Because the valve closure in mitral regurgitation is INcomplete, S1 is often quieter. On the other side of the heart, a TRICUSPID valve regurgitation has similar timing and shape, but it is loudest in the tricuspid area and the sound radiates up, along the left sternal border. - AORTIC valve stenosis, when the aortic valve does NOT OPEN properly and blood is forced through a narrow opening. The blood flow starts small, rises to a maximum in mid-systole at the peak of ventricular contraction, then attenuates toward the end of systole. This results in a crescendo-decrescendo, or a diamond-shaped, murmur which starts a short moment after S1. It is often preceded by an ejection click caused by the opening of the STENOTIC valve. Aortic stenosis murmur is loudest in the aortic area and the sound radiates to the carotid arteries in the neck following the direction of blood flow. Again, on the other side of the heart, a PULMONIC stenosis has the same characteristics but is best heard in the pulmonic area and does NOT radiate to the neck. Other conditions that cause audible systolic murmurs include ventricular septal defect and mitral valve prolapse. An example of diastolic murmurs is aortic valve regurgitation. This is when the aortic valve does NOT CLOSE properly, resulting in blood flowing back to the left ventricle during diastole- the filling phase. As the blood flows in the REVERSE direction, the murmur is best heard NOT in the aortic area, but rather along the left sternal border. It peaks at the beginning of diastole when the pressure difference is highest, then rapidly decreases as the equilibrium is reached. Other common diastolic murmurs are associated with pulmonic regurgitation, mitral stenosis and tricuspid stenosis.

Today, the County of San Diego and its partners provided free blood pressure screenings to the public at select sites throughout the region. It was all part of the Love Your Heart program which urges residents to “know their numbers” and take charge of their own heart health. The screenings took place at over 170 sites around the county. Now in its sixth year, Love Your Heart has grown to include sites in Orange and Riverside counties, Massachusetts and in each of the states on both sides of the United States-Mexico border. Heart disease is the leading cause of death in the United States, and the second leading cause of death in San Diego County. Obesity, high blood pressure, high cholesterol, diabetes and a family history of heart disease can put an individual at greater risk of heart disease. Getting a blood pressure check is a simple step to take for heart health. Last year, a record 30,000-plus people got their blood pressure checked at Love Your Heart in San Diego County and Mexico.
